Incisional Hernia- Study Guide ✔️An incisional hernia is an iatrogenic condition where the peritoneal sac and its contents, such as bowel or omentum, protrude through an acquired scar in the abdominal wall.✔️This typically results from a previous surgical operation or accidental trauma. These hernias occur in approximately 10 to 20 percent of all laparotomies and represent a failure of tissue and wound healing rather than a simple anatomical gap.✔️Risk Factors for DevelopmentThe development of an incisional hernia is driven by a combination of patient and technical factors.1. Patient Factors: These include obesity, diabetes, smoking, malnutrition, chronic cough, and the use of steroids or immunosuppressants.2. Technical Factors: These involve wound infection, the type of suture material used, emergency surgery, and improper suturing techniques such as mass closure or placing a drainage tube through the main wound.3. Biological Factors: Late-onset hernias occurring five to ten years after surgery are often associated with tissue failure due to abnormal collagen production.✔️Evaluation and ClassificationClinical features typically include a swelling and pain at the site of a previous scar. The hernia is usually reducible and demonstrates an expansile impulse on coughing.The European Hernia Society framework provides a systematic classification based on three criteria:Location: Midline, lateral, or subxiphoid sites.Width: Categorized as small when under 4 centimeters, medium between 4 and 10 centimeters, large between 10 and 15 centimeters, and giant when exceeding 15 centimeters.Status: Defined as either a primary or a recurrent hernia.✔️Mandatory computed tomography imaging is the cornerstone of preoperative planning. It is essential to identify multiple defects known as Swiss cheese hernias, measure rectus muscle width, and assess the volume of hernia contents to determine if there is a loss of domain.✔️Management PrinciplesMesh reinforcement is the non-negotiable standard of care for all incisional hernias regardless of size. Primary suture repair alone is associated with unacceptable recurrence rates as high as 50 percent. The ultimate goal of surgery is functional restoration of the abdominal wall rather than just closure.✔️Surgical Algorithm by Defect WidthThe recommended surgical technique is determined primarily by the width of the fascial defect.Defects under 10 centimeters: Primary repair with mesh is recommended, which can be performed as an open Rives-Stoppa repair or a laparoscopic IPOM-Plus procedure. The IPOM-Plus technique, which involves primarily suturing the fascial defect before mesh placement, is superior to standard bridging techniques because it reduces recurrence and seroma formation.Defects between 10 and 14 centimeters: Transversus Abdominis Release, also known as posterior component separation, is the preferred technique. It offers excellent results with significantly lower wound morbidity compared to anterior approaches.Defects exceeding 14 to 15 centimeters: Open Anterior Component Separation is generally required for these massive defects to achieve a tension-free midline closure.✔️Optimization for Complex Cases-Loss of domain is a serious condition where chronic herniation causes the abdominal cavity to shrink, forcing viscera to reside outside the cavity. Forcible repair in these patients risks fatal abdominal compartment syndrome. Preoperative optimization includes Botox injections into the lateral muscles to relax them and preoperative progressive pneumoperitoneum to stretch the abdominal cavity.-Recurrent hernias are viewed as opportunities to identify specific mechanical or biological failures. Management involves identifying the cause and converting the repair to a different surgical plane. Because recurrence can occur years after a repair, annual long-term follow-up is considered mandatory.

The Joint Readiness Training Center is pleased to present the sixty-fourth episode to air on ‘The Crucible - The JRTC Experience.' Hosted by the Commander of Ops Group (COG), COL Matthew Hardman. Today's guest is the company radio-telephone operator for B Company, 1-504th PIR, 1st BCT, 82nd Airborne Division, SPC Kevin Mitchell.   Established during the fierce fighting in the Italian campaigns of World War II, 1st Battalion of the 504th Parachute Infantry Regiment was employed in multiple brush wars throughout the Cold War as well as in Operation Desert Storm and later as part of Operation Joint Guardian in Kosovo before deploying in support of the Global War on Terror. 1-504 PIR now serves as part of 1st Brigade Combat Team of the 82nd Airborne Division. They have the Hollywood call-sign of “Red Devils” (the BCT are the “Devils”) and the motto of “Strike and Hold.”   In this episode we discuss how airborne forces would be utilized at the commencement of large-scale combat operations for a joint forcible entry. Specifically, we look at how Airborne forces allow the US Army to set conditions for follow-on operations to meet national objectives anywhere in the world. While in other episodes we have focused at the macro level, in this episode we focus on the micro level, specifically at the platoon and company echelons. Focusing on brilliance in the basics is paramount for the success of airborne joint forcible entry operations, ensuring that every paratrooper is proficient in fundamental skills and tactics. I hope this information will help readers decide what biases I may have and subject my arguments to the appropriate degree of critical scrutiny. I first heard about EA upon attending Aaron Swartz's memorial in January 2013. One of the speakers at that event was Holden