In this powerful and deeply personal episode of You're Not Broken, Dr. Kelly Casperson is joined by psychiatrist Dr. Judith Joseph to unpack the complex relationship between trauma, high-functioning depression, and the systemic challenges facing both patients and providers in healthcare. Dr. Joseph shares her unique career journey from anesthesiology to psychiatry—a shift fueled by the pursuit of deeper fulfillment and connection. Drawing from her new book High Functioning, she explores how trauma can show up in unexpected ways: perfectionism, strained relationships, and even the culture of medicine itself. Together, Dr. Joseph and Dr. Casperson dive into the mental health crisis within the medical profession, the difference between complex PTSD and traditional PTSD, and why trauma-informed care must become the standard in healthcare. They highlight the need for preventative mental health care and why joy—not just happiness—is vital to healing. Key Takeaways: Dr. Judith Joseph's new book High Functioning explores hidden depression and the masks people wear. A desire for deeper meaning and purpose sparked her transition from anesthesiology to psychiatry. Trauma can subtly impact daily life, relationships, and career satisfaction. Complex PTSD differs from traditional PTSD and requires a nuanced approach. Perfectionism may be a trauma response rooted in avoidance and fear. Many healthcare professionals operate with unresolved trauma, often unacknowledged and invalidated. The medical system itself can contribute to both patient and provider trauma. High-functioning depression is widespread but often misunderstood or overlooked. Preventative mental health care is crucial—waiting for a crisis is not sustainable. Joy is a core component of mental health, not a luxury or afterthought. Understanding one's own trauma can make you a more effective and empathetic clinician. Identifying and naming emotions can be profoundly therapeutic. Self-care and setting boundaries are necessary acts of preservation, not selfishness. https://www.instagram.com/drjudithjoseph/ High Functioning Book by Dr. Joseph https://drjudithjoseph.com/ Sedona in October - only five slots left: https://www.ascendretreats.com/menopause-and-sexual-health-2025 Order my book "You Are Not Broken: Stop "Should-ing" All Over You Sex Life" Listen to my Tedx Talk: Why we need adult sex ed Take my Adult Sex Ed Master Class: My Website Interested in my sexual health and hormone clinic? In this enlightening episode of You're Not Broken, Dr. Kelly Casperson sits down with Dr. Steven Gundry, renowned cardiothoracic surgeon turned gut health advocate. Dr. Gundry shares the personal and professional journey that led him from the operating room to the forefront of nutritional science. He breaks down the concept of leaky gut and how it contributes to autoimmune diseases, emphasizing how dietary changes can radically transform health outcomes. Together, Dr. Gundry and Dr. Casperson dive into the deep connection between gut health and mental health, the underrated power of vitamin D, and the hidden dangers lurking in our food supply and personal care products. They also tackle the pharmaceutical industry's influence on medical training and why it's time for a paradigm shift in how we understand disease and wellness. If you've ever wondered how your gut impacts everything from anxiety to skin health—or why your mouthwash might be raising your blood pressure—this episode is for you. Top Takeaways: Dr. Gundry left a thriving surgical career after witnessing remarkable patient healing through nutrition. Leaky gut is a real, measurable condition that drives many autoimmune disorders. Genetics are less impactful than lifestyle, behavior, and environmental factors. Whole food diets can reverse chronic illness and improve quality of life. The gut microbiome plays a central role in immunity, skin health, and brain function. Modern agriculture and personal care products can damage the microbiome. Vitamin D is a foundational nutrient for both gut and mental health. Gut health is deeply tied to emotional states like anxiety and depression. Mouthwash can disrupt the oral microbiome and raise blood pressure. Probiotics and prebiotics are helpful—but only when used with care and intention. We must become more aware of what we eat, apply, and ingest daily. Personalized medicine is the next frontier in optimizing gut health. https://www.instagram.com/drstevengundry/ The Gut Brain Paradox - Releases April 15th https://gundrymd.com/ Sedona in October - only three slots left: https://www.ascendretreats.com/menopause-and-sexual-health-2025 Order my book "You Are Not Broken: Stop "Should-ing" All Over You Sex Life" Listen to my Tedx Talk: Why we need adult sex ed Take my Adult Sex Ed Master Class: My Website Interested in my sexual health and hormone clinic? Women deserve to feel turned on for their own pleasure, not just for others. Men's enjoyment in relationships often comes from seeing their partner's pleasure. Communication about sexual needs is crucial for both partners. Exploring sexuality can enhance intimacy and connection in relationships. Emotional honesty leads to a more expansive and fulfilling relationship. Hormone therapy can reignite passion and feelings in midlife. Nothing makes us lose desire for sex faster than having bad sex. Indeed, many cases of low sexual desire are simply due to people having sex that isn’t worth having. In this episode, we’re going to talk about the three main types of bad sex, what it means to have good sex, and how to stop having bad sex. Today, we're revisiting a fantastic conversation I had with Dr. Kelly Casperson back in episode 128. Kelly is a urologist, sex educator, author, and podcaster whose mission is empowering women to live their best love