hVIVO Chief Executive Officer Mo Khan joined Steve Darling from Proactive to share significant news about the company's latest developments. hVIVO has signed a £2.5 million contract with a mid-sized pharmaceutical company to initiate an Omicron characterisation study. Intro A few months ago, the other (former) Alvea executives and I made the difficult decision to wind Alvea down and return our remaining funding to investors. In this post, I'll share an overview of Alvea's story and highlight a few of our wins and challenges from my perspective as an Alvea Co-Founder and former CTO, in hopes that our experiences might be useful to others who are working on (or considering) ambitious projects to solve the biggest problems in the world. I'm sharing everything below in a personal capacity and as a window into my current thinking - this is not the definitive or official word on Alvea and it doesn't necessarily represent the views of any other Alvea team members. I expect my reflections to continue evolving as I further process the journey and outcomes of this project, and hope to share more along the way. Alvea's Story First vaccine sprint and decision to continue Alvea (December 2021 through April 2022) We launched Alvea in response to the Omicron wave of COVID-19 (the most transmissible and immune-evasive variant then to have arisen) as a short-term, high-risk project with the goal of developing an easily-deployable, room temperature-stable DNA vaccine against Omicron as quickly as humanly possible, without compromising on safety and quality. We ultimately carried this vaccine candidate into Phase I clinical trials in less than 6 months, becoming the fastest startup ever to take a new drug candidate from company launch to a human clinical trial. Our success in this initial sprint caused us to expand our ambitions for Alvea and explore ways of building on the track record and momentum we'd built up. One of our initial strategies was to deploy our first vaccine (which was optimized for the Omicron BA.2 variant), and then leverage our development platform to roll out updated versions in response to the emergence of new variants. However, a few key updates convinced us that this was not a promising path. First, the time between waves of new variants continued to drop, making it more difficult to keep up with viral evolution. Second, the FDA and other regulators began an expedited approval process for updated versions of the mRNA vaccines that had already been authorized, making it more difficult for new vaccines to break into the commercial market (a great pandemic regulatory move, though!). Additionally, early efficacy results for our vaccine were underwhelming and suggested that it was unlikely to provide sufficient protection to justify continued investment, particularly against the newest variants in low- and middle-income countries. In light of these updates we decided to stop development of our candidate and consider other paths forward. Product pursuits (May 2022 through July 2022) Despite discontinuing our first vaccine program, we believed we'd landed on a compelling model for general-purpose acceleration of promising drugs and vaccines into the clinic, so we set out to find other high-impact products we could accelerate with this approach. We specifically targeted products and technologies with early published preclinical data that were nearing readiness for clinical testing, with the idea that we could pick them up and dramatically speed up their development and deployment. We ran multiple "product pursuits" in parallel to explore possible technologies, with particular focus on nucleic acid vaccines that could be formulated as dry powders for inhaled administration and on therapeutic interfering particles, a class of RNA therapeutics/prophylactics that are expected to be highly resistant to viral evolution. It works by activating the immune system against the spike protein, so it will be ready to fight the actual virus when you get infected. Regardless of vaccine type, the shots are administered in the upper arm and have been demonstrated, for most people, to be safe and effective. There have now been hundreds of million vaccines administered in the US