Okay.. our top story this week: XX Another indication for Mounjaro – the FDA approved it to lower the risk of major adverse cardiovascular (CV) events (MACE), including CV death, non-fatal heart attack, or non-fatal stroke in adults with type 2 diabetes who are at high risk for these events. Mounjaro – the brand name of tirzepatide - is already approved as an adjunct to diet and exercise to improve blood sugar in adults and children 10 years of age and older with type 2 diabetes. The approval was based on results from the largest and longest tirzepatide study to date, enrolling more than 13,000 participants across 30 countries over more than four and a half years. In the trial, Mounjaro demonstrated non-inferiority to Trulicity (dulaglutide), a GLP-1 treatment with established cardiovascular benefit, with an 8% lower rate of cardiovascular death, heart attack or stroke (MACE-3). The estimated hazard ratio for time to first MACE was 0.92 (95.3% CI: 0.83, 1.01) for Mounjaro compared to Trulicity (dulaglutide). https://www.prnewswire.com/news-releases/fda-approves-lillys-mounjaro-tirzepatide-to-reduce-cardiovascular-risk-in-adults-with-type-2-diabetes-302862415.html XX The FDA approves the Libre Duo – the first device designed to continuously monitor both blood sugar levels and ketone levels in people with diabetes. The Libre Duo 10-Day Continuous Dual Glucose Ketone Monitoring System is now approved for those ages 2 and older. Until now, the agency said, people had to monitor ketone levels with separate tests that only provided a single measurement at one point in time. The new system combines blood sugar and ketone tests and monitors them continuously. Readings -- and alerts in case of dangerous trends -- are sent wirelessly to a smartphone app. The FDA previously granted "breakthrough device" status to the system, which is meant to expedite the device's development and review. https://www.upi.com/Top_News/US/2026/08/25/fda-oks-blood-sugar-ketone-monitor/5521787688375/ XX Diabetes tech firms Dexcom and Abbott are watching closely for a Medicare coverage expansion that could significantly broaden access to their glucose sensors. The Centers for Medicare and Medicaid Services is expected to expand Medicare coverage of continuous glucose monitors to people with diabetes who do not take insulin. Currently, coverage is limited to people with Type 1 or Type 2 diabetes who take insulin or have a history of problematic low blood sugar. Dexcom CEO Jake Leach told investors in a late July earnings call that the company expects to hear back from the CMS before the end of the year, with coverage going into effect in mid-2027.     Dexcom presented results of a randomized, controlled trial earlier this summer at the American Diabetes Association's Scientific Sessions that it expects will support the CMS' decision. The 26-week study found that people who used a CGM had a larger hemoglobin A1C reduction than the control group and spent five more hours per day in a normal glucose range than the control group. https://www.medtechdive.com/news/dexcom-abbott-await-medicare-coverage-expansion-for-cgms/828775/ XX A new study suggests continuous glucose monitors (CGMs) may offer benefits for people with type 1 diabetes that go beyond improving blood sugar and reducing hypoglycemia.   Researchers analyzed health records from more than 8,400 adults with type 1 diabetes receiving care through the Veterans Health Administration. Over one to four years of follow-up, people who started using a CGM had a 10% to 16% lower risk of death from any cause compared with those who did not use one.   The association appeared strongest among people over age 65 and those who were not using insulin pumps. Researchers did not find meaningful differences based on A1C, race or ethnicity, or frailty.   Importantly, this was an observational study using medical records, not a randomized clinical trial, so it cannot prove that CGM use directly caused the lower death rate. Research published in JAMA finds that this wellness trend isn't helpful for people without a diabetes diagnosis. They found that the strongest evidence for CGM use is for people who are already living with diabetes, but not for those without the condition who use them for wellness purposes. It's also worth noting, though, that this new study is a narrative review. This means that the researchers drew their conclusions from the findings reported in several other studies. More randomized controlled trials would be necessary to see whether there are any meaningful health benefits of using a CGM if you don't have diabetes.   