lives. She combines education, humor, and candor in her book and podcast, both of which are titled, You Are Not Broken. Some of the specific topics we explore include: What are the three main kinds of bad sex? How can we stop thinking of sex as something we’re obligated to do? Why is it bad idea to think of sex as a bargaining tool? What kind of sex should we be aiming for? How can we get closer to having good (or “good enough”) sex? “Demonstrates how some of the most accomplished and elite scientific gatekeepers may have lied, cheated, squandered trust and endangered lives.” —The Wall Street Journal In this episode of the You Are Not Broken podcast, Dr. Kelly Casperson interviews investigative journalist Charles Piller about his newly released book, 'Doctored,' which delves into the misconduct in Alzheimer's research. They discuss the amyloid hypothesis, the role of whistleblowers, the FDA's controversial drug approvals, and the systemic issues in medical research. Piller emphasizes the importance of scientific integrity and the need for independent oversight in research while also exploring emerging theories in Alzheimer's treatment and the impact of lifestyle factors on prevention. Takeaways The amyloid hypothesis has dominated Alzheimer's research but has significant flaws. Whistleblowers play a crucial role in exposing misconduct in scientific research. The FDA's approval process for drugs can be influenced by industry relationships. Lifestyle factors such as diet and exercise can impact Alzheimer's risk. There is a growing interest in alternative theories for Alzheimer's treatment. AI can assist in detecting research fraud but also poses risks for falsification. In this episode, Shelley Craft sits down with Dr. Kelly Casperson to debunk the myths, break down the science, and—most importantly—explore real, effective solutions to improve bladder health at any age. In this episode of the You Are Not Broken podcast, Dr. Kelly Casperson interviews Dr. Jocelyn Fitzgerald, a urogynecologist specializing in pelvic floor disorders. They discuss the importance of open conversations about women's health, particularly regarding prolapse and the misconceptions surrounding childbirth. Dr. Fitzgerald shares her journey into social media advocacy, the challenges women face in understanding their health, and the need for better education and separate specialties in women's health. The conversation highlights the systemic issues in healthcare that affect women's treatment and the importance of empowering women with knowledge about their bodies. In this conversation, Dr. Jocelyn Fitzgerald and Dr. Kelly Casperson discuss the complexities of women's health, particularly focusing on menopause, the role of healthcare providers, and the challenges faced in the healthcare system. They emphasize the need for a multidisciplinary approach to women's health, the importance of vaginal health, and the radical nature of urogynecology as a feminist act. The discussion also touches on the future of OB-GYN, reproductive rights, and the necessity for women to take action in advocating for their health. Takeaways Prolapse is often misunderstood and not openly discussed. Education can reduce anxiety about childbirth and its consequences. Women often feel ashamed or broken due to health issues post-childbirth. IUD placement pain is often inadequately addressed in women's health. Obstetrics and gynecology should be separate specialties for better care. The healthcare system often undervalues women's health procedures. Women need to be informed about the risks associated with childbirth. Empowering women with knowledge can lead to better health outcomes. Ob-Gyns often lack knowledge about menopause-related issues. A multidisciplinary approach is essential for women's health. The healthcare system can be toxic and burdensome for providers. Urogynecology plays a crucial role in women's health post-childbirth. Vaginal estrogen is vital for maintaining vaginal health. Incontinence is a leading cause of nursing home admissions. Reproductive rights are under threat and require advocacy. Reliable male birth control could change societal dynamics. https://www.instagram.com/jjfitzgeraldmd/ Sedona in October: https://www.ascendretreats.com/menopause-and-sexual-health-2025 Order my book "You Are Not Broken: Stop "Should-ing" All Over You Sex Life" Listen to my Tedx Talk: Why we need adult sex ed Take my Adult Sex Ed Master Class: My Website Interested in my sexual health and hormone clinic? In this conversation, Dr. Kelly Casperson discusses a significant study on the effects of transdermal testosterone therapy on mood and cognitive symptoms in peri- and postmenopausal women. She emphasizes the importance of addressing hormonal changes and their impact on women's mental health, particularly during menopause. The study reveals that testosterone therapy can lead to improvements in mood and cognitive function, challenging the notion that there is insufficient data on testosterone's benefits for women. Dr. Casperson also critiques the study's limitations and calls for further research in this area, while engaging with her audience on related questions and concerns. Takeaways Testosterone therapy can improve mood and cognitive symptoms in women. The study involved 510 women with persistent low libido and mood symptoms. Mood improvements were more significant than cognitive improvements. Testosterone does not drop off a cliff with menopause; it declines gradually. Hormonal changes during menopause can lead to increased risk of depression and cognitive impairment. Testosterone therapy may be a safer alternative to SSRIs for mood issues in perimenopausal women. The study highlights the need for more research on testosterone's effects in women. Libido is closely linked