BUFFALO, NY — April 9, 2025 — A new #research paper was #published in Aging (Aging-US) Volume 17, Issue 3, on March 5, 2025, titled “Reproductive aging, preimplantation genetic testing for aneuploidy, and the diameter of blastocysts: does size matter?” In this study, a team led by first author Jakub Wyroba from the Malopolski Institute of Fertility Diagnostics and Treatment and Andrzej Frycz Modrzewski Krakow University, and corresponding author Pawel Kordowitzki from Harvard Medical School, Nicolaus Copernicus University, and Charité, found that the size of an embryo and whether it has started hatching can help predict its genetic health. This insight could help fertility clinics select better embryos during in vitro fertilization (IVF), especially in countries or situations where advanced genetic testing is not available. As more women are choosing to have children later in life, fertility challenges related to age are becoming more common. Older maternal age is linked with a higher risk of chromosomal problems in embryos, which can reduce the success of IVF. To identify healthy embryos, many clinics use a test called preimplantation genetic testing for aneuploidy (PGT-A). However, PGT-A is expensive and not available in all countries. This study explored whether embryo quality could be predicted using physical features alone. During IVF, embryos develop in the lab for several days before being transferred into the uterus. Around day five or six, the embryo reaches a stage called the blastocyst. At this point, it begins to break out of its outer shell, called the zona pellucida. This process is called hatching, and it is an important step before the embryo can attach to the uterus and begin a pregnancy. The researchers examined 1150 embryos from women aged 26 to 45 who underwent IVF. They looked at whether the embryos were already starting to hatch and how big they were. They then compared these features with results from genetic tests. They found that smaller embryos that were already hatching were more likely to be chromosomally normal, also called “euploid.” “Of the 1150 blastocysts that underwent PGT-A analysis in this study, 49% were aneuploid.” For women over 35, 51% of small hatching embryos were euploid, compared to just 38% of larger ones that had not started to hatch. Among younger women under 35, the difference was even greater—73% of small hatching embryos were euploid, compared to 58% of large, unhatched ones. The research team also looked at what happened after the embryos were transferred. When embryos were already known to be euploid, both large and small embryos led to similar pregnancy rates. This means the size and hatching behavior mostly matter when genetic testing is not done. This study offers new guidance for IVF clinics. Choosing a small hatching embryo may improve the chances of success, especially for women of advanced age and in clinics that do not use PGT-A. This finding could help make fertility treatment more accessible and affordable. As fertility science continues to advance, insights like this provide practical tools to improve outcomes and bring new hope to individuals and families trying to conceive through IVF. This week Kate is joined by Clinical Embryologist Victoria Wigley a.k.a @allaboutembryology, to take you on a whirlwind of an episode on, what is quite a confusing topic, Pre-implantation Genetic Testing for Aneuploidy (PGT-A). PGT-A was first used for those who had a family history of a condition, such as cystic fibrosis. PGT-A takes a look at the chromosomes that are in the cells of the embryo. Since we all have a set amount, PGT-A makes sure that there are no aneuploidies, or in other words - an abnormal amount of chromosomes.Victoria talks about why clinics started using PGT-A testing and she goes into detail on what's involved with this testing and how it is done. Kate and Victoria discuss the HFEA grading system, which currently grades PGT-A testing as ‘red', meaning that ‘there are potential safety concerns and/or, on balance, the findings from moderate/high quality evidence shows that PGT-A testing may reduce treatment effectiveness'. Victoria also discusses her concerns that patients are being offered PGT-A when they do not need it. Victoria tells Kate how she believes that all patients should know everything about what they are being offered, including the risks and hidden costs. She goes on to explain how many of her patients struggle to find the confidence to say ‘no' to their clinics, when being offered treatment that they don't necessarily think they need. Dr. Natalie Crawford answers voicemail questions about miscarriage. Miscarriage is so hard and something Dr. Crawford has experienced herself. Understanding when you should seek help and what that help should look like can be so advantageous as you try to advocate for yourself. Questions answered: Is it common to have something retained in the uterine cavity after a miscarriage? What could cause this? I am having recurrent implantation failure with PGT-A tested embryos. My REI recommends a new cycle because there could be a cohort issue. What are your thoughts? After two healthy pregnancies, I lost a pregnancy at 19.5 weeks. They told me my next pregnancy would be low risk, which concerns me that I'll only get two ultrasounds. I had a miscarriage at around 6 weeks after my first IVF transfer. Is it normal to experience some spotting while waiting for my next period. I had a miscarriage after my first pregnancy. Could my PCOS have played a role in the loss? PGT-A, PGT-M, PGT-SR, Carrier Screening, Karyotype — oh my! The options available for genetic screening in fertility testing and IVF treatment can get confusing. Understanding testing available to you BEFORE treatment as well as DURING the IVF process is key. These tests can decrease risk of disease and miscarriage and improve success rates with a single embryo transfer, but there are pros and cons for every intervention. It can be difficult to keep it all straight and overwhelming to know what to do. I'm so delighted to have Meaghan Doyle back on the show today to talk about the truth about abnormal embryos. Meaghan is a genetic counselor, and founder of DNAide, where she specializes in infertility, preimplantation genetic testing, and preconception genetic counseling. In