Segment 1: Blocks used in Head and Neck Surgeries Scalp Block: The scalp block involves blocking six nerves that provide sensory innervation to the scalp. It is performed by subcutaneous infiltration of local anesthetics (such as bupivacaine, ropivacaine, or levobupivacaine) for each nerve. Ultrasound guidance has improved the precision of block administration. The main indication for a scalp block is awake craniotomy, but it is also used in other procedures like deep brain stimulation and cranioplasty surgery. Scalp block offers advantages such as accurate neurological evaluation, pre-emptive analgesia, and hemodynamic stability during surgery. It also reduces postoperative pain, the need for rescue analgesics, and pain scores in the early postoperative period. Infraorbital Block (IOB): The infraorbital nerve block targets the infraorbital nerve, which supplies the skin and mucous membrane of the upper lip, lower eyelid, and cheek. The IOB can be performed using the classical landmark technique or ultrasound guidance. Ultrasound guidance provides real-time visualization and accurate needle placement. IOB combined with general anesthesia is beneficial for postoperative pain relief in procedures like endoscopic trans-nasal trans-sphenoidal (TNTS) approach for pituitary tumor excision. Other regional techniques like sphenopalatine ganglion block and maxillary nerve blocks have also been attempted for transsphenoidal surgeries. Trigeminal Nerve Block: Trigeminal nerve block is used for patients unresponsive to medical management of trigeminal neuralgia. Traditionally performed using the paresthesia technique, ultrasound guidance allows real-time visualization and confirmation of local anesthetic spread. Ultrasound guidance helps locate the Gasserian ganglion and visualize the trigeminal ganglion, providing a safe and radiation-free procedure for pain relief. Segment 2: Blocks used for Spinal Surgeries Cervical Plexus Block (CPB): CPB is commonly used in carotid endarterectomy (CEA) and cervical spine surgery. Different levels of CPB can be performed depending on the depth of injection. Superficial CPB involves injecting local anesthetic superficially into the deep cervical fascia. Deep CPB requires depositing local anesthetic deep to the prevertebral fascia. CPB helps in monitoring cerebral blood flow during CEA and provides postoperative pain relief. Ultrasound guidance can be used for superficial CPB, ensuring accurate needle placement and local anesthetic spread. Erector Spinae Block (ESB): ESB is used for pain control in spinal surgeries. It involves depositing local anesthetic in the plane between the erector spinae muscle and the transverse process. ESB provides effective postoperative analgesia and reduces opioid consumption. Regional anesthesia techniques play a crucial role in neurosurgery, providing effective pain relief and improving patient outcomes. Blocks like scalp block, infraorbital block, trigeminal nerve block, cervical plexus block, and erector spinae block offer numerous advantages in specific procedures. Ultrasound guidance has enhanced the precision and safety of block administration. These techniques contribute to improved surgical outcomes and patient satisfaction in neurosurgical procedures. Abstract: Cell-to-cell communication it is a fundamental mechanism by which unicellular and multicellular organisms maintain relevant functions as development or homeostasis. Tunneling nanotubes (TNTs) are a type of contact-mediated cell-to-cell communication defined by being membranous structures based on actin that allow the exchange of different cellular material. TNTs have been shown to have unique structural features compared with other cellular protrusions and to contain the cell adhesion molecule N-Cadherin. Here, we investigated the possible role of N-Cadherin and of its primary linker to the actin cytoskeleton, alpha-Catenin in regulating the formation and transfer function of TNTs. Our data indicate that N-Cadherin through its downstream effector alpha-Catenin is a major regulator of TNT formation, ultrastructure, as well as of their ability to transfer material to other cells. Abstract: Tunneling nanotubes (TNTs) are open actin- and membrane-based channels, connecting remote cells and allowing direct transfer of cellular material (e.g. vesicles, mRNAs, protein aggregates) from cytoplasm to cytoplasm. Although they are important especially in pathological conditions (e.g., cancers, neurodegenerative diseases), their precise composition and their regulation were still poorly described. Here, using a biochemical approach allowing to separate TNTs from cell bodies and from extracellular vesicles and particles (EVPs), we obtained the full composition of TNTs compared to EVPs. We then focused to two major components of our proteomic data, the CD9 and CD81 tetraspanins, and further investigated their specific roles in TNT formation and function. We show that these two tetraspanins have distinct functions: CD9 participates in the initiation of TNTs, whereas CD81 expression is required to allow the functional transfer of vesicle in the newly formed TNTs, possibly by regulating