Dr. Rosenblum reviews several key studies, providing insights into the efficacy of these regenerative treatments and their implications for clinical practice. Featured Studies:  1.  A Comparison of Functional Outcomes in Rotator Cuff Repairs Using Adjunctive Bone Marrow Aspirate Concentrate vs. Bone Marrow Aspirate Concentrate With Platelet-Rich Plasma: A Systematic Review and Meta-Analysis**      -     Authors:       Seth J. Spicer, Sara Soliman, Robert Malek, Mitchell Kaplan, Jensen Clark, Nicholas Averell, Brandon Goodwin, Richard Jermyn      -  Published in:  Cureus. 2024 Aug 23;16(8):e67594. doi: 10.7759/cureus.67594      -  Summary: This systematic review evaluates the outcomes of rotator cuff repairs (RCR) using BMAC alone versus BMAC combined with PRP. The analysis included three studies and found no significant difference in functional outcomes between the two groups, suggesting that BMAC alone may be adequate, potentially reducing costs and resource use. 2.        Bone Marrow Aspirate Concentrate Versus Platelet-Rich Plasma for Treating Knee Osteoarthritis: A One-Year Non-Randomized Retrospective Comparative Study            -  Authors: Abed El-Hakim El-Kadiry, Carlos Lumbao, Natasha Salame, Moutih Rafei, Riam Shammaa      -  Published in:  BMC Musculoskeletal Disorders, 2022.      -  Summary:   This study compared the effectiveness of intra-articular BMAC and PRP injections in patients with knee osteoarthritis (OA). Results indicated that patients receiving BMAC experienced significant improvements in pain and functionality scores over 12 months, while the PRP group showed nonsignificant improvements. BMAC was found to be more effective than PRP in managing knee OA symptoms. 3.  Patients With Knee Osteoarthritis Who Receive Platelet-Rich Plasma or Bone Marrow Aspirate Concentrate Injections Have Better Outcomes Than Patients Who Receive Hyaluronic Acid: Systematic Review and Meta-analysis**      -     Authors:  John W. Belk, Joseph J. Lim, Carson Keeter, Patrick C. McCulloch, Darby A. Houck, Eric C. McCarty, Rachel M. Frank, Matthew J. Kraeutler      -       Published in:   World Journal of Stem Cells, 2021.      -    Summary:  This systematic review assessed the efficacy of PRP and BMAC compared to hyaluronic acid (HA) injections for knee OA. The findings demonstrated that both PRP and BMAC led to significantly better patient-reported outcomes (PROs) compared to HA, with no significant differences between PRP and BMAC in terms of effectiveness. 4.          Platelet-Rich Plasma vs Bone Marrow Aspirate Concentrate: An Overview of Mechanisms of Action and Orthobiologic Synergistic Effects             - Authors: José Fábio Santos Duarte Lana, Lucas Furtado da Fonseca, Rafael da Rocha Macedo, Tomas Mosaner, William Murrell, Ashok Kumar, Joseph Purita, Marco Antonio Percope de Andrade      -  Published in:  World Journal of Stem Cells, 2021.      - **Summary:** This overview discusses the mechanisms of action for PRP and BMAC and their potential synergistic effects in musculoskeletal healing. So, let's talk about career pivots and the value of continuous education and training with Dr. Sharon Smith... Unlike many who enter medicine unsure of the path they're going to take, today's guest knew from the start that she wanted to be an obstetrician-gynecologist. Even when people around her tried to invite her to do something else, she didn't falter and continued with what she wanted to do. But then, the pandemic came, a.k.a. one of the toughest times in medicine. And with the extreme burnout and having to deal with the pain of losing her father, she knew she needed to make a change. And so, she did. She pursued aesthetics and served in locum tenens of obstetrics and gynecology. This shift later changed her life for the better, helping her achieve work-life balance and better self-care. So, if you're curious about how she was able to take the leap, or if you just want to learn about aesthetic medicine and procedures associated with it, tune in and join us as we talk about her story. Guest's bio: Dr. Sharon Smith is a board-certified obstetrician-gynecologist with over 15 