A quiet July 5 holiday morning was marred by the crackle of the police radio, alerting lawmen and firefighters of a wreck with entrapment on the Bashi Road in northwestern Clarke County shortly after 11: 30 a. m. Soo n, one fatality was acknowledged, and then two. Both drivers had been killed in the head-on crash near 12407 Bashi Road , about 12 miles west of Thomasville. It was a warm evening with occasional breezes, singing birds, and buzzing insects as the cold mountain stream trickled by. The low call of a male Sooty Grouse can be heard in the distance. There is a little noise pollution in the form of air traffic on this recording, but I felt that it wasn't too bad not to release it. This was recorded on International Dawn Chorus Day at Bigelow Meadow Botanical Area in Shasta-Trinity National Forest, Siskiyou County, California. International Dawn Chorus Day is held on the first Sunday in May and was created to encourage people to get up early and listen to the beautiful sounds of birdsong. You can learn more about it here- https://www.wildlifetrusts.org/dawn-chorus-day It was a cool and breezy morning and I have to confess there weren't quite as many birds on this morning as I have heard at this location in the past. Nevertheless, this recording was made on International Dawn Chorus Day and I didn't want to replace it with a recording which had more abundant birdsong. So in the spirit of the day I am sharing this with you. That said it was a beautiful morning at a very beautiful and secluded meadow and I look forward to gathering recordings at this location in the future. As an aside, I did have an interesting Black Bear encounter while I was there. I set up my recording rig and moved to the other side of the meadow so as not to disturb the natural behavior of the birds and other wildlife. As I was quietly sitting and taking in the scenery, a large Black Bear silently walked out onto the meadow not more than fifty feet from me. I was looking in the other direction when my dog alerted me to it's presence with a bark and scared the bear into the brushy forest which borders the meadow. I see Black Bears frequently but have never had one approach so quietly. It was an interesting and unnerving experience. Circulation Bariatric Surgery and Cardiovascular Outcomes in Patients With Obesity and Cardiovascular Disease: A Population-Based Retrospective Cohort Study Circulation 2021 Apr 13;143(15)1468-1480, AG Doumouras, JA Wong, JM Paterson, Y Lee, B Sivapathasundaram, JE Tarride, L Thabane, D Hong, S Yusuf, M Anvari Patients with CVD who underwent bariatric surgery were matched 1:1 with similar CVD patients who did not undergo bariatric surgery. primary outcome was major adverse cardiovascular events (MACE) (first occurrence of all-cause mortality, myocardial infarction, coronary revascularization, cerebrovascular events, and heart failure hospitalization) n follow-up of 4.6 years, the primary outcome was lower in the surgery group compared with the control group occurred in 11.5% (151/1319) of the surgery group and 19.6% (259/1319) of the controls that is roughly a NNT of 12. But enough with the observational trials- do what we want to see or don’t do it. And while observation studies annoy me, sometimes they are necessary evil lead to practice or board changing answers, Girometti N et al. And At 30 days after completion of therapy, 91% of patients receiving parenteral penicillin therapy and 100% of doxycycline recipients achieved symptomatic resolution. But enough of the observation data lets move onto a viewpoint in JAMA Industry-Sponsored Speaker Programs—End of the Line? | Law and Medicine | JAMA | JAMA Network November 16, 2020 for only the 6 times in 20 years the the Office of Inspector General (OIG) for the US Department of Health and Human Services (HHS) issued a Special Fraud Alert on “abuse risks associated with the offer, payment, solicitation, or receipt of remuneration” relating to industry-sponsored speaker programs Now when I was a resident I took a free dinner but I was never invited back because I would ask a bunch of hard questions about the methods of the study that the speaker wasn’t prepared to answer. It was entertainment, education, and a free meal. SINCE becoming an attending I have not been to one and I said that with pride. industry-sponsored speaker programs dates back to the 1950s and they have always been ‘dirty’ with physician kick backs and bias. And off these CME or speaker sponsored programs are “offered under circumstances that are not conducive to learning. ALSO lets be very clear often these drugs are not better than current standard of practice and they are more expensive which is a losing situation for our patients. But why this fraud alert issued?? Well July 1, 2020, During covid peak, Novartis quietly agreed to pay 678$ million dollar settlement for fraud charges of payment to physician and speak programs. And I quote- Novartis “violated the