Vi har nu de officielle børspapirer for SpaceX, der børsnoteres den 12. juni på teknologibørsen Nasdaq i New York i verdenshistoriens største notering med en samlet markedsværdi på op mod 2.000 milliarder dollars. Vi kigger også på techgiganten Nvidia, der netop har aflagt et regnskab, der nok engang slog analytikernes forventninger. I Danmark går det også fremragende: Nye bnp-tal viser, at dansk økonomi er den hurtigst voksende i Europa og slår både Kina og USA i vækst. Men kritikerne siger, at BNP-tallene mudres af regnskabsteknik og reelt ikke kan bruges til tage temperaturen på dansk økonomi. Hvorfor? Og hvis ikke bnp giver mening, hvad skal politikerne så stile efter og økonomerne holde øje med? Ti lsidst snakker vi om de knap 3.700 aktiehandler, som præsident Trump har foretaget i første kvartal af 2026. Men skatten risikerer også at få konsekvenser langt ud over det umiddelbare provenu. Chính quyền Trump hôm 20/04/2026 bắt đầu tiếp nhận đơn của các công ty nhập khẩu Hoa Kỳ đòi hoàn trả 166 tỷ đô la đã phải nộp cho chính phủ từ một năm qua. Tối Cao Pháp Viện Mỹ bác bỏ thẩm quyền của tổng thống áp đặt các các hàng rào thuế quan với toàn thế giới nhưng chiến tranh thương mại do Nhà Trắng khai mào chưa tới hồi kết. Washington thất bại trên ba mục tiêu : kiềm tỏa xuất khẩu Trung Quốc, thu hẹp thâm hụt mậu dịch của Hoa Kỳ và đưa nước Mỹ trở lại trung tâm bàn cờ công nghiệp thế giới. Thay đổi duy nhất là trật tự thương mại quốc tế đã rẽ sang một khúc quanh mới.     Chuyển biến trong một năm, từ Ngày Giải Phóng nước Mỹ 02/04/2025. Đúng hai tháng sau phán quyết ngày 20/02/2026 của Tối Cao Pháp Viện, các công ty nhập khẩu của Mỹ bắt đầu có thể đòi Sở Quan Thuế và Biên Phòng (CBP) hoàn trả 166 tỷ đô la mà họ đã phải nộp từ khi ông Donald Trump ban hành biện pháp thuế đối ứng 02/04/2025. Hiện tại, hơn 330.000 công ty nhập khẩu của Mỹ, đã có 56.500 đòi CBP bồi hoàn. Thủ tục đòi lại tiền thuế cho các doanh nghiệp Mỹ sẽ không đơn giản. Nhìn từ góc độ của người tiêu dùng, câu hỏi quan trọng nhất là liệu các công ty nhập khẩu của Mỹ có hoàn trả lại cho các khách hàng khoản thuế hải quan « không hợp pháp » mà họ đã thu vào từ một năm nay hay không. Trong Vườn Hồng của Nhà Trắng cách nay hơn một năm tổng thống Donald Trump tuyên bố : « Mồng 2 tháng Tư năm 2025 sẽ mãi mãi được khắc khi trong ký ức như một ngày công nghiệp của Hoa Kỳ hồi sinh, ngày để chinh phục lại vận mệnh của nước Mỹ và là ngày đòi lại tài sản cho Hoa Kỳ ». Lập tức thuế hải quan đánh vào hàng nhập khẩu vào Mỹ tăng 10 rồi 50 %. Trong đợt đầu tiên này, Việt Nam bị lãnh mức thuế 46 %. Cam Bốt và Lào là 49 và 47 %. Cả thế giới, từ Liên Hiệp Châu Âu, Anh Quốc, Thụy Sĩ … đến Nhật Bản, Hàn Quốc hối hả đến Washington đàm phán với hy vọng thuyết phục được chính quyền Trump về thiện chí thu hẹp thặng dư mậu dịch với Mỹ. Các nền kinh tế Đông Nam Á không là một ngoại lệ. Sau nhiều vòng đàm phán, tháng 7/2025 Việt Nam cuối cùng đạt được thỏa thuận để hàng xuất khẩu sang Mỹ chịu mức thuế 20 % (thuế 40 % nếu là hàng hóa trung chuyển). Liên Hiệp Châu Âu, Nhật Bản và Hàn Quốc bằng lòng với mức thuế hải quan 15 %. Ấn Độ, Brazil, là những nhà đàm phán không dễ vâng lời. Riêng Trung Quốc là một ngoại lệ : Chẳng những Bắc Kinh không vội vàng thương lượng với Nhà Trắng mà còn tung đòn đáp trả, chuyển từ thế thủ sang thế tấn công đúng nghĩa « ăn miếng trả miếng » để rồi đôi bên tuyên bố « tạm ngừng ». Tháng 10/2025 tổng thống Donald Trump và chủ tịch Tập Cận Bình gặp nhau tại Hàn Quốc và trên nguyên tắc trong hai tuần nữa (tháng 5/2026) nguyên thủ Mỹ công du Bắc Kinh. Thương mại vẫn là trọng tâm thượng đỉnh Mỹ-Trung Cuộc chiến còn dài Tháng 2/2026, gần một năm « Giải Phóng nước Mỹ » với đòn thuế đối ứng, tổng thống Trump phẫn nộ trước phán quyết của Tối Cao Pháp Viện. Ông cho rằng phán quyết này cho phép các đối tác nước ngoài tiếp tục « bòn rút » tài nguyên của Hoa Kỳ. Vào lúc mà các công ty nhập khẩu Mỹ bắt đầu đòi « hoàn trả » 166 tỷ đô la đã nộp « oan uổng » cho chính phủ, thì cũng ông Trump cảnh báo, ông sẽ « để bụng » những doanh nghiệp nào dám đòi được hoàn trả thuế hải quan. Những công ty đó, không « thông minh » và không « yêu nước ». Nhà kinh tế Denis Ferrand giám đốc công ty tư vấn Rexecode-Paris, trên đài phát thanh France Info hôm 21/02/2026 ghi nhận : « Đối với Donald Trump phán quyết của Tối Cao Pháp Viện là một vố đau, bởi vì ông luôn coi thuế quan là cột trụ trong chính sách của mình. Ngay nhiệm kỳ đầu tổng thống Mỹ đã sử dụng lá bài này, nhưng chỉ là để đối đầu với Trung Quốc. Rồi Washington và Bắc Kinh đã liên tục đàm phán … Trong nhiệm kỳ 2 Trump lại đem chiêu bài này ra nhưng là để áp đặt với toàn thế giới. Ông coi đây là một công cụ để đạt được những mục tiêu đã đề ra, thậm chí là công cụ để gây áp lực, để đòi các đối tác thương mại của Hoa Kỳ đầu tư trên đất Mỹ, làm sống lại nền công nghiệp của Hoa Kỳ, để rồi Mỹ lại đóng vai trò đầu tàu dẫn dắt thế giới ». Nhưng tổng thống Mỹ không chịu buông tay. Ngay sau phán quyết của Tối Cao Pháp Viện, Donald Trump lập tức ban thêm một đợt thuế mới 15 % (ban đầu là 10 %) có hiệu lực trong vòng 150 ngày. Cho đến tuần này, lãnh đạo Nhà Trắng vẫn dùng đòn thuế quan, công cụ ông yêu thích nhất, để đòi phạt các đối tác không ủng hộ Mỹ trong cuộc chiến chống