The first medical description of pernicious anaemia is from 1821. It is written by Dr James Combe and describes a previously well 47 year old corn-merchant who presented with pallor, weakness, and neurological symptoms. It would take over a hundred years before we would know how to treat this condition, and another decade after that to understand why the treatment worked. Vitamin B12 is an essential component of our diet and only found in animal products. Deficiency in vitamin B12 can present with fatigue, anaemia (macrocytic), and neurological symptoms. The most at risk from a dietary perspective are vegetarians, vegans, and the elderly. Patients may also have an autoimmune condition (pernicious anaemia) that interferes with gastric parietal cells and intrinsic factor; the way our body can absorb vitamin B12 from our diet. This is the story of vitamin B12. Our special guest: Our special guest is Professor Ken Sikaris who is a chemical pathologist and Director of Chemical Pathology at Dorevitch Pathology.See omnystudio.com/listener for privacy information.

In this enlightening conversation, Andrea Nakayama breaks down the often confusing world of functional nutrition and explains how it can transform healthcare practices. Rather than focusing on restrictive diets or one-size-fits-all protocols, functional nutrition takes a personalized approach that considers each individual's unique history, physiology, and circumstances. Andrea shares how practitioners can integrate these services into their practices to better serve patients with chronic health issues, while working collaboratively within appropriate scopes of practice. Key Highlights What functional nutrition really is: A systems-based approach focused on individual experience rather than dietary protocols, guided by the principles that "everything is connected, we are all unique, and all things matter" The three-tier framework: Tier 1: Non-negotiables (sleep, digestion, blood sugar balance) Tier 2: Deficiency to sufficiency Tier 3: Dismantling dysfunction The "three roots, many branches" model: Addressing genes, digestion, and inflammation as the root causes while symptoms and diagnoses are the branches Scope of practice collaboration: How functional nutrition practitioners can work alongside medical providers to fill gaps in care without overstepping boundaries Making nutrition accessible: Moving away from expensive testing and supplements toward a way of thinking that can be democratized for all income levels The bike lane analogy: Understanding your personal "path" versus "poison ivy" - what foods and choices work for your individual physiology and circumstances Business integration options: How practices can incorporate functional nutrition through hiring trained practitioners, referral networks, or additional training Patient empowerment: Helping individuals understand their personal "mediators" - what makes them feel better or worse - rather than following rigid good/bad food rules Memorable Quotes "We may be asking too much of our doctors... when we ask them about nutrition, that's an out of scope question." "If somebody's having an issue with compliance, I say we didn't match the pacing of our recommendations to that person's aptitude to be able to take them on." "Your body is likely more functional than it's not. It's doing a lot for you. And when it comes to a functional practice, we're fine tuning the dial." "We're not replacing [medical care] - food is better, lifestyle should do it. We're augmenting all the different things that person is experiencing." "It's not about a protocol, it's about making steps forward and working within frameworks that help people know where to start." "We can't walk a very narrow path for a very long time, and we shouldn't." "When we know our mediators, we're making the decisions, we're not farming out our agency for somebody else to tell us we're good or bad." Bio: As the host of the 15-Minute Matrix Podcast and the founder of Functional Nutrition Alliance, Andrea is leading thousands of students and practitioners around the globe in a revolution to offer better solutions to the growing chronic illness epidemic. For several years I was focusing heavily on the relationship between copper and iron while neglecting and even demonizing zinc. Given that our body stores a much higher concentration of zinc than copper, that was not a wise approach. Over the years I have learned that even in the case of a copper deficiency, one should still supplement zinc in addition to copper, and the clinical research clearly proves this. It is very easy to get caught in the anti-supplement fear campaigns that big influencers spread and I have since repented for my sins in this regard. In this episode I share my experience taking high dose zinc for the past few years and the copper deficiency symptoms I have noticed recently, specifically in relationship to ultraviolet light exposure from sunlight. On day one of supplementing copper again I noticed an increased resilience to sun exposure and less of a burning sensation being outside in the mid-day summer sun. I break down the science of different types of melanin, the exact biochemical processses involved in copper supporting melanin production, what melanin does after it absorbs UV light, what other nutrients we should be supplementing to protect ourselves from sunburn and skin photoaging, the complexity of the zinc/copper ratio and where I stand with it, how you can diagnose yourself for deficiencies using symptoms instead of blood tests, and more. Short term weight loss sounds great BUT it's not all fat – it's muscle! Muscle will be much harder to regain as we age because of anabolic resistance.. Clothes might feel loose and you get weight loss compliments. But short term weight loss is just giving a “false positive” honeymoon period. This might mean you divorced muscle, the love of your life. 1. Muscle Mass Loss (Sarcopenia) Protein Deficiency Protein is essential for building and maintaining muscle tissue. Insufficient protein intake exacerbates muscle atrophy and increases the risk of falls and injuries. Excessive Cardio Prolonged cardio can lead to a breakdown of muscle tissue for energy, particularly if glycogen stores are depleted. This can worsen age-related muscle loss and counteract maintaining strength and function. 2. Bone Health (Osteoporosis) Protein Deficiency Protein is needed for bone health and bone density. Inadequate protein intake, especially after menopause, increases the risk of osteoporosis and fractures. Osteoporosis Risks After menopause, risk of osteoporosis increases due to declining estrogen levels and can weaken bones prone to fracture. Sarcopenia and Osteoporosis Link Having both increases the risk of falls and fractures. Poor nutrition leads to sarcopenic obesity and increases the risk of osteoporosis. 