this discussion, Meaghan and I are talking about the different kinds of embryo testing, what genetic test results should be used for, and why all fertility patients need to ask for reports on "abnormal" embryos. The truth is that sometimes, patients are told that their embryos are abnormal but never learn much more about the embryo itself. In some situations, those abnormal embryos can become healthy pregnancies and healthy, live babies, so it's important to learn as much as you can about all of the results of the genetic testing done on your embryos. To set the stage for this conversation, I asked Meaghan what it means to genetically test an embryo, and here's what she explained: "It means that we've taken a sample from the outer layer of the embryo, usually a few cells, and sent that sample to a genetic testing lab. And there's actually a few different types of genetic testing that can be done on that sample. Generally, when we say an embryo has had genetic testing, we're referring to a test called PGT-A. And so when we say an embryo has been genetically tested, usually that's what we're referring to." So what happens when an embryo comes back as "abnormal," and what can you do if it happens to you? I'm so delighted to have Meaghan Doyle back on the show today to talk about the truth about abnormal embryos. Meaghan is a genetic counselor, and founder of DNAide, where she specializes in infertility, preimplantation genetic testing, and preconception genetic counseling. In this discussion, Meaghan and I are talking about the different kinds of embryo testing, what genetic test results should be used for, and why all fertility patients need to ask for reports on "abnormal" embryos. The truth is that sometimes, patients are told that their embryos are abnormal but never learn much more about the embryo itself. In some situations, those abnormal embryos can become healthy pregnancies and healthy, live babies, so it's important to learn as much as you can about all of the results of the genetic testing done on your embryos. To set the stage for this conversation, I asked Meaghan what it means to genetically test an embryo, and here's what she explained: "It means that we've taken a sample from the outer layer of the embryo, usually a few cells, and sent that sample to a genetic testing lab. And there's actually a few different types of genetic testing that can be done on that sample. Generally, when we say an embryo has had genetic testing, we're referring to a test called PGT-A. And so when we say an embryo has been genetically tested, usually that's what we're referring to." So what happens when an embryo comes back as "abnormal," and what can you do if it happens to you? Emma Whitney, Laboratory Manager, is an Embryologist with nearly 20 years of practical IVF laboratory experience specialising in pre-implantation genetic testing of embryos with a passion to prevent the transmission of inherited genetic disease and enhancing the capabilities of embryo selection. She breaks it down for us in a way that can help us to understand what the different genetic test on embryos are and which of these tests require tests on parents to then be able to look for the conditions within the embryos and which tests are screening tests for a selection of conditions. We talk about biopsies of embryos and how this procedure is carried out and how being good on a playstation is helpful when training to biopsy as an embryologist, I'll let you listen to it to find out more. Busting myths of the unknown and what really happens behind those lab doors. “Fear gets reduced with knowledge”. Humanising the process of what happens beyond egg collection really can help to bring reduce the fear and worry felt by patients. We talk about how to better manage expectations during the embryo development stage and is it better to receiv news on day 3 and 5 or to wait until day 5? PGT-A does not increase the number of genetically normal embryos, but it does reduce the number of abnormal embryos being transferred in the first place helping to reduce failed transfers/pregnancy loss. Emma also helps us to understand what receiving a mosaic result back from genetic testing and what that can mean and how it's not always the end of the road. There are extra layers of testing should you receive Mosaic results back to further help you make the best decision on whether or not to transfer these embryos. Emma's commitment to genetics, coupled with her multitude of experiences - allows for a pragmatic and honest approach when discussing embryology. Emma believes IVF patients should have the opportunity to understand what goes on in an IVF laboratory. Her goal is to help patients alleviate some of the stress and anxiety caused by so many unknowns. “IVF is as much an emotional journey as it is a scientific one. PGT-A is a hot topic in fertility medicine and I'm so happy that Dr. Eric Forman joined me to talk to us about a recent Chinese study published in The New England Journal of Medicine. Takeaways: - this study is getting a lot of attention because of the prestigious journal it's published in - the study design isn't how we do things in the real world: we don't just biopsy three embryos - the average age was 29 of the patients and mean egg number 20 and we know from previous studies that PGT-A as a selection tool may help women over age 37 more than younger women - the cumulative live birth rate was higher in the PGT arm - the study provided reassurances about PGT-A: the biopsy is safe, the group who did it had a higher implantation rate, similar cumulative live birth rate with fewer transfers, and fewer miscarriages. There were more congenital abnormalities in the non-biopsy group. A big thank you to Dr. Forman for breaking this all down for us and for sharing how PGT-A can improve the live birth rate on the first transfer and make elective single embryo transfer the standard of care across ages. This week we try getting my left ovary to spit out an egg so that we can proceed with our ERA! And it honestly feels like it's taking forever. We talk about our PGS (or I guess it's technically called PGT-A) results, what it was like getting our those results, almost missing our consult appointment, Ally's saline sonogram, and what the heck an Endometrial Receptivity Analysis is!