Iran, không giúp Hoa Kỳ mở cửa eo biển Hormuz đang bị Iran phong tỏa. Cần lưu ý rằng phán quyết hôm 20/02/2026 của Tối Cao Pháp Viện Mỹ không liên quan đến một số trường hợp như thuế đánh vào nhôm, thép (vẫn là 50 %) và xe hơi (vẫn là 25 %) của nước ngoài bán sang thị trường Mỹ. Nói cách khác, cuộc chiến thuế quan Donald Trump khởi xướng chưa tới hồi kết. Isabelle Méjean chuyên gia về thương mại quốc tế trường Khoa Học Chính Trị Paris giải thích trên kênh truyền hình Pháp Public Sénat :  « Người tiêu dùng Mỹ trả giá cho các đòn thuế quan đó. Hơn nữa chính sách này đã không cho phép khởi động lại đầu tư vào các nhà máy công nghiệp ở Hoa Kỳ như từng hứa hẹn, và cũng không đem lại việc làm cho người lao động. Trong năm qua kinh tế và chứng khoán phát triển nhờ những khoản đầu tư rất lớn vào công nghệ trí tạo thông minh (…) Cả thế giới lập tức lao vào một cuộc chạy đua với thời gian để đàm phán với Washington, mong được giảm mức thuế khi xuất khẩu hàng sang Hoa Kỳ, để tránh phải trực diện lao vào một cuộc đối với chính quyền Trump. Do vậy có thể nói về phương diện ngoại giao, Donald Trump đã ghi được một bàn thắng khi củng cố vị trí của nước Mỹ ». Bộ Tài Chính Mỹ thu tiền của dân Mỹ Kinh tế trưởng của ngân hàng Pháp BNP, Nathalie Benatia phân tích thêm :   « Rất rõ ràng là người tiêu dùng Mỹ gánh chịu các khoản chi phí phụ trội. Thành công duy nhất là khi tăng thuế hải quan, bởi đây là một hình thức thu thuế, nên tiền mà chính quyền liên bang thu vào được nhiều hơn, ước tính là đã thu thêm được khoảng 264 tỷ đô la trong năm 2025. Để so sánh, hồi 2024 khoản thu vào từ thuế hải quan này chỉ là 80 tỷ đô la. Nhưng chỉ cần đặt câu hỏi, ai đã chuyển số tiền 264 tỷ đó vào công quỹ Nhà nước thì câu trả lời hiển nhiên là người dân Mỹ, là người tiêu dùng ở Mỹ. Có điều với quyết định của Tòa Án Tối Cao hôm 20/02/2026 thì 2 phần ba khoản bội thu 264 tỷ vừa nói, có thể sẽ phải hoàn trả lại cho các công ty nhập khẩu của Hoa Kỳ, nếu có yêu cầu, để rồi số này phải trả lại cho khách hàng … » Theo nghiên cứu của viện nghiên cứu của Đức, Kiel Institute, 96 % thuế hải quan mà tổng thống Trump ban hành là do các công ty và người tiêu dùng ở Mỹ gánh chịu, 4 % còn lại đè nặng lên các nhà sản xuất và xuất khẩu muốn chen chân vào thị trường Hoa Kỳ. Báo cáo của đại học Yale chỉ ra rằng trong năm 2025, trung bình mỗi hộ gia đình ở Mỹ phải chi thêm 1.751 đô la vì các biện pháp thuế quan nói trên. Điều đó cũng có nghĩa là vì loại thuế này mà lạm phát ở Mỹ tăng lên thêm trong năm qua. Giá tiền ga tăng 11 % (trước khi nổ ra chiến tranh Trung Đông), tiền điện tăng thêm 6,7 %. Thất bại 3 KHÔNG  Về câu hỏi thuế đối ứng của Mỹ có cho phép Hoa Kỳ thu hẹp nhập siêu với các bạn hàng hay không thì câu trả lời là không. Báo cáo Brookings Papers on Economic Activity công bố tháng 3/2026 kết luận « chính sách thuế quan của ông Trump khiến 2025 trở thành một năm đặc biệt có nhiều biến động về kinh tế ». Trung Quốc cũng như châu Âu không phải là những bên gánh chịu nặng nhất thuế hải quan của ông Trump. Theo Cục Dự Trữ Liên Bang Hoa Kỳ, thâm hụt mậu dịch của Mỹ trong năm 2025 tăng thêm 2 % so với hồi 2024 và đạt ngưỡng kỷ lục 1.240 tỷ đô la. Trong khi đó ngành xuất khẩu của Trung Quốc vẫn an toàn : xuất khẩu sang Mỹ giảm 20 % năm ngoái nhưng lại tăng mạnh tại châu Âu (+28 %)  và nhất là sang các nước Đông Nam Á (+29 %). Thuế hải quan có thôi thúc các nhà sản xuất quốc tế ồ ạt đầu tư vào Mỹ để phát triển mạng lưới công nghiệp của Hoa Kỳ hay không, câu trả lời là không. Trong năm 2025 công nghiệp Mỹ sa thải 100.000 nhân viên. 15 hiệp định tự do mậu dịch, Mỹ đứng ngoài  Tuần báo kinh tế Anh The Economist ghi nhận, chính sách thương mại của Hoa Kỳ mang nặng màu sắc bảo hộ nói trên là động lực thúc đẩy cộng đồng quốc tế ký kết 15 thỏa thuận thương mại quan trọng trong năm vừa qua. Từ tháng 5 đến tháng 12/2025 tổng trao đổi mậu dịch giữa Anh, Canada Liên Âu, Nhật Bản, Hàn Quốc và Thụy Sĩ tăng thêm 5 %. Theo thống kê của hải quan Trung Quốc công bố tháng 1/2026, trong năm 2025, thặng dư mậu dịch nước này lên tới 1.189 tỷ đô la - tương đương với GDP của quốc vương dầu hỏa Ả Rập Xê Út, tăng thêm 20 % so với tài khóa 2024. Ông Trump thất bại trong việc kiềm tỏa các nhà xuất khẩu của đối thủ thương mại lớn nhất và nguy hiểm nhất trong mắt Hoa Kỳ.  Có điều trong một năm qua tổng thống Hoa Kỳ đã làm đảo lộn trật tự thế giới trên bàn cờ thương mại, như Erwan Benazet của báo Le Parisien ghi nhận : « Chúng ta đang đứng trước một chuyển biến, thay đổi cục diện thương mại toàn cầu, một bước ngoặt trong giao thương quốc tế từ hơn 50 năm qua. Trong thời gian gần đây, cụm từ thường được nhắc đến nhiều nhất là ‘chủ quyền', là ‘tự chủ'. Các nền kinh tế càng lúc càng tìm cách đa dạng hóa các nguồn cung ứng và tìm kiếm thêm những thị trường thay thế ». Công cụ thuế quan của chính quyền Donald Trump không phân biệt « các nước bạn và thù » mà điển hình là Canada hay Mêhicô sát cạnh đều lãnh búa rìu của Nhà Trắng. Trong tinh thần đó hơn bao giờ hết, Canada đã thắt chặt thêm quan hệ với Liên Âu. Du khách Canada thăm Mỹ vắng dần. Theo hiệp hội du lịch USA Travel Association, ngành lịch Mỹ thất thu 4 tỷ đô la năm ngoái do lượng du khách Canada giảm 20 %.