alone and the effectiveness of the vaccine to reduce the risks of severe illness, hospitalization, and death has been well documented. The most common side effects consist of mild to moderate cases of fever, chills, headache, and tiredness that are self-resolving.What is new about COVID-19 Vaccines?The updated vaccine is known as “bivalent”. This term is important because it refers to the vaccine's ability to confer protection against both the original COVID-19 virus as well as new variants Omicron BA.4 and BA.5. Rollout of the updated vaccine began in September 2022 for those aged 12 years and older and became widespread in March 2023 with approval granted for use in children aged 6 months – 4 years. Selected individuals over age 65 or those who are immunocompromised may receive additional doses to provide comparable and safe protection. It was determined that the latest circulating variant currently making rounds is from the Omicron group known as XBB. The committee decided it is prudent to proceed with a preference for the XBB 1.5 variant. The updated vaccine will be a monovalent version available in the Fall of 2023. As with the previous version, the FDA will provide strict oversight and safety monitoring of the vaccine._______________________________Conclusion: Now we conclude episode number 149, “COVID Vaccines Updates.” Future Dr. Williams explained that the bivalent COVID vaccines are currently recommended for unvaccinated patients, or for those who were previously vaccinated with monovalent vaccines. 幾個關於變種病毒的速報。 美國最新一週XBB.1.5佔比有修正,還有台灣,日本針對中國入境驗到的病毒株初步都是BA.5.2還有BF.7為主。 1.美國新的一週病毒定序有修正。過去三周XBB.1.5 校正回歸為11.5-->18.3-->27.6%,增加占比的速度沒有上週直接衝到40%看起來這麼嚇人。BQ家族還是占最多 55.8%,BF.7,BA.5都繼續減少。但東北新英格蘭區域以紐約為主的確XBB.1.5已經成主流,也開始看到長輩的住院人數開始增多,值得繼續追蹤。 2.台灣指揮中心昨天公布,自1月1日至1月4日針對中國入境採檢,共採檢3999件,825名陽性,陽性率為20.6%。入境陽性率約莫在17.6%至25%之間挪移,每日陽性率會隨著入境地區感染情況不同而有所異動,這4天觀察陽性率約20%左右。1月1日自中國來台陽性個案有100多件,指揮中心針對Ct值26以下的10多件進行定序,目前以BA.5.2、BF.7為主,另外有少數BA.5.1,都是已知且常見的病毒株,不過目前尚未完全完成定序,詳細數據待累計更多資料後再公布。 3.日本從12月30日開始中國入境日本做落地快篩,七天檢查了4895人,其中陽性是408人,約8%。半數以上是無症狀。厚勞省目前公布了主要是12月30日入境的病毒監測,來自中國的64人中,全部都是Omicron。BA.5.2佔28例,BF.7佔32例。沒有驗到XBB或XBB.1.5。 04b解讀: 1.台灣和韓國針對中國入境的PCR陽性率大概是20~30%之間。台灣前幾天採檢657人時新聞有寫其中陽性164人,陽性率為25%,其中Ct值小於或等於30者計有42人,就是其中大概只有四分之一是病毒量比較高的。其他四分之三可能多半是幾個月內感染過的陰陽人。當然也有機會是正好抓到病毒量正在上升的人就是了。但我們大概在現階段是不太需要大費周章去釐清這件事了。 2.對比日本這個數字是落地快篩病毒量較高的才會抓出,落在8%,是很合理的數字。 3.如果後續資料大概就和中國自己公布的差不多,都以BA.5.2、BF.7為主,沒有看到令人擔心的新朋友,累積資料量再大一些後,我想應該就可以比較安心一點了。反而美國這邊XBB.1.5的動向更值得關注。 歡迎追蹤前台大感染科醫師。04b的發聲管道! 我的電子名片 https://lit.link/linshibi 希望大家當我的種子教師,推廣正確的新冠衛教。科學防疫,不要只以恐懼防疫! 歡迎贊助林氏璧孔醫師喝咖啡,讓我可以在這個紛亂的時代,繼續分享知識努力做正確新冠相關衛教。 https://pay.firstory.me/user/linshibi Powered by Firstory Hosting

Coronavirus Update #70 GOOD NEWS: This information is so important to help us all understand risk. Stratified risk is the only true way to measure personal risk. Let us look at some CDC data from the spring Omicron BA.1 and .2 spikes versus the fall 2021 delta wave. Median age of hospitalization has increased from 60 years old with Delta to 64 with BA.1 and 71 with BA.2. Any underlying medical condition associated with hospitalization increased from 89% with Delta to 92% with BA.1 to 95% with BA.2 respectively. Length of hospital stay decreased from 4.8 to 3.9 to 3.3 days. ICU admission was down from 24% to 18% to 13% of admitted patients. Mechanical ventilation decreased from 14% to 8% to 6% of admitted patients. And Finally, death from 12% to 8% to 5% overall. What we can glean from this data set is very clear. With successive SARS2 mutations coupled to increased population based exposure to virus via infection or vaccine, we are now seriously in a reduced risk state unless you are older than 65 years with a comorbid disease or younger than 65 with a serious disease. 95% of hospitalizations were related to a comorbid disease regardless of age. The other big takeaway was this: if you are in this high risk group, getting every available booster is vital to your survival based on the risk reduction data. For everybody else, the data is clear, you are ok - to boost or not to boost is up to you. But, absolutely work on your general health. To your health, Dr. M

