In this episode, we sit down with Dr. Iván Cuesta, MD. Dr. Cuesta was born and raised in the Dominican Republic (DR) where he attended and graduated from the Instituto Tecnológico de Santo Domingo. He completed his residency in Neurology at Temple University in Philadelphia and his fellowship in Neurologic Critical Care at Baylor College of Medicine in Houston, Texas. Currently, Dr. Cuesta serves as a Neurocritical Care Attending at Advocate Lutheran General Hospital here in the greater Chicago area as well as on faculty at the Rosalind Franklin University Chicago Medical School. We spoke extensively with Dr. Cuesta about the process he took to match into a US residency. Toward the end of medical school in the DR, his interest in pursuing a US residency was sparked after a rotation in Miami, Florida. We talk about the different hurdles that International Medical Graduates (IMGs) have to overcome to match into a US residency, including obtaining a visa and learning medicine in a new language. Additionally, he gives his advice to others who dream of following a similar path. Dr. Joshi was a practicing physician at a clinic in rural Indiana when his world was turned upside down. The DEA, using a fake patient and a prescription monitoring system, investigated Dr. Joshi for his opioid prescription practices. He was eventually arrested, tried in court and served a prison sentence. While in prison he wrote the book Burden of Pain, which is his side of the story. After serving his time, he is back to practicing medicine. No matter your views on the causes and perpetrators of the opioid epidemic, in this episode Dr. Joshi provides valuable insights on the complexity of the epidemic; how we view and treat pain; the patient-Doctor relationship, the role of the DEA; and what he feels are the best steps forward. This week Alicia had a guest host! She was joined by friend of the show and past guest- Lauren Erbach Barnfield and they spoke to Dr. Abdul Aziz Elkadri! Dr. Elkadri is an Assistant Professor at the Medical College of Wisconsin where he specializes in pediatric IBD. We talked to Dr. Elkadri about his personal connecting to IBD, how he inadvertently chose GI as his specialty and why he prefers treating kids. We also talked to him about receiving his medical training in Lebanon and how that created some hurdles to him practicing in the US post training. We discussed his research into genetics and into very early onset (VEO) IBD and working with little kids (and their parents). We also talked about talking to teens about weight and eating when they have IBD and the impact of societal pressure/reward on weight. Lauren and Alicia had such a good laugh with Dr. Elkadri and always appreciate his irreverent sense of humor. "Jeffrey A. Singer is a senior fellow at the Cato Institute and works in the Department of Health Policy Studies. He is President Emeritus and founder of Valley Surgical Clinics Ltd., the largest and oldest group private surgical practice in Arizona and has been in private practice as a general surgeon for more than 35 years. He is also a visiting fellow at the Goldwater Institute in Phoenix. Singer is a member of the Board of Scientific Advisors of the American Council on Science and Health. From 1994 to 2016, he was a regular contributor to Arizona Medicine, the journal of the Arizona Medical Association. He served on the Advisory Board Council of the Center for Political Thought and Leadership at Arizona State University from 2014 to 2018 and is an adjunct instructor in the Program on Political History and Leadership at ASU. He writes and speaks extensively on regional and national public policy, with a specific focus on the areas of health care policy and the harmful effects of drug prohibition. He received his BA from Brooklyn College (City University of New York) and his MD from New York Medical College. He is a fellow of the American College of Surgeons." Medical training and practice habituates physicians to a culture that narrows the possibilities we see for finding joy and meaning in our work. We often become efficient task completers, stuck in routines, and prone to burnout.  Saul and Stefan discuss a set of questions that challenge physicians to look at their work and themselves in fresh ways, can be used for mentoring or teaching purposes, as prompts for reflective writing exercises, or to engage thoughtful colleagues (perhaps over a beer).  10 Questions (selected from On Becoming a Healer: The Journey from Patient Care to Caring about Your Patients)  Think about a brief account of a patient interaction you recently had in which you think you functioned as a healer rather than just a task completer – meaning that you were able to help the patient beyond the narrowly biomedical aspects of care? Was there something you learned from this visit that you could apply more broadly? Think of interactions with patients that are rewarding and meaningful? Are they rare or common? Can you think of a specific one? Was there something you did differently that made the encounter memorable? If so, can you think of ways you could modify how you practice and interact with other patients so that more of your interactions are as satisfying? Do you see yourself as someone friends turn to when they are in distress or need guidance? If so, what is it