At the 19th World Congress of the Societies of Anaesthesiologists (WCSA 2026) in Marrakesh, TopMedTalk welcomes Desiree Chappell back alongside Kate Leslie to interview Professor Palesa Motshabi-Chakane, Associate Professor and Head of Anaesthesiology at the University of the Witwatersrand and her colleague, Dr Mullai Slave a PhD candidate at University of the Witwatersrand. They discuss a study of 629 women undergoing cesarean section with spinal anesthesia at Chris Hani Baragwanath Hospital, where 23–33% are HIV positive. Using standard monitoring plus BD APM noninvasive continuous hemodynamic monitoring, they compared HIV-positive and HIV-negative patients and found higher hypotension incidence in HIV-positive women (68% vs 64%), with lower heart rate and lower cardiac index over 60 minutes. Additional testing included echocardiography with speckle tracking, pro-BNP, and autonomic assessments, with HIV-positive patients showing stiffer ventricles; Apgar scores did not differ. They discuss replication, multicenter research, and potential machine-learning tools to predict hypotension risk, and describe BD Advanced Patient Monitoring grant support enabling equipment, sensors, staffing, and training (~200 staff) to complete data collection in about six months. Contributor: Aaron Lessen, MD Educational Pearls: Patients with pulmonary embolism (PE) are divided into three risk categories Low risk (non-massive PE): patients are stable Treatment: prescribe anticoagulants and discharge home Intermediate risk (submassive PE): patients are stable but display evidence of clot burden such as elevated troponin, elevated BNP, and/or right heart strain Treatment is controversial High risk (massive PE): patients are unstable with hypotension, hypoxia, and/or respiratory distress Treatment: IV thrombolysis to prevent decompensation A recent randomized controlled trial evaluated treatment of intermediate risk PE patients Patients were randomized to receive either thrombectomy with anticoagulation or anticoagulation alone The primary outcome evaluated changes in right ventricular enlargement at 48 hours A controversial primary outcome because it does not speak to mortality or incidence of other necessary aggressive interventions Low clinical significance The study found that thrombectomy significantly reduced right ventricular enlargement faster than anticoagulation alone. Episode 215: Meth-associated HFrEF.   Abishak and Zat (medical students) explain the cardiotoxic effect of methamphetamine and the diagnosis and treatment of heart failure with reduced ejection fraction (HFrEF). Dr. Arreaza adds insight into the reversibility of meth-associated HFrEF.   Written by Abishak Govindarajan, MSIV and Zat Akbar Shaw. American University of the Caribbean. Edits and comments by Hector Arreaza, MD. Welcome Dr. Arreaza: Welcome to Rio Bravo qWeek. My name is Hector Arreaza, family physician, faculty and associate program director of the Clinica Sierra Vista/Rio Bravo Family Medicine Residency Program. Today we will explore heart failure with reduced ejection fraction, a high-yield and clinically relevant topic in medicine. We will discuss the role of methamphetamine use in the development of HFrEF. This is a pressing issue because about 0.8% of the population 12 and older in the US reported using methamphetamine within the past 12 months in 2024 (National Survey on Drug Use and Health, NSDUH), that's about ≈2.4 million people!We are joined by two aspiring physicians who will help explore this topic. By the way, we will refer to methamphetamine in this episode as “meth”. [Abishak and Akbar introduce themselves] Abishak: [Introduce yourself] The role of meth in HFrEF Dr. Arreaza: Meth is a growing problem in many places, including Bakersfield, where we live. Meth is also known as Meth Crystal, Poor man's cocaine, Ice, Glass, Crank, Speed, Chalk, and Tina. How does meth contribute to the development of HFrEF? Abishak: So, first, let's understand how methamphetamine works. It has a chemical structure similar to dopamine and norepinephrine, and it gets taken up through the neuron transporter proteins. Once it enters the synaptic vesicles (storage sacs for neurotransmitters), it displaces and forces the release of large amounts of dopamine, norepinephrine, and serotonin into the synapse (the space between neurons). Additionally, meth blocks the reuptake of those neurotransmitters into the neuron, ensuring they remain in the synapse for a prolonged period. All this causes a downstream effect of increased sympathetic pathways in the body. Diagnosis Dr. Arreaza: The diagnosis starts with collecting a good history and performing a complete physical exam, and then we confirm with an echocardiogram.  