She arrived in Guinea in December  2023.  She was taking doxycycline for malaria prophylaxis and says that she has not missed any doses On October  2024 she presented with fever and dry cough.  Lab work was done and follow up planned for the following day.  The patient slept poorly, was febrile to 104 and had ongoing cough.  The next day she went to the hospital and was evaluated in the ER for acute febrile illness of unclear etiology.   In the hospital, VS were 97.9F, BP 105/70, P 94 Oxy sat 98%, normal physical exam. She was started on Augmentin and Coartem. Pause here to think about the differential at this point and maybe some more history and what testing you might want WBC 14, Hb 13, HCT 40, PLT 285, Neut abs 8, Eos Abso0.80; BUN/creat  normal, AST normal; ALT 44, GGT 125 Stool parasite screen + for some sort of eggs, malaria smear negative, CXR with b/l infiltrates She was given a medication (vomited 30 min after dose received).  She then received a second dose of medication 5 hours after the first) and was discharged. The following day the patient returned to the ER, stating that she felt worse.  Her temperature had climbed to 104 overnight, and she developed watery diarrhea and nausea.  There were no additional episodes of vomiting.  She was given an additional dose of a medication, ibuprofen, and started on ceftriaxone 1 gm IV Q12 hrs.  During the day she continued to have low grade fevers and developed abdominal pain.  That night she was again febrile to 104 F.    She remained admitted for 5 days with ongoing symptoms of diarrhea, nighttime fevers and diffuse abdominal discomfort.  Three more malaria tests were negative (rapid test and slide review) Blood cultures collected – no growth She continued to have mild elevation of WBC and slight elevation of AST and ALT. The patient was transferred to a different hospital. They give her a different medication, and within 24 hours symptoms resolve.  What is the diagnosis and what happened here with management? Holmes says that Watson added too much romanticism, and believes that Watson should have only left in the parts pertaining to Holmes' detective abilities. Holmes then mentions that he has also written several pieces about his detective abilities on the distinction between ashes of various tobaccos, and the like.Watson then asks Holmes to explain the difference between observation and deduction. Holmes obliges, and says that Watson went to the post office to send a telegram, and then explains how he came to these conclusions. With a goal to challenge his flat mate, Watson gets Holmes to identify his watch, and describe its former owner. After looking at the watch, Holmes concludes that the watch belonged to Watson's brother, that the man had times of poverty and prosperity, and was also a drunk. After a brief outburst from Watson, Holmes goes to explain how he arrives at the answer he gave. At some point during most audits, taxpayers will ponder whether, or to what extent, they should “cooperate” with the IRS. They also might ask what, exactly, cooperation means in a particular situation. These are critical questions to which many taxpayers lack clear answers, and this type of unawareness can lead to bad decisions. This article describes duties associated with foreign accounts, standards for reducing penalties, a new case in which taxpayers were stuck with higher sanctions because they failed to fully cooperate during a voluntary disclosure program, and other contexts in which cooperation has a significant effect on IRS disputes.

This suit was referenced in Lil Rod's lawsuit, with a note that a suit would be forthcoming. The suit's allegations stem from incidents on the Holiday Yacht trip that are also detailed in Lil' Rod's suit. Unsurprisingly, this plaintiff and Lil' Rod have the same attorney, currently under fire in New York Federal court. The attorneys for UMG brought to the court's attention a court order in an unrelated case brought by Attorney Blackburn wherein the court ordered him referred to the discipline committee of the federal court. In a comment given to the media, attorney Blackburn states that the two things have nothing to do with the other. One thing is clear: these cases aren't slowing down. Is notorious serial killer Dennis Rader, better known as BTK, responsible for even more unsolved disappearances? A deep dive into the concerns over evidence collection and the heart-wrenching quest for answers unveils the true complexity of such high-profile cases. The "Hidden Killers" podcast, featuring retired FBI Special Agent Robin Dreeke, provides a rare window into the professional and emotional challenges investigators face.    In a recent episode of "Hidden Killers," a significant point of contention arose surrounding BTK's potential link to the 1976 disappearance of Cynthia Kinney. The District Attorney for Osage and Pawnee counties, Mike Fisher, voiced his reservations about implicating Dennis Rader in the case, due to the lack of substantial evidence to press charges. Meanwhile, Sheriff Eddie Virden of Osage County, having spearheaded a nationwide task force, seems convinced of Rader's involvement in not just Kinney's disappearance, but potentially other cold cases.    The heart of the matter, as presented by hosts Tony Brueski and Robin Dreeke, revolves around two central issues. Firstly, there's the concern of procedural integrity. The manner in which evidence has been collected, especially as the Osage County Sheriff's Office apparently stepped outside their jurisdiction, raises serious questions. Are the obtained search warrants valid? Would the evidence be admissible in court, given the concerns about how it was collected?    Robin Dreeke touched upon the nuances, emphasizing, "If it's been done that way, yeah, that's a concern." He further elaborated on the danger of investigators becoming overly emotionally invested in cases, potentially leading to oversights or missteps in the process. In a particularly poignant moment, Dreeke noted, "Emotionally being invested in this versus being professionally invested in this and doing things the right way... there's much bigger things that could go wrong here."    In light of such concerns, the episode delved into the quintessential challenge of balancing the pursuit of justice with procedural propriety. While the pressing urgency to "right wrongs in society" is paramount, there are processes and rules designed to ensure that justice isn't just served, but served right.    Further adding to the conundrum is the revelation that some leads, directly linked to BTK, have been gathering dust for years. The existence of "trophies" — a chilling term used by killers to refer to mementos from their victims — buried in Rader's backyard shed was noted in a 2008 letter. Yet, it took until 2023 for any action to be taken on this information.    One could argue, why the delay? Dreeke offers a simple, albeit troubling, answer: resources. "There are not nearly enough people working these things to do it all," he states. The demands of prioritizing, often driven by public and media outcry, means some cases, or leads, are left by the wayside.    Reflecting on his time with the FBI, Dreeke offers insight into the agonizing choices that law enforcement personnel must make daily, juggling a multitude of "urgent" tasks. They're in a perpetual race against time, forced to make heart-wrenching decisions about where to allocate their attention. While the public's cries for justice echo loudly, law enforcement is constrained by the practicalities of resource allocation and the ever-present, towering backlog of cases.    It's a haunting reminder that behind every unresolved case, there's a team of individuals grappling with the knowledge that they couldn't do it all. As Dreeke poignantly remarked, referencing the film "The Guardian": "Sometimes you just can't do it all." The pursuit of justice is never black and white, and as the podcast episode reveals, even the most notorious cases aren't exempt from the shadows of uncertainty.
