The impact of epidurals on the second stage of labor has sparked a lot of discussion. Research suggests that epidurals can prolong this stage... and a prolonged second stage can be associated with increased risks of infection, severe tears and the need for interventions like forceps-assisted delivery or Cesarean. But does an epidural mean that you are higher risk for complications during the second stage of labor, or are there ways to lessen your risks? In this episode, Dr. Dekker explores the physiological effects of epidurals (and how they impact your body's birthing reflexes), the correlation between epidurals and the duration of the second stage of labor, and nuanced perspectives from professional guidelines worldwide— including a brief discussion of "laboring down" (aka delayed pushing). Join me, Lindsay McCoy as I hit “record” and jam out on all things professional birth support. There is a revolution in birth support. While holding the space and offering sips of water and a cool cloth is really important, there's also so much more a doula can be doing! In this episode, I talk about the role of a doula and their impact on the client's birth experience, and what it really looks like to support birth physiology as a doula. I believe we can do more than just randomly using comfort measures. If a doula knows the biomechanics of labor and birth; the science of what to do when in labor and the “whys” of it, they can really make a difference in the client's birth experience! What do clients really need in a doula? Our job is to help the clients protect their space, help them find their voice and achieve the birth they want; and that not only means hands on labor support but also working with the client prenatally to prepare their body for labor so that they can have a smoother, uneventful birth. I also talk about the essential skills in professional birth support. With over a decade of being a doula and helping pregnant families achieve the birth they desire, I have seen it all! In this episode I wanted to share why this role is not “less than” but complimentary to the provider role and should be treated as such. In this episode, I am joined by Katie Kane, a parent from Cherry Hill, New Jersey who took the Evidence Based Birth Childbirth Class when she was pregnant with her daughter. Katie worked with EBB Instructor Cat LaPlante. From the beginning of her pregnancy, Katie planned for an unmedicated hospital birth - leading her on a path to researching doulas and eventually finding the EBB Childbirth Class. She was intentional in choosing her birth team, and ended up switching providers when she knew her first was not the best fit for her. With the support of her husband and doula, Katie was able to navigate roadblocks that may have resulted in different outcomes for her birth if she had not been so well prepared. In light of all the craziness currently happening with COVID-19, we decided to re-release this episode about comfort measures in labor. Many people will be having their babies with little or no support because of the current hospital restrictions and we hope that these tips will help with managing the intensity of labor, especially if you’re doing it without the support you’d planned on having. Additionally, here are a few helpful resources that we mentioned at the top of the episode: COVID-19 resources page on Evidence Based Birth The WHO’s page on COVID-19 The CDC’s page on COVID-19 The CDC’s page on Pregnancy & Breastfeeding with COVID-19 ACOG’s Practice Guidelines on COVID-19 RCOG’s recommendations on COVID-19 during pregnancy If you’re interested in working with Ali for digital doula support, email her here. --- Karly and Ali get down to the nitty gritty details of coping with the pain of labor and birth. They explain why it’s essential for every birthing person to learn about non-medical coping techniques (even if they plan to use pain medication during their birth) and they review the best tools and techniques for pain management. Here’s an overview of everything that’s covered in this episode: The reasons why it is SO important to know a variety of non-medical pain management coping techniques, including times when birth goes too quickly to use medication or when meds don’t work effectively What the 5 elements of birth are and why they’re an essential part of pain management in labor Why you should consider hiring a doula for your birth (including this great article on the evidence for doulas) The reasons it’s SO important to do some sort of childbirth education during pregnancy Helpful tools in labor: Bucky 40 Blinks – the best eye mask in all the land! Episode 12 - In our 12th episode, Jessica tackles her first (but far from her last) science fiction Comfort Media as she takes a deep dive into the premiere episode of the (criminally-cancelled-too-soon - Thanks, Frelling SciFi Channel!) television series, FARSCAPE. The Comfort Food is buttermilk biscuits. And the Community Comfort is (a bit on the nose, we know) Stargazing...so pull some buttermilk biscuits out of the oven, cue up the first episode of FARSCAPE (now available on Amazon Prime) and join Jess for a discussion of a Broken DRD, a Helium-farting Hinerian, the luminous Claudia Black, and action-figure wedding cake toppers.* *A word from Jess about spoilers: Usually, I don't worry about spoilers. I assume if you're listening to the podcast, you've already consumed the media so I'm not particularly careful about what I say. However, for some reason with this episode (maybe because it's science fiction (and we sci-fi geeks are pretty spoiler-phobic), maybe because we're in the middle of the final season of GoT and Avengers:Endgame literally just opened so I'm hyper aware of spoilers, but whatever the reason), I was concerned that people watching might not have seen any future episodes and I didn't want to ruin anything for anyone, so I tried to be very careful about not revealing things that are to come. So! There are a couple of places that had mild spoilers, where I give the heads up to fast forward 30 seconds just to avoid hearing those little nuggets. Here are the three articles Jess mentioned in the pod: Who killed Farscape? http://strangehorizons.com/non-fiction/articles/who-killed-farscape/ The Entertainment Weekly review that started it all (at least for Jessica) https://ew.com/article/1999/03/19/farscape/ The Ethical Drama of John Crichton (Warning: Spoiler-ific if you haven't watched the full series) https://www.tor.com/2019/01/16/the-ethical-drama-of-farscapes-john-crichton/ As always, thank you to The Don't Tell Darlings for use of their songs "Never Saw Nothing" and "Till I Get Home" in the intro and outro (respectively) to the pod.