Abishak: Yes, diagnosis requires both symptoms consistent with heart failure and objective evidence of reduced ejection fraction. Echocardiography is the primary diagnostic tool. We also measure BNP. In certain cases, cardiac MRI is used to evaluate myocardial fibrosis and exclude infiltrative or inflammatory etiologies. Coronary angiography may be performed if ischemic disease is suspected.Guideline-Directed Medical Therapy Dr. Arreaza: GDMT Guideline-Directed Medical Therapy started around 1987 when ACE inhibitors were proven to improve mortality in patients with heart failure. Then, during the following decades, many medications have been added to GDMT. Until around 2019–2022 we came out with the main 4 groups of medications that we know as GDMT. Let's talk about GDMT. Akbar: There are four core pillars in GDMT. First, an angiotensin receptor-neprilysin inhibitor, such as sacubitril with valsartan (Entresto), is preferred over ACE inhibitors when tolerated. This medication reduces mortality and heart failure hospitalizations. Second, evidence-based beta blockers including carvedilol, metoprolol succinate, or bisoprolol are used to reduce sympathetic overactivity and improve ventricular remodeling. Third, mineralocorticoid receptor antagonists such as spironolactone or eplerenone reduce fibrosis and improve survival. The Fourth pillar is SGLT2 inhibitors such as dapagliflozin or empagliflozin, which provide significant reductions in heart failure hospitalizations and cardiovascular mortality, regardless of diabetes status. Abishak: Other main parts of the treatment are diuretics, which are used for symptom control but do not reduce long-term mortality. Dr. Arreaza: As a recap: The current 4 pillars of GDMT are: ARNI/ACEi + β-blocker + MRA + SGLT2i)  Beta Blocker Considerations Dr. Arreaza: Sometimes we may be concerned about using beta blockers in active meth users. What did you read about it? Abishak: Historically, there was concern about unopposed alpha stimulation. However, in chronic heart failure, beta blockers remain essential. Carvedilol is often favored because it provides both alpha and beta blockade. Careful titration and close monitoring are critical.Reversibility and Remodeling Dr. Arreaza: Regarding meth-associated HFrEF, we have good news for meth users. Tell us about how reversible this condition is.  Akbar: It can be reversible. One of the most important aspects of this condition is that significant reverse remodeling may occur if the patient stops methamphetamine use and adheres to medical therapy. The Left ventricular ejection fraction can improve substantially and, in some cases, normalize. On the other end of the spectrum, continued meth use may lead to progressive fibrosis, ventricular dilation, and potentially irreversible damage, leading to death.Complications of meth-associated HFrEF Abishak: These patients are at increased risk for ventricular arrhythmias, sudden cardiac death, left ventricular thrombus formation, and progressive pulmonary hypertension. If the ejection fraction remains below 35 percent after at least three months of optimized therapy, implantable cardioverter-defibrillator (known as ICD) placement should be considered for primary prevention.Addiction Treatment as Core Therapy Dr. Arreaza: It sounds like GDMT cannot be done without talking about meth use disorder treatment. Akbar: Absolutely. Treating the myocardium without addressing the substance use disorder is ineffective. Primary care providers can be trained to manage addictions, but if resources are available, you can place a referral to addiction medicine, psychiatric support, behavioral therapy, and social support services. This is an essential part of the treatment. Sustained abstinence is the single most powerful predictor of recovery.Prognosis Abishak: Prognosis is highly dependent on abstinence. Patients who stop using methamphetamine often experience meaningful improvement in EF and even return to normal.  Dr. Arreaza: Yes, the key factor is complete abstinence, plus standard heart failure treatment. If the damage is mostly functional and inflammatory, recovery is possible. If there is extensive fibrosis (scar) recovery is less likely. Observational studies have shown that patients with meth-associated cardiomyopathy who stop using meth have significant improvement in EF over 3–12 months, fewer hospitalizations, and lower mortality. Akbar: Absolutely. Not all meth-associated cardiomyopathy behaves the same way. The extent of fibrosis determines recovery potential. Cardiac MRI with late gadolinium enhancement can help us estimate scar burden. Patients with minimal fibrosis often have better improvement with abstinence and medical therapy. Dr. Arreaza: So, MRI can actually help us determine the prognosis. Abishak: Yes, very much so. If MRI shows extensive fibrosis, the likelihood of full EF recovery is lower. That information helps us counsel patients more accurately. Akbar: Another key issue is right ventricular involvement. Methamphetamine can affect both ventricles. When the right ventricle fails, patients may develop severe peripheral edema, ascites, and hepatic congestion. Right ventricular dysfunction also worsens prognosis significantly. Dr. Arreaza: And pulmonary hypertension can also worsen the whole picture.  Akbar: That's correct. Meth is associated with pulmonary arterial hypertension independently of left-sided heart failure. In some patients, you may see a combined picture of both pulmonary vascular disease and right ventricular dysfunction. That can make management more complicated because pulmonary pressures may remain elevated even after EF improves. Dr. Arreaza: Tells us about the role of BNP in monitoring these patients.  