years of experience and has always had a strong interest in the beauty industry. This later on led her to transition to aesthetic medicine after realizing the need to pivot, especially after the pandemic. She's currently serving as a part-time obstetrician and gynecologist while working full-time on her aesthetic and obesity medicine practice. Why you need to check this episode: Discover the work obstetrician-gynecologists and aesthetic physicians do; Recognize the importance of continuous education and training to achieve excellence in what you do; Understand the significance of building strong and nurturing relationships with patients; and Learn about aesthetic procedures while learning what the terms “OB Deserts”, “Platelet Rich Plasma” (PRP), and O-Shot mean “For you to become proficient and skilled at what you do, you have to put training upfront, and you have to know that you're going to train and train again and update yourself and learn new techniques and hone your craft. That's the only way you're going to get good and become excellent at what you do.” – Dr. Berry Pierre Notable Quotes: “Just the awareness itself of what can happen and how I'd have to handle it, to me, [it] gives me an edge.” – Dr. Sharon Smith “It's been good. Even transitioning to locums and growing my aesthetic practice, it's been great. I sleep more now, I have more time for self-care, I'm not running on empty, I'm not missing out on much.” – Dr. Sharon Smith Mentions: Leading Ladies Lounge (Dr. Smith's podcast) The Medical Collective (podcast collaboration with other physicians) Connect with Dr. Sharon Smith: Website Instagram TikTok Facebook Sign up at www.listentodrberry.com  to join the mailing list. This breaking field offers exciting options for chronic pain WITHOUT drugs or surgery. In this episode, Dr. Kristy explains how the use of Human Cellular Tissue Products (HCTP's) such as Stem Cells and Platelet Rich Plasma (PRP) can be used to repair, rebuild and heal human cells, tissues or organs to restore your injured and painful area to as close to normal as possible. Dr. Kristy also explains how HCTP's are humanely harvested and do NOT involve enbryos or festal tissues. When we are born we have millions and millions of these stem cells attached to our capillaries. As we age the stem cells become dormant or non-functional. After we have reached full bone growth, in our late teens or early twenties, we only have 10% active to help us heal. Thus our bodies begin to degenerate rather than regenerate. By transplanting the correct tissue, in the correct amount, into the injured or painful part of your body we can activate dormant cells that can regenerate new tissue, reduce inflammation and restore normal function. Regenerative Medicine is definitively a HOT and marketable offering across a variety of medical disciplines. But it is equally as controversial. The most well known type of regenerative medicine is the use of Platelet Rich Plasma (PRP). This has been proposed as a novel care therapy for musculoskeletal pain disorders, and a variety of GYNECOLOGICAL applications- including POI, pelvic floor dysfunction, uterine infertility treatment, and even some forms of vaginal fistulas. AND YES... RPR is even being used as a type of cosmetic fascial (with a new CDC health investigation underway!). In this episode, we will review the concept of Regenerative Medicine in gynecology, summarizing some key points from a brand new narrative review of the subject published in the Green Journal (Obstetrics Gynecology).

Platelet-Rich Plasma (PRP) remains a topic of controversy in Sports Medicine. One of the limitations surrounding this therapy is the paucity of high-quality data to support or refute its efficacy. We welcome Dr. Dean Wang, MD from UC Irvine to discuss his study looking at the fragility of the highest quality data we have on PRP. A fragile study is one in which the study's conclusion would change with the reversal of only a few outcomes. Our medical community holds randomized-controlled trials in high regard, however if these studies are fragile, how much can we rely on them to be true?

