This week I'm talking with Dr. Maggie Sibley, a clinical psychologist and professor at the University of Washington School of Medicine. Dr. Sibley has spent over two decades studying ADHD, and is author or co-author of over 120 research papers on the topic. And she is the author of Parent-Teen Therapy for Executive Function Deficits and ADHD: Building Skills and Motivation. So recently, when I was working on the newsletter for the show, I came across an article about ADHD titled "Study describes fluctuations, remissions seen with ADHD,” and that felt like it was worth investigating more. While reading through the paper that was linked into the article I got to thinking, “hey, I'd love to ask some more questions about the findings in this paper,” and it occurred to me, hey, I can just reach out to the author of the paper for a conversation on the podcast. And so that's what today's show is all about, we dig into that paper, titled “Characteristics and Predictors of Fluctuating Attention-Deficit/Hyperactivity Disorder in the Multimodal Treatment of ADHD (MTA) Study” that looks into symptom fluctuation based on the a review of the Multimodal Treatment of ADHD (MTA) Study. We talk about how ADHD symptoms don't just disappear but actually tend to fluctuate — a lot more than many researchers expected. We also dive into why having more going on in life might actually make your ADHD symptoms less severe (or how that's just one interpretation of the results), how motivation works for us, and what it means to find your own “sweet spot” of structure. Symptoms can often fluctuate, sometimes improving for years and then intensifying again. Expect waves, not a straight line, and don't blame yourself when experiencing higher-than-normal symptoms. When you're in a phase where ADHD feels more manageable, that's a great time to try and take on more meaningful responsibilities — like work, school, or parenting — that can help create external structure and reinforce good patterns. While having more life demands (like a busy schedule, work responsibilities, or kids) can improve ADHD functioning by creating natural urgency and external motivators, it's also important to make sure it doesn't tip into overload. Not all clinicians are trained to recognize ADHD in adults, especially when childhood histories are murky. If your concerns are dismissed, it's okay — and important — to seek out a more knowledgeable provider. And on that note, look out for updated diagnostic and treatment guidelines for adult ADHD from APSARD (American Professional Society for ADHD and Related Disorders). While these guidelines won't change any of the diagnostic criteria in the DSM, they will help give clinicians clearer, evidence-based advice on how to apply them when evaluating ADHD in adults.

Earlier this year, Hyperfocus took a field trip to San Diego for an ADHD conference called APSARD, which stands for the American Professional Society of ADHD and Related Disorders (quite a mouthful).A conference of ADHD professionals might not sound fascinating, but it was — and we learned a ton. But there was one person and one talk I really wanted to find: A keynote speech about how ADHD affects women's bodies by Dr. Sandra Kooij. What Sandra is doing is something so sensible and radical: Looking at ADHD as a whole-body issue. Digging into how it can affect women's hormones, our health, and especially our hearts. I find Sandra's research fascinating and was thrilled when she agreed to sit down for a conversation with me. MBP refers to when a caregiver abuses a child by pretending they are ill in order to get attention for themselves. With the rise and prevalence of social media there are more and more cases of Munchausen by Proxy and Munchausen syndrome happening in the digital world. There are cases of people pretending to be sick themselves to get attention and cases of caregivers pretending their children are ill to get attention, and sometimes to raise money. To talk about all of this I am joined by Andrea Dunlop, the host and executive producer of NOBODY SHOULD BELIEVE ME an investigative true crime podcast about Munchausen by proxy. Parenting the neurotypical teenager is not a job for the faint at heart, but when your teen has an ADHD diagnosis, it can feel downright daunting. In this week's episode, although my guest, Dr. Mark Bertin, (a board certified developmental behavioral pediatrician, on the board of directors for the American Professional Society of ADHD and Related Disorders) specifically addresses parents of teens with ADHD, much of his wisdom applies to most parents of teens and tweens.  