Abishak: Serial BNP levels can help track response to therapy. Additionally, troponin may be elevated at times in meth users due to myocardial injury. Monitoring renal function is critical because many heart failure medications affect kidney function and potassium levels. Akbar:Other lifestyle modifications include sodium restriction, regular follow-ups, vaccination, and avoidance of other cardiotoxic substances such as alcohol or cocaine. Sleep disorders, especially OSA, should be evaluated because untreated OSA worsens heart failure outcomes. Dr. Arreaza: WhatIs there any role for wearable devices or remote monitoring? Abishak: Yes, increasingly so. Remote weight monitoring, blood pressure tracking, and symptom reporting can reduce hospitalization. In select patients, implantable hemodynamic monitors may help detect rising filling pressures before symptoms occur. Dr. Arreaza: It was a great discussion. Pulmonary embolism (PE) remains a leading cause of cardiovascular mortality and a frequent diagnostic challenge, often masquerading as myocardial infarction or a benign illness. Over the past decade, PE care has evolved from anticoagulation-only strategies to nuanced, risk-stratified, multidisciplinary management. Modern approaches integrate hemodynamics, biomarkers, and advanced imaging to guide therapy, including catheter-directed interventions and large-bore thrombectomy. The Pulmonary Embolism Response Team (PERT) model addresses historical gaps by coordinating rapid, multispecialty decision-making and standardizing care pathways. The PERT Consortium further advances PE care through education, research, and the world's largest PE registry, while fostering leadership and research opportunities for trainees. Pearls PE is a “master masquerader”—maintain suspicion for atypical presentations like myocardial infarction, heart failure, flu, or anxiety. How has the clinical approach to PE changed over the past decade? PE is the third leading cause of cardiovascular death and historically under‑recognized. Symptoms mimic MI, HF, asthma, syncope, and more.PE is a silent killer, and it should be recognized more as a cause of spontaneous cardiac arrest. Where life threatening disease like stroke which is owned by neurological specialists and MI is primarily managed by cardiac specialists, PE is an entity without a professional home. The PERT Consortium brings the specialties together for PE care. 2. Ten years ago, a 58-year-old patient with a large bilateral PE, RV dilation, and positive biomarkers might have been managed with anticoagulation and close observation alone. Today, with evolving—but still uneven—data on advanced therapies, PE care feels far more nuanced and highly dependent on where you practice. What are the major gaps in traditional PE management that clinicians should recognize, and what care pathways should they be aware of across different hospital systems? Care has shifted from anticoagulation‑only to multidisciplinary approaches like catheter directed thrombectomy. Risk‑based pathways and the use of CT angiogram has improved early recognition. Risk stratification tools must be used as tools for early recognition of intermediate risk PE. Untreated PE leads to chronic complications like chronic thromboembolic disease and chronic thromboembolic pulmonary hypertension, which requires long term clinic follow up. 3. What is the role of risk stratification tools such as PeSI, sPeSI scores, cardiac biomarkers, and imaging findings in PE, and how do they guide treatment decisions in real world practice? Integrate vitals (blood pressure and heart rate), biomarkers (troponin, pro-BNP), RV/LV ratio assessment, acid‑base status, and scores. Tools include PESI, sPESI, BOVA, HESTIA, FAST, Geneva, NEWS, shock index. Vitals, lactate, acid-base status, and tools like NEWS or shock index track clinical evolution. PESI/sPESI estimate 30-day mortality and help identify low-risk patients who may be candidates for early discharge or outpatient therapy. Clinical judgment matters—scores don't fully capture clot burden, trajectory, or bleeding risk. 4. How was the pulmonary embolism response team created, and since its creation, what evidence or outcome data became available to support the PERT model? Originated after a sentinel case at MGH: A young, pregnant woman in her 30s, who collapsed at home, underwent thrombectomy, and had to be on ECMO for a few days. The case brought cardiology, cardiac surgeons and critical care physicians together for planning and improvement in her health, which was rewarding. Thereby, it was decided to bring specialties involved in PE care together to create a response team. The name of the team, Pulmonary Embolism Response Team (PERT), was coined by Richard Channick in the first meeting. Posters were set up all over the hospital to call a centralized line when an acute PE is recognized A meeting was held to present the concept of putting together a consortium, with development of action items and a PERT database. Enabled rapid multidisciplinary input using early teleconferencing tools. 