Taking folic acid prior to conception and during pregnancy can help protect the unborn baby from developing abnormalities. Supplements are particularly important for women who have epilepsy, as anti-seizure medication (previously known as anti-convulsants or anti-epileptic drugs) can lead to a deficiency in folic acid. Until 2023, high doses of 4-5 mg per day were recommended. However, this has changed as the data has changed. Did you know the SMFM no longer recommends “high dose” folic acid preconceptionally for patients on seizure medications? This is also highlighted in a recently released epub from Obstetrics and Gynecology (Green Journal) on October 31, 2025. Listen in for details. 1. Send comments and feedbackIs there a fast, simple way to measure whether someone with epilepsy is doing "better" or "worse" compared with a past clinic visit? Not really. Seizure count is often considered a standard, but multiple small seizures may have less impact on someone's life than a single generalized tonic-clonic seizure. In this episode, we talk with authors of a study on an outcome measure that factors the impact of seizures as well as the impact of treatments. The study: Validation of the seizure-related impact assessment scale Sharp Waves episodes are meant for informational purposes only, and not as clinical or medical advice.Let us know how we're doing: podcast@ilae.org.The International League Against Epilepsy is the world's preeminent association of health professionals and scientists, working toward a world where no person's life is limited by epilepsy. HEADLINE: China Retaliates Against Dutch Chipmaker Seizure Amid European Fragmentation GUEST NAME:Theresa Fallon SUMMARY: Theresa Fallon discusses China imposing export controls on Nexperia after the Dutch government seized control of the chipmaker, which was owned by China's Wingtech. The Dutch acted due to fears the Chinese owner would strip the technology and equipment, despite Nexperia producing low-quality chips for cars. Fallon notes Europe needs a better chip policy but struggles to speak with one voice, as fragmented policy allows China to drive wedges and weaken the EU. 1966 TIBET

2. The Seizure of Black Powder: Lexington, Concord, and the First Shots of the Revolution Author: Patrick O'Donnell Book Title: The Indispensables: The Diverse Soldier Mariners Who Shaped the Country, Formed the Navy, and Rowed Washington across the Delaware Black powder was the most crucial resource for the nascent revolution, as it did not exist in the colonies, making guns useless without it. The British actively sought to seize all gunpowder in New England. The Marbleheaders, utilizing their established supply lines, brought in the bulk of the powder and arms the British were trying to confiscate. A key early action occurred on December 14, 1774, during the raid on Fort William and Mary, which was a main powder source for the British. Led by John Sullivan and John Langdon, the raiders—tipped off by Marblehead intelligence—stormed the fort and seized the powder and cannon; the few shots fired here are considered the "first shots" of the American Revolution. Following this, in April 1775, the British marched toward Lexington and Concord. Marblehead leaders, including Elbridge Gerry, were meeting nearby and fled into a cornfield, fearing arrest. The returning British column faced thousands of angry militiamen, including the Marbleheaders, who fought them during their retreat along the Battle Road. This included a deadly stand at Menotomy (Arlington), where many patriots were slaughtered.

Send comments and feedbackSomatic genetic epilepsies arise from mutations that occur early in fetal development. They are usually only detectable by genetic sequencing of tissue. For these epilepsies, the timing of the mutation is key: For example, research has shown that focal cortical dysplasia type IIB and hemimegaloencephaly are genetically the same disease, but arise from somatic mutations at different developmental time points. PREVIEW: This file features a conversation between John Batchelor and Jim Fanell, a retired US Navy intelligence captain, regarding China's aggression in the South China Sea. They discuss China's 2012 seizure of Scarborough Shoal and its continued efforts to provoke the Philippines and assert "irregular" claims. Jim Fanell explains that China's declaration of Scarborough Shoal as its own, controlling access, is based on PRC law and constitutes aggression, aiming to dominate the Philippines militarily or diplomatically. Recently, a Chinese Navy destroyer and coast guard cutter chased a Philippine Coast Guard vessel, demonstrating China's intent to take full possession of Scarborough Shoal and deny Filipino access, showing a willingness to use increasing military force. This echoes the 2012 event when China took Scarborough Shoal from the Philippines without firing a shot under the Obama administration, with the awareness of Secretary Clinton and Kurt Campbell. More recently, the Chinese state government declared Scarborough Shoal a nature reserve, a "political warfare move" to force Filipinos to seek permission for access and threatening the use of force under Chinese law if they enter without authorization.