fusion with the opposing cell. Abstract: Tunneling Nanotubes (TNTs) facilitate contact-mediated intercellular communication over long distances. Material transfer via TNTs can range from ions and intracellular organelles to protein aggregates and pathogens. Prion-like toxic protein aggregates accumulating in several disorders, such as Alzheimer's, Parkinson's, and Huntington's diseases have been shown to spread via TNTs not only between neurons, but also between neurons-astrocytes, and neurons-pericytes, indicating the importance of TNTs in mediating neuron-glia interactions. TNT-like structures were also reported between microglia, however their roles in neuron-microglia interaction remain elusive. In this work, we quantitatively characterise microglial TNTs and their cytoskeletal composition, and demonstrate that TNTs form between human neuronal and microglial cells. We show that -Synuclein (-Syn) aggregates increase the global TNT-mediated connectivity between cells, along with the number of TNT connections per cell pair. Homotypic TNTs formed between microglial cells, and heterotypic TNTs between neuronal and microglial cells are furthermore shown to be functional, allowing movement of both -Syn and mitochondria. Quantitative analysis shows that -Syn aggregates are transferred predominantly from neuronal to microglial cells, possibly as a mechanism to relieve the burden of accumulated aggregates. By contrast, microglia transfer mitochondria preferably to -Syn burdened neuronal cells over the healthy ones, likely as a potential rescue mechanism. Besides describing novel TNT-mediated communication between neurons and microglia, this work allows us to better understand the cellular mechanisms of spreading of neurodegenerative diseases, shedding light on the role of microglia. Abstract: Protein import into mitochondria is an intricate and highly conserved process essential for organellar biogenesis, and maintenance of its structure and function. Defects in the import apparatus impact the assembly of the respiratory chain and ATP synthase complexes required for oxidative phosphorylation, compromising the ready supply of ATP to the cell. The consequences of reduced bioenergetic function are particularly severe for cells with high energetic demands such as neurons. However, relatively little is known about howdefectiveimportcontributestoneurodegeneration, or how neurotoxic proteins characteristic ofneurodegenerative diseases impact mitochondrial import efficiency. Here, we used HeLa cells to investigate how expressing high levels of Tau variants affect mitochondrial import activity, morphology, and function. We found that the variant associated with neurodegeneration (TauP301L) colocalises with mitochondria. TauP301L, but not wildtype Tau, interacts with TOM40, the protein-channel component of the outer membrane protein import complex. Interestingly, TauP301L expression had no discernible effect on overall mitochondrial import function, despite associating with TOM40 and altering mitochondrial morphology, suggesting that a rescue mechanism is at play. This rescue could be explained by the appearance of microtubule and actin containing tunnelling nanotubes (TNTs), used to recruit healthy mitochondria from neighbouring cells and/or dispose of mitochondria with aggregated Tau. Furthermore, in primary neuronal cultures TauP301L induces morphological changes that resemble a neurodegeneration like phenotype, and this is mirrored in cells where the import sites are blocked artificially. These results reveal an intriguing link between the production of aggregation prone protein variants, such as Tau, and the mitochondrial protein import machinery relevant to neurodegenerative disease. Seine Echos waren in einem Umkreis von 20 Kilometern zu hören. Auch die Einwohner der Hauptstadt Zyperns, Nikosia, spürten die Explosion trotz der Entfernung von 250 Kilometern in Beirut. Die Chronologie der Ereignisse am Hafen von Beirut Der russische Staatsbürger Igor Greschushkin aus Chabarowsk, der jetzt mit seiner Ehefrau Irina auf Zypern lebt und dort als Direktor der UNIMAR SERVICE Ltd. agiert, erhielt 2013 den Auftrag 2.750 Tonnen Ammonium Nitrat von der georgischen Firma Rustavi Azot LLC von Georgien nach Afrika zu transportieren. Zum Vergleich: Amatol ist ein militärischer Sprengstoff und eine kriegsbedingte Streckung des sehr teuren TNTs mit prieswertem Ammoniumnitrat (Kunstdünger-Komponente) erfolgte nicht nur im Deutschen Reich. I høst innlemmes TNT i Rockheim Hall of Fame! Jernverket møtte den klassiske utgaven av TNT under Tons of Rock i 2014: Ronni Le Tekrø, Tony Harnell og Diesel Dahl. Hvorfor spilles ikke "Tell No Tales" live? Hva gjorde jubileumsshowet i Trondheim til en god, men også dårlig idé? Hva gjorde TNT i en fornøyelsespark midt på natta? Og hvorfor i all verden tilbyr menn konene sine til Ronni? Siden intervjuet ble utført har bandet byttet ut flere medlemmer, og i dag er Baol Bardot Bulsara vokalist. Historisk sett ser det derimot ikke ut til at kjernetrioen klarer å holde seg unna hverandre, så hvem vet hva som skjer i fremtiden? Jernverket har i hvert fall sluttet med spekuleringen for lengst.