5. Given concerns about having too many ‘cooks in the kitchen' during the initial PE call—especially with rotating teams—how can institutions reconcile workflow complexity with standardized pathways in a way that meaningfully supports and justifies the added burden on frontline clinicians? Every hospital's PERT is different, catering to their needs and workflow At least two disciplines are needed to make a PERTData is currently being collected to guide further on how the workflow can be standardized Most importantly, the team brings in resources that were not available prior to PERT formation. 6. What are the main goals of the PERT consortium, and how does it support clinicians and institutions involved? To improve care and improve outcomes for patients with PE Expand education, refine algorithms, standardize care with Centers of Excellence. Maintain the largest PE registry for research and outcomes improvement. 7. Beyond global networking, shared learning from successful systems, and the pathway toward Center of Excellence designation, what additional benefits can clinicians and health systems gain by participating in the PERT Consortium? The ability to learn from other systems, the ability to share experiences. Allow people to develop their professional careers like leadership experience, becoming a member of the trainee council Initiate projects and receive funding for your ideas 8. For trainees interested in pulmonary embolism care, how can a trainee be a champion at their institution? Does PERT provide assistance and how can they really contribute meaningfully even before becoming a fellow/attending? The global ship recycling market saw another shift in Week 7 of 2026 as key fundamentals moved in different directions across the sub-continent. The Baltic Dry Index declined by 0.6 percent, mainly due to weaker Capesize and Panamax performance, while Supramax rates improved. Oil prices held near USD 62.8 per barrel as markets continued to monitor U.S. and Iran tensions. In this week's episode, Ingrid and Henning discuss how the U.S. Dollar strengthened against most recycling nation currencies, with India being the exception as the Rupee improved to around INR 90.6. Steel plate prices reversed course in India, falling nearly USD 10 per ton, while Pakistan maintained the strongest fundamentals in the region with plate prices holding near USD 594 per ton. Bangladesh reached a political milestone as the BNP secured a more than two-thirds majority in the general elections. The result is expected to support long-delayed infrastructure projects and could improve domestic steel demand in the months ahead. The country also adopted the International Ready for Recycling Certificate framework, aligning with regional compliance requirements under the Hong Kong Convention. Steel plate prices in Bangladesh remained flat near USD 494 per ton, while the Taka weakened slightly. Pakistan continued to lead pricing tables, supported by firm steel levels, stable currency performance near PKR 279.6, and rising anchorage activity totaling nearly 30,000 LDT across multiple bulk carriers. India's anchorage activity also remained active with more than 47,000 LDT present, despite softer steel prices. Turkey remained quiet, with limited activity in Aliaga and the Lira weakening toward TRY 44. Bangladesh will have its general elections on February 12. Nearly 2,000 candidates will contest on 300 seats of the Jatiya Sangsad. Alongside the parliamentary vote, electors will also vote in a referendum – on whether or not to adopt the July National Charter.  Bangladesh has two major legacy parties -- the Bangladesh National Party (BNP) and the Awami League. With the Sheikh Hasina-led Awami League banned from contesting, the two main players are the BNP, which is contesting from 288 seats, and the Jamaat-e-Islami, which is seeking 224 seats. There is also the student-led National Citizens Party (NCP), which will contest in 32 seats, as part of an alliance with the BNP. What are the main issues in this election? Just imagine, the heart is a pump, blood gets into the heart through the veins, the ventricles fill up and then squeeze the blood out. So, the percent of blood that is pumped out is the EF. Let's start at the beginning. What is HFpEF?Mike: HFpEF stands for heart failure with preserved ejection fraction. Basically, these patients squeeze normally—their ejection fraction is 50% or higher—but here's the thing: the heart can't relax and fill the way it should. The muscle gets stiff, almost like a thick leather boot that just won't stretch. And because the ventricle can't fill properly, pressure starts backing up into the lungs and the rest of the body. That's when patients start experiencing shortness of breath, leg swelling, fatigue—all those classic symptoms.Dr. Arreaza: And this is where people get fooled by the ejection fraction.Mike: Exactly. The ejectionfraction tells you total left ventricular emptying, not just forward flow.Jordan: The classic example is severe mitral regurgitation. You can eject 60% of your blood volume and still be in cardiogenic shock because most of that blood is leaking backward into the left atrium instead of going into the aorta. So, you get pulmonary edema, hypotension, fatigue, all with a “normal” EF. Which is honestly terrifying if you're over-relying on echo reports without thinking clinically.Dr. Arreaza: And in HFpEF, functional mitral