In this episode of A Shot in the Arm Podcast, hosts Yvette Raphael and Ben Plumley discuss the resilience of South Africa's healthcare system amidst U.S. aid cuts, particularly through USAID and PEPFAR. They highlight the devastating impacts on HIV treatment, TB care, and broader healthcare services due to the sudden cessation of funding. But the country is markedly more prepared than critics might have feared, to assume full responsibility for its infectious diseases strategies - including procurement and surveillance strategies that the US maintained control over in exchange for the aid. Their conversation extends to cover issues around mental health, future healthcare innovations like long-acting antiretrovirals, and the broader geopolitical implications of donor aid cuts.   00:00 Introduction and Setting the Scene 00:33 Impact of US Aid Cuts on South Africa 03:08 Healthcare Challenges and Government Response 07:04 The Role of Civil Society and Future Preparations 10:21 Consequences of Sudden Aid Withdrawal 14:17 Future of HIV Treatment and Advocacy 16:55 The Threat of Drug-Resistant TB 17:35 Government Investment in Healthcare 19:01 Mental Health Crisis Among Youth 19:41 Impact of USAID Funding Cuts 20:57 Soft Power and International Relations 22:37 South Africa's Self-Reliance 26:43 Addressing Racism and Emigration 32:42 Parting Thoughts and Optimism

The latest episode of The Top Line, sponsored by ViiV Healthcare, features Dr. Luis Buzón, an infectious disease specialist, discussing the critical role of real-world evidence in HIV treatment and prevention. While clinical trials establish the safety and efficacy of new therapies, real-world data help bridge the gap between these highly controlled environments and patient care in the real world. Buzón explains that collecting long-term data is essential, especially for lifelong conditions like HIV. Not only do real-world studies track how medicines perform over longer periods of time than clinical trials, they include a broader range of people, revealing potential side effects not seen in trials and ensuring long-term effectiveness for the different people who can be impacted by HIV. Beyond the science, Buzón shares how his father was a pioneer in Spain’s early response to the HIV epidemic, and why, inspired by his father’s dedication, he continues to advocate for compassionate, evidence-based care. For those interested in the evolving landscape of HIV treatment and prevention and the power of real-world data in shaping better health outcomes, this episode is a must-listen. They examine various innovative approaches that are transforming the landscape of HIV care.Dr. Darrell Tan, a clinician scientist at St. Michael's Hospital and associate professor at the University of Toronto, discusses the article he co-authored, "Long-acting injectable antiretroviral therapy for HIV-1 infection in adults." He explains the newly approved long-acting injectable cabotegravir for PrEP, an integrase inhibitor administered as a deep intramuscular injection administered every two months. Dr. Tan highlights its superior effectiveness compared to daily oral PrEP, emphasizing its adherence benefits and potential to revolutionize HIV prevention. Additionally, he covers the long-acting injectable treatment regimen for HIV, which combines cabotegravir with rilpivirine, administered as two intramuscular injections every one to two months. This regimen offers an alternative to daily oral medication, easing the treatment burden for individuals living with HIV.Dr. Maxime Billick, a graduating infectious diseases fellow at the University of Toronto and co-author of "HIV postexposure prophylaxis-in-pocket," introduces the concept of "PIP" (prophylaxis-in-pocket). She explains how this approach allows individuals who might rarely have high-risk encounters to keep a 28-day supply of PEP (post-exposure prophylaxis) on hand, enabling immediate use after potential HIV exposure. Dr. Billick discusses the importance of ongoing patient-provider relationships and the role of PIP in empowering patients to proactively manage their sexual health.Throughout the episode, the hosts and guests explore the challenges and opportunities presented by these new treatments. They emphasize the importance of accessibility, awareness, and the need to address health inequities in HIV prevention. The conversation underscores the potential of these advancements to significantly impact public health and improve the quality of life for individuals at risk of or living with HIV.Join us as we explore medical solutions that address the urgent need to change healthcare. This week on “The Top Line,” we're speaking with Jared Baeten, MD, Ph.D., Vice President, HIV Clinical Development, Gilead Sciences about how advancements in HIV treatment research have enabled individuals to lead longer and healthier lives by taking and staying on medication. Nevertheless, HIV persists as a significant public health challenge. To end the HIV epidemic, it is crucial to go beyond a biomedical approach and fully integrate social determinants of health into the collective response. Not everyone is achieving long-term success.  Long-term success involves meeting more than the United Nations' goals of ending the epidemic by 2030. Treatment selection can help prioritize long-term outcomes from the very beginning. There are several other key factors for people with HIV and their healthcare providers to consider when assessing treatment choices, such as the resistance and safety profile of a treatment, drug-to-drug interaction potential, and comorbid conditions. This podcast is brought to you by Gilead Sciences.