Karnofsky, co-founder of GiveWell, a charity evaluator for which Aaron had volunteered. Karnofsky described Aaron as someone who "believed in trying to maximize the good he accomplished with each minute he had." I resonated with that phrase, and in conversation with some friends after the memorial, I learned that Aaron's approach, and GiveWell's, were examples of what was, at the time, a new movement called "effective altruism." Despite my sympathy for EA, I never got very involved with it, due to a combination of introversion and the sense that I hadn't much to offer. I have donated a small fraction of my income to the Against Malaria Foundation for the last nine years, but I have never taken the Giving What We Can pledge, participated in a local EA group, or volunteered or worked for an EA organization. I decided to write this article after a friend forwarded me one of Torres's critical pieces on longtermism. I knew enough about this movement -- which emerged out of EA -- to quickly identify some falsehoods and misrepresentations in Torres's polemic. So I was surprised to find, when I checked the comments on Twitter, that no one else was pointing out these errors. A few weeks later, I discovered that this was just one of a growing number of articles by Torres that attacked these ideas and their proponents. Since I also noticed several factual inaccuracies in these other publications, I got curious and decided to look into Torres's writings more closely. I began to follow Torres's Twitter presence with interest and to investigate older Twitter feuds that Torres occasionally references. After looking into these and systematically checking the sources Torres cites in support of their various allegations, I found Torres's behavior much more troublin...

The Joint Readiness Training Center is pleased to present the forty-first episode to air on ‘The Crucible - The JRTC Experience.' Hosted by CPT Benjamin Malcolm of 1-509th IN (OPFOR) on behalf of the Commander of Operations Group. Today's guest is the Deputy Commanding Officer – Operations for the 82nd Airborne Division, COL Andrew Saslav, All American 07. COL Saslav was the COG from 2021-2022 and the commander of 11th DTG  (OPFOR notional higher headquarters).   The 82nd Airborne Division specializes in joint forcible entry operations via vertical envelopment, both airborne and air assault, into denied areas with a U.S. Department of Defense requirement to respond to crisis contingencies anywhere in the world within 18 hours. They have the Hollywood call-sign of “All American” Division and the motto of “In Air, On Land.”   In this episode we discuss how airborne forces would be utilized at the commencement of large-scale combat operations. Specifically, we look at how Airborne forces allow the US Army to set conditions for follow-on operations to meet national objectives anywhere in the world. The individuals that become paratroopers are some of the most versatile, capable, and lethal lightfighters in the world that pride themselves in seizing the initiative and aggressively bringing the fight to the enemy on the complex, dynamic modern battlefield. GEN McGavin, the longest serving CDR of the 82nd ABN said, “You show me a man willing to jump out of an airplane and I'll show you a man that will fight [and win] for his country.”   An interesting point that is discussed is that previously the DoD's took for granted that the US would have air supremacy immediately, but lessons from Ukraine has forced the DOD to reevaluate this concept. The 82nd ABN operates under the premise that they will conduct a JFE into a contested environment to seize a key piece of terrain and establish a lodgment for US forces. The Israeli government, its security forces, and Israeli settlers are actively targeting Palestinian communities through a growing matrix of tools geared towards making their lives so economically untenable, functionally unbearable, and fundamentally unsafe that they are compelled to abandon their homes and lands, disperse their communities, and give up their means of earning their livelihoods.  Israel's long-used methods include forbidding Palestinians from legally building homes and other structures, preventing access to infrastructure, including electricity, water, and roads, and ongoing demolitions in 60% of the West Bank. Targeted violence carried out by settlers, and tolerated if not actively supported by Israeli security forces -- - including harassment, threats, destruction of property, and outright terrorism in the formed of armed, lethal attacks -- has been used to suppress and threaten Palestinian life in the West Bank – and it has been accelerating in frequency and brazenness under the new Israeli government. Together, these factors have already led to the displacement of multiple Palestinian shepherding communities in the West Bank. An increasing number of states in India have implemented anti-conversion laws, supposedly in an effort to stop Hindus from being forcibly converted to other religions. In reality, however, these laws are often used as an excuse to harass, arrest, imprison, and intimidate Christians who distribute aid among their communities or hold private church meetings. Additionally, these laws do not seem to protect Christian converts from being coerced to return to Hinduism. Often, persecuted followers of Jesus in India do not have the financial means to hire lawyers who can help defend them against false charges. As a result, Ramchand Saranu – a trusted ministry partner of VOMC and long-time Christian leader in central India – founded the Sui Juris Law Firm in Indore, Madhya Pradesh, with the help of Sushil George.
