Most clinicians crush the evaluation… and then spend the next several visits wondering why their treatment sessions feel flat. In this episode, we sit down with J.J. Mowder-Tinney, a true wizard of crafting salient, meaningful, movement-rich sessions, to talk about the moment that actually matters: How do you turn your evaluation into a treatment session that changes something? J.J. breaks down how to shift from checking boxes to building experiences—sessions that engage the brain, challenge the body, and make every rep count. We explore: Why salience isn't fluff — it's the lever that drives neuroplastic change How to translate your eval findings into interventions that actually matter Using standing rests, strategic intensity, and real movement retraining Why even a single well-placed high-intensity burst can outperform a whole session of underdosed "activity" What happens when patients stay physically engaged instead of passively seated The power of controlling your session, even when you can't control the system This conversation is a masterclass in doing more with the minutes you have, ditching the excuses, and designing sessions that older adults feel, remember, and grow from. As rehab professionals, we often face unique challenges when working with patients who have ataxia. In today's episode Claire McLean, PT, DPT, NCS, J.J. Mowder-Tinney, PT, PhD, NCS and Erin Gallardo, PT, DPT, NCS have a group discussion about working with people with it. One of the primary challenges is that often there can be slow progress. This can be exacerbated by the diverse underlying causes and presentations of ataxia, making it difficult to develop standardized treatment approaches. In today's show we'll discuss a couple of main points the research emphasizes to incorporate in your treatments for people with ataxia. We'll discuss the question of how to divide up your time in the session, how to uncover what their true capabilities are, and how to balance care partner training into sessions. Some treatment examples include incorporating sensory input, start-stop techniques, resistance training, whole task versus part task training, ocular motor training and visual tracking exercises to address the challenges patients face with balance and stability. In this episode, hosts Erin Gallardo and Claire McLean are joined by guest Dr. JJ Mowder-Tinney, PT, PhD, NCS, a highly experienced physical therapist specializing in neurological conditions. Are you afraid to get your patients on the floor? Are you nervous it will be too painful for them (or you)? Join us for this engaging discussion on the importance of getting patients down on the floor and practicing floor transfers, as well as strategies to overcome the common fear and hesitation clinicians may have around this. Some of the key takeaways include: - The significance of building patients' self-efficacy and confidence in getting up from the floor, as fear of falling is closely linked to increased fall risk. - The value of strong movement analysis skills that allow therapists to tailor floor activities to each patient's unique presentation and abilities. - Practical tips for breaking down the task into smaller, more manageable parts and adjusting the environment (e.g. using padded mats) to facilitate success. - The collaborative approach of problem-solving with patients to find the best methods for them, rather than prescribing a one-size-fits-all technique. - The importance of regularly assessing floor skills and incorporating floor exercises into treatment, even for higher-functioning patients. - Encouragement for clinicians to reflect on their own comfort level with floor transfers, seek support from colleagues, and continuously expand their skills in this area. We are also excited to announce a free webinar on September 24th titled "Gettin' Down and Dirty: Mastering Floor Transfers and Exercises in Neuro Without the Fear," which will give you the opportunity to brainstorm live with hundreds of neuro colleagues from around the world, see some helpful videos, and give you specific takeaways you can use the next day. Do you like new, creative ideas? In this episode we're talking about why we're on a kick with resistance bands. Like a lot of equipment we use in neuro rehab it was first intended to be used in the fitness and general rehab population, and because neuro clinicians are crafty we found the uses far surpass strengthening isolated muscles. There are a lot of different types of resistance bands so we'll talk about some of our favorites. We'll also discuss different ways these can be used and how they improve movement and function. Resistance bands can be used for error augmentation which engages the cerebellum and leads to learning. They also offer a sensory component that improves movement and function by giving the person using it feedback. Bands can provide assistance for movement an individual may not be strong enough to do on their own allowing their brain to feel what that movement is like. Pick up some new ideas and incentives for using resistance bands in your sessions now! Join us for a free webinar! https://www.neurocollaborative.com/precision-therapy