Dower also warned that constantly monitoring your blood sugar when you don't have diabetes could lead to unintended health anxiety. In some cases, you may even make accidental lifestyle changes that could be harmful. "[Blood sugar] spikes may occur after meals, but if this leads someone to replace healthy food such as fruit with unhealthy food such as something high in fat but low in carbohydrate, the glucose information has done more harm than good," Dower said. https://www.verywellhealth.com/blood-sugar-monitor-for-people-without-diabetes-12058575   XX   Could metformin somehow influence the biological processes that drive aging? A new review found evidence that metformin may affect several key features of aging, including how cells use energy, inflammation, cellular damage and even some changes in gene activity. Animal studies have shown longer lifespans or improvements in markers of biological aging, while some human studies have also found promising results.   But researchers say there's still no proof that metformin actually slows aging or helps healthy people live longer. In rats, curcumin reduced inflammation, improved cellular signaling, and restored vascular health to levels resembling those of nondiabetic animals. The results hint at a possible new way to reduce the long-term cardiovascular risks of diabetes, but human trials are still needed. Curcumin, the natural compound responsible for turmeric's vivid yellow color, is widely studied for its anti-inflammatory and antioxidant properties. https://www.sciencedaily.com/releases/2026/08/260824065543.htm XX New research estimates the number of people living with type 1 diabetes (T1D) worldwide will increase by 29% from 9.4 million in 2025 to 12.1 million in 2049. This will be presented at EASD, the Annual Meeting of The European Association for the Study of Diabetes (EASD) in Milan, Italy (Sept 28 – Oct 2 The USA will see an increase of over half a million, going from 1,492,825 to 2,055,235. The authors suggest that the reasons for the predicted high incidence and prevalence in 2049 in countries such as Australia, US, UK are likely to be a combination of genetics, environmental triggers and also high case detection rates (due to the good health care infrastructure here and in other HIC). Unlike previous estimates, the DIAMOND-T1D model takes historical patterns of insulin availability into account and aims to provide global, regional and country-specific estimates of T1D incidence, prevalence and mortality from 1900 to 2050. Although the majority of people with T1D live in high-income countries (HIC) and middle-income countries (MIC), the largest relative increase in the T1D prevalence is expected to occur in LIC as access to insulin, glucose monitoring and survival all improve in those countries. https://www.news-medical.net/news/20260828/Type-1-diabetes-cases-projected-to-increase-worldwide-by-2049.aspx XX Early initiation of statins after a diagnosis of type 2 diabetes was associated with a lower risk of dementia, according to a study published in The Lancet Regional Health – Europe. "Detecting and treating high levels of low-density lipoprotein cholesterol (LDL-C) from midlife is one such highlighted risk factor," From Danish registers, researchers identified 132,585 statin-naive individuals who developed type 2 diabetes from 2006 through 2019, with follow-up until the end of 2021.   Medical records were searched for diagnoses of all-cause dementia. A website created by President Donald Trump's DOGE to tally its purported cost savings contained myriad data quality issues and inaccuracies, a congressional watchdog reported Thursday. In a review of the Department of Government Efficiency's “Wall of Receipts” website, the Government Accountability Office found the group didn't use transparent methods to produce its figures, included lease terminations that predated DOGE's creation, and listed savings with unknown origins. The findings again call into question the roughly $215 billion DOGE claimed to have saved the government and adds to reporting on the deficiencies of the website's accounting. The New York Times, for example, has previously documented errors and deletions on that page. FedScoop has reported inconsistencies between officials' publicly stated savings claims and the figures included on the website. GAO concluded its report by recommending that the website prominently display the existence of the data-quality issues and limitations. “Prominently displaying data limitations would enhance the value of the Wall of Receipts by providing users with the information that they need to interpret and use the data appropriately,” the report