Today's episode covers a case I saw in the clinic in which a patient presented with symptoms consistent with a calf muscle strain, but ended up having a blood clot or deep vein thrombosis (DVT). Deep Vein Thrombosis or DVT describes a situation in which a blood clot (thrombus) forms in one of our deep veins. In many instances, this occurs in the calf region and happens after a person has undergone a surgical procedure. Typical symptoms include pain, swelling and warmth in the region. Besides surgery, blood clots can occur when we have been sedentary for extended periods (plane flight, bed rest after an injury, etc). DVTs can become a life-threatening if they break loose and move through the blood stream to the lungs. This situation is referred to as a pulmonary embolism (PE) and blocks blood flow to a portion of the lungs. In this episode I have discussed about Deep Vein Thrombosis- about epidemiology, etiopathogenesis, clinical features, diagnostic investigations, Wells scoring, differential diagnosis, complications and treatment.
PE's (or Pulmonary Emboli) are a key part of Emergency Care, something that many of us will consider as a differential diagnosis multiple times of a daily basis, in a similar way to acute coronary syndrome, so we need to be absolute experts on the topic! A PE normally occurs when a Deep Vein Thrombosis shoots off to the pulmonary arterial tree, occurring in 60-120 per 100,000 of the population per year The inhospital mortality is 14% and the 90 day mortality is around 20%. But this is proportional to its size, and risk stratifying PE's once we've got the diagnosis is really important. PE is a real diagnostic challenge and less than 1 in 10 who are investigated for a PE end up with the diagnosis, so knowing the risk factors, associated features and thresholds for work up are really important. There are some key concepts in risk stratification and particularly in test thresholds that we'll cover in this episode that are applicable to all of our practice…..we're excited! Getting these right helps us to avoid missing the diagnosis and equally importantly ensure we aren't ‘over testing' & ‘over diagnosing' because investigation and treatment for a PE isn't without it's own risks. In the episode we'll talk in depth about factors associated with presentation, risk factors, investigations and finally onto treatments, covering the whole spectrum from low risk PE's up to those with massive PE's and cardiac arrest. This is merely a nuisance for some, while others experience significant morbidity, leading to hemodynamic instability and potential death. Alissa shares her first birth story of her son, born in November 2019. Her partner and her decided to stop taking any precautions after their May 2018 wedding. She believed she would come home with a honeymoon baby, but that did not happen. Being the planner she is, she quickly turned to tracking her cycle with 7 months of negative pregnancy tests. She took her concerns to her OBGYN and her OB agreed to run a blood screening and sperm mobility test. She was diagnosed with PCOS and her partner had few head sperm, which gave them a 2% chance of conceiving naturally. They decided to move right onto IVF with a successful first transfer. Around 35 weeks pregnant, Alissa woke up with her left leg swollen to the size of a “Frankenstein Leg” and had Deep Vein Thrombosis. Between the DVT and a breech baby, she was given a c-section date at 39 weeks. She had to deliver in a normal Operating Room over an OB Operating Room and the team did not allow her husband in the room. She struggled with postpartum anxiety and credits that mostly to issues with breastfeed that led to pumping and decreased sleep. This is Part 1 of 2. The single most common, preventable cause of in-hospital death is a hospital-acquired venous thromboembolism; making this an important target for prevention when people are admitted to hospital. An updated Cochrane Review from January 2022 looks at the evidence for one of the possible interventions, which combines anti-coagulant drugs with intermittent pneumatic leg compression to prevent these blood clots. Stavros Kakkos from the University of Patras in Greece tells us more.

Objective Two: In patients with high probability for thrombotic disease (e.g., extensive leg clot, suspected pulmonary embolism) start anticoagulant therapy if tests will be delayed. Objective Three: Identify patients likely to benefit from DVT prophylaxis. Objective Four: Utilize investigations for DVT allowing for their limitations (e.g., Ultrasound and D-dimer). Objective Five: In patients with established DVT, use oral anticoagulation appropriately, (e.g., start promptly, watch for drug interactions, monitor lab values and adjust dose when appropriate, stop warfarin when appropriate, provide patient teaching). Objective Six: Consider the possibility of an underlying coagulopathy in patients with DVT, especially when unexpected. Objective Seven: Use compression stockings in appropriate patients, to prevent and treat post-phlebetic syndrome.
