They reflect on the evolution of CGM technology, from its early use primarily in type 1 diabetes to its expanding role in type 2 diabetes management, and explain why this trial represents an important step forward for patients who have historically had limited access to CGM.The hosts review the randomized controlled trial findings, emphasizing the significant improvements in glycemic outcomes, including a 1.6% reduction in A1c from baseline and an approximately 0.9% greater reduction compared with standard care. You can't erase the betrayal. You can't undo what happened. And you can't force yourself to forget the fact that someone you loved broke your trust. But that doesn't mean the betrayal has to define the rest of your life. In this episode, I talk about the idea of “include and transcend” and how it applies to infidelity recovery. Healing after being cheated on is not about pretending it never happened, suppressing the pain, or trying to go back to who you were before. It's about reaching a point where the betrayal is included as part of your story, but it no longer becomes the lens through which you see yourself, your relationship, your past, or your future. We'll also talk about one of the hardest parts of betrayal trauma: holding conflicting truths at the same time. Maybe there was love and betrayal. Maybe parts of the relationship were real, and parts were deeply distorted. Maybe this chapter changed you, but it doesn't have to become your whole identity. Dr. Spencer and Karl Nadolsky sit down with David W, a nurse practitioner and one of the actual patients enrolled in the Triumph 1 retatrutide phase 3 trial, to break down the data that was just presented at the American Diabetes Association conference and explain why everyone in obesity medicine is paying very close attention. In this episode they cover what retatrutide actually is and why adding glucagon agonism to the GLP-1 and GIP dual agonism of tirzepatide creates a meaningfully different drug with direct effects on liver lipid metabolism, insulin sensitivity, blood pressure, and fat catabolism that you do not see with semaglutide or tirzepatide alone, what David's personal experience in the 12 milligram arm looked like from dose escalation through steady state including the GI side effects that faded by month seven and the heartburn that a low dose PPI fixed quickly, how David went from 240 pounds and a BMI of 35 down to 167 pounds by the end of the trial representing roughly 30 percent weight loss which is right at the trial average, what the Triumph 1 obesity trial found at 80 weeks with the nine and 12 milligram doses delivering nearly 26 and 28 percent average weight loss respectively and almost half of patients on the highest dose losing 30 percent or more, why the 104 week extension data showing patients who stayed on 12 milligrams reaching 30.3 percent average weight loss is being compared to bariatric surgery outcomes, what the Transcend type 2 diabetes trial showed with average A1C dropping to 5.9 percent on the 12 milligram dose in patients who were on no other diabetes medication, why the 41 percent triglyceride reduction and 20 percent LDL reduction are particularly interesting given that the mechanism appears to involve multiple pathways in the liver that tirzepatide and semaglutide do not touch, what the 70 percent reduction in WOMAC knee arthritis pain scores and 60 percent reduction in sleep apnea events mean for patients who have been told their only option is surgery, how Spencer plans to use retatrutide clinically once it is approved and which patients he thinks are the right candidates, why the gray market research peptide version currently circulating is something both doctors strongly advise against, and what Triumph 2 and Triumph 3 are measuring and when that data is expected. Alexandra is an artist, educator, and somatic guide working with movement as a way to access creative authority, embodied clarity, and deep personal voice. Her work supports artists, teachers, activists, facilitators, and guides to strengthen their inner compass while leading, creating, and communicating. She is fascinated by how decision-making lives in the body: how gesture holds belief, habit becomes pedagogy, and intuition is a fluent language.She is a choreographer, director, and educator working at the intersection of dance, theater, and somatics and was a member of the Bill T. Jones/Arnie Zane Dance Company from 1995–2001. She founded Alexandra Beller/Dances in 2002, creating more than forty dance-theater works across the U.S., Europe, and Asia.Her theater credits include Off-Broadway productions of Sense and Sensibility (Folger Shakespeare, A.R.T., Portland Center Stage; Helen Hayes Award, Lortel nomination), How to Transcend a Happy Marriage (Lincoln Center Theater), and The Mad Ones, as well as regional work with Hudson Valley Shakespeare Festival, La MaMa, La Jolla Playhouse, Milwaukee Rep, and The Goodman Theatre. Her directing credits include Macbeth (Theater Row) and A Midsummer Night's Dream (92Y).She has taught at institutions including Barnard, SUNY Purchase, Rutgers, UC Santa Barbara, the University of Michigan, The New School, and Princeton University, where she taught for seven years. Alexandra currently serves on faculty at the Laban Institute for Movement Studies and Gibney Dance Center and teach internationally through residencies and master classes. So why does it feel like you're running on empty most days?On this episode of All About Capital Campaigns, Amy Eisenstein sat down with Indra Lahiri — an organizational psychologist, certified traumatic stress specialist, and founder of Indraloka Animal Sanctuary — to talk about what burnout really looks like, why it's more dangerous than most leaders realize, and what you can actually do about it.Most people think of burnout as simply being tired. And while exhaustion is part of it, Lahiri explains that burnout is more nuanced than that. It shows up as cynicism about your work, loss of motivation, and behavioral shifts you might not even recognize in yourself—like becoming overly controlling or short-tempered with colleagues and donors. For nonprofit professionals, burnout often stems from financial strain, overwhelming caseloads, or the emotional toll of serving people in crisis.But there's a related condition that's even more insidious: secondary traumatic stress. This occurs when you absorb the trauma of the people you're trying to help. Unlike burnout, which builds gradually, secondary traumatic stress can rewire your brain in ways that mirror the effects of direct trauma. Your amygdala—the part of your brain responsible for detecting danger—gets stuck in fight-or-flight mode. And when that happens, your prefrontal cortex, the region you rely on for strategic thinking, clear communication, and relationship-building, gets hijacked.Think about what that means for fundraising. The skills you need most—asking donors for major gifts, coordinating with board members, explaining your capital campaign vision with clarity and confidence—are exactly the skills that suffer when your nervous system is in overdrive. You might think you're functioning normally, but the people around you can tell something is off.Lahiri frames the choice every leader faces in stark terms: when you encounter trauma or chronic stress, you will either transcend it or transmit it. Transmitting looks like snapping at staff, micromanaging, withdrawing from donor relationships, or creating a culture of fear and reactivity. Transcending means doing the internal work required to regulate your nervous system, process what you've experienced, and lead from a place of stability rather than survival.That framing matters because it makes burnout a leadership issue, not just a personal one. When a nonprofit leader transmits their stress, it ripples through the entire organization—staff morale drops, donor relationships suffer, and the mission takes a hit. When they transcend it, they model resilience for their team and create the conditions for everyone to do their best work.So what does transcending actually look like in practice? Lahiri offers several accessible starting points. First, give yourself permission to take time—even an hour—that is entirely for you, not for your team, your board, or your donors. Second, learn basic nervous system regulation techniques. Deep breathing, meditation, physical movement, or even turning on music and dancing around your living room all count. The point is consistency, not perfection.One of her most practical recommendations is creating what she calls a self-care itinerary. Think of it like a project plan, but for your own wellbeing. Identify the emotional states you need throughout the day—calm, energized, soothed—and map specific activities to each one. Then schedule three small acts of self-care daily: morning, midday, and evening.
