This epsiode includes content related to reproductive organs and sexual health and may not be suitable for all listeners. In this podcast, we welcome Dr. Jose Flores, an expert in sexual and reproductive medicine at New York's Memorial Sloan Kettering Cancer Center. We dive deep into the often challenging but crucial topic of male sexuality, particularly in the context of cancer treatment and graft versus host disease (GVHD).Dr. Flores begins by distinguishing between sexual dysfunction and erectile dysfunction (ED). He emphasizes that sexual dysfunction encompasses various issues beyond just achieving erections, such as ejaculation problems, orgasm issues, decreased sex drive, and changes in penile shape. This broader understanding is essential when discussing sexual health with patients. The prevalence of ED after cancer treatment is notably high, with up to 60-70% of men experiencing it post-chemotherapy, and even higher rates following pelvic surgery, specifically. Low testosterone levels also contribute significantly to sexual dysfunction, particularly after treatments targeting the pelvic area.A critical aspect of addressing ED is breaking the wall of silence around it. Dr. Flores stresses the importance of open communication between patients and healthcare providers. Initiating conversations about sexual health can lead to better assessments and treatments. He outlines the initial steps in diagnosing ED, including patient questionnaires and lab tests, and underscores the need for healthcare providers to proactively ask patients about their sexual health. The discussion then shifts to the impact of GVHD on sexual function. Dr. Flores notes that about 70% of men post-bone marrow or stem cell transplant suffer from low testosterone and ED. Unfortunately, without intervention, these issues do not typically resolve on their own. Patients must seek help to explore treatment options.Dr. Flores explains the ED treatment model, which follows a stepwise approach. The first step includes lifestyle modifications and the use of PDE-5 inhibitors like Viagra and Cialis. If these are ineffective, injection therapy is the next step, followed by mechanical aids like penile pumps and, as a last resort, penile implants.Myths and realities of PDE-5 inhibitors are addressed, clarifying that these medications require proper usage, including an empty stomach for Viagra and sufficient time for absorption for Cialis, along with sexual stimulation to be effective. Proper education on these aspects can significantly enhance their effectiveness.Low testosterone, particularly after cancer treatment, is another major topic. Dr. Flores discusses the complexities of testosterone replacement therapy (TRT), including potential risks like polycythemia, effects on fertility, and the necessity of screening for conditions like sleep apnea and prostate cancer before starting treatment. He advises patients to preserve fertility before undergoing cancer treatments and outlines options for those with compromised fertility post-treatment.The conversation concludes with Dr. Flores urging patients to be proactive about their sexual and reproductive health. He highlights the importance of seeking specialized care and the positive impact it can have on overall quality of life. Throughout the discussion, Dr. Flores's compassionate approach underscores the importance of addressing these sensitive issues openly and effectively.Memorial Sloan Kettering Cancer Center: https://www.mskcc.orgInternational Index of Erectile Dysfunction: https://www.uptodate.comTestosterone Replacement Therapy Information: https://www.urologyhealth.orgThis season is made possible thanks to donations from Syndax and Incyte.https://syndax.com/https://incyte.com/ Follow the nbmtLINK on Instagram! https://www.instagram.com/nbmtlink/

