Venetoclax plus decitabine was associated with superior safety and non-inferior response rates compared to intensive chemotherapy. Is it time to consider lower-intensity therapy beyond older and unfit patients? Finally, a focus on venous thromboembolism. Researchers link BGAT, an enzyme pivotal to determining blood type, to risk of future VTE. Le groupe sanguin pourrait influencer le risque de développer un cancer et la mortalité associée en raison de divers facteurs biologiques, immunologiques et génétiques. Plusieurs études ont exploré cette corrélation et ont mis en évidence des différences entre les groupes sanguins A, B, AB et O concernant certaines formes de cancer.1. Influence des antigènes du groupe sanguinLes groupes sanguins sont définis par la présence ou l'absence d'antigènes spécifiques (A, B, AB ou O) à la surface des globules rouges et d'autres cellules du corps, y compris celles des tissus épithéliaux, où se développent de nombreux cancers.- Le groupe A est associé à un risque accru de plusieurs cancers, notamment le cancer gastrique. Picture this: you're on the admit shift on a Tuesday morning, and your next patient is a 6-month-old Doberman for routine neutering. Is there anything you'd be particularly worried about before surgery? Is there anything you'd discuss with the client or any tests you'd speak to your vet about? Dobermans are a classic breed affected by von Willebrand's disease, which is a disease that sits somewhere between thrombocytopenia and coagulopathy. If they have it, bleeding is a very real risk. Thankfully, if we know beforehand, we can implement strategies to manage the patient's disease and prevent severe haemorrhage. N-acetyl cysteine (NAC) shows promise for heart attack preventionHeart attacks and strokes are a leading cause of death in developed countries - accounting for ~20% of all deaths in the U.S. Frequently, heart attacks and strokes are caused by blood clots (blood platelets that form when they are not needed, causing a narrowing/blockage of blood vessels) formed through arterial thrombosis. Current antiplatelet agents (e.g., aspirin) are effective but can increase major bleeding risk. As such, there is a growing need to prevent arterial thrombosis different from antiplatelet agents.A recent study published in Arteriosclerosis, Thrombosis, and Vascular Biology investigated the potential of NAC as an agent to prevent arterial thrombosis. Utilizing both [cell-based] human blood models and mouse (in vitro) models, the research demonstrated that NAC significantly delayed and even prevented thrombus formation dose-dependently without increasing bleeding risks.Findings:*NAC treatment extended clot formation times by up to 3.7 times compared to controls in the human blood [cell] model.*NAC entirely inhibited platelet aggregation and occlusive clot formation at higher doses than the above point in the cell model.*A 400 mg/kg (via injection in rodents) dose of NAC in mice effectively prevented arterial occlusion post-injury.*Lower doses (200 mg/kg - via injection in rodents) of NAC reduced clot stability, suggesting working as both an acute and preventative clotting therapy.The study showed that NAC interferes with von Willebrand factor (a large protein crucial for blood clotting) activity. NAC acts against VWF rather than platelets to prevent clot formation. Thus, providing a safer alternative to traditional antiplatelet drugs that carry higher bleeding risks. Support the Show.

About our guest … Dr. Lasky joined the Cure the Kids Foundation in 2017. He attended medical school at the University of Illinois in Chicago and did his pediatric residency at the University of Chicago. He completed his Pediatric Hematology Oncology fellowship at UCLA in 2006 and was director of the Pediatric Neuro-Oncology Program at UCLA until 2009 when he moved to Harbor-UCLA Medical Center in Torrance, CA, and was named Chief of the Division of Pediatric Hematology Oncology. While there, Dr. Lasky became involved with a number of clinical trials for patients with bleeding disorders (including hemophilia and von Willebrand disease) as well as sickle cell anemia. While continuing to have research interests in pediatric brain tumors and immunology, he also is interested in running and developing clinical trials for patients with bleeding disorders and sickle cell anemia. Dr. Lasky is a member of several professional societies including the Children's Oncology Group, Society for Neuro-Oncology, the American Society of Hematology, and the American Academy of Pediatrics. He enjoys watching sports, reading, music, and outdoor activities with his family. In September of 2022, Dr. Lasky is now the Medical Director at Cure 4 The Kids (C4K).

Emergencies happen in hematology and oncology. This is a fact. But how do we manage these emergencies? Look no further. In this episode, we talk all about our fourth hematologic emergency: thrombotic thrombocytopenic purpura (TTP). Thrombotic thrombocytopenic purpura (TTP):- Be sure to check out episode 009 on thrombocytopenia for a general approach and differential!- New anemia and thrombocytopenia should raise concerns for TTP! Workup: - Peripheral smear - concern for schistocytes. Look at this first! Example of these cells from ASH image bank here- ADAMTS13 level - always draw ASAP before any intervention- Repeat CBC- Reticulocyte count - will have elevated retic count- Citrated platelet count - CMP- PT, PTT, INR- Fibrinogen- Haptoglobin - LDH- Viral serologies Clinical manifestations: - Fever, Anemia, Thrombocytopenia, Renal (AKI), Altered Mental Status- If you see this - the patient is in bad shapeMechanism:- Tiny blood clots form in the body, causing platelet shearing- Loss of ADAMTS13 - This protein normally is responsible for chopping up von Willebrand's factor (vWF)- In the absence of ADAMTS13, vWF multimers are extra long, therefore interacting with platelets/collagen more and causing activation of platelets and clotting system- This causes red blood cell shearing due to small vessel microthrombi (brain, kidneys, heart)- Cytokine release causes fevers Management:- Do not reflexively transfuse platelets; can make situation worse - PLASMIC Score: helps to stratify likelihood of TTP; MDCalc link (https://www.mdcalc.com/plasmic-score-ttp)Treatment: - Plasma exchange: replacing ADATMS13-deficient plasma with ADAMTS13-rich plasma- This is different than plasmapheresis, where we replace plasma with albumin- Steroids: 1mg/kg prednisone daily to stop auto-antibody (against ADAMTS13) production- Confirm with ADAMTS13 levels; if

Dr. Bertino starts by reviewing the difference between the compositions of arterial versus venous clots. Arterial clots are formed as a response to endothelial injury and exposure of von Willebrand factor, so these clots are usually platelet-rich. On the other hand, venous clots are formed due to stasis, and these are usually platelet-poor. Therefore, antiplatelet therapy may not be ideal for venous clots. However, Dr. Bertino notes that stent placement can cause endothelial injury at the apposition points of the stent, so the treatment algorithm can become more complex in these cases. The doctors note that there are non-thrombotic diseases that require venous stenting, such as May Thurner syndrome. Dr. Bertino says that addressing this early in the pediatric population can be a safe way to prevent future DVT, as long as children are monitored carefully. Next, Dr. Bertino walks us through his preferred anticoagulation routine for stent placement. Four hours before the procedure, he starts with a dose of Factor Xa inhibitor (apixaban or rivaroxaban) to prevent in-stent thrombosis. The patient is maintained on heparin during the procedure. After the procedure, anticoagulation varies depending on whether a stent was placed, or solely mechanical thrombectomy was performed. Finally, the doctors discuss preferred anticoagulation for special scenarios such as covered stents (which can be more thrombophilic) and patients with malignancies.