3. Other Negative Effects Reduced Physical Function Since inadequate protein leads to muscle loss, this reduces strength, impaired balance, and decreased ability to perform daily activities. Slow-Healing Injuries Protein repairs tissues. Deficiency can slow wound healing and recovery from injuries. Weakened Immune Function Amino acids from protein build antibodies and maintain a healthy immune system. Low protein intake can lead to frequent illnesses and infections. Potential Cardiac Issues (Excessive Cardio) Associated with potential adverse cardiac effects, such as myocardial fibrosis and an increased risk of atrial fibrillation, in some individuals. Musculoskeletal Injuries (Excessive Cardio) Increases the risk of musculoskeletal issues like osteoarthritis and stress fractures. What Can You Do Instead of A Short Term Weight Loss Recommendations: Prioritize protein intake with 30 grams each meal. Balance cardio and strength training to build and maintain muscle mass. Listen to your body and avoid pushing yourself too hard or engaging in prolonged, strenuous exercise if it causes excessive fatigue or pain. If you want to lower your chances of developing high blood pressure, atherosclerosis, heart attack, arrhythmias, malignant tumors, kidney stones, cholesterol problems, depression, attention problems or other psychiatric disorders, fatigue, and poor athletic performance, you need enough magnesium. 80% of the American population does not get enough magnesium. BUFFALO, NY – June 9, 2025 – A new #research paper was #published in Volume 16 of Oncotarget on May 20, 2025, titled “Cigarette smoke and decreased DNA repair by Xeroderma Pigmentosum Group C use a double hit mechanism for epithelial cell lung carcinogenesis.” In this study, led by first author Nawar Al Nasralla and corresponding author Catherine R. Sears, from the Division of Pulmonary, Critical Care, Sleep and Occupational Medicine, Indianapolis and the Richard L. Roudebush Veterans Affairs Medical Center, researchers investigated how cigarette smoke and reduced DNA repair capacity contribute together to the development of lung cancer. They found that when a critical DNA repair protein called XPC is decreased and lung cells are exposed to cigarette smoke, the combination causes extensive damage and significantly increases cancer risk. Non-small cell lung cancer (NSCLC) develops through both genetic and environmental factors. This study focused on how cigarette smoke affects the body's natural ability to repair DNA. The researchers studied the role of XPC, a protein essential for recognizing and repairing harmful DNA changes caused by tobacco smoke. They found that low levels of XPC — commonly seen in lung cancer patients — made lung cells less capable of repairing DNA. This made the cells unstable and more likely to become cancerous. These changes were most pronounced in normal lung cells, suggesting that the earliest stages of disease occur before cancer is even detected. The findings support a “double hit” model, where both cigarette smoke and reduced DNA repair work together to drive cancer development. In laboratory experiments, normal lung cells with low XPC levels showed more damage and cell death after cigarette smoke exposure. By contrast, lung cancer cells were more resistant to smoke damage, even when XPC was low, indicating that critical changes had likely occurred earlier in the disease process. “Our study suggests that cigarette smoke exposure leads to decreased XPC mRNA expression, exacerbates total and oxidative DNA damage, hinders NER, and may contribute to lung cancer development.” The study also showed that DNA repair ability declined significantly in healthy cells after smoke exposure, but this effect was not seen in cancer cells. In addition, the researchers confirmed that XPC gene activity was lower in actual lung tumor tissue compared to nearby healthy lung tissue. This pattern was consistent across both adenocarcinoma and squamous cell carcinoma, the two main types of NSCLC. These results add to our understanding of how lung cancer begins at the molecular level. By showing how cigarette smoke and reduced DNA repair combine to create genetic instability, the research points toward new strategies for prevention. A better understanding of XPC's role could help identify high-risk individuals and inform future efforts to stop lung cancer before it begins. These new creatures that were devised are better than the Deficiency because they come from the union of the entire Fullness, whereas the Deficiency came out of a mistaken solo effort by Logos alone. Out of this union of the Fullness and Logos came what are called the Second Order Powers, and these powers are better and greater than those of the imitation because they did not come into being out of ego and out of lack of thought. This new fruit of the Fullness and Logos was fitted purposely into the boundary that surrounded the Fall, and so because they were fitted to live within this boundary, they were able to have a cooperative structure the way the Fullness had a cooperative structure.

The literature shows that the incidence of heart failure with preserved ejection fraction (HFpEF) increases significantly in postmenopausal women, but how can researchers study the underlying mechanisms? In our latest episode, Dr. Jonathan Kirk (Loyola University Chicago) interviews lead author Dr. Mei Methawasin (University of Missouri, Columbia) and expert Dr. Glen Pyle (University of Guelph) about the recent study by Methawasin and co-authors investigating sex differences, menopause and HFpEF. The authors created an animal model that resembles HFpEF in women undergoing natural menopause by using 4-vinylcyclohexene dioxide (VCD) to induce “ovary-intact” menopause, combined with the 2hit regimen (HFpEF inducing regimen) to model postmenopausal HFpEF. Combining echocardiography, pressure-volume and single-cell analyses, the authors found that VCD mice, compared to ovariectomized mice and premenopausal mice, have higher testosterone levels compared to other models. By developing this robust phenotype animal model, the authors open new avenues for investigating therapeutic targets in other hormones beyond estrogen alone. Ready to explore the estrobolome, the importance of animal models of human disease, and the complex family of hormones comprising estrogen? Listen now. Mei Methawasin, Joshua Strom, Vito A.
