No one could forget the confronting vision of the Grenfell apartment building in London going up in flames in 2017, when 72 people lost their lives. It emerged that aluminium composite panelling, used as part of the building's façade, enabled the original fire to spread rapidly around the apartment block because it was highly combustible. The discovery sparked a worldwide audit of buildings fitted with several types of this cladding, including here in WA. In March 2019, the WA Health Department identified a number of its assets were fitted with the cladding and therefore posed a major fire risk. "Core samples of the ACP cladding were taken for laboratory testing to confirm their material type. Immediate action has been taken to ensure the ongoing safety of staff and patients within the hospital," a government statement said afterwards. "Work will now commence on mitigation planning and assessing replacement options. The department will follow due process to determine the scope of remediation works required." Opposition health spokesperson Libby Mettam asked in Parliament yesterday if the public could be confident that a tragedy such as Grenfell will not happen at buildings in WA, and was accused of scaremongering and mis-truths by Health Minister Amber-Jade Sanderson. "This is a government that has consistently been slow to act, particularly when it comes to the Health portfolio," Mettam told 6PR Mornings host Gary Adshead.  "Here we are three years on [from the audit], and the Minister can't even answer a simple question about whether this work is in hand, despite having the funds to do so.  "There are so many questions to ask [the Health Minister] ... we've got the highest level of ambulance ramping on record, we have a health system on its knees, we have a demoralised workforce... it is unfortunate when it comes to answers, this minister and this government give no confidence they have these issues at hand or take patient safety seriously."See omnystudio.com/listener for privacy information.

The fight for higher wages in the public sector continues, particularly for nurses and midwives. Australian Nursing Federation Secretary WA Janet Reah said some of them are holding a stop-work rally at 2pm at Fiona Stanley Hospital today. "We're expecting hundreds of nurses today at the rally, it's the first in a series of rallies leading up to a mass meeting of nurses on the 12th of October," she told Liam Bartlett on 6PR Mornings. Staffing levels at Perth Children's Hospital are again in the spotlight after a mother claims her nine-month-old baby was sent home with a fever and struggled to stay conscious. Jasmyn Lawrence told Mornings host Liam Bartlett her daughter Evelyn was unable to keep her eyes open and was basically in an unconscious state after PCH failed to detect she had a potentially deadly virus in their emergency department last week. After waiting for six hours at the hospital on August 26, Jasmyn and her family were told their baby girl had bronchiolitis and they were sent home, but they then took the girl to Fiona Stanley Hospital. A man in his 60s is in intensive care in Fiona Stanley Hospital after what appears to have been a medication bungle. It is believed Darshan Arora was given 30ml of blood pressure medication on Monday morning, six times the normal dose. His insulin medication was also reported halved. It sent Arora into renal failure and he now needs dialysis. Family friend Philip Couper told Liam Bartlett on 6PR Mornings the mistake should have been caught. We recorded the September Journal Club during Healthcare Simulation Week – a chance to reflect on the amazing work of the healthcare simulation community.   Our first paper showcased the work of the STEPs (Simulation To Enhance Patient Safety) team at Fiona Stanley Hospital in Perth. Rory Trawber et al. write about Improving Simulation Accessibility in a Hospital Setting - Implementing a Simulation Consultation Service. They give us a practical guide to leveraging the impact of a small, skilled simulation delivery team through a formalised consultation service for clinical units seeking to use translational simulation for their quality and safety challenges in clinical care. Rory makes an ( audio) appearance in the podcast and describes the importance of using a quality ansafety based reporting tool, based on the work of Mel Barlow and colleagues on a documentation framework for healthcare simulation quality improvement activities.  As a nice example of using simulation as a test bed for planned clinical process improvements, Ben-Haddout and a team from Roeun, France offer us A Cognitive Aid Improves Adherence to Guidelines for Critical Endotracheal Intubation in the Resuscitation Room. We were impressed with the opportunities afforded by the in situ simulation setting, and the interesting ways to look at cognitive aids.  