to mood; improving one can enhance the other. Transdermal testosterone is generally safe and easy to administer. Women should be informed about their options for hormone therapy. Study LINK Glynne S, Kamal A, Kamel AM, Reisel D, Newson L. After many years of practice, a pivotal encounter with a patient ignited her deep curiosity about female sexual wellness, leading her to learn everything she could to help others. Compelled to do more, she realized that helping patients one-on-one in her clinic wasn't enough to address the widespread gap in sexual and hormonal education. Recognizing that women aren't broken in the bedroom but simply misinformed, she launched the You Are Not Broken podcast. The show quickly gained popularity, consistently ranking in the top 10 in Apple Podcasts' “Medicine” category in the U.S., and has been nominated for an AASECT award three years in a row. In addition to the podcast, Dr. Casperson has developed online courses, a book, and a membership that empowers women by teaching them the fundamentals of their anatomy, physiology, and sexual function. Her approach helps women dismantle limiting beliefs and live their best love lives. Combining her medical expertise with certifications from The Life Coach School and the North American Menopause Society, Dr. Casperson uses a holistic, evidence-based approach to help women achieve sexual wellness and confidence. Known for her engaging and humorous storytelling, Dr. Casperson is a sought-after speaker who makes people feel at ease discussing often uncomfortable topics. In episode 301 of the You're Not Broken podcast, Dr. Kelly Casperson discusses 15 things she wishes would change in women's health, focusing on hormonal health, the healthcare system, sexual health, and education. She emphasizes the importance of choice regarding hormone therapy after breast cancer, the need for better sex education, and the challenges posed by the current healthcare system. Dr. Casperson also shares her aspirations for future clinics and the importance of addressing orgasmic inequality and healthcare costs. Takeaways Hormones after breast cancer should be a choice. There is a need for FDA-approved female-dosed testosterone products. Healthcare costs are a significant barrier to accessing care. Sex education in the U.S. is inadequate and needs reform. Orgasmic inequality is prevalent, particularly in heterosexual relationships. Vaginal estrogen is safe and should not carry a cancer warning. Healthcare providers need better work-life balance to prevent burnout. Education is key to improving sexual health and understanding. The healthcare system should prioritize patient care over profits. Future clinics are needed to support women's health and hormonal optimization. She emphasizes the need for longer patient visits and the significance of understanding the nuances of healthcare. Additionally, she discusses upcoming events and retreats focused on women's health education. Takeaways Vaginal estrogen is safe and effective for postmenopausal women. Testosterone is present in both men and women, and is crucial for health. Short medical visits hinder effective healthcare delivery. Menopause is a natural process, and suffering through it is not necessary. Alcohol consumption is linked to several types of cancer. Osteoporosis can be improved with the right interventions. Estrogen does not inherently cause cancer; it can support health. Regular exercise is essential for overall health and well-being. However, increasing numbers of women dealing with hot flashes, insomnia, painful sex, and loss of libido are using non-FDA-approved hormone pellets. Millions of women have no idea that they are using a form of hormone therapy that is unregulated and associated with side effects such as an enlarged clitoris, and male pattern baldness, as well as an increased risk of hysterectomy. How pellets are different than FDA-approved commercial hormone therapy Why pellets are so popular The problem with pellets Side effects from pellets Irreversible side effects Typical hormone levels when using pellets Why women are more likely to bleed when using pellets Why women are more likely to need a hysterectomy when using pellets Menopause Retreat with Dr. Streicher, Dr. Kelly Casperson, and Dr. Ellen Gendler! In this episode, Dr. Katie Kennedy, a pathologist, discusses the intersection of aging, health, and the effects of alcohol on the body. The conversation explores how alcohol impacts various organs, particularly the liver and brain, and its connection to inflammation and cancer. The role of estrogen in women's health and the implications of aging on inflammation are also examined, providing insights into how these factors affect quality of life. In this conversation, Dr. Kelly Casperson and Dr. Katie Kennedy discuss various aspects of women's health, particularly focusing on postmenopausal issues such as bladder health, the importance of vaginal estrogen, and the role of pathology in understanding these conditions. They delve into the significance of bone health and osteoporosis, emphasizing the need for preventative measures against hip fractures and frailty. The discussion also highlights the connection between breathing, oral health, and overall well-being, advocating for a holistic approach to health that considers the entire body. Takeaways Pathology is the study of diseases and their effects on tissues. Alcohol has a significant impact on liver health, leading to conditions like fatty liver disease. Fatty liver disease can be reversed if caught early and with proper support. Alcohol is classified as a carcinogen, contributing to various cancers. Chronic alcohol consumption can lead to memory loss and increase the risk of dementia. The loss of estrogen during aging creates a synergistic