He reports headaches for several months, possibly nearly 1 year. Over the past 2 weeks, he has had intermittent numbness on the left side of his face and in his left arm and foot, with difficulty standing due to sensory changes in his feet. He states these symptoms are new. He reports a fall related to the numbness but denies balance problems. He denies fever or chills. He has intermittent nausea and vomiting but denies diarrhea, abdominal pain, or other complaints. He works as a busboy in a restaurant and states that he only cleans and does not prepare food. In Guatemala, he used to eat street tacos containing pork and beef, sometimes with undercooked meat. He is a former smoker who quit 4 years ago after smoking for 10 years. He drinks alcohol 3–4 times per week, typically 2–4 drinks per occasion, but not daily. He reports no sexual activity for 1–2 years. General: no acute distress HEENT: NC/AT, anicteric, vEEG Neck: supple, normal ROM Lungs: clear to auscultation bilaterally  Heart: S1, S2 present, normal rate/rhythm Abdomen: Soft. Nondistended. Nontender. Neuro: AAOx3, no obvious focal deficits Extremities: No cyanosis. No edema. Skin: Warm. Dry. No visible rash. Psychiatric: Appropriate affect and mood for situation. CT HEAD WO IV CONTRAST-Cystic structures scattered along the subarachnoid space, for example, right sylvian fissure/frontotemporal cortical sulci, right > left suprasellar cistern, interpedicular cistern, basal cistern and left sylvian fissure. Associated mass effect with minimal leftward midline shift (0.2-0.3 cm). Calcifications at the left temporal region. There is no obvious acute intracranial hemorrhage. David McKnight kicks this episode off by explaining how, for decades, conventional financial wisdom has been saying that, as you approach retirement, you should begin dialing down your stock exposure and increasing your bond allocation. A 60-year-old, for example, would have 40% of their portfolio in stocks and 60% in bonds.  Historically, bonds served three primary functions: They provided income, they reduced portfolio volatility, and they protected retirees from so-called sequence of returns risk. David touches upon how the sequence of returns risk works. Retirees who get hit early often run out of money earlier – in some cases, even 15 years prior to life expectancy. The old approach to retirement planning assumes that bonds could provide meaningful returns while still acting as a stabilizer. However, recent years have shown that bonds are not risk-free. Back in 2022, for instance, the Bloomberg U.S. Aggregate Bond Index lost 13%. Long-term treasuries did even worse, as many lost between 25 to 30% due to rapidly rising interest rates. David stresses that an annuity can do something bonds cannot do: It can guarantee income that you cannot outlive. It's important to realize that whenever your basic living expenses are covered, something profound happens psychologically: You stop depending on your investment portfolio to solve every problem. Furthermore, you feel as if you now have permission to spend. Studies show that those who have guaranteed lifetime income spend 22% more than those who rely strictly on a stock bond portfolio. A properly funded IUL can create a pool of tax-free money that's insulated from stock market loss and available during downturns. David unpacks a strategy that can increase the sustainable withdrawal rate on your stock portfolio from 4% to as high as 8% with a 95% success rate. When you combine guaranteed lifetime income from annuities with a volatility shield in the form of IUL, you are no longer reliant on bonds, says David. He also touches upon why retirees who adopt the no-bond power of zero approach begin to take a lot more risk in their stock market allocations. This comes from an article by Derek Tharp at Kitces.com titled “The Flaws In Using A 0% Discount Rate To Justify Delaying Social Security”. It takes a hard look at why the common advice to “wait until 70” might not always hold up in the real world.  Tharp argues that the assumptions baked into much of the research—especially the idea that a future Social Security dollar is worth the same as a dollar today—can tilt the math toward delay, while ignoring very real risks like mortality, sequence of returns, policy changes, and even health-span. She arrived in Guinea in December  2023.  She was taking doxycycline for malaria prophylaxis and says that she has not missed any doses On October  2024 she presented with fever and dry cough.  Lab work was done and follow up planned for the following day.  The patient slept poorly, was febrile to 104 and had ongoing cough.  The next day she went to the hospital and was evaluated in the ER for acute febrile illness of unclear etiology.   In the hospital, VS were 97.9F, BP 105/70, P 94 Oxy sat 98%, normal physical exam. She was started on Augmentin and Coartem. Pause here to think about the differential at this point and maybe some more history and what testing you might want WBC 14, Hb 13, HCT 40, PLT 285, Neut abs 8, Eos Abso0.80; BUN/creat  normal, AST normal; ALT 44, GGT 125 Stool parasite screen + for some sort of eggs, malaria smear negative, CXR with b/l infiltrates She was given a medication (vomited 30 min after dose received).  She then received a second dose of medication 5 hours after the first) and was discharged. The following day the patient returned to the ER, stating that she felt worse.  Her temperature had climbed to 104 overnight, and she developed watery diarrhea and nausea.  There were no additional episodes of vomiting.  She was given an additional dose of a medication, ibuprofen, and started on ceftriaxone 1 gm IV Q12 hrs.  During the day she continued to have low grade fevers and developed abdominal pain.  That night she was again febrile to 104 F.    She remained admitted for 5 days with ongoing symptoms of diarrhea, nighttime fevers and diffuse abdominal discomfort.  Three more malaria tests were negative (rapid test and slide review) Blood cultures collected – no growth She continued to have mild elevation of WBC and slight elevation of AST and ALT. The patient was transferred to a different hospital. They give her a different medication, and within 24 hours symptoms resolve.  What is the diagnosis and what happened here with management? Holmes replies to this outburst by claiming that his mind rebels at stagnation, and that because he doesn't have a case to solve, he yields to this form of Mental Exaltation. He then says that he made his detective job for himself because he takes great pleasure in solving problems.The two men quickly discuss the Jefferson Hope Case which Watson wrote about, "A Study in Scarlet." Holmes says that Watson added too much romanticism, and believes that Watson should have only left in the parts pertaining to Holmes' detective abilities. Holmes then mentions that he has also written several pieces about his detective abilities on the distinction between ashes of various tobaccos, and the like.Watson then asks Holmes to explain the difference between observation and deduction. Holmes obliges, and says that Watson went to the post office to send a telegram, and then explains how he came to these conclusions. With a goal to challenge his flat mate, Watson gets Holmes to identify his watch, and describe its former owner. After looking at the watch, Holmes concludes that the watch belonged to Watson's brother, that the man had times of poverty and prosperity, and was also a drunk. After a brief outburst from Watson, Holmes goes to explain how he arrives at the answer he gave. Following this, their Land Lady Mrs. Hudson, comes in and announces that a Miss Mary Morstan wants to see Holmes... A New Case