that you offer them that enables you to be such a valuable resource? Is that part of you accessible to your patients during medical encounters? Can you think of an example? If not, why do you think that is? If you couldn't be a physician, what would you most want to do instead? How would it be similar or different from what you have sought in a medical career? Can you draw connections between your second choice and medicine to gain perspective on what you most love to do? Assuming you stay in medicine, how can you be sure you are most likely to find it? What's happened to your curiosity during medical training? What are you more curious about? What are you less curious about? Specifically, what questions do you find yourself asking or wanting to ask as you go through the day? How do you think your curiosity or lack of curiosity affects how you relate to and care for your patients and how you feel about your work? Do you feel your patients are benefiting from the distinct qualities that make you the unique person you are, or is that uniqueness not really a part of the way you relate to them? Do you feel you are interacting with patients in a manner that gives you a window into what makes each of them unique? Are many of your interactions rewarding? If so, in what ways? Are there certain types of patients who “get under your skin,” making you cringe when you see their names on your appointment calendar? Consider what might be going on during your interactions with them, utilizing the framework described in this chapter. Is it that you can't engage with them? Do you struggle with maintaining boundaries when they make incessant demands? How might you alter your behavior so that these encounters become opportunities to model healthy interaction and to provide them a brief respite from the chaos that is likely present in their other relationships? Have you ever felt resentment that a patient didn't show appreciation after you significantly helped them? If so, why do you think their show of gratitude is important to you? Does the doctor-patient relationship include an expectation that patients make their doctors feel good too? Could their indifference reduce your investment in their care? What if you learned from a patient's family member that the person actually does appreciate you but just isn't able to show it? Given what you know now, do you think you can have a career in medicine in which you find patient interaction rewarding and meaningful much of the time? If yes, are you on course to experience those rewards, or do you need to make some changes? If the latter, what are you going to do to make those changes? Are you going to live with low expectations or look for something more rewarding? Many, if not most, work environments have a fair amount of hassle, meaning you spend a good deal of time doing nuisance work and coping with difficult colleagues and bosses. These are manageable challenges, and they even provide an opportunity to learn to negotiate and adapt. Sometimes, however, workplaces become too dysfunctional to do your job effectively or facilitate meaningful change. They are beyond repair. How would you know when that line has been crossed? Our cultural conditioning tells us that women aren't capable of giving birth independently and that we should apply every possible intervention to reduce the difficulties associated with childbirth. For Michel Odent, that's a neutralization of the laws of natural selection. Early in his career, Michel worked as the only doctor in a maternity unit with six midwives. He took a deep interest in their experiences and asked them questions that changed his thoughts on birth work. As a general surgeon, he credits a lot to his interdisciplinary perspective. He believes that's what allows him to make connections that other overly-specialized people do not. Michel is a legend in the maternity care space and has been a hero of mine for many years. "No one anticipated how the pandemic would impact their Deaf colleagues' work environment and career opportunities. Yet, 15% of adults report some form of hearing difficulty. This number increases dramatically with advancing age, with 50% of those aged 75 years or older experiencing hearing difficulty. The exact number of Deaf/Hard of Hearing (DHH) physicians and other health care providers is unknown. However, according to the Association of Medical Professionals With Hearing Losses, the numbers have increased over the past few decades. This increase is in part due to legislation (such as the Americans With Disabilities Act of 1990) and evolving technologies (such as electronic and visual stethoscopes and speech-to-text applications). Although these changes have enabled more DHH professionals to work in health care, none was sufficient to address the COVID-19 pandemic's effect on DHH physicians. I grieved this change in my life. I could not rely on my usual communication methods to understand people. To succeed in this new reality, I had to advocate for full-time interpreters at work to treat my patients, urgently help a colleague with their shift, or attend a last-minute meeting. I needed to select skilled, compatible interpreters while consciously redesigning my work environment to consider their needs, methods, and availability." Carolyn Stern is a family physician.