Uvalde, the SBC & Prayers of the People. Mike and Tim are joined by Executive Pastor and Elder of Journey Church Suzie Lind to discuss the events of the past week, including the sexual abuse report from the Southern Baptist Convention and the mass shooting in Uvalde Texas. It is a very raw conversation as everyone tries to sort out postures and directions forward. At the end of the conversation, Suzie, Mike and Tim engage in another 'Prayers of the People.' Lord, we pray for the people and children who have been subjugated and abused by institutions or those in power, but specifically within your church. Lord that you would bring healing and peace and freedom to those who have been hurt and who have been hiding or have been made to feel shameful by their abusers. Father, we pray that there would be Justice and a complete dismantling of the church that hides and protects those that prey on others, that marginalizes and exerts power over. Lord have mercy. Father God, we pray for the families of Uziyah Garcia, Jose Flores, Amerie Jo Garza, Xavier Javier Lopez, Nevaeh Bravo, Alithia Ramirez, Tess Marie Mata, Alexandria Aniya Rubio, Layla Salazar, Makenna Lee Elrod, Jayce Luevanos, Jailah Nicole Silguero, Eliana “Ellie” Garcia, Eliahana Cruz Torres, Annabell Guadalupe Rodriguez, Jacklyn “Jackie” Cazares, Maite Yuleana Rodriguez, Rogelio Torres, Miranda Matthis, Eva Mireles and Irma Garcia. Lord, that you would hear their cries; That you would bring mercy; That you would sit with them; That others would walk in grief with them. Lord we pray that American hearts would be empathetic and would bring about the change necessary to keep our babies safe, to keep our neighbors safe, to keep our elderly safe. God, show us how to be peacemakers and not just peace keepers. As always, we encourage and would love discussion as we pursue. The Daily Still Podcast - Guided Christian Meditations and Devotions
Today we offer a meditation on Grieving a Tragedy. Here in the United States, we have recently experienced the pain and trauma of gun violence in an elementary school in Texas, a grocery store in New York, and a church in California. We can become desensitized and unable to process so much tragic news. Often words alone seem inadequate. We need silence in the presence of a Holy God. So today we make space to grieve and mourn for those who are mourning. We make space to get in touch with our own feelings and emotions. And we make space to consider how God is calling us to action and to show compassion to those hurting. We'll be meditating on a prayer from Psalm 57, TPT. We invite you to prepare a quiet place for this time of meditation. Perhaps light a candle. Don't rush or hurry this time. Linger as long as you need to process your feelings. Have mercy on me, O God…for in you my soul takes refuge. I will take refuge in the shadow of your wings. All gunned down by young men who should have had no access to an assault weapon in the first place. I have no meaningful response to such cruel slaughter. When I am in the most profound mourning, I turn to the composers whose music directly confronts that despair. Today that composer is Gustav Mahler. Since the victims were almost exclusively people of color, today for solace I turn to the extraordinary voices of women of color singing the music of Mahler. Roberta Alexander, Marian Anderson, Carol Brice, Oralia Dominguez, Jessye Norman, Florence Quivar, Shirley Verrett, Lucretia West provide balm for the depths of despair that we are all feeling right now. They are joined on the podium by some of the greatest conductors of the twentieth century: Leonard Bernstein, Christoph von Dohnányi, Bernard Haitink, Paul Kletzki, Hans Knappertsbusch, Erich Leinsdorf, Zubin Mehta, Pierre Monteux, Fritz Reiner, and Frieder Weissmann. This episode is also offered in memory of and in gratitude for the life of the great Lucretia West, who died on 21 February 2022 at the age of 99. Professor Leonardo Vieira, is a Brazilian Jiu-Jitsu black belt under Romero Cavalcanti, being famous for combining one of the most crowd-pleasing games in BJJ with an amazing track record. Leo Vieira was also the co-founder of important teams such as Brasa Clube de Jiu-Jitsu and Checkmat BJJ, becoming in the process one of grappling's most important coaches by developing athletes such as Marcus Almeida ‘Buchecha', Lucas Leite, João Assis, and many others. Leozinho is also one of the Top BJJ Competitors of All Time by BJJ Heroes.Lineage: Carlos Gracie > Helio Gracie > Rolls Gracie > Romero Cavalcanti > Leonardo VieiraMain Achievements:1st Place ADCC (2003, 2005)1st Place IBJJF World Championship (1999)1st Place IBJJF Pan Championship (2002/2004)1st Place CBJJ Brazilian Nationals (1998)2nd Place ADCC (2011/2007)2nd Place IBJJF World Championship (1997)3rd Place IBJJF World Championship (2000)1st Place IBJJF World Championship (1996 brown)Weight Division: Peso Leve (76 kg / 167 lbs)Favorite Position/Technique: Brabo Choke, Guard Passing.Association/Team: CheckmatThe following year, his first as a black belt, Leo fought his way to the final against Marcio Feitosa in the first of many historical duels between the two. This first fight went to the Gracie Barra fighter, Feitosa, but in 1998 when the two met once again in the final, Leonardo was the victor in one of the hardest fights of the event. 