Ever wondered what PRP is and how it can help you on your fertility journey? Well, today, I'm excited to share a conversation all about Platelet Rich Plasma (PRP) therapy that I recently had with Dr. Lorne Brown on his Conscious Fertility Podcast. PRP is making waves in the world of fertility, and here's why you should consider it: 1. Every woman should have the opportunity to explore PRP before opting for egg donation. It might just be the key to helping you have a baby with your own eggs!  2. Wondering when to start PRP? It's all about your individual response. Depending on your age and other factors, you can start PRP before IVF, you can use it ahead of egg freezing, or you can use it during an IVF cycle. Some patients see better results after several PRP therapy treatments.  3. The magic lies in the technique! It's not a one-size-fits-all approach. I've been tracking data and seeing promising results like increased AMH levels, lower FSH levels, and higher egg quality.  4. The PRP technique matters. In my practice, I use a specialized nine-inch spinal needle, carefully targeting the ovary's cortex, aiming to impact as much surface area as possible.  5. Considering a series of treatments? It might make sense. Just like with other therapies, consistency and the right timing seem to be key for optimum results.  Remember, everyone's fertility journey is unique. PRP could be the missing piece to the puzzle for some, especially those with a high egg count or those looking to improve egg quality.  We're excited about the possibilities that PRP offers on the path to parenthood! Dr. Hemal V. Mehta, M.D., the Medical Director of Vibrant Health of North Carolina, shares valuable insights on the intersection of regenerative medicine, Hyperbaric Oxygen Therapy (HBOT), and fasting in a recent episode of the HBOT News Network. Dr. Mehta's medical journey has evolved from interventional pain management to regenerative medicine, where he utilizes Platelet Rich Plasma (PRP), stem cells, and HBOT to promote tissue regeneration and holistic healing for his patients.Key Takeaways:Regenerative Medicine and HBOT: Dr. Mehta initially ventured into regenerative medicine and PRP therapy, witnessing remarkable improvements in his patients' health and the ability to reduce or eliminate opioid pain medications. Recognizing the regenerative potential of HBOT, he began incorporating it into individualized patient treatment plans. HBOT contributes to tissue healing by upregulating anti-inflammatory genes and downregulating inflammatory genes.Inflammation as a Health Challenge: Dr. Mehta emphasizes that inflammation is a pervasive health concern driven by factors such as poor dietary choices, injuries, and stress. Managing inflammation is crucial for overall well-being. HBOT is an effective therapy for addressing inflammation, and it complements other treatments like regenerative therapies involving stem cells, PRP, ozone injections, and exosomes.Nutrition vs. Pharmaceuticals: Dr. Mehta points out the disproportionate emphasis on pharmaceutical drugs compared to nutrition in medical education. He highlights the importance of nutrition in health, expressing concern about the toxicity and pro-inflammatory nature of many processed foods. Dr. Mehta underscores the impact of diet on health and advocates for a diet rich in natural, unprocessed foods to reduce inflammation. The Benefits of Fasting: Dr. Mehta believes that fasting is an excellent approach to maintaining health. He challenges the conventional three-meals-a-day norm and notes that fasting can serve as the ultimate detoxification process. Fasting allows the liver to cleanse toxins and offers a reprieve to the body's organs for healing and rejuvenation. Fasting has historical and religious significance, being a practice embraced across various cultures and religions.Fasting and Reduced Cancer Risk: Dr. Mehta hints at the strong association between fasting and reduced cancer risk. In Part 2 of the conversation, he plans to delve deeper into how fasting can starve cancer cells by depriving them of the sugars and nutrients they require for growth and proliferation.Dr. Mehta encourages individuals to consult with their healthcare providers before embarking on fasting regimens, as it may not be suitable for everyone. He emphasizes that a combination of fasting and HBOT can have a profound impact on the body, contributing to improved health and overall well-being.

Skin aging is a natural process that affects everyone as they grow older, however, there are several factors that can accelerate the aging process, including cellular degeneration. Our skin is composed of various types of cells, each playing a crucial role in maintaining its health and appearance. Understanding these cellular mechanisms can help us take steps to slow down the aging process and maintain healthier, more youthful-looking skin. Welcome to another episode of the Biohacking Beauty podcast brought to you by Young Goose Skincare! In this episode, host Amitay Eshel, the CEO of Young Goose, is joined by Dr. Sandra Kaufmann to discuss the cellular causes of skin aging, and her work in developing the Kaufmann protocol. We'll also cover why both internal and external care is necessary for optimizing skin health, and why mixing up your skincare can be beneficial for aging skin. Dr. Sandra Kaufmann began her academic career in the field of cellular biology, earning a Master's Degree from the University of Connecticut in Tropical Ecology and Plant Physiology. Turning to medicine, she received her medical Degree at the University of Maryland, and completed a residency and fellowship at Johns Hopkins in the field of pediatric anesthesiology. For the last five years she has been the Chief of Pediatric Anesthesia at the Joe DiMaggio Children's Hospital, a nationally recognized center of excellence. Most recently, she was recognized as “Best in Medicine” by the American Health Council. What we discuss:[00:00] Dr. Sandra Kaufmann's work on cellular degeneration and aging can revolutionize skin care.