In this episode of the Advancing Surgical Care Podcast, ASCA Chief Executive Officer Bill Prentice talks with orthopedic surgeon and ASCA Board member David Weinstein, MD, about performing outpatient total shoulder surgeries in both hospitals and ASCs.Tens of thousands of patients with commercial insurance undergo safe, effective total shoulder surgeries in ASCs each year, saving themselves and their insurers millions of dollars. Yet the Centers for Medicare & Medicaid Services (CMS) has refused to extend its insurance coverage to ASCs—the less costly site of care and the one that most patients prefer. Dr. Weinstein discusses several clinical studies that include findings that support making this change, as well as his own experience performing thousands of shoulder surgeries, in this compelling case for finally allowing Medicare patients the same choice that private pay patients enjoy today.Dr. Weinstein is a graduate of the University of Colorado School of Medicine, where he completed his residency in orthopedic surgery, as well as an associate clinical professor at the University of Colorado Department of Orthopedics in Denver.

Darrell has worked as an Emergency Manager for nearly 20 years at the local, regional, and state level when he was with Spokane Emergency Management and for the last 8 years at Providence. As an Emergency Manager for Providence Inland Northwest Washington, he was responsible for the Emergency Management Program for the five (5) Providence hospitals in Spokane and Stevens County that represent just over 1,000 licensed beds: a level 2 and 3 trauma centers for adults and pediatrics, two 25-bed Critical Access Hospitals (CAH), a specialty rehab facility, and stand-alone Ambulatory Surgery Center. He started working for Providence in September 2015. Prior to work at Providence, Darrell worked for the Spokane Department of Emergency Management for 11 years, as the Regional Coordinator for Washington State Homeland Security (HLS) Region 9 (6 years) and as a Program Coordinator (5 years). Region 9 is comprised of the 10 counties and three tribes of Eastern Washington. In that role, he supported regional collaboration, coordination, and an interagency approach to all-hazard emergency preparedness. He has worked in all phases of emergency management, supporting planning, training, and exercises, HLS grants, and grant-related projects. Darrell continues to support community-wide planning on behalf of Providence. Darrell has an undergraduate degree in Construction Science from Texas A&M University, a master's degree in business, and retired from the naval reserve as an Explosive Ordnance Disposal Officer December 1, 2015 (12 years active, 13 years reserve). He is most proud to be happily married (30+ years) and the father of three kids +1 (married daughter). Ryan Tuchmayer, MPH, CEM Director of Emergency Management Cedars-Sinai Medical Center Ryan is the Director of Emergency Management for Cedars-Sinai Medical Center. In this episode, cohosts Dr. Aaron Fritts and Dr. Krishna Mannava interview vascular surgeon Dr. Sean Hislop about building an ambulatory surgery center, including where to purchase property, how to plan your build, and how to prepare for expansion. --- CHECK OUT OUR SPONSOR Medtronic OBL https://www.medtronic.com/obl --- SHOW NOTES We begin by discussing Dr. Hislop's current practice in Charleston, South Carolina. He is part of a group of eight vascular surgeons, and is also chief of vascular surgery at a local hospital. Their group has 8 offices, 2 of which are outpatient based labs (OBLs). They are currently working on building an ambulatory surgery center (ASC) that is projected to open in April 2023. Dr. Hislop describes how ownership of the ASC was determined. Five interested partners in their group used their personal funds (5 equal parts) and in turn all 5 are on the board of directors. They keep 100% of their profits and work with a local banker that they have built a trusted relationship with from their prior experience with OBLs. To plan for future expansion, each partner will devote a certain percentage of their shares which will go into a pot to provide shares for future partners to buy in. When it came to deciding where to buy property and build their ASC, they factored in weather, price, and proximity to patients. They did market research to evaluate where to build that would be close to their target patient population. They were able to find an affordable property in an area with a high concentration of retirees. Their LLC leased the land, and their practice leased space from the building owner. They built out one procedure room with a portable 9900 OEC C-arm, 4 prep and 4 recovery bays