federal False Claims Act and Anti-Kickback Statute by providing doctors with cash payments, recreational outings, lavish meals, and expensive alcohol to induce them to prescribe Novartis cardiovascular and diabetes drugs reimbursed by federal healthcare programs.” From 2017 to 2019, drug and device companies reported paying health care professionals nearly $2 billion in compensation for services other than consulting but I suspect this will be going down significantly in the near future as it is hard to continue to issue kickbacks when you have the attention of the office of the inspector general. SOO if you are a big pharma industry sponsored event attendee that like a free industry sponsored meal, enjoy it while you can because this for the betterment of medicine and the well being of our patients is likely going away. And while we are talking about government lets talk politicians and something they did positive which is usually few and far between but on December 27, 2020, congress passed the Surprises Act — which banned “surprise billing” and in order to talk about how this is a good thing lets quickly make sure we are all on the same page. Lets say a patient gets sick and goes to the ER and then gets admitted to the hospital. Once in the hospital that patient has no control over what doctors they see. IF they see a doctor or two that are out of their network. Since insurance plans aren’t required to pay out-of-network providers their full charges, clinicians may bill the patient for the difference between the insurance payment and their charges. This ends of up being a surprise bill and usually a surprise that is a lot of money. This is terrible. You get sick, you just want to get better, you are at a hospital and you have no control if the pulmonologist or cardiologist is in your insurance plan but yet SURPRISE you get stuck with the bill. Episode 72: McCloud River Trail- Spring Morning Walk in the Rain

This was recorded on a cold and rainy spring morning while walking along the McCloud River Trail in Shasta-Trinity National Forest, Siskiyou County, California. The rain fell steadily as I strolled along, at times mixed with wet and slushy snow, and heavy water drops fell from the trees as the rain collected in the foliage before dripping down to the forest floor. This recording was made using a binaural microphone configuration which means the microphones were placed at ear level on either side of the head in order to reproduce a lifelike stereo recording which comes as close as possible to what someone would hear were they actually at the location themselves. In this case the microphones were clipped below the brim of a hat I wore both to protect the microphones and my head from getting soaked by the rain and heavy water droplets dripping from the trees. The rain had softened the trail surface which muffled my footsteps and allowed for a much quieter walk than if the trail had been dry, with leaves, twigs, and pine needles crunching underfoot. The frogs sang in chorus continuously before an unidentified animal walked onto the wet and grassy stage and compelled them to end their performance. There is unfortunately some noise pollution on this recording, in the form of air traffic, but the frogs were singing loudly and I didn't think it overshadowed their performance. This recording was gathered on a chilly night in early spring beside the campfire at my campsite in the Crystal Springs Campground in Ahjumawi Lava Springs State Park, Shasta County, California. A large deciduous tree had fallen across the trail not far from my campsite, and the park maintenance workers had cut out the section blocking the trail, which I used for firewood. I am not quite sure what kind of tree it was, but the wood, some of which was studded with acorns by busy acorn woodpeckers, burned nicely without a lot of loud snaps and pops, and the fire hissed as the moisture within the wood heated and escaped as steam.  Sound By Nature is produced by me with the generous support of listeners like you. Episode 67: Ahjumawi Lava Springs State Park: Morning Beside Ja She Creek

This was recorded on an early spring morning next to the stone fish traps at Ja She Creek in Ahjumawi Lava Springs State Park, Shasta County, California. Ja She Creek is more of a series of interconnected ponds than a creek in the classic sense, all of which are fed by countless underwater springs. The stone fish traps were constructed by the indigenous people who have inhabited the area for thousands of years, the Ajumawi(as spelled on the tribal website) band of the Pit River Tribe, for whom the park is named. The traps were made to catch Sacramento sucker fish, which were actively spawning as this recording was being made and can be heard splashing in the shallows at water's edge. You will also hear the raucous calls of both Stellar and Scrub Jay's, Acorn Woodpecker's, numerous songbirds, an abundance of waterfowl, and more.