regurgitation often shows up later in the disease. It's not usually the primary cause; it's more of a marker of advanced disease. Moderate to severe MR in HFpEF independently predicts worse outcomes, including a higher risk of mortality or heart failure hospitalization. So, let's contrast this with HFrEF. How are these two different?Mike: HFrEF—heart failure with reduced ejection fraction—is a pumping problem. The heart muscle is weak and can't contracteffectively. Ejection fraction drops below 40%, and this is your classic systolic dysfunction.Jordan: HFpEF, on the other hand, is diastolic dysfunction. The heart muscle is thick, fibrotic, and noncompliant. It squeezes fine, but it just doesn't relax, even though the EF looks reassuring on paper.Mike: I like to explain it this way: HFrEF is a weak heart that can't squeeze. HFpEF is a stiff heart that can't relax. Totally different problems.Dr. Arreaza: And then there's the gray zone: heart failure with mildly reduced EF, or HFmrEF. That's an EF between 41 and 49% with evidence of elevated filling pressures. It really shares the features of both worlds. So, what actually causes HFpEF versus HFrEF?Jordan: HFpEF is basically what happens when all the problems of modern living catch up with you. You've got chronic hypertension, obesity, diabetes, metabolic syndrome, aging, systemic inflammation—all of these things slowly remodel the heart over years. The muscle gets thick and stiff, and eventually the ventricle just loses its ability to relax. So, HFpEF is really a disease of metabolic dysfunction and chronic stress in the heart. Mike: HFrEF is more about direct injury. Think about myocardial infarctions, ischemic cardiomyopathy, viral myocarditis, alcohol toxicity, chemotherapy like doxorubicin, genetic cardiomyopathies, or chronic uncontrolled tachycardia. These insults actually damage or kill heart muscle cells, leading to a dilated, weak ventricle that can't pump effectively.Dr. Arreaza: So the short version: HFpEF is caused by chronic metabolic and hypertensive stress, while HFrEF is caused mainly by myocardial damage. A question we get a lot: does HFpEF eventually turn into HFrEF? What do you guys think?Mike: In most cases, no. HFpEF patients usually stay HFpEF throughout their disease course. They don't just “burn out” and turn into HFrEF.Jordan: They're generally separate disease entities with different pathophysiology. A patient with HFpEF can develop HFrEF if they have a big myocardial infarction or ongoing ischemia that damages the muscle, but that's not the natural progression.Mike: Interestingly though, the opposite can happen. Some HFrEF patients actually improve their ejection fraction with good medical therapy—that's called HF with improved EF—and it's a great sign that treatment is working.Dr. Arreaza: Another question. How do HFpEF and HFrEF compare to restrictive cardiomyopathy and constrictive pericarditis?Jordan: Clinically, they can all look very similar: dyspnea, edema, fatigue, but the underlying mechanisms are completely different.Mike: In HFpEF, the myocardium itself is stiff from hypertrophy and fibrosis. The problem is intrinsic to the heart muscle, and EF stays preserved. Echoshows diastolic dysfunction with elevated filling pressures.Jordan: In HFrEF, the myocardium is weak. The ventricle is often dilated and contracts poorly, with a reduced EF.Mike: Restrictive cardiomyopathy is different. Here, the myocardium gets infiltrated by abnormal stuff—amyloid, iron, sarcoid—and that makes it extremely stiff. It can look like HFpEF on the surface, but it's usually more severe. On Echo You'll see biatrial enlargement, small ventricles, and preserved EF. And importantly, it's a pathologic diagnosis, so you need advanced imaging or biopsy to confirm it.Jordan: Constrictive pericarditis is another mimic, but here the myocardium is usually normal. The problem is that the pericardium is thickened, calcified, and rigid. This will physically prevent the heart from being filled. Imaging shows pericardial thickening, septal bounce, and respiratory variation in flow, and cath shows equalization of diastolic pressures, which is the hallmark of constrictive pericarditis.Dr. Arreaza: So the takeaway is: HFpEF is a clinical syndrome driven by common metabolic and hypertensive causes, while restrictive and constrictive diseases are specific pathologic entities. If “HFpEF” is unusually severe or not responding to treatment, you need to think beyond HFpEF. Which type of heart failure is more common right now?Mike: Good question, the answer is: HFpEF. It now accounts for up to 60% of all heart failure cases, and it's still rising.Dr. Arreaza: Why is that?Jordan: Because people are living longer, gaining weight, and developing more metabolic syndrome. HFpEF thrives in older, or people with obesity, hypertension, or diabetes: basically, the modern American population. At the same time, better treatment of acute MIs means fewer people are developing HFrEF from massive heart attacks.Mike: HFpEF is the heart failure epidemic of the 21st century. It's honestly the cardiology equivalent of type 2 diabetes.Dr. Arreaza: Let's talk aboutCOVID-19. (2025 and still talking about it) Does it actually increase