1998 was the last time Leonardo won the World Championship, although he did make the final again in 1999 (losing to Royler Gracie) and placed 3rd in 2000.His career's highest point had not been reached however, that came in 2003 when Leonardo won the World's most prestigious Submission Grappling title, the ADCC (Abu Dhabi Combat Club), a gold medal achieved again in 2005.In between this period, Leozinho moved to Sao Paulo and was training under Fabio Gurgel at the Team Alliance HQ there until 2002 when a group of competitors joined up against the head coach Gurgel. Leonardo left the team together with most of the black belts that trained there. The argument (in broad strokes) was over the ‘Mundial‘ (World Championships by IBJJF federation) which occurred a week after the Copa do Mundo (World Cup). As the Copa do Mundo offered a good prize money to its competitors and the Mundial didn't, Leonardo and his teammates felt they should support this organization (CBJJO – Confederação de Jiu Jitsu Olimpico) and compete in it, Gurgel however decided against it for reasons that were never made public. The fighters went on to fight at the ‘Copa' and Gurgel banned them from the gym, and so Leozinho and the others formed another team, ‘Master Team' to honor the gym where this group had started training in the 1980's. The Master Team later became Brasa Clube de Jiu Jitsu.

Jose Flores Interviews Gabriel Almeida a Brazilian jiu-jitsu black belt under Ricardo and Leonardo Vieira, having also worked extensively with Eduardo Flexa while being a representative of the Checkmat team in the sport's international circuit. Almeida earned the reputation of being one of the top competitors of his generation by conquering medals in important events such as the International Brazilian Jiu-Jitsu Federation (IBJJF) World, Pans, and European Championships as well as United Arab Emirates Jiu-Jitsu Federation (UAEJJF) Abu Dhabi Pro.Gabriel Almeida Jiu-JitsuFull Name: Gabriel Salles Muniz AlmeidaNickname: Gabriel is often referenced as “Todd”, the byname started being used by Almeida's first coach, Mr. Eduardo Flexa, right at the start of his BJJ training. The reason for the nickname came from Gabriel beginning his BJJ tuition together with his cousin Raphael, the two sharing very similar features, being mistaken as brothers. Gabriel and Raphael were also raised Catholic, and were known as regular churchgoers; given these characteristics coach Flexa (who was known for handing nicknames to all his students) compared the two cousins to The Simpsons characters, Rod and Todd, sons of Ned Flanders. This week, we have stories about immigrants facing misunderstandings and confusion in their interactions with the criminal justice system. Plus, the popular Hulu series “The Handmaid’s Tale” prompts us to look back at the New England Puritan culture that provided the underpinnings for Margaret Atwood’s dystopia. And we take in nature’s bounty at one of New England’s underappreciated destinations: Long Island Sound. Women dressed like “handmaids” — fertile women forced into sexual slavery in the Hulu drama “The Handmaid’s Tale” — ascend the steps of the New Hampshire statehouse on June 1 to protest a bill that would define a fetus as a person in cases of homicide. Photo by Casey McDermott for NHPR Charged We've been following the stories of immigrants to New England who face obstacles while trying to navigate the American law enforcement and criminal justice system. New Hampshire Public Radio’s Emily Corwin is following the story of Joyce Chance, a refugee from the Congo, who's now out on bail after spending three weeks in jail for charges of child abuse. The state says she's a danger to her community, but many in that community disagree. As Emily reports, cultural misunderstandings and language barriers are getting in the way. Jose Flores speaks to WBUR about the events leading up to his arrest by federal immigration agents with his wife Rosa Benitez. Photo by Jesse Costa for WBUR A felony, or even a lesser conviction, could get a refugee like Chance sent out of the country. But what if you're in the country illegally, and don't have