Contributor: Alec Coston, MD Educational Pearls: Hepatic encephalopathy (HE) is defined as a disruption in brain function that results from impaired liver function or portosystemic shunting. Manifests as various neurologic and psychiatric symptoms such as confusion, inattention, and cognitive dysfunction Although ammonia levels have historically been recognized as important criteria for HE, the diagnosis is ultimately made clinically. An elevated ammonia level lacks sensitivity and specificity for HE Trends in ammonia levels do not correlate with disease improvement or resolution A 2020 study published in the American Journal of Gastroenterology evaluated 551 patients diagnosed with hepatic encephalopathy and treated with standard therapy Only 60% of patients had an elevated ammonia level, demonstrating the limitations of ammonia levels However, a normal ammonia level in a patient with concern for HE should raise suspicion for other pathology. In patients with cirrhosis presenting with neuropsychiatric symptoms, consider HE as the diagnosis after excluding other potential causes of altered mental status (i.e., Seizure, infection, intracranial hemorrhage) The primary treatment is lactulose Works by acidifying the gastrointestinal tract. Ammonia (NH₃) is converted into ammonium (NH₄⁺), which is poorly absorbed and subsequently eliminated from the body Also exerts a laxative effect, further enhancing elimination References: Haj M, Rockey DC. //The Wire//2300Z August 19, 2025////ROUTINE////BLUF: PENTAGON DEPLOYS NAVAL ASSETS TO SOUTHCOM TO COMBAT DRUG TRAFFICKING. PARK RANGER STABBED IN COLORADO.// -----BEGIN TEARLINE------HomeFront-Colorado: A manhunt is underway for an assailant which attacked a park ranger this morning at Staunton State Park in Jefferson County. Local authorities state that an unidentified man stabbed a park ranger this morning, before egressing on foot into the woods of the park. The suspect is described as a White Male, 6'0" tall, 200lbs and is wearing a grey shirt and blue jeans.Washington D.C. - Yesterday evening the White House announced the deployment of US Navy forces to the SOUTHCOM area of responsibility. A Task Force comprising of 3x Destroyers is currently transiting to their patrol areas off the coast of Venezuela, with the stated goal of combatting drug trafficking by sea.-----END TEARLINE-----Analyst Comments: Though it has been de-prioritized in the news cycle lately, the escalation of force to combat narco and cartel activity in Central and South America has continued to grow, with multiple Intelligence, Surveillance, and Reconnaissance (ISR) flights being observed deep inside Mexico over the past few weeks. A drone pilot leaving their transponder turned on while conducting a spying mission in southern Mexico is message, and a bold one at that.Last week, the Mexican government stated that they will hand over 26x cartel shotcallers to the United States, so that they cannot continue to run their vast narco empires from prison (or at least make it a little more challenging for them to continue operations). This is very likely an attempt at an olive branch...Mexico may be trying to play ball so that the United States doesn't start dropping Hellfire missiles in their country.More strategically speaking, one of the early indications and warnings of a very sharp increase in combat operations in an area, is the arrival of the US Navy. In the case of counter-narco operations in Central and South America, controlling the seas will be absolutely mandatory for any sort of escalation of conflict, should that course of action be the one taken later on. Arleigh-Burke Class Destroyers serve as a critical part of the Aegis defense doctrine; these vessels are mostly used for missile defense and providing protection for much more vulnerable assets (such as aircraft carriers). In this role they excel, however Destroyers also deploy with VBSS teams onboard. Visit, Board, Search, and Seizure operations are a mainstay of the counter-narco mission on the high seas, and this is very likely what these Naval vessels will be engaged in for the foreseeable future.Analyst: S2A1Research: https://publish.obsidian.md/s2underground//END REPORT//
