Im Oval Office haben sich Donald Trump und sein Vize JD Vance mit Wolodymyr Selenskyj gestritten. Der US-Präsident und sein Vize haben den ukrainischen Präsidenten vor der versammelten Presse gedemütigt und ihm Undankbarkeit vorgeworfen. In dieser Sonderfolge von Streitkräfte und Strategien spricht Host Carsten Schmiester mit dem ehemaligen USA-Korrespondenten der ARD, Stefan Niemann. Er analysiert die Szene und erklärt den Unterschied zwischen der ersten und zweiten Amtszeit Trumps. Wir erleben jetzt einen ungebremsten Präsidenten, der von Ja-Sagern und Loyalisten umgeben sei, niemand widerspreche, so Niemann. Trump habe kein Werte- und Koordinatensystem und keine Prinzipien, denen er sich verpflichtet fühle. Über die Reaktionen in der Ukraine berichtet die ARD-Korrespondentin Rebecca Barth aus Kiew. Die Demütigung sei für viele Ukrainer schmerzhaft. Viele versammelten sich hinter ihrem Präsidenten. Gleichzeitig beschreibt sie die Szenen in Washington als „sehr erhellend“. Natürlich müsse sich die ukrainische Führung die Frage stellen, ob sie die Trump-Administration nicht völlig falsch eingeschätzt habe. Mit Blick auf die Europäer sagt Rebecca Barth, dass im Endeffekt alle in einem Boot sitzen. Die Ukraine habe aber das Problem an der vordersten Front zu sitzen. Auf die Reaktionen in Europa schaut Astrid Corall. Viele Politiker haben der Ukraine ihre Unterstützung zugesichert, es gebe aber auch Ausnahmen. Having had a previous Omicron natural infection is beneficial towards preventing moderate to severe disease from current circulating Omicron strains, but not helping against getting mild infection at all. Coronavirus Update #70 GOOD NEWS: This information is so important to help us all understand risk. Stratified risk is the only true way to measure personal risk. Let us look at some CDC data from the spring Omicron BA.1 and .2 spikes versus the fall 2021 delta wave. Median age of hospitalization has increased from 60 years old with Delta to 64 with BA.1 and 71 with BA.2. Any underlying medical condition associated with hospitalization increased from 89% with Delta to 92% with BA.1 to 95% with BA.2 respectively. Length of hospital stay decreased from 4.8 to 3.9 to 3.3 days. ICU admission was down from 24% to 18% to 13% of admitted patients. Mechanical ventilation decreased from 14% to 8% to 6% of admitted patients. And Finally, death from 12% to 8% to 5% overall. What we can glean from this data set is very clear. With successive SARS2 mutations coupled to increased population based exposure to virus via infection or vaccine, we are now seriously in a reduced risk state unless you are older than 65 years with a comorbid disease or younger than 65 with a serious disease. 95% of hospitalizations were related to a comorbid disease regardless of age. The other big takeaway was this: if you are in this high risk group, getting every available booster is vital to your survival based on the risk reduction data. For everybody else, the data is clear, you are ok - to boost or not to boost is up to you. But, absolutely work on your general health. To your health, Dr. M

Quick Hits and other musings - 1) In the interview with Dr. Offit there were a few big take aways: Boosters for non risk based teen and young adults are unlikely to provide benefit while offering a small but real level of risk from myocarditis Boosters are offering minimal benefit to the nation from a transmission perspective. At best 8-12 weeks of protection against symptomatic disease Dr. Offit voted against adding omicron antigens to this fall's booster as there was limited data that it would any benefit. He was in the minority at the FDA advisory meeting, thus this fall's booster will have new strain genetics in it The boosters could, not shown yet in humans, block future variant immunity to newer strains through viral immune imprinting A lot more this week to digest. Full Hour | In today's second hour, Dom welcomes back Lancaster County Commissioner Ray D'Agostino after an announcement that the county, along with Berks and Fayette counties, have been sued by the Department of State for being ‘outlier counties' that have not correctly certified ballots from the primary elections. Ray offers the lawsuit from his perspective, telling that the county has properly certified the results in accordance with the PA Election Code, and tells that he and his office plan to continue to uphold the law as they see it after being sued. Then, Dr. Marty Makary, professor at Johns Hopkins University, author of The Price We Pay, and columnist for the New York Post and other national sources, joins the Dom Giordano Program to discuss the re-igniting of Coronavirus warnings by the Biden administration and Dr. Anthony Fauci. First, Dom asks Dr. Marty to tell why he believes this is making a return. Then, Giordano and Dr. Makary discuss the push to vaccinate children 6 months to 5 years of age, with Makary explaining the shocking results of vaccinating children and the strange response by the CDC. Coronavirus Update 63 plus other stuff - Lots on boosters today while the persistent world of Covid exposure drags on. Omicron US strains: newer variant BA.2 makes up 35% of current case volume based on different parts of the country while omicron BA BA 1.1.529 is at 6% and B 2.12.1 is 59%. Delta is gone by competition. Cases are increasing in some cities with little increase in hospital morbidity and mortality. In essence we are still fairing quite well in the new norm. Coronavirus Update #56 – This audiocast is a deeper dive into covid and children's mental health. In my opinion, our children's mental and physical health need to take primacy over pandemic fear at this time. We will dive deeply here. Also, Omicron's sibling is the new variant BA.2. It appears to be very very similar to Omicron from the same lineage genetically with 50 amino acid changes. It is more infectious by 10-30% by most recent data. No data that it is more deadly to date in humans although a preprint article from a Japanese research group noted that in an animal model the disease is worse than BA.1 Omicron. (Yamasoba et. al. 2022) They noted more cellular fusion increasing infectivity and also more lung cell infection increasing morbidity. The immune protection from BA.1 against BA.2 was reduced by 6X. This does not mean that humans will respond the same way as the animal model results. It bears watching. So far, I have seen no data that humans are suffering worse disease. United States BA.2 numbers remain less than 10% likely due to the quality immunity achieved following the massive Omicron BA.1 surge the past 8 weeks. Our T and B cell immunity from BA.1 should serve to protect most of us from any issues with BA.2. Time will tell, but so far so good. Be well, Dr. M
