Contributor: Alec Coston, MD Educational Pearls: Hepatic encephalopathy (HE) is defined as a disruption in brain function that results from impaired liver function or portosystemic shunting. Manifests as various neurologic and psychiatric symptoms such as confusion, inattention, and cognitive dysfunction Although ammonia levels have historically been recognized as important criteria for HE, the diagnosis is ultimately made clinically. An elevated ammonia level lacks sensitivity and specificity for HE Trends in ammonia levels do not correlate with disease improvement or resolution A 2020 study published in the American Journal of Gastroenterology evaluated 551 patients diagnosed with hepatic encephalopathy and treated with standard therapy Only 60% of patients had an elevated ammonia level, demonstrating the limitations of ammonia levels However, a normal ammonia level in a patient with concern for HE should raise suspicion for other pathology. In patients with cirrhosis presenting with neuropsychiatric symptoms, consider HE as the diagnosis after excluding other potential causes of altered mental status (i.e., Seizure, infection, intracranial hemorrhage) The primary treatment is lactulose Works by acidifying the gastrointestinal tract. Ammonia (NH₃) is converted into ammonium (NH₄⁺), which is poorly absorbed and subsequently eliminated from the body Also exerts a laxative effect, further enhancing elimination References: Haj M, Rockey DC. //The Wire//2300Z August 19, 2025////ROUTINE////BLUF: PENTAGON DEPLOYS NAVAL ASSETS TO SOUTHCOM TO COMBAT DRUG TRAFFICKING. PARK RANGER STABBED IN COLORADO.// -----BEGIN TEARLINE------HomeFront-Colorado: A manhunt is underway for an assailant which attacked a park ranger this morning at Staunton State Park in Jefferson County. Local authorities state that an unidentified man stabbed a park ranger this morning, before egressing on foot into the woods of the park. The suspect is described as a White Male, 6'0" tall, 200lbs and is wearing a grey shirt and blue jeans.Washington D.C. - Yesterday evening the White House announced the deployment of US Navy forces to the SOUTHCOM area of responsibility. A Task Force comprising of 3x Destroyers is currently transiting to their patrol areas off the coast of Venezuela, with the stated goal of combatting drug trafficking by sea.-----END TEARLINE-----Analyst Comments: Though it has been de-prioritized in the news cycle lately, the escalation of force to combat narco and cartel activity in Central and South America has continued to grow, with multiple Intelligence, Surveillance, and Reconnaissance (ISR) flights being observed deep inside Mexico over the past few weeks. A drone pilot leaving their transponder turned on while conducting a spying mission in southern Mexico is message, and a bold one at that.Last week, the Mexican government stated that they will hand over 26x cartel shotcallers to the United States, so that they cannot continue to run their vast narco empires from prison (or at least make it a little more challenging for them to continue operations). This is very likely an attempt at an olive branch...Mexico may be trying to play ball so that the United States doesn't start dropping Hellfire missiles in their country.More strategically speaking, one of the early indications and warnings of a very sharp increase in combat operations in an area, is the arrival of the US Navy. In the case of counter-narco operations in Central and South America, controlling the seas will be absolutely mandatory for any sort of escalation of conflict, should that course of action be the one taken later on. Arleigh-Burke Class Destroyers serve as a critical part of the Aegis defense doctrine; these vessels are mostly used for missile defense and providing protection for much more vulnerable assets (such as aircraft carriers). In this role they excel, however Destroyers also deploy with VBSS teams onboard. Visit, Board, Search, and Seizure operations are a mainstay of the counter-narco mission on the high seas, and this is very likely what these Naval vessels will be engaged in for the foreseeable future.Analyst: S2A1Research: https://publish.obsidian.md/s2underground//END REPORT//

Contributor: Aaron Lessen, MD Educational Pearls: Prehospital seizures are typically managed with intramuscular midazolam (Versed) Seizures theoretically involve the NMDA pathway, and ketamine is a potent NMDA antagonist A recent retrospective cohort study analyzed a Florida EMS protocol that uses ketamine in seizures refractory to midazolam One group received two doses of midazolam for seizure control The other group received a dose of midazolam followed by a dose of ketamine After matching, 82% of the midazolam-only group patients had resolution of convulsions prior to ED arrival 94.4% of patients in the midazolam + ketamine group experienced resolution Absolute difference between groups was 12.4% (95% CI 3.1% to 21.7%) Limitations to the study include its prehospital setting and limited long-term follow-up References Zitek T, Scheppke KA, Antevy P, et al. On this episode, Vince speaks with Alex Szinnyev, a U.S. Marine Corps veteran and security professional with 13 years of experience in military service, government contracting, executive protection, and weapons instruction. In July, Alex is scheduled to face trial for human smuggling charges after a transport operation he believed was affiliated with a church turned out to involve undocumented migrants. Alex shares his account of what happened—how he was contacted, what he was told, and why he believed the operation was legitimate. He says those who recruited him claimed to be moving refugees and even provided church references and documentation. According to Alex, he later learned that some of the individuals involved in organizing the transport had previously worked under Operation Lone Star—the controversial Texas state initiative aimed at curbing illegal immigration. He discusses the confusion this created, the legal jeopardy he's now facing, and the personal and professional fallout, including the loss of his career, security clearance, and the impact on his family and future. Greta Thunberg is a burr in the ass of the powerful throughout the world—and not only for rightwing sociopaths. Increasingly, as she grows up, learns more, and networks with other activists around the world, she's become a scourge for liberals as well. Many of them would prefer she remain a child, providing a theatre of emotional catharsis for a climate crisis industry more interested in raising money from green economy donors than forcing change. Now, as Thunberg broadens her target to capitalism and colonialism writ large, it's more and more clear that she's confronting what autistic philosopher Robert Chapman calls “the empire of normality,” from her lived experience of autism, about which so many of the conspiracy theories we cover here swirl. In this episode of Talk About It, actor/ comedian/ influencer and good friend of the show Stanzi Potenza returns to talk about something that can cause both celebration and disappointment alike... milestones. Everybody naturally loves to celebrate milestones. They can motivate us to keep pushing or show us that our progress is leading to positive outcomes, whether they are for an addiction that you have overcome, professional progress, even time between seizures. But are they designed by default to lead us to disappointment when we don't reach the next one? In the case of seizures, there is inevitability that a breakthrough seizure may - and will - occur, so is this really a healthy thing for patients or caregivers to do? Greg chats with Stanzi, because she recently posted that she had her first seizure in almost two years, which lead to some feelings of disappointment. The call she has to make to her mom, telling her followers about it, even coming out of a postictal state and realizing what has just happened. It can be discouraging when your pride and self-fulfillment is attached to not having seizures. So what is the right thing to do? Greg also has another friend of the show, Jonathan Shapiro, on to briefly give some hopeful perspective about reframing how we think about milestones. This is something we all deal with, so you do not want to miss this episode! Talk About It with Greg Grunberg is excited to be sponsored by Neurelis and by Jazz Pharmaceuticals.