[06:28] Using molecules to affect aging pathways is the key to longevity.[17:17] Improving skin health requires both internal and external care.[22:12] Lactoferrin can help block Advanced glycation end products (AGEs) and clear them from the skin.[32:31] Mixing up skincare is important for aging skin[37:57] Hypoxic training can enhance mitochondrial function for high altitude climbing.[48:24] Starting a longevity company to provide access to preventive medicine[53:16] Tailored dosing strategy is important for longevity medicines[1:03:40] Hyaluronic acid is an important molecule for skin health.Key Takeaways:When it comes to achieving and maintaining healthy, radiant skin, it's essential to adopt a comprehensive approach that includes both internal and external care. While topical treatments like injectables and Platelet-Rich Plasma (PRP) therapies can enhance skin appearance, internal care through supplements can play a pivotal role in protecting and repairing the skin from damage. As we age, our skin undergoes various changes and requires different approaches to maintain its health and vitality. One effective strategy for aging skin is to mix up your skincare routine. This involves incorporating a range of products and techniques to address different concerns and adapt to the changing needs of your skin.Addressing skin concerns like creepiness requires a combination of internal and external approaches. However, in order to stimulate collagen, it is important to provide the necessary nutrients for fibroblasts, the cells responsible for collagen production. Huberman Lab Podcast Notes Key Takeaways Different hairs on your body have different lengths of growth phases (for example, hair on the head will grow for 2-8 years, and hair on the eyebrows will fall out every few months) The rate of hair growth is relatively stable across hair – what differs is the length of the growth phaseMicroneedling procedures, PRP injections, and Minoxidil all center around the general idea of increasing blood flow, increasing oxygen and delivery of nutrients, or increasing inflammation just enough locally to set off a cascade of regrowthCombination treatments that combine a mechanical stimulus (such as microneedling) with chemical stimulus (such as minoxidil) will always be better than either alone But be realistic, it's unlikely to grow a full head of hair again if you are baldPeople are highly individual in response and side effects to hair growth treatments – start with minimum effective dose and be patient with experimentation timeAndrogens (especially DHT) are potent inhibitors of IGF-1 and cyclic AMP which stimulate hair growthRead the full notes @ podcastnotes.orgIn this episode, I explain the biology of hair, hair growth, why hair growth slows and what cause hair to stop growing and/or “fall out.” I discuss the essential role of hair stem cells and other supporting biological factors for healthy hair growth. Then I describe various approaches (mechanical and chemical) to slow hair loss by increasing blood flow to hair stem cells, including minoxidil, tadalafil, PRP, microneedling, Botox and ketoconazole treatments. I also discuss how age-related hormone changes cause hair loss and explain the effectiveness of treatments such as caffeine, saw palmetto, growth hormone, finasteride and dutasteride. For all hair growth options, I describe potential side effects, how soon to expect results and the amount of hair regrowth to expect and I highlight effective combination treatments for hair regrowth even in hair “dead” (bald) zones. For many listeners, thinning, brittle hair, or pattern baldness are a source of anxiety and stress. This episode explains the mechanisms underlying hair regrowth tools and the science behind them so that you can evaluate potential treatments and associated side-effect profiles and select the best one(s) for you. If you have pain at the bottom of the big toe joint, up along the arch, just above the plantar fascia, you may have been told that you have this thing called FHL tenosynovitis. "Tenosynovitis" just means that you have inflammation of the tendon sheath or tube that carries the flexor tendon down to your big toe. It helps you pull your big toe down when you push off while running. When the tendon sheath gets inflamed, it can be painful. If you have tenosynovitis, your doctor may recommend a cortisone injection, or a Platelet Rich Plasma (PRP) injection. So what's the difference between these two injections and why would you want one versus another? Today on the Doc On The Run Podcast we're talking about FHL tenosynovitis injections, corticosteroids versus PRP injection.

Platelet Rich Plasma (PRP) therapy is a HOT and TRENDY item right now. PRP use was initiated in sports medicine in the 1970s. It has since expanded into many medical specialty fields, and now it is being heralded as the new hope for diminished ovarian reserve for those desiring pregnancy…even in menopause! Can injecting the ovaries with PRP really rejuvenate the ovary? We'll examine the evidence as it sits as of January 2023.