As the parent of an adolescent, it can be so frustrating to deal with the daily homework battles, checking behind them, constantly reminding, not to mention the emotionality and the behavior issues that often intensify beginning in middle school. But for parents of a teen with ADHD, these issues are 10-fold because of the additional delay your kid has is self-management skills. Add in IEPs or 504s, evaluations, doctor visits, medication issues, communications with the school…and you have a recipe for major stress, overwhelm and exhaustion.Learning to manage your reactivity to your teen's behavior is crucial for their wellbeing and yours. This is true not only for parents of a teen with ADHD, but for parents of any teen. And so much of our behavior as parents, depends on our own thoughts and emotions – expectations for ourselves, judgment from others, questions about our teen's behavior, etc. Dr. Bertin gives us guidance for reframing the way we think about these issues, discusses mindfulness and its impact on our reactivity, and addresses specific ADHD-related concerns as well. Julie talks about what she observes during trainings that would be helpful for interviewers and investigators as we think about the multi-disciplinary approach.Julie Kenniston, MSW, LISW is a Program Manager for the National Criminal Justice Training Center of Fox Valley Technical College. Julie has worked 15 years in child protection in two different counties in Ohio. She began as a caseworker on the Sexual Abuse Investigation Team, conducting forensic interviews for their MDT. Later in her career, she went back to child protection as a Training Director and one of her duties was to manage a nonprofit that provided services to families dealing with abuse and violence. Julie has trained nationally and internationally on topics related to child abuse, domestic violence, and child sex trafficking. She is a faculty member for three nationally recognized forensic interview programs: the American Professional Society on the Abuse of Children (APSAC), Ohio Attorney General's Finding Words, and the Child Forensic Interview Training (formerly known as The Childhood Trust Forensic Training Institute). She has conducted over 3,000 forensic interviews and continues to interview as a contract forensic interviewer for the Northern Kentucky Children's Advocacy Center. A former APSAC board member, she co-chairs forensic interviewing committees. Julie has written many articles and curricula regarding forensic interviewing, including her work with Modell Consulting Group, LLC where she has co-authored multiple curricula on interviewing individuals with disabilities. Rita Farrell has over 23 years of experience working with child abuse investigations as a forensic interviewer and founding executive director of a child advocacy center. In her role as forensic interview specialist and trainer for Zero Abuse Project, Rita admits that her role as a forensic interviewer of child abuse victims is her “true passion.” After graduating from Ball State University with a degree in secondary education, Rita began teaching in secondary and post-secondary classrooms. While raising her two sons in Fishers, IN, Rita held various leadership positions throughout the community. In July 2000, Rita undertook the challenge of opening Chaucie's Place, a child advocacy center in Hamilton County, IN. The combination of her roles as executive director and forensic interviewer naturally led Rita to become involved with state and national organizations in the field of child abuse. Rita serves as the Director, ChildFirst® for Zero Abuse Project, managing the national ChildFirst® Forensic Interview Training program. She is a co-author of the ChildFirst® Forensic Interview Protocol and has conducted more than 2,900 child interviews. In addition, she manages the development of advanced training courses and programs for forensic interviewers and investigators. Rita is a certified law enforcement instructor and still conducts forensic interviews in the field. She has testified in numerous court cases and has authored many peer reviewed articles specific to child abuse investigations and forensic interviewing of child abuse victims. As a national speaker, Rita continues to champion the cause of child advocacy and forensic interviewing. Rita serves as a board member for the Indiana Coalition of Child Advocacy Centers. She is a member of the American Professional Society on the Abuse of Children (APSAC) and serves on numerous councils, task forces and boards in the areas of child abuse and domestic violence. Her team has published several seminal publications on the impact of disasters on youth mental health (JAMA Network Open), and the implementation and dissemination of in-person and telehealth delivery of Trauma Focused Cognitive Behavioral Therapy (TF-CBT) in Latin America and US (American Psychologist). She is a co-author in the first telehealth manual available in Spanish published in January of 2022 (Manual de Telesalud Mental). She is the 2022 Association for Behavioral and Cognitive Therapies (ABCT) Program Chair and serves on the Executive Board of the American Professional Society on the Abuse on Children (APSAC). Victor gained national recognition for his work in addressing child abuse in small communities as a prosecutor in rural Minnesota, and has been named to the President's Honor Roll of the American Professional Society on the Abuse of Children. In 2012, Victor was awarded the Pro Humanitate Award from the North American Resource Center for Child Welfare. After several years in general pediatrics he completed fellowship in neurodevelopmental behavioral pediatrics at the Children's Evaluation and Rehabilitation Center (Rose Kennedy Center) at the Albert Einstein College of Medicine in New York. Dr. Bertin is an Assistant Professor of Pediatrics at