The booster shots will be released months before trials on humans are completed, according to a new report. New COVID-19 booster shots that target the latest Omicron variant will be approved by the FDA this week, the Wall Street Journal reported. The new booster shot will be "bivalent" – which means it can target the original COVID-19 strain and the Omicron BA.5 subvariant. The booster shot will likely be available to Americans despite not having been tested on humans. Instead, the FDA will rely on testing on mice, data from current COVID-19 vaccines, and earlier iterations of boosters. The Wall Street Journal noted, "The Food and Drug Administration is expected to authorize new COVID-19 booster shots this week without a staple of its normal decision-making process: data from a study showing whether the shots were safe and worked in humans." FDA Commissioner Robert Califf issued a statement on Twitter regarding how the government agency will likely make a decision in granting emergency use authorization (EUA) for booster shots from Moderna and Pfizer. "FDA will rely on the totality of the available evidence in making a decision, including: Clinical trial data from other bivalent mRNA COVID-19 boosters RWE from current COVID-19 vaccines administered to millions of people, non-clinical data for the bivalent BA.4/5 vaccines, bivalent and multivalent vaccines are very common and modifying a vaccine to include different virus strains often does not require a change in other ingredients," Califf said on Twitter. Coronavirus Update 67 Natural infection provides a much better route to transmission protection than intramuscular systemic vaccination because of the induction of mucosal immunity via IgA and tissue resident T cells. (Tang et. al. 2022) If you can tolerate a natural infection without much morbidity, this seems to be a reasonable choice at this time. This is especially important now with the immune escaping variants of Omicron lineage and the reality of the current vaccine booster pool is lackluster in its effect on transmission. The public has already been primed to believe that lockdowns were great for generating energy savings. We saw the evidence of that with our own eyes. Traffic jams disappeared. Oil prices went negative. Air pollution reversed. In the face of late Soviet-style chaos on the streets, unconstrained inflation, not enough electricity to heat the homes of the vulnerable, the prospect of order emanating from the “temporary” suspension of a market economy might seem appealing. It’s even easy to predict the messaging that might feel compelling: ‘Stay Home. Don’t queue. Save Energy.’ Or, ‘Bread and energy is cheap if you stay home!’ But here is why we must not fall for this line of logic. Planned economies are what got us into this mess to begin with. Covid, the war in Ukraine and sanctions against Russia may have all added accelerants to the fire, but the smoulders were burning ever since the 2008 global financial crisis nearly brought down the system. It’s just that the consequences of papering over the flaws in the system rather than properly addressing them only became visible in late 2021. It took 70 years for the communist system to fall apart under the weight of its own capital misallocation. We’ve managed to achieve it in about 14 years. At the heart of the problem is the poorly thought-out subsidisation of negative-sum business models propelled by excessive cheap money in the system. In the communist period, this sort of misdirection was the fault of state bureaucrats who had no idea about what people really wanted. This time it’s been driven by deep-pocketed Western venture capitalists who became convinced that outsized rents from monopoly interests could compensate for short-term non-profitability. That’s not to say the market economy is perfect or free of its own negative externalities. It definitely needs political guidance and a moral backbone to stay on course. But locking people down is not the answer. This only addresses the symptoms, not the cause. Our best path out of this mess is to keep the system as free as possible so that the people themselves can innovate their way out of trouble. Necessity, as they say, is the mother of invention. 近來很多人擔心Omicron的表妹們。洪副院長昨天和我討論,說我擔不擔心邊境改為3+4,這些新的變種病毒會進來?
