Drug-induced seizures are a major concern for central nervous system active pharmaceuticals during development. Currently, there are no guidelines for interrogating and mitigating seizure liability associated with new pharmaceuticals. Because of the risks to patient safety during clinical trials, the understanding of nonclinical characterization that informs the translation of risks to humans is an area of intense interest among drug developers and regulators. A Continuing Education course was held at the 2022 ACT Annual Meeting in Denver, Colorado, where Dr. Delatte presented on identifying seizure liability in nonclinical studies. In this interview with Dr. Delatte, we discuss nonclinical findings that are indicative of seizure liability and appropriate approaches to characterize risks. Many drug classes carry seizure liability which may trigger preliminary studies, such as kindling studies, to understand risks prior to conducting a repeat-dose toxicology study. A description of premonitory signs of seizure in dose-range findings and repeat-dose toxicology studies is also discussed. Finally, this talk briefly discusses when nonclinical EEG studies are warranted and how to leverage the data from general toxicology studies to design an informative EEG study. Learning objectives of this talk are: (1) set up a drug development program for drugs in a class with seizure liability; (2) identify seizure liability in nonclinical studies; and (3) examine the elements from general toxicology studies that inform the pivotal EEG study.

While in practice he completed a Master's Degree in Public Health at the Medical College of Wisconsin. He began doing side jobs while in practice as a hospital utilization management physician advisor and a medical director for a family planning clinic and an occupational medicine clinic. He worked as the VP for Medical Affairs and then full-time as the Chief Medical Officer for a 300-bed independent nonprofit hospital outside of Chicago Illinois, leaving clinical medicine completely for 4 years. He now works part-time as a co-owner and Medical Director for an urgent care network. In 2017, he began producing a weekly podcast called Physician Nonclinical Careers with John Jurica, where he presents interviews and solo episodes addressing physician burnout and moral injury, unconventional jobs and careers for physicians, and related topics. When he worked as a hospital CMO, there was always a new consulting firm or two working for the organization. And many of the teams included physician consultants. She completed her pediatric residency at the University of Maryland and then went on to practice outpatient pediatrics for 3 years before deciding to make her transition into the pharma industry. She started by working as a contracted part-time employee before deciding to move full-time into a role in Pharma Safety involving vaccines. Her temporary position helped greatly in making the transition from clinical work to Pharma Safety Associate Medical Director. In this week's show, John interviews Dr. Jeffrey Gazzara. Dr. Gazzara attended the Philadelphia College of Osteopathic Medicine for medical school and completed his internship at the Michigan State University College of Osteopathic Medicine. He is a General Practitioner who has been practicing in telemedicine for five and a half years. Over that time, he has been promoted to a number of leadership roles including Medical Director and Supervising Physician Leader. In spite of the progress he made in his career, he has faced challenges during his medical education and professional life as a result of his vision impairment. During our conversation, Dr. Gazzara describes those challenges and some of the steps he has taken to overcome them. And he provides advice to those with similar physical impairments.