said. According to a letter included within the report, the audit was ordered by Sens. Gary Peters, D-Mich., and Richard Blumenthal, D-Conn. Peters is the ranking member on the Committee on Homeland Security and Governmental Affairs and Blumenthal is ranking member of its subcommittee on investigations. The report came a day after GAO released another report that revealed little available documentation of financial disclosures and completion of ethics and records management training by DOGE personnel. A House-passed bill to scrap educational requirements in federal contracting jobs cleared a key Senate panel Thursday, putting it on track for a vote before the full chamber. The Skills-Based Federal Contracting Act (H.R. 5235) advanced out of the Senate Homeland Security and Governmental Affairs Committee by a 10-0 vote. No senators commented on the legislation during the business meeting. The bill, led by Rep. Nancy Mace, R-S.C., would block federal solicitations from including any minimum education standards, unless the contracting officer provides a written justification on why the agency needs an exemption. “Washington created the paper ceiling. This bill tears it down,” Mace said in a statement. “A four-year degree should not be a government-mandated checkbox when hardworking Americans have already proven they can do the job. Demonstrated ability should open doors in this country.” The legislation passed the House in February and has been a pet project of Mace's for years. This episode is part of our comprehensive Decipher the Guidelines Series covering the 2025 ACC/AHA/ACEP/NAEMSP/SCAI Guideline for the Management of Patients With Acute Coronary Syndromes.  The following question refers to Section 5.2.1 of the 2025 ACS Guidelines. The question is asked by Thomas Jefferson medical student and CardioNerds Academy Intern Dr. Grace Qiu, answered first by Henry Ford Interventional cardiology fellow and member of the CardioNerds Interventional Cardiology Council Dr. Li Pang, and then by expert faculty Dr. Michelle O'Donoghue. Dr. O'Donoghue is a cardiologist, senior investigator with the TIMI Study Group, and Associate Professor of Medicine at Harvard Medical School who holds the McGillycuddy-Logue Endowed Chair in Cardiology at Brigham and Women's Hospital. She was the Vice Chair of the Writing Committee for the 2025 ACS Guidelines. Question #2 A 63-year-old woman presented to the emergency room for chest pain. She described having exertional chest pain for the past two months and had an episode of severe pain after dinner 3 days ago. She went to bed and slept it off.  She told her children today at a family gathering, and was immediately brought to the ED by her daughter. She has a history of hypertension and hyperlipidemia. She was asymptomatic and normotensive in the ED. Labs show a down-trending troponin and an elevated NT-proBNP but are otherwise unremarkable. Her ECG showed Q waves with ST elevation in V2-V4. She was treated with aspirin and heparin drip, and taken to the cath lab. Coronary angiogram showed complete proximal LAD occlusion with right-to-left collaterals, without significant residual disease elsewhere. She remains asymptomatic and is stable, both hemodynamically and electrically. What is the next best step with regard to reperfusion and anti-thrombotic management? A Proceed with primary PCI to LAD  B Medical management with aspirin and enoxaparin  C Medical management with aspirin and clopidogrel D Medical management with aspirin and ticagrelor   Answer #2 Explanation  The Correct answer is D In patients who are stable with STEMI and have a totally occluded infarct-related artery >24 hours after symptom onset and are without evidence of ongoing ischemia, acute severe HF, or life-threatening arrhythmia, PPCI should not be performed due to lack of benefit. (Class 3, LOE B-R) The benefit of PPCI begins to diminish after >12 hours from symptom onset, but there appears to be continued benefit through approximately 24 hours.  In stable asymptomatic patients with an occluded artery >48 hours after symptom onset, routine PCI has not been shown to be beneficial in the absence of ongoing ischemia. The relative utility of routine PCI for asymptomatic patients with STEMI between 24 and 48 hours from symptom onset is less rigorously tested. PCI is not recommended for an occluded infarct-related artery if the patient is asymptomatic and has a completed infarct. MACE outcomes were similar in those with an occluded infarct-related artery who underwent medical therapy versus those who underwent PCI 3 to 28 days after an MI (Occluded Artery Trial [OAT]), and results were no different at 7-year follow-up. Similar findings were noted in the DECOPI (Desobstruction Coronaire en Post-Infarctus) trial, which enrolled patients with an occluded artery and Q waves on the ECG presenting 2 to 15 days after symptom onset. However, coronary revascularization should be considered for patients with late presentations with continued signs and symptoms of ischemia, including cardiogenic shock, acute severe HF, persistent angina, and life-threatening arrhythmias.  