where patients can stay for up to 48 hours. South Carolina is a certificate of need (CON) state, meaning they had to apply for a CON to do all their procedures. Their current CON is procedure specific, not specialty specific, though it does not currently include coronary interventions. This allows them to bring in interventional cardiologists, interventional radiologists or podiatrists in the future. Lastly, Dr. Hislop talks about the hiring process in the ASC. Throughout the COVID-19 pandemic, there has been a huge surge in travel nursing, which has caused retention problems throughout the country. Dr. Hislop remarks that he has recently seen the tides shift back towards normal employment. He believes that in order to recruit and retain high quality staff, it is vital to understand the market and offer competitive salaries. Some of the benefits to working at an ASC instead of a hospital is the lack of nights, weekends, and call coverage. For Dr. Hislop and his partners, they believe that efficiency and work satisfaction are more important than a big financial outcome, which is why they are passionate about building this ASC. DR. MARY WILSON Dr. Mary Wilson began her nursing career in 1987. She was employed by San Antonio Hospital in Upland California in June of that year. She quickly transitioned into the CCU there by the following year. She moved on to the Operating Room at St. Bernardine's Medical Center by the year 1989. There she was promoted to Open-Heart Coordinator for the hospital and worked with Dr. Leonard Bailey performing pediatric open-heart procedures. She left that position in 1992 and moved with her extended family to Colorado where she continued as the Open-Heart Coordination at Memorial hospital in Colorado Springs. Missing the sunny beach air of California, she and her family moved back to California and she accepted a position once again at St. Berardine's, but this time in an administrative role as the Manager of the Outpatient Surgery Center. One of the highlights of her career is having served in Haiti after the catastrophic earthquake in 2010. Seeing the devastation and poverty there gave her life a new meaning. Holding injured children in her arms and helping transport them to a higher level of care at the University of Miami's mobile units was an experience that shaped her life moving forward. Working with vulnerable populations has been a professional goal of hers that she was able to fulfill in 2016 when she began giving anesthesia at the Indio Surgery Center to pediatric patients. In this podcast, Dr. Wilson discussed her award-winning Outstanding Doctor of Nursing (DNP) Project, titled Pediatric Dental Anesthesia Guideline Development and Evaluation in an Ambulatory Surgery Center. He founded the Gastrointestinal Associates in Knoxville and led the group for four decades. A native of Allentown Pennsylvania, Dr. Ernest Normington attended medical school at Hahnemann University in Philadelphia. After medical school, Dr. Normington trained in general surgery at the Geisinger Medical Center for eight years. He trained in plastic surgery at the world-renowned Cleveland Clinic Foundation known for excellence in plastic surgery. Dr. Normington is board certified by the American Board of Plastic Surgery (ASPS) and is a member of the American Society of Plastic and Reconstructive Surgeons. He is an adjunct staff member of the Department of Plastic Surgery in the Dermatology and Plastic Surgery Institute at the Cleveland Clinic. Dr. Normington's areas of expertise include facial surgery, breast surgery, body sculpting, laser surgery and skin care. He is also trained in advanced hand reconstruction. He has published numerous medical journal articles on his research in plastic surgery and has lectured to national and international audiences. This podcast highlights original research published in the June 2020 issue of Otolaryngology–Head and Neck Surgery, the official journal of the American Academy of Otolaryngology—Head and Neck Surgery (AAO-HNS) Foundation. Surgical care is increasingly shifting to freestanding ambulatory surgical centers (ASCs). The extent to which otolaryngologists use ASCs has implications for patient safety and health care spending. This study characterizes trends in utilization and resultant financial implications for common otolaryngologic procedures performed at ASC and hospital outpatient departments (HOPDs). In conclusion, otolaryngologists shifted outpatient surgical care from HOPDs to ASCs between 2010 and 2017, with resultant reductions in Medicare expenditures. Further research is necessary to examine the impact of this shift on patient safety. Click here to read the full article.