We discussed a paper on Online-synchronized clinical simulation: an efficient teaching-learning option for the COVID-19 pandemic time and: beyond. This is one of many articles currently being published that seek to tease out the benefits and drawbacks of online scenario-based activities for learning engagement and impact.  We covered a scoping review on Healthcare Provider Stress and Virtual Reality Simulation, and concluded that - despite a small literature base at present – this is a space to watch for those seeing to improve healthcare providers ability to mange stress in the clinical workplace. A highlight was a really useful table listing various tools used to measure healthcare professionals stress – in either simulation settings or the real world.    Happy Listening

A man who presented at Fiona Stanley Hospital in Perth and posted on social media he had COVID-19 has been revealed as a historical case.Western Australia health authorities confirmed late on Monday night investigations had determined the man’s infection was historical.It means the man was not infectious in Australia and at no time posed a public health risk, WA Health said in a statement.“Members of the public who attended Fiona Stanley Hospital and the same entertainment venues as the individual can be reassured he did not pose a public health risk,” WA Health said.Authorities believe the man contracted the virus in Mexico in December last year.The man returned to Australia from overseas travel and his completed two weeks hotel quarantine in NSW before returning to Western Australia.“Initial PCR tests were very weak, in keeping with him being non-infectious and having infection some time ago,” WA Health said.“Subsequent test results received (on Monday) confirm the person is now PCR negative, but serology positive.“This provides confirmation that the case was historical and non-infectious in WA.”The man has now been released from hotel quarantine in Perth.The suspected case, which turned out to be historical, caused quite a stir in Perth after the man took to Facebook on Sunday to urge people he had been in contact with to get tested.It comes as Perth and two neighbouring regions in the state’s south enter their second day of a snap five-day lockdown after a separate case was confirmed on Sunday in a security guard working at the Four Points by Sheraton quarantine hotel in the CBD.Health officials suspect the man, aged in his 20s, may have contracted the mutant UK strain from an active case at the hotel.He visited 18 locations over several days before being diagnosed, sparking fears he may have spread the deadly illness widely and prompting a massive wave of community testing.No new cases were announced on Monday from 3171 tests, with the state’s total remaining at 903.People in the Perth metropolitan area, Peel and South West regions will remain locked down until at least 6pm on Friday, with Premier Mark McGowan warning the easing of restrictions will likely be gradual.

Professor Fiona Wood AM is one of Australia’s most innovative and respected surgeons and researchers. A highly skilled plastic and reconstructive surgeon and world leading burns specialist, she has pioneered research and technology development in burns medicine. It was an absolute privilege to be able to discuss her inspirational journey and the approach to life that has enabled her to make a great contribution to the world. We had a connection through our involvements with gymnastics, so it is also great to speak to her about her passion for sport and trampoline gymnastics. Professor Wood’s positions include Director of the Burns Service of Western Australia (BSWA), Consultant Plastic Surgeon at Fiona Stanley Hospital (previously at Royal Perth Hospital) and Princess Margaret Hospital for Children and Winthrop Professor (Burns Injury Research Unit) at the School of Surgery, Faculty of Medicine UWA. She was awarded Member of the Order of Australia in 2003, the Australian Medical Association’s ‘Contribution to Medicine’ award in 2003, the 2003 and 2004 West Australian of the Year and 2005 Australian of the Year. She was voted Australia’s Most Trusted Person for six successive years (2005-2010) and has been recognised as an Australian Living Treasure. Many parents are impacted by children with peanut allergy. There has been a general rise in allergies in Australia over the last 20 years. It has changed the way we live. Schools have put bans on peanut butter sandwiches to avoid cross-contamination between children. Parents have been seen giving the first dose of peanut to their children in hospital car parks or outside medical clinics. This podcast looks at who is at risk of peanut allergy and the best tests available along with prevention strategies that parents can consider. Dr. Michael O'Sullivan is a leading consultant immunologist in Australia who works at the Perth Children's Hospital and Fiona Stanley Hospital. This week’s