My name is Margarita Fedorova and I'm a neurology resident at the Cleveland Clinic. Today we're reviewing a randomized trial that provides high quality evidence for treatment we've been using for decades, shunting for idiopathic normal pressure hydrocephalus. The PENS trial, a placebo controlled effectiveness and iNPH shunting trial was published in the New England Journal of Medicine in December 2025 by Luciano and colleagues. This international multicenter study enrolled 99 patients across the United States candidate in Sweden. While idiopathic normal pressure hydrocephalus or iNPH is characterized by triad of gait impairment, cognitive decline in urinary continence, these findings can be non-specific and we mass factor in radiological findings too. Furthermore, while CSF shunting has long been the standard treatment, its effectiveness has never been rigorously confirmed in a large well-powered randomized trial. In this trial, patients with a clinical improvement in gait velocity after temporary CSF drainage were deemed eligible for shunting and randomizing the trial. What makes this trial particularly elegant is its blending strategy. All 99 participants underwent the same surgical procedure with the same commercially available programmable shunt valve. After surgery, the valve was set either to an open functioning position or to a high resistance placebo setting. Neither patients nor assessors knew who had a working shunt. This is about as close to a true double-blind design as neurosurgery can get. The primary outcome was changing gait velocity at three months. The open shunt group improved by 0.23 meters per second on average, while the placebo group showed essentially no change in 0.03 meters per second. That's a treatment difference of 0.21 meters per second, both statistically significant and clinically meaningful. To put that in perspective, a change of 0.10 meters per second is considered the threshold for substantial meaningful change in the elderly. 80% of the open shunt group exceeded that threshold compared to only 24% of the placebo group.  The Tenet scale, which measures gait imbalance, also showed significant improvement in the open shunt group. However, screening measures for good condition using the MoCA scale and bladder symptoms did not reach significance at three months, though tertiary outcomes for cognitive testing, quality of life and functional independence tended in favor of shunting. Importantly, falls were more common in the placebo group at 46% compared to 25% in the open shunt group. This is a meaningful safety signal given how dangerous falls are in older adults. There were also real risks with active shunting. Subdural hematomas occurred in 12% of the open shunt group versus 2% of placebo and three even required surgical intervention. Positional headaches from low CSF pressure were more common in the open shunt group at 59% versus 28%. The good news is that the adjustable valve allowed non-invasive management of many of these complications. While this trial gives us reasons to be cautiously optimistic about shunting for appropriately selected iNPH patients, it's worth noting that we only have evidence for improvement in gait and follow-up is only three months.  Longer-term data is still being collected so we don't know yet how durable these benefits are. Cat Lo spent 20 years learning that execution excellence can actually hold you back. After burning out at Amazon doing beautiful work that nobody noticed, she distilled her hard-won lessons into five tenets, a framework that helped her shape billion-dollar opportunities and drive $218 million in impact.What if the very thing that made you a great designer is the reason you're being left out of the decisions that matter most?Cat Lo's career has taken a path most designers don't follow, starting with a bus ticket to New York City and no job waiting, through agency work, entrepreneurship, and eventually to Amazon, where she designs how millions of new products find their first customers. On this retrospective, the AW team take one last trip to the past for the year to talk about the pandemic year of cinema that saw one of the worst Oscar telecasts of all time. Baffling decision after another, the night ended on a sour note instead of one of celebration for Nomadland winning the top prize and Chloé Zhao becoming the second female director to win Best Director. Still, even with the ceremony being so divisive, the year presented plenty of films the team wanted to highlight and discuss over the course of changing the Academy's nominations, with films such as The Nest, Tenet, Miss Juneteenth, Never Rarely Sometimes Always, Clemency, Dick Johnson is Dead, I'm Thinking of Ending Things, The Father, Sound of Metal, Minari, Judas and the Black Messiah, and more. In their in-depth discussion, the AW team talked about the film year of 2020, briefly discuss talk about Nomadland as a Best Picture winner, how this year is full of celebrities that are problematic or "cancelled" and how that speaks to the legacy of their nominates and or wins, do an extensive conversation over the below the line categories and nominees for the year, and then the new version of the AW Shoulda Woulda Coulda game, where instead of individual replacements, they must decide as a group who the nominees and winners should be in the top eight categories. The rules of the game state they can only replace two of the nominees that year from each category, except in Best Picture, where the group could replace up to four films to make up the final set of eight nominated films. Like past retrospective episodes, it was a fascinating, fun conversation including spirited debates, alliances, vote swinging, celebrating various movies, performances that aren't normally talked about and more that we all hope you enjoy. How does a conversation start with a Silence of the Lambs quote hunt and end with an extremely detailed review of a bathroom upgrade that becomes a full debate about the physics of aiming, the ethics of “priming,” and what qualifies as a peanut-butter situation. It opens in full movie-nerd mode with Buffalo Bill voice impressions, actor trivia spirals, and side quests through Monk, Wings, and why a forgotten talking-parrot movie deserves more respect than it got. That turns into horror recommendations that feel like a trap, including Bring Her Back and the kind of scenes that permanently live in your brain once you have heard them, plus the rage that follows when a movie's logic falls apart the moment you think too hard about basic parental supervision. Then it slides into reality TV discomfort and social dynamics. Sister Wives gets analyzed like a hostage negotiation, especially the boyfriend meeting that turns into a job interview, complete with power plays, weird hypotheticals, and the kind of performative masculinity that makes everyone in the room sweat.The film talk gets meaner and more specific with a reassessment of Tenet that goes from “genius” to “actually terrible” once you start rewatching it, plus the broader Christopher Nolan problem of confusing the audience on purpose.
