Drug-induced seizures are a major concern for central nervous system active pharmaceuticals during development. Currently, there are no guidelines for interrogating and mitigating seizure liability associated with new pharmaceuticals. Because of the risks to patient safety during clinical trials, the understanding of nonclinical characterization that informs the translation of risks to humans is an area of intense interest among drug developers and regulators. A Continuing Education course was held at the 2022 ACT Annual Meeting in Denver, Colorado, where Dr. Delatte presented on identifying seizure liability in nonclinical studies. In this interview with Dr. Delatte, we discuss nonclinical findings that are indicative of seizure liability and appropriate approaches to characterize risks. Many drug classes carry seizure liability which may trigger preliminary studies, such as kindling studies, to understand risks prior to conducting a repeat-dose toxicology study. A description of premonitory signs of seizure in dose-range findings and repeat-dose toxicology studies is also discussed. Finally, this talk briefly discusses when nonclinical EEG studies are warranted and how to leverage the data from general toxicology studies to design an informative EEG study. Learning objectives of this talk are: (1) set up a drug development program for drugs in a class with seizure liability; (2) identify seizure liability in nonclinical studies; and (3) examine the elements from general toxicology studies that inform the pivotal EEG study.

On this episode of The Good, The Bad and The Pure Evil, I look at the Minamata disaster and disease that came from the wastewater of Chisso factory. Symptoms started to be seen in cats who went crazy and died, it then was seen in humans with numbness in limbs, difficulty walking, difficulty breathing, seeing and hearing. Convulsion was next and then the patient slipped into a coma and died. All this came from mercury poisoning which the company was aware was deadly. The patients put up a fight with not only the company but also the government winning recognition of the disease, who caused it and compensation for the victims. This win would take years to achieve and is still going on today.

Epilepsy in the New Millennium: Emerging Treatments and Guidelines for Effective Diagnosis and Disease Management - PDF Course Materials

>Does having a seizure mean epilepsy, or is it an isolated neurological event? Failure to recognize diagnostic uncertainty between the epilepsies and non-epileptic events may be a factor in high rates of misdiagnosis. Many patients who have been misdiagnosed as having epilepsy have had previous EEGs interpreted as epileptiform that contributed to the misdiagnosis of epilepsy. Patients are often faced with the need to tolerate complex cocktails of medications over long periods of time causing side effects of these chronic medications to be of concern for both physicians and their patients. Pharmacologic management of patients with epilepsy is one of the greatest unmet needs of the disease. It is important that clinicians are aware of the evidence for current and emerging therapies so they can individualize care of their patients. There is no real "Gold Standard" of guidelines for improving diagnosis and treatment of epilepsy, but clinicians need to be aware of the most recent guidelines and how they can incorporate them in their practice.