guest, Australian intensivist Dr Ed Litton, truly amazes and inspires me. Despite having a full-time clinical and research career, and a young family, Ed pursues his passion for adventure mostly through ultra-endurance exercise. Many intensivists run, swim, cycle or do other sorts of vigorous exercise in their spare time. Some even run marathons, swim regularly with a squad or cycle long distances to and from work. Some do all 3 by competing in triathlon events. Yet not too many intensivists take on ironman triathlons like Ed does. And how many cycle across Australia from Sydney to Perth as he did a couple of years ago? Ed uses adventure and exercise to keep refreshed for his busy medical career. So to me this podcast conversation is a real treat. Hearing about this massive cross-continent bike ride, the recent family cycling trip across the New Zealand Alps he and his wife did with their 2 young children, and his love for other physical pursuits like surfing and climbing, is both educational and inspiring. Ed Litton is a Staff Specialist in Intensive Care Medicine and Director of ICU Research at Fiona Stanley Hospital in Perth, Australia, and a Clinical Research Fellow in the ICU at St John of God Hospital in Subiaco. He is the recipient of a National Health and Medical Research Council Early Career Fellowship and is a Clinical Senior Lecturer at the University of Western Australia. Ed is a member of the executive committee of the Australia and New Zealand Intensive Care Society Centre for Outcomes Research Evaluation where he is clinical director of the Critical Care Resources Registry. He’s received over $6M in research support and published over 60 manuscripts in the peer reviewed literature. I have admired Ed and his thoughtful approach to intensive care and to life for several years now. Apart from his approach to adventure and exercise he has many valuable perspectives on his bedside approach including: What fascinates him about Intensive Care Some of the things he learnt from his key mentors The importance of reassurance for patients Mixing patient-centred and digital information The 2 phase ward round at his ICU The internal pressure he felt as a new consultant to make fast decisions Creating the right atmosphere in communicating with colleagues His tendency to ruminate about his patients Fatigue during ward rounds His thoughts on sleep How he uses meditation The value of reading books The difficulty in trying to be a jack of all trades in our careers. THOMAS BAKER ORATION "Striving for Excellence in Health Care" Professor Fiona Wood has been a burns surgeon and researcher for the past 20 years and is Director of the Burns Service of Western Australia (BSWA). She is a Consultant Plastic Surgeon at Fiona Stanley Hospital (previously at Royal Perth Hospital) and Princess Margaret Hospital for Children, co-founder of the first skin cell laboratory in WA, Winthrop Professor in the School of Surgery at The University of Western Australia, and co-founder of the Fiona Wood Foundation (formerly The McComb Foundation). Professor Fiona Wood's dedication to improving outcomes for burns patients and expanding the knowledge of wound healing began in 1991 when she became trained as West Australia's first female plastic surgeon. Professor Wood's ability to lead a team and direct innovation for future clinical care was recognised as she quickly became a leader in her field becoming Director of the Burns Service of Western Australia at an early point in her career. In this position Professor Wood has led the Burns Service of Western Australia to be recognised internationally as a leader in burns care. Professor Wood's greatest contribution and enduring legacy is her work pioneering the innovative ‘spray-on skin' technique (Recell), which greatly reduces permanent scarring in burns victims. Professor Wood patented her method in 1993 and today the technique is used worldwide. In October 2002, Fiona was propelled into the media spotlight when the largest proportion of survivors from the 2002 Bali bombings arrived at Royal Perth Hospital. She led a team working to save 28 patients suffering from between 2 and 92 per cent body burns, deadly infections and delayed shock. Fiona was named a Member of the Order of Australia (AM) in 2003. In 2005 she won the Western Australia Citizen of the Year award for her contribution to Medicine in the field of burns research. That same year her contribution to burns care was recognised through Australia's highest accolade when she was named Australian of the Year for 2005 by Australian Prime Minister John Howard at a ceremony in Canberra to mark Australia Day. She is an Australian Living Treasure. Professor Wood was voted the most-trusted Australian in a Reader's Digest poll for six successive years from 2005 to 2010. Fiona is married to fellow surgeon Tony Kierath and is mother to four boys and two girls.