Main Takeaway In patients who are stable with STEMI who have a totally occluded infarct-related artery >24 hours after symptom onset and are without evidence of ongoing ischemia, acute severe HF, or life-threatening arrhythmia, PPCI should not be performed due to lack of benefit. The following question refers to Section 7.1 of the 2025 ACS Guidelines. The question is asked by Thomas Jefferson medical student and CardioNerds Academy Intern Dr. Grace Qiu, answered first by University of Michigan fellow and CardioNerds FIT Ambassador Dr. Kayla Secrest, and then by expert faculty Dr. Sunil Rao. Dr. Rao is an interventional cardiologist, Professor of Medicine at NYU Grossman School of Medicine, Deputy Director of the Leon H. Charney Division of Cardiology, and the Director of Interventional Cardiology for the NYU Langone Health System. He is the Editor-in-Chief for Circulation Cardiovascular Interventions and was the Chair of the Writing Committee for the 2025 ACS Guidelines. This episode is part of our comprehensive Decipher the Guidelines Series covering the 2025 ACC/AHA/ACEP/NAEMSP/SCAI Guideline for the Management of Patients With Acute Coronary Syndromes. Question #1 A 68-year-old man with a history of hypertension, hyperlipidemia, stage III chronic kidney disease, and prior tobacco use presents to a local emergency department with reports of chest pain while raking leaves at home. Upon arrival, he is hemodynamically stable with a heart rate of 86 beats per minute and a blood pressure of 133/85 mmHg. His EKG reveals ST elevations in the septal and anterior leads (V1-V4). He is given 324mg of aspirin and is promptly evaluated by the interventional cardiology team, who elects to take him emergently to the catheterization lab. Upon arrival to the catheterization lab, the nurse asks the interventional fellow which access sites they should prep for this case? How should the interventional fellow respond? A Right radial artery only B Radial + bilateral femoral C Bilateral femoral only Answer #1 Explanation  The correct answer is B. Radial and bilateral femoral Radial artery access is the preferred vascular access site for coronary angiography and PCI in patients with ACS. Transradial access has been shown to reduce mortality, bleeding, and vascular complications compared with transfemoral access (Class I, LOE A). Radial access also allows earlier ambulation and is associated with greater patient comfort. Although the right radial artery is the most widely studied upper-extremity access site, alternative sites such as the ulnar and distal radial arteries have demonstrated similar outcomes. However, the radial artery may be required as a bypass conduit for CABG. In institutions where the radial artery is routinely used for surgical grafting, this potential future use should be considered when selecting vascular access. In addition, transfemoral access—preferably performed with ultrasound guidance—should be considered in patients in whom temporary mechanical circulatory support (MCS) is anticipated or in those for whom radial access is not feasible due to anatomical or technical constraints. Prepping bilateral groins in addition to the radial artery provides a backup strategy for urgent MCS placement or for transition to femoral access should radial access fail. For these reasons, prepping both the radial artery and bilateral groins is the most appropriate response. Radial-only preparation is incorrect because, although radial access is preferred, patients with STEMI may still require emergent MCS or alternative access if the radial artery is unsuitable. Preparing only the wrist without backup femoral access may delay care should hemodynamic instability occur. Femoral-only preparation is incorrect because transradial access provides superior outcomes in ACS, including significant reductions in all-cause mortality, major bleeding, and vascular complications. RCTs and meta-analyses, including MATRIX (which showed lower MACE and net adverse clinical events with radial access) and SAFARI-STEMI (which showed no difference in mortality but was underpowered)—support radial as first-line access when feasible.