heart failure risk?Mike: Yes, absolutely. COVID increases both acute and long-term heart failure risk.Jordan: During acute infection, COVID can cause myocarditis, trigger massive inflammation, and precipitate acute decompensated heart failure, especially in patients with pre-existing disease. It also causes microthrombi, which can injure the myocardium.Mike: And after infection, even mild cases are linked to a significantly higher risk of developing new heart failure within the following year. Both HFpEF and HFrEF rates go up.Dr. Arreaza: I remember seeing this in 2021, we had a patient with acute COVID and HFrEF, her EF was about 10%, I lost contact with the patient and at the end I don't know what happened to her. What's the pathophysiology of COVID and heart failure?Mike: COVID causes direct viral injury through ACE2 receptors, triggers massive inflammation that damages the endothelium and heart muscle, leads to microvascular clotting and fibrosis—all mechanisms that promote HFpEF.Jordan: Add autonomic dysfunction, persistent low-grade inflammation, and worsening metabolic syndrome, and you've got a perfect storm for heart failure.Dr. Arreaza: Bottom line: COVID is a cardiovascular disease as much as a respiratory one. If someone had COVID and now has unexplained dyspnea or fatigue, think about heart failure. Get an echo, get a BNP, start treatment. Last big question: why did we have so many therapies for HFrEF but essentially none for HFpEF for years?Mike: HFrEF is mechanistically straightforward. You've got a weak heart with excessive neurohormonal activation going on — so you block RAAS, block the sympathetic system, drop the afterload. The drugs make sense.Jordan: HFpEF is messy. It's not one disease. It's stiffness, fibrosis, inflammation, microvascular dysfunction, metabolic disease, atrial fibrillation, all overlapping. One drug can't fix all of that.Mike: And some drugs that worked beautifully in HFrEF actually made HFpEF worse. Take Beta blockers, for example.  They slow heart rate, which is a problem because HFpEF patients rely on heart rate to maintain their cardiac output.Jordan: The breakthrough came with SGLT-2 inhibitors: diabetes drugs that unexpectedly addressed multiple HFpEF mechanisms at once: volume, metabolism, inflammation, and myocardial energetics.Dr. Arreaza: The miracle drug for HFpEF! Alright, let's wrap up.Mike: Bottom line: HFpEF is common, complex, and dangerous: even if the EF looks “normal.”Jordan: And if you're relying on ejection fraction alone, HFpEF will humble you every time.Dr. Arreaza: If you liked this episode, share it with a friend or a colleague and rate us wherever you listen. En mai 2014, un événement inédit a secoué le monde bancaire : BNP Paribas, première banque française, a accepté de payer une amende colossale de 8,97 milliards de dollars au gouvernement américain. Le motif ? La banque avait contourné les embargos imposés par les États-Unis à Cuba, l'Iran et le Soudan entre 2004 et 2012. Ces pays étaient considérés par Washington comme des États soutenant le terrorisme ou violant les droits humains.Quels faits étaient reprochés exactement ? BNP Paribas avait réalisé, via certaines filiales, des transactions en dollars pour le compte de clients liés à ces pays. Or, toute opération en dollars transitant à un moment donné par le système financier américain est soumise à la législation des États-Unis. Cela signifie que même une banque étrangère peut être poursuivie à partir du moment où elle utilise la monnaie américaine. C'est l'un des points clés de ce dossier.La banque a reconnu avoir non seulement effectué ces paiements, mais parfois mis en place des procédures visant à masquer l'identité des clients ou l'origine réelle des fonds pour éviter les contrôles américains. Sur le plan du droit américain, la sanction était donc légale : la banque avait violé les règles de l'Office of Foreign Assets Control (OFAC), qui encadre les embargos.Là où le dossier devient explosif, c'est sur le plan du droit international. Beaucoup d'experts et de gouvernements ont dénoncé une sanction « extraterritoriale ». Autrement dit, les États-Unis appliquent leurs lois à des entités étrangères, opérant hors de leur territoire, simplement parce qu'elles utilisent la monnaie américaine ou un serveur situé aux États-Unis. Pour nombre de juristes, cela revient à imposer au reste du monde la politique étrangère américaine.Les critiques soulignent que BNP Paribas n'a pas violé le droit français ni le droit international, et que les embargos américains n'engageaient que les États-Unis. Pourtant, Washington a considéré que l'utilisation du dollar suffisait à justifier son intervention. Ce type de sanction a depuis été utilisé contre de nombreuses entreprises européennes, provoquant un réel malaise diplomatique.L'affaire BNP a ainsi mis en lumière un rapport de force : les États-Unis disposent d'une arme économique puissante — le contrôle du dollar — qui leur permet d'étendre leur influence bien au-delà de leurs frontières. Elle a également relancé le débat sur la souveraineté européenne et la capacité du continent à protéger ses entreprises des pressions américaines.Une sanction financière, donc, mais aussi un choc géopolitique.
