In the future, pharmacists are going to provide more direct patient care, like pharmacogenomics, such as point of care testing for influenza and strep throat, and collaborative practice agreements that allow us, through a physician sign-off or medical practice sign-off, to manage and monitor medications." Steven Simenson, BPharm, FAPhA, FACA, DPNAP is the CEO and Managing Partner of Goodrich Pharmacy, Inc., with five community pharmacies in Minnesota. He is a graduate of the University of Minnesota College of Pharmacy along with his wife, Wendy, also a Pharmacist. Steve was the 2013-2014 President of the American Pharmacist's Association and is on the Board of Directors of Pharmacists Mutual Insurance Companies and the Community Pharmacy Foundation. She completed her MScMed (clinical pharmacy) 1997. Sonya was employed at Netcare Linksfield as the clinical pharmacist since Mid-2010 until April 2017. She successfully implemented the antibiotic stewardship programme at Netcare Linksfield in the ICU and roll out to the rest of the hospital is underway. She was part of the Netcare leadership group for antibiotic stewardship. Sonya moved to the Nelson Mandela Children Hospital as the clinical pharmacist in 2017. There, she is responsible for clinical pharmacy activities in the hospital especially in the NICU and PICU. She is responsible for all antimicrobial stewardship activities in the hospital and is chairperson of the hospital's AMS committee. Sonya is a founding member of the Society of Clinical pharmacy (SASOCP). She was the chairperson 2011 – 2015, Vice chairperson 2016-2019 and national treasurer from 2019-2021 She is an external examiner for the MscMed clinical pharmacy programme at Medunsa university, a part time lecturer at School Pharmacy Wits University and a member the Gauteng Antimicrobial stewardship committee and part of the guideline subcommittee. Our host Scott Vondeylen is joined by Chief Scientific Officer for Tabula Rasa HealthCare (TRHC), Jacques Turgeon, BPharm, PhD to discuss how MedWise® Science is empowering pharmacists to impact patient outcomes—now and in the future. MedWise Science, TRHC's proprietary technology, is a clinical decision support system that provides pharmacists—and other health professionals—with highly valuable information that helps them make the proper decisions for the pharmacotherapy of their patients. Beyond ensuring patients receive the proper medication for a particular disease state, MedWise Science also helps ensure that the combination of drugs prescribed—the patient's drug regimen—is optimal. This is critical because when patients have several conditions, they are often required to take multiple medications, which can put a patient's health at risk by significantly increasing the risk of an Adverse Drug Event (ADE), the 4th leading cause of death in the United States. Preventing and decreasing ADEs is important for multiple reasons. When patients start to experience side effects, they may stop taking the medication, which means patients are no longer receiving the intended benefit of their medication. Further, non-adherence leads to poor outcomes, an increase in doctor and emergency room visits, and an increase in medical expenditure—it's all linked.Utilizing decision support tools backed by science in conjunction with taking a holistic view of drug regimens can have a major impact on the way pharmacists deliver care to improve medication safety and patient outcomes. To that end, MedWise Science utilizes a one-of-a-kind algorithm to establish the MedWise Risk Level and to cumulatively compare how patients' medications interact together, all at the same time.Pharmacists can leverage the MedWise Risk Level to determine a patient's risk for experiencing an ADE, and then be properly alerted to intervene and recommend a Medication Safety Review™ to address concerns and improve medication efficacy. TRHC provides training to ensure that pharmacists can properly use the MedWise decision support tools to conduct Medication Safety Reviews in the pharmacy, as well as offering telephonic support via its national clinical contact centers. The value of MedWise Science has been proven and now there is scientific data to show an association between the level of risk and outcomes. As a patient's MedWise Risk Level increases, so does their risk of experiencing an ADE, which ultimately leads to an increase in medical expenditure. In fact, TRHC's Precision Pharmacotherapy Research and Development Institute recently conducted a study in conjunction with DARTNet Institute and found a correlation between risk of premature death and higher medication risk scores, affirming that the appropriate use of medications is critical to avoid harm. The current use and value of the MedWise technology is clear, but that begs the question: What's next? According to Dr. Turgeon, it appears the future value of MedWise Science could be virtual. Imagine being able to virtually add different medications to a patient's regimen and then conduct simulations to determine how the MedWise Risk Level may change, altering ADE risk and subsequently impacting outcomes. Well, this is exactly how Dr. Turgeon and his team have already utilized the MedWise technology in two studies—one with elderly patients and another with Medicare patients. By continuing to generate data in large cohorts with multiple disease states, who are managing various drug regimens, can ultimately become a tool for the pharmaceutical industry to test the impact of a new medication coming on the market. Imagine being able to know in advance if and how a medication could change a patient's MedWise Risk Level. In this scenario, prescribers would have a good idea of potential risk factors, pre-identify populations for ADEs, and be able to limit exposing patients to unwanted side effects. Cristina was born in Madrid, Spain and came to Canada with her parents at the age of 7. The second of six children, she grew up in Ottawa, Montreal, and Vancouver. In 1990, Cristina graduated from pharmacy school at the University of Montreal, and after spending 7 years working in the hospital setting in Montreal and Toronto, she moved out West and discovered her love for the preventive side of medicine.  Early in her pharmacy career, Cristina realized that her vision for excellent and ethical healthcare would conflict with the growing anti-life and anti-Catholic sentiments prevalent in society. This lead her to seek support from Pharmacists for Life as a student, later acting for several years as coordinator for Western Canada. Cristina went on to serve briefly on several prolife Boards, most recently finishing a 6- year term with COLF (Catholic Organization for Life and Family).  However, her greatest passion lies in seeking to imbue the professional world with upright principles and fighting for conscience rights, so as to safeguard healthcare's intrinsic life-giving mission. In the year 2000, Cristina's refusal to dispense abortifacient drugs brought her head-to-head with her professional licensing College. With the help of like minded professionals, she lobbied the College for change, raising awareness by putting out press releases under the auspices of “Pharmacists for Conscience”.  In 2007 Cristina equipped herself with a masters in Bioethics from the University of Navarre in Spain. Her thesis examined the Codes of Ethics of Pharmacy Licensing bodies across Canada, bringing to light the lacks of tolerance towards religious believers and conscientious objectors. A summary was published in 2009 in the Annals of Pharmacotherapy under the title “The Hijacking of Moral Conscience from Pharmacy Practice: A Canadian Perspective”, and can be accessed at the Protection of Conscience Project website. http://www.consciencelaws.org/ethics/ethics094.aspx  Thankfully, Cristina's lengthy battle with her licencing College over the importance of respect for conscience and religion has not prevented her from enjoying a successful and rewarding pharmacy career. She worked at the same community pharmacy from April 2000, managing its services since 2012, until it was sold to a Corporation in December of 2018. She sat on the Ethics Advisory Committee of the College of Pharmacists of British Columbia for many years, and sits on the Ethics Committee of Vancouver Coastal Health.  She is a clinical pharmacy instructor for the University of British Columbia, and thoroughly enjoys mentoring students. Cristina writes regularly on a variety of topics for the Canadian Healthcare Network in her Blog “From Pill to Till”, and has been published in various newspapers and ethics blogs over the years.