New York Medical College, is on the faculty of the Windward Teacher Training Institute, and from 2003 to 2010 was Director of Developmental Behavioral Pediatrics at the Westchester Institute for Human Development, working in their foster care program. He is on the advisory board for Reach Out and Read, a national organization promoting child development and literacy, and also for Common Sense Media. He is on the board of directors for the American Professional Society of ADHD and Related Disorders (APSARD). He is a frequent lecturer for parents, teachers and professionals on topics related to child development including autistic spectrum disorders, ADHD, parenting and mindfulness. In this second episode of our two-part conversation with Michele Thames and Christine Rouse, forensic interviewers from Virginia, we discuss the importance of leadership to the success of a forensic interviewing program. Episode Summary When child abuse occurs, cases may be investigated using a Children's Advocacy Center (CAC) and a Multidisciplinary Team (MDT). Forensic Interviewers are often employed at CACs and tasked with gathering reliable information from children. An interviewer is required to have knowledge in child development, memory formation, cultural influences, state and federal laws, and skills in questioning children. During this part of our series on forensic interviewing skill development, Michele Thames and Christina Rouse share their insights on the importance of leadership, program support, and proper training programs to the enhancement of any forensic interviewing program. Guest Bios Michele Thames is the Executive Director of SafeSpot Children's Advocacy Center in Fairfax, VA/ She holds a Master's Degree in Education from Cambridge College. She was previously the Supervisor of the Forensic Interview Program at the Children's Hospital of The King's Daughters Child Abuse Program in Norfolk, VA. In Search of the Perfect Interview Is your team searching for the perfect forensic interview? Although a “perfect” outcome might not be possible, the journey is important. In this episode, I speak with two forensic interviewers from the Commonwealth of Virginia about that journey. We discuss some of the challenges that might get in the way and what interviewers and team members can do to overcome the obstacles that impact the journey toward perfection. Episode Summary In the last 5 years, over 1 million children have received forensic interview services at Child Advocacy Centers across the United States. In this second episode of a 4-part series, I speak with Michele Thames and Christina Rouse about what forensic interviews and multidisciplinary teams can do to remove some of the hurdles that get in the way of forensic interviewing programs operating at their best. We discuss tools and resources that forensic interviewers can utilize to enhance their skills as well as what team members can do to support the interviewers on their team. We begin to talk about what the team can do on a systems level, to help ensure that children and families are having the best possible experience they can when coming to the CAC for a forensic interview, but you will have to tune in to part 3 of our series to hear more on that topic! He received his Ph.D. in Human Development from the University of Maryland, College Park with a specialization in Developmental Science. His program of research focuses on the dynamic interplay between development and learning for children who experience early maltreatment, and how elucidation of such processes can inform trauma-informed classroom instruction and prevention efforts. J. Bart Klika is the Chief Research Officer with the national organization Prevent Child Abuse America and a research faculty at Florida State University College of Social Work. Prior to joining PCAA, Dr. Klika was an Assistant Professor in the School of Social Work at the University of Montana. His research examines the causes and consequences associated with child abuse and neglect in an effort to prevent its occurrence. In 2011, he was selected for the inaugural cohort of fellows for the Doris Duke Fellowship for the Promotion of Child Well-Being, a national fellowship providing support and mentorship for doctoral students seeking innovations in the prevention of child abuse and neglect. Dr. Klika is on the national Board of Directors for American Professional Society on the Abuse of Children (APSAC) and is the chair of the APSAC prevention and publications committees. He is the senior editor for the APSAC Handbook on Child Maltreatment (4th Ed.) and an Associate Editor for the Journal of Interpersonal Violence. Christy Tirrell-Corbin, PhD, is the Executive Director of the Center for Early Childhood Education and Intervention and the Director of the Early Childhood/Early Childhood Special Education program at the University of Maryland. She has worked extensively with Title I schools to increase family engagement and raise awareness of teacher beliefs and practices relative to race, culture and socioeconomic status. She has also taught courses on culture and community, as well as studied teacher candidates’ beliefs and practices around race and culturally responsive and respectful pedagogy.

