Emergency podcast! We've been asked by many people, mostly junior/mid career faculty, to quickly record a podcast on ageism and the elections.  People are feeling conflicted.  On the one hand, they have concerns about cognitive fitness of candidates for office.  On the other hand, they worry about ageism.  There's something happening here, and what it is ain't exactly clear. We need clear eyed thinking about this issue. In today's podcast, Louise Aronson, author of Elderhood, validates that this conflict between being concerned about both fitness for the job and alarmed about ageism is exactly the right place to be.  We both cannot ignore that with advancing age the prevalence of cognitive impairment, frailty, and disability increase.  At the same time, we can and should be alarmed at the rise in ageist language that equates aging with infirmity, and images of politicians racing walkers or a walker with the presidential seal.  Ken Covinsky reminds us that we should not be making a diagnosis based on what we see on TV, and that if a patient's daughter expressed a concern that their parent “wasn't right,” we would conduct an in depth evaluation that might last an hour.  Eric Widera reminds us of the history of the Goldwater Act created by the American Psychological Association in the 1960s which states that psychiatrists should refrain from diagnosing public figures, and the American Medical Association code of ethics which likewise discourages armchair diagnosis (rule established in 2017). We frame today's discussion around questions our listeners proposed in response to our Tweets, and are grateful for questions from Anand Iyer, Sandra Shi, Mike Wasserman, Ariela Orkaby, Karen Knops, Jeanette Leardi, Sarah McKiddy, Cecilia Poon, Colleen Christmas, and Kai Smith. What's the role of geriatrics and palliative care in the care of individuals with COPD? We talk this week with Anand Iyer, the lead author of this weeks JAMA IM article on this subject. It's a little off from our ongoing COVID topics, but given that his along with his co-authors (Randy Curtis and Diane Meier) JAMA IM piece just got published, we figured now is the right time to highlight #PalliPulm. What is #PallPulm? #PalliPulm is something that Anand Iyer founded, and is an online community of clinicians, investigators, patients, and others interested in the intersection of palliative care and geriatrics. Anand is also an Assistant Professor in Division of Pulmonary, Allergy, and Critical Care Medicine at the University of Alabama at Birmingham (UAB) and faculty at the Center for Palliative and Supportive Care at UAB. Anands research on this subject has shown a lot of things, including that COPD patients are often referred too late to palliative care and hospice, pulmonologists often equate hospice with palliative care, there is high symptom and emotional needs in COPD patients, and that patients and family members feel palliative care should be integrated early on in the care of COPD. Last time I looked this up online, there were more than 5000 companies offering digital medicine tools. Which ones worked? Which ones are less good than others? Which ones have not been tested? How were they tested: apples to apples or a whole fruit basket of standards? If you’re a clinician or the head of population health or an insurance carrier and you’re trying to figure out whether a digital tool could help solve a problem your patients are having … at this juncture, may the force be with you and I hope you know how to use Excel. The Digital Therapeutics Alliance (DTA) aims to put definitions and standards around what can be legitimately called a digital therapeutic and how these tools are best deployed so that patients are best equipped to get the best possible outcomes. William Gibson, the noted science fiction author wrote, "the future is already here, it’s just not widely distributed yet."  Our guest this week describes a future revolution in healthcare that is, in fact, already here.  Digital health therapeutics are transforming the delivery and experience of medical care and rapidly gaining traction in healthcare delivery.Anand Iyer, the Chief Strategy Officer for Welldoc, has been engaged in digital health for well over a decade.  He is a respected global digital health leader.  Anand has a doctorate in computer and electrical engineering as well as a MBA from Carnegie Mellon.  Anand and his colleagues are part of a large and growing community of cutting-edge digital entrepreneurs that are building the technologic infrastructure and capabilities to enable a completely different approach to medical care.  In this interview, Anand describes the digital therapeutics that are being used to provide clinical care to patients with chronic diseases such as diabetes and hypertension.  To date, Welldoc’s FDA approved digital therapeutic, Blue Star, has been used by tens of thousands of healthcare consumers with diabetes.In addition, in this episode Anand shares the following:How digital health software and algorithms are becoming FDA-approved and regulated therapeutics, similar to medications and medical devices.The three main facets of the Blue Star digital health program - targeted at patients, providers and health systems.The requirements for the acceptance and adoption of digital health into mainstream clinical delivery.Why digital health is the only viable option in addressing numerous chronic diseases that are advancing at epidemic proportion.Why digital health presents a great solution to some of the challenges in providing optimal care to the Medicaid population - addressing some of the existing inequities in quality, accessibility, affordability and engagement.How different digital health platforms are beginning to coalesce, affording users a singular, non-fragmented, seamless experience.It is remarkable that Anand and his colleagues understood this potential well over a decade ago - the use of digital apps to treat diabetes, hypertension, depression, heart failure and numerous other chronic diseases.  Given their forethought, Anand asks us to imagine what digital technology and advanced analytics will be able to accomplish in the next 5 years. Anand is a respected global digital health leader—most known for his insights on and experience with technology, strategy and regulatory policy. Anand has been instrumental in WellDoc's success and the development of BlueStar®, the first FDA-cleared mobile prescription therapy for adults with type 2 diabetes. Since joining WellDoc in 2008, he has held core leadership positions that included Chief Data Science Officer, President and Chief Operations Officer. In 2013, Anand was named “Maryland Healthcare Innovator of the Year” in the field of mobile health. Prior to joining WellDoc, Anand was already an established thought leader in the field. He had served as the Director of PRTM's wireless practice, where helped companies take advantage of disruptive technologies, business models and process models offered by and enabled by advanced wireless communications. Anand was the founder and immediate-past president of the In-Building Wireless Alliance, and teaches advanced wireless courses to senior officers in the US Department of Defense at the Institute for Defense and Business. Prior to joining PRTM, Anand was a member of the scientific staff at Bell Northern Research and Nortel Networks.
