Is it a good movie if you have to watch it ten times? That's the question Chris and Daniel keep circling back to as they take on Christopher Nolan's The Odyssey, skipping the marketing noise and the wireframe-cyclops conspiracy theories to actually ask whether the thing works as a film. They dig into what Nolan is doing as a filmmaker, structurally and emotionally, in his adaptation of the oldest story in Western literature, and why a three hour runtime built around a decade-long wait might be the whole point rather than a flaw. The real conversation is about adaptation itself: what happens when a director spends the entire movie making an audience feel exactly what an intractable wait feels like, and why Nolan's obsession with time connects everything from Memento through Tenet to this one. Daniel makes the case that Nolan has finally figured out how to deliver Tenet's disorienting rewatch effect without forcing anyone to actually rewatch anything. Chris and Daniel also compare Nolan's spectacle-forward approach to what a Denis Villeneuve version might have looked like, and get into why single-vision directors are becoming Hollywood's new IP. The end of our Nolan series approaches, as you can tell by the fact we've reached a film that doesn't care about beginnings, or endings, or - more than one of us might argue - what happens in the middle, either.This week we're watching/have watched/will have been about to be watching Tenet, which is like a time-based counterpart to the globe-spanning, dream-plumbing action of Inception. Not content with simply folding Paris back upon itself, Tenet goes one further and folds all of reality, with a war of annihilation moving back along the timeline from the future and about to engulf the present.What does this mean? On the one hand, we sincerely have no idea. On the other, it means bullets that fire backwards, fight scenes between opponents moving in contrary directions through time, car chases where both cars pursue each other at the same time(s), and a final assault that's finished before it begins.Obviously, Bill and Ted would have had this wrapped up in 90 minutes. That's one of the reasons Tenet is seen as a rare Nolan misfire, but as we look ahead to The Odyssey, and Nolan's status as a Hollywood blockbuster juggernaut, we will re-examine Tenet and what it can tell us about the director. In an attempt to make his own spy film, Nolan creates an uncompromising, complex, loose thriller about an unnamed secret agent working to take down a rich, Russian billionaire who has discovered objects from the future and brought them to the past to end the world as we know it. Along the way, our Protagonist enlists the help of our villain's wife and a fellow spy (unknowing it's his best bud) and with just the titular word as his guild, doors open to a puzzle he finds himself at the center of. Complicated, confusing, and downright brilliant, Nolan stops explaining everything in this film, instead, opting for the audience to "just feel it" and make a film off good vibes and technical wonder; and doing so, made one of his best films. Ryan, Jay, and Diego break down their love of this film, when they saw it and how they saw if given the COVID-19 virus, and it's questionable release schedule, Kenneth Branagh's accent, the use of Elizabeth Debicki's height, what Hollywood is going to do with John David Washington, how good looking Aaron Taylor-Johnson is, what makes a modern movie 5 stars on Letterboxd, their excitement over the phrase "temporal pincer movement," if they like hot sauce with their meals, and whether the movie makes sense and if that even matters at all. Dude, this episode hit different. I sat down with Jacob Albaugh Tenet, founder of Tenet Products, and what started as a guy from Ohio racing four-wheelers turned into one of the wildest entrepreneurial grinds I’ve heard on this podcast — and that’s saying something. Jacob was a national champion ATV racer before he ever picked up a rifle. Then he had a kid, watched his buddy crash at 45 mph, and walked away from racing cold. He poured that same obsessive, pro-level mentality into western hunting — and that’s where this story really takes off. After blowing out his eardrums on a mule deer at 32 yards, losing an animal off a cliff because he couldn’t spot his own shot, and surviving a blizzard SOS situation in Idaho that he says changed his life, Jacob became convinced there had to be a better way to hunt suppressed without sacrificing accuracy. For decades, suppressor design has forced a trade-off — get recoil control from a brake, or get hearing protection from a suppressor, but rarely both without adding serious weight. Tenet’s approach of engineering the suppressor itself (not just the brake) for shot control is what let them break that curve. Passion-driven problem solving can outperform decades of industry expertise. Jacob had no suppressor background and no entrepreneurial ambition when he started — he was just trying to fix his own hunting setup. His point that someone solving their own problem out of obsession will often out-innovate someone doing it as a job is a great reminder for anyone building something in any industry. Recoil management matters more than caliber size for accuracy in the field. The current trend toward smaller cartridges (6.5s, 6mm) exists because lower recoil equals better shot placement. Jacob’s insight — that you can get that same manageability out of a 7 PRC or 300 PRC with the right muzzle system — means hunters don’t have to give up knockdown power to get accuracy anymore.

In doing this, he created an undeniable masterpiece that took away some of his bad habits and makes a lean film that feels like an action set piece all the way through. Ryan, Jay, and Trace break down their thoughts on the film, Hoyte van Hoytema first collaboration with Nolan, each section of the war, how awesome Tom Hardy is in the plane we see in the film, the acting of Harry Styles, the sound work, and Jay's continued quest to remind us all that WWII is still underrated when we talk about important events in the world. My name is Margarita Fedorova and I'm a neurology resident at the Cleveland Clinic. Today we're reviewing a randomized trial that provides high quality evidence for treatment we've been using for decades, shunting for idiopathic normal pressure hydrocephalus. The PENS trial, a placebo controlled effectiveness and iNPH shunting trial was published in the New England Journal of Medicine in December 2025 by Luciano and colleagues. This international multicenter study enrolled 99 patients across the United States candidate in Sweden. While idiopathic normal pressure hydrocephalus or iNPH is characterized by triad of gait impairment, cognitive decline in urinary continence, these findings can be non-specific and we mass factor in radiological findings too. Furthermore, while CSF shunting has long been the standard treatment, its effectiveness has never been rigorously confirmed in a large well-powered randomized trial. In this trial, patients with a clinical improvement in gait velocity after temporary CSF drainage were deemed eligible for shunting and randomizing the trial. What makes this trial particularly elegant is its blending strategy. All 99 participants underwent the same surgical procedure with the same commercially available programmable shunt valve. After surgery, the valve was set either to an open functioning position or to a high resistance placebo setting. Neither patients nor assessors knew who had a working shunt. This is about as close to a true double-blind design as neurosurgery can get. The primary outcome was changing gait velocity at three months. The open shunt group improved by 0.23 meters per second on average, while the placebo group showed essentially no change in 0.03 meters per second. That's a treatment difference of 0.21 meters per second, both statistically significant and clinically meaningful. To put that in perspective, a change of 0.10 meters per second is considered the threshold for substantial meaningful change in the elderly. 80% of the open shunt group exceeded that threshold compared to only 24% of the placebo group.  The Tenet scale, which measures gait imbalance, also showed significant improvement in the open shunt group. However, screening measures for good condition using the MoCA scale and bladder symptoms did not reach significance at three months, though tertiary outcomes for cognitive testing, quality of life and functional independence tended in favor of shunting. Importantly, falls were more common in the placebo group at 46% compared to 25% in the open shunt group. This is a meaningful safety signal given how dangerous falls are in older adults. There were also real risks with active shunting. Subdural hematomas occurred in 12% of the open shunt group versus 2% of placebo and three even required surgical intervention. Positional headaches from low CSF pressure were more common in the open shunt group at 59% versus 28%. The good news is that the adjustable valve allowed non-invasive management of many of these complications. While this trial gives us reasons to be cautiously optimistic about shunting for appropriately selected iNPH patients, it's worth noting that we only have evidence for improvement in gait and follow-up is only three months.  Longer-term data is still being collected so we don't know yet how durable these benefits are.
