Menopause and age raise our cardiovascular disease risk. Athletes and highly active people also tend to have a higher prevalence of coronary artery calcium and atherosclerotic plaque (though it tends to be the benign kind). But what does that all mean? How can we know what's really going on in those coronary arteries of ours? In this episode of the podcast, we are discussing the gut microbiome. The gut and the microbiome have gotten a lot of attention over the last 10 years and for good reason. Our digestive system plays a vital role in our overall health and dysfunctional digestion can lead to a variety of health problems. One of the most fascinating aspects of our digestive health is the microbes that live in our gut which refer to as our "gut microbiome". These communities of microbes make a home out of our intestinal tract and are responsible for helping us to digesting and absorbing nutrients from our food food and they also synthesize some nutrients and neurotransmitters, help to regulate our immune system and more. Bottom line these little bugs living in our gut are very important for our health. In this two-part episode, Zachariah DeFilipp, MD, and Andrew Brunner, MD, with Massachusetts General Hospital discuss their findings on a committee of the American Society for Transplantation and Cellular Therapy (ASTCT) to publish an evidence-based review on guidelines for the use of allogeneic hematopoietic cell transplantation in the management and treatment of myelodysplastic syndromes (MDS) from the perspective of both transplant and hematology/oncology. Contributor: Travis Barlock MD Educational Pearls The QT interval represents phases 2 and 3 of ventricular plateau and repolarization, respectively. As the QT interval lengthens, more sodium and calcium channels are available and susceptible to action potentials. Prolonged QT interval is more concerning in the setting of bradycardia. This scenario increases the likelihood of R on T phenomenon. R on T phenomenon occurs due to an early afterdepolarization event in which a premature ventricular contraction (PVC) occurs during the repolarization period (superimposed on the T wave), leading to an aberrant re-entry circuit. The re-entry circuit leads to Torsades de Pointes (polymorphic ventricular tachycardia with prolonged QT) and subsequent ventricular fibrillation. Treatment for Torsades de Pointes is 2g MgSO4. The preferred antiarrhythmic for VTach is IV lidocaine 1.5 mg/kg over 2 minutes. Avoid amiodarone due to risk of further QT prolongation. At present, treatment for myelopathy varies widely. Although various surgical treatments are well known, there are currently no guidelines for the many non-operative domains of care (e.g. pre- and post-operative rehabilitation, pain relief, physiotherapy, nursing). In a powerful initiative led by Prof Chad Cook from Duke University, the RECODE-DCM Perioperative Rehabilitation Incubator is bringing together experts from all relevant disciplines to write management guidelines. Iwan shares his story, as a reminder of why filling this important knowledge gap could provide many simple, immediate gains for people living with myelopathy.

Aspirgillosis is a deadly fungal infection that is a major cause of mortality in patients with impaired immune systems. The current standard of care has been voriconazole due to the favorable side effect profile compared to amphotericin. However, posaconazole may not have there adverse effects and be a reasonable course of therapy. Is voriconazole just just a phase? Their success is enviable, but breaking the cycle of poverty is a mammoth task; one that requires educational opportunities that compensate for the disadvantages created by the socio-economic gap, appropriate structural support, and exposure to the larger world. Every young mind has the right to dream big, but not all dreams are destined to become a reality.On this episode, Elise Davis-McFarland, the Immediate Past President of American Speech-Language Hearing Association (ASHA 2019) and an ASHA Fellow, discusses how poverty is a serious condition and a potential cause of deprivation and educating students from low income families warrants more than just tolerance, but strong cultural competence. Robust Executive Function and self-regulation are essential ingredients for raising independent children, but ongoing environmental stressors and economics adversity can prove to be an obstacle in bringing forth future-ready children.About Dr. Elise Davis-McFarlandDr. Elise Davis-McFarland has enjoyed a rewarding career in higher education that includes teaching, development, and leadership of speech-language pathology programs; research; and executive-level college administration. She began her career as a school speech-language pathologist (SLP) in North Carolina where she provided diagnostic and therapeutic services for children in pre- and elementary schools. Following an audiology internship at the VA and Duke Hospitals and graduate study, she joined the faculty of the University of Houston as an assistant professor where she taught graduate courses in language development, childhood language disorders, early literacy development, and assessment and diagnosis of childhood communication disorders.In Charleston, South Carolina—in the absence of an academic program in her discipline—she took advantage of new experiences, first as vice president of Governmental Affairs for the Charleston Chamber of Commerce and later as Director of Institutional Research at The Citadel. Dr. Davis-McFarland was also elected as a commissioner for the state's Medicaid program by the South Carolina Legislature. At the Medical University of South Carolina (MUSC), as an associate professor the opportunity to develop and lead the interdisciplinary graduate Communication Sciences and Disorders program in the Department of Rehabilitation Sciences where Occupational and Physical Therapy programs were housed led to a teaching award and to her research at the MUSC hospital. Later, she became Vice President for Student Affairs at Trident College, where she provided executive-level leadership and supported the successful matriculation of students with autism and Asperger's Syndrome into the college. Dr. Davis-McFarland is an ASHA Fellow. Her service to ASHA includes membership on the Committee on Practice Guidelines for SLPs, the Professional Practices Committee, the Ethics Committee, the Executive Board Subcommittee on Examination Performance, and the Multicultural Issues Board. She was one of ASHA's representatives on the committee formed by ASHA and the International Association of Physicians in AIDS Care (IAPAC). She chaired the Committee on Honors and was the coordinator for SIG 14. Until her election as ASHA's president-elect she served on the SLP Advisory Council as a representative from South Carolina, and the SIG 17 Coordinating Committee. She has also been a reviewer for Language, Speech, and Hearing Services in Schools and the African Journal of AIDS Research.