The emergency pager goes off - code blue medical on the gynaecology surgical ward. As you enter the ward a frantic grim faced nurse waves you into the side room where you discover an elderly woman who is obviously cyanosed, not really conscious and struggling vigorously to breath. She has a hudson mask on her face and there is an obvious tracheostomy tube protruding from her neck. One of the nurses tell you she had a laparotomy for ovarian cancer earlier that day and that she has a long term tracheostomy after having had a throat cancer resected 5 years ago..... Hi Everyone, This week I am joined by another anaesthetist, Dr James Anderson to discuss the perioperative management of tracheostomies. Before seeing the light and jumping the drapes to become an anaesthetist James spent a year working as an ENT registrar and now he has an interest in teaching both perioperative and crisis management of patients with tracheostomies. He helps run tracheostomy crisis management courses at Fiona Stanley Hospital and helped author a recent article in the famous ANZCA blue book (see the link below): Unfortunately patients with surgical airways and tracheostomies are not confined to just the ENT wards - they all often have associated comorbidities and medical problems which mean they can be encountered anywhere in the hospital or healthcare settings. Whether we like it or not we could all get called to deal with an emergency - and so we all need to have some basic understanding of tracheostomies and skills in dealing with any crises or problems which could occur! In this episode we discuss the perioperative management and emergency management of these airways. You phone goes off - you roll over it is 2am - when you pick up it is a theatre nurse calling to ask if you can urgently come to the hospital immediately - the team are too busy to talk to you. The nurse tells you a woman has just arrived via ambulance from another small peripheral hospital. She had an emergency caesarean about 6 hours ago and hasn't stopped bleeding since. She has had 4 units of red cells and 4-5 litres of saline but nothing else. When you arrive 15min later surgery is underway but the surgical team tell you "everything we touch is bleeding" and you notice that she is even bleeding from the skin around her iv...... The anaesthetic registrar turns to you and says - "lets give her the fibrinogen concentrate -  we need to get on top of this coagulopathy right now!......" (*Fictitious case example) Hi Everyone, This week we are joined by a colleague and a great friend of mine Dr Hamish Mace, one of the co-authors of an article in the 2017 edition of Australasian Anaesthesia (aka the Blue Book) entitled "Fibrinogen concentrate for the treatment of acquired hypofibrinogenaemia". Hamish is a consultant anaesthetist working in Western Australia - he works at Fiona Stanley Hospital a large tertiary centre in metropolitan Perth - but has also worked in the past in remote regional centres in WA, in retrieval medicine for the RFDS (Royal Flying Doctor Service) and spent time on fellowship in Toronto Canada. Hamish co-ordinates the preoperative anaemia correction service, is on the hospital transfusion committee, has a strong interest in many aspects of patient blood management. In the interview we briefly discuss the role of fibrinogen in the acquired coagulopathy that often develops during major haemorrhage, the historical treatments used (FFP, cryoprecipitate), the history of fibrinogen concentrate and where we think fibrinogen concentrate might fit into the current management of major haemorrhage - both in tertiary but also remote, regional and retrieval situations. Bedrija Parsons is a Clinical Neuropsychologist who currently works at the new Fiona Stanley Hospital in Perth, Western Australia. Bedrija's role is diverse and includes covering the Mental Health Assessment Unit, Mental Health Consultation Liaison, Youth Mental Health and the Mother and Baby Unit. Bedrija completed her Master of Clinical Neuropsychology at Latrobe University, Melbourne Australia in 2012. She has since worked at the Royal Talbot Rehabilitation Centre's Acquired Brain Injury Unit, the Royal Melbourne Hospital including working with inpatients and outpatients, and at Eastern Health in a community based rehabilitation program. Bedrija is an early career Clinical Neuropsychologist currently in her 4th year of working. She discussed with me how the role of Clinical Neurospychology is still fairly new in mental health settings, and how much of her current role at Fiona Stanley Hospital has been about flying the flag for Neuropsychology and educating other practitioners about her role and purpose.