effect with alcohol on brain health. Inflammation is a common response in aging and is linked to estrogen loss. Women metabolize alcohol differently than men, making them more susceptible to its effects. Inflammation can be observed in various tissues, not just localized areas. Understanding the impact of alcohol on health is crucial for making informed choices. Postmenopausal women often suffer from bladder issues that are under-discussed. Vaginal estrogen should be a standard part of women's health care. Thin vaginal mucosa can lead to increased infections and complications. Preventative measures for osteoporosis should start early in life. Hip fractures are a significant health risk for older women. Mouth breathing can lead to various health issues, including sleep apnea. Strengthening the tongue is crucial for preventing aspiration and improving swallowing. Holistic health considers the entire body and its interconnected systems. Education and awareness are key to improving women's health outcomes. Regular health evaluations should include assessments of bone and muscle strength. They discuss the evolving definition of feminism, the impact of cognitive dissonance on women's health, and the collaboration between Dr. Tavris and Dr. Avrum Blooming on hormone therapy. The conversation highlights the importance of staying curious, the role of fear in controlling women, and the challenges women face in navigating medical information. Dr. Tavris emphasizes the need for women to advocate for their health and to question outdated medical warnings. In this conversation, Dr. Kelly Casperson and Carol Tavris discuss the complexities surrounding hormone therapy, particularly estrogen, and the societal and medical biases that influence women's health. They explore the implications of labeling estrogen as hazardous, the challenges of mobilizing women for advocacy, and the cognitive biases that affect perceptions of hormone therapy. The discussion emphasizes the need for informed activism, understanding causation versus correlation, and the evolution of medical beliefs over time. Dr. Tavris offers insights on how to engage effectively with both the medical community and the public to foster change in women's health policies. Takeaways Feminism is about equality and justice for all genders. Cognitive dissonance can hinder our ability to accept new information. Women have historically been misrepresented in medical research. Collaboration between psychology and medicine can improve women's health. Fear is a powerful tool used to control women's choices. Women must navigate a complex landscape of medical information. The medical community often overlooks women's health needs. Outdated medical warnings can create unnecessary fear. Women should advocate for their health and question misinformation. Understanding cognitive dissonance can help in making informed decisions. Estrogen patches are being labeled as hazardous without proper justification. Women need to mobilize and organize for advocacy efforts. Apathy among women in advocacy is a significant barrier to change. Cognitive biases can distort perceptions of hormone therapy. Correlation does not imply causation, especially in health matters. Personal anecdotes can shape beliefs about medical treatments. The evolution of medical beliefs often requires a critical mass of evidence. Activism should focus on engaging those who are open to change. Mistakes in medicine are common, and learning from them is essential. Holding beliefs lightly allows for adaptation to new evidence. There is a misconception that hormone therapy is only for post-menopausal women; it can be beneficial during perimenopause as well. Vaginal estrogen can significantly reduce the risk of urinary tract infections. Patients should advocate for their health and seek second opinions if necessary. Hormone therapy can improve quality of life and reduce symptoms of menopause. Education about hormone therapy is essential for both patients and healthcare providers. In this episode of the You're Not Broken podcast, Dr. Kelly Casperson and Dr. Corinne Menn discuss the recent paper by Sasha Taylor and Susan Davis that revisits the recommendations for initiating menopausal hormone therapy. They explore the arbitrary nature of age cutoffs for hormone therapy, the timing hypothesis, and the implications of the Women's Health Initiative data. The conversation emphasizes the importance of understanding the risks and benefits of hormone therapy, particularly for women over 60, and addresses common misconceptions surrounding hormone therapy and its effects on bone health and dementia. The episode concludes with a call to action for women to advocate for their health and seek appropriate hormone therapy. Takeaways Millions of women feel they have been overlooked in hormone therapy discussions. The age cutoffs for hormone therapy initiation are arbitrary and not based on solid evidence. The timing hypothesis suggests that starting hormone therapy earlier can lead to better health outcomes. The Women's Health Initiative data is often misinterpreted and does not reflect the current understanding of hormone therapy. Hormone therapy can provide long-term benefits for bone health and reduce the risk of osteoporosis. Many women experience symptoms beyond hot flashes that can persist for years after menopause. The risks associated with hormone therapy are often overstated and misunderstood. Women over 60 should not be denied hormone therapy based solely on age or years since menopause. Dementia risk associated with hormone therapy is not as significant as previously thought, especially with proper medication choices. Women need to advocate for their health and seek out hormone therapy options that are safe and effective. Taylor S, Davis SR.