Many of us are uncomfortable talking candidly with our children and teens about sex, perhaps hoping sex education programs at school will do the job. And yet, research shows that, as parents, we often overestimate what our children learn at school. And we underestimate the impact we will have on our children’s attitudes and behaviors if we muster the courage and confidence to talk openly about this important aspect of human development.   Author Janet Rosenzweig, Executive Director of the American Professional Society on the Abuse of Children and a longtime sex educator and author, joins Marti & Erin to talk about what it means to speak comfortably and accurately about sex with children and teens. For example, she emphasizes the importance of teaching children about sexual arousal as an autonomic (involuntary) response that has nothing to do with thought or reason – a critical element in discerning between arousal and love. Kenneth Lanning is currently a consultant in the area of crimes against children. Before retiring in 2000, he was a Special Agent with the FBI for more than 30 years. He was assigned to the FBI Behavioral Science Unit and the National Center for the Analysis of Violent Crime at the FBI Academy in Quantico, Virginia for 20 of those years (longer than any other agent doing behavioral analysis). He is a founding member of the Board of Directors of the American Professional Society on the Abuse of Children (APSAC) and is a former member of the APSAC Advisory Board. He is a former member of the Advisory Board of the Association for the Treatment of Sexual Abusers (ATSA). Mr. Lanning is the 1990 recipient of the Jefferson Award for Research from the University of Virginia, the 1996 recipient of the Outstanding Professional Award from APSAC, the 1997 recipient of the FBI Director’s Annual Award for Special Achievement for his career accomplishments in connection with missing and exploited children, and the 2009 recipient of the Lifetime Achievement Award for Outstanding Service from the National Children's Advocacy Center. He has testified on seven occasions before the U.S. Congress and many times as an expert witness in state and federal court. Mr. Lanning has authored numerous publications and consulted on thousands of cases involving the sexual victimization of children. He has lectured before and trained tens of thousands of law enforcement officers, prosecutors, social workers, mental health and medical personnel, judges, and other professionals. Retired agent Ken Lanning served in the FBI for more than 30 years. For twenty years his Bureau career, he was assigned to the Behavioral Science Unit (BSU) as a profiler specializing in the study of the sexual victimization of children. In this episode of FBI Retired Case File Review, Ken reviews the controversial topics of satanic ritual child abuse, recovered repressed memory, and the suggestibility of children. His work in this area and the ramifications of confirmation bias has divided and polarized many child advocates, the media, and the American public. His professional study of the criminal aspects of deviant sexual behavior included assignments to the Missing and Exploited Children’s Task Force from 1996 to 1998 and the National Center for the Analysis of Violent Crime from 1998 to 2000. He is a founding member of the Board of Directors of the American Professional Society on the Abuse of Children (APSAC) and a former member of the APSAC Advisory Board and the Advisory Board of the Association for the Treatment of Sexual Abusers (ATSA). He has testified before the U.S. Attorney General’s Task Force on Family Violence, the President’s Task Force on Victims of Crime, and the U.S. Attorney General’s Commission on Pornography. He has also testified on seven occasions before the U.S. Senate and House of Representatives and many times as an expert witness in state and Federal court. Ken Lanning has consulted on thousands of cases involving deviant sexual behavior, the sexual victimization of children, missing and exploited children, and the use of computers and the Internet to facilitate the sexual exploitation of children. He has also been retained as an expert in numerous civil cases concerning alleged negligence by youth-serving organizations. Ken Lanning has published articles in the FBI Law Enforcement Bulletin and other professional journals. He has authored two monographs on child abuse that have been widely distributed by the National Center for Missing and Exploited Children (NCMEC). Ken Lanning is the 2009 recipient of the Outstanding Service Award for Lifetime Achievements from the National Children's Advocacy Center. He has lectured before and trained tens of thousands of law enforcement officers, prosecutors, social workers, mental health and medical personnel, judges, and other professionals on the sexual victimization of children, child abuse and neglect, satanic ritual child abuse, and missing and exploited children. Tonight's special guest is Charles J Stecker Jr from Marshalls Creek, Pennsylvania, a child abuse survivor and returning NAASCA family member. In his childhood Charlie survived neglect, physical violence, emotional and spiritual traumas and sexual abuses. He experienced murder, repeated loss of family, friends and belongings, stolen educational opportunities, failures of the Child Welfare System, injustice, and abandonment. Charlie explains, "On February 28, 1967 at the tender age of 2 years old, my younger brother, Edward John Stecker, was murdered by our foster mother. We had been assigned to her home by the Philadelphia Department of Human Services contract agency, the Philadelphia Society to Protect Children, now known as Turning Points for Children." He goes on, "I was with my brother and witnessed his murder. I suffered a severely shattered left elbow causing me limited use of my arm to this very day." One of the most involved abuse survivor / activists we know, Charlie has a web site called "Chahlies Angels" where he offers 'A Voice for the Voiceless.' He's also listed on the Eagle Rare web site where he's devoted to 'Inspiring Victims to become Empowered Victors'. Charlie stays busy.