a criminal record? What if the biggest mark against you is an accident you had on the job? Jose Flores, a Honduran immigrant living in Massachusetts, fractured his femur in March when he fell from a ladder at a Boston construction site. After Flores filed for worker’s compensation, a manager at the construction company that employed him asked for a meeting. Just after the meeting, Flores was apprehended by ICE. WBUR's Shannon Dooling has been following his story — and what it tells us about the new realities of undocumented workers during the Trump administration. Don’t Hate on Long Island Sound Meigs Point at Hammonasset Beach State Park is a glacial moraine that you can hike across. The boulders here were pushed south from the Connecticut hills during the last ice age. Photo courtesy of Patrick Lynch We've heard in recent episodes how nitrogen gets into the Connecticut River, and flows downstream into Long Island Sound. There, it feeds algae, which reduces oxygen in the water, killing off fish and other marine life in a condition called hypoxia. Nitrogen pollution, among other factors, has contributed to the Sound’s less-than-glowing reputation among the region’s waterways. The good news is the overall health of Long Island Sound is improving. Since the EPA implemented a nitrogen reduction program in 2001, hypoxia has decreased by 40 percent, and fish and marine mammals that had been absent or struggling for decades are coming back. A beach at Hammonasset Beach State Park is made up entirely of slipper shells. Photo by Ryan Caron King for NEXT Author, photographer and illustrator Patrick Lynch. Photo by Ryan Caron King for NEXT But while oxygen levels improve, sea level rise and warming waters are causing other changes to the Sound that are much more difficult to reverse. Author Patrick Lynch provides a snapshot of these changes, as well as beautiful illustrations of birds, fish and the variety of habitats along the shore in his new Field Guide to Long Island Sound. We caught up with Lynch at beautiful Hammonasset Beach State Park in Madison, Connecticut. The Handmaid’s Tale and the Puritans New England is both the literal and symbolic the setting of “The Handmaid's Tale,” the hit Hulu drama starring Elisabeth Moss as Offred, a young woman forced into sexual slavery in a dystopian near-future. In the series, the United States has been overthrown and replaced by an oppressive new country called Gilead. The patriarchal leaders have used the near total collapse of fertility in the country to institute a kind of theocracy. The handmaids, including Offred, are women who are still fertile. The series is based on the eponymous novel by Margaret Atwood, written in 1985 as a response to the Islamic Revolution in Iran and the rise of the Christian right in the United States. With the launch of the Hulu series in April, many have reflected on the series as a commentary on the current political moment. In recent months, women dressed in handmaids’ red cloaks and white bonnets have shown up at state houses to protest legislation that would restrict abortion or grant person-hood to fetuses. But here at NEXT, we were curious about the story’s connections to New England's Puritan past. In Atwood's new introduction to the book, she writes: “The Republic of Gilead is built on a foundation of the 17th-century Puritan roots that have always lain beneath the modern-day America we thought we knew.” An illustration depicting the execution of Ann Hibbins for witchcraft on Boston Common in 1656. Public hanging is a common punishment in the Handmaid’s Tale. Sketch by F.T. Merril, 1886. Atwood has said that her setting for the book was inspired by the time she spent as a graduate student at Radcliffe College in Cambridge, then the women's counterpart to Harvard. She dedicated the novel to Perry Miller, a Harvard scholar of American Puritans with whom she studied, and also to Mary Webster, a Massachusetts woman who was hanged for witchcraft and survived — and may have been one of Atwood's ancestors. So how closely does the dystopia of “The Handmaid's Tale” reflect the utopia the Puritans attempted to create on these shores? For answers, we turn to two historians who have studied women, sexuality, and religion in 17th Century New England. Rebecca Tannenbaum is a Senior Lecturer of History at Yale, and Kathy Cooke is a professor of History at Quinnipiac University. About NEXT NEXT is produced at WNPR.
