Welcome back to April's Papers of the Month! First up this month we have a think about posterior circulatory strokes, which can mimic benign vertigo, and can be really tricky to differentiate between a completely benign issue or a stroke that is really important to pick up. Clearly imaging everyone is one option but completely impractical when you consider the gold standard of MRI. So having a bedside tests to rule in or out the diagnosis of stroke would be a huge help. Our first paper looks at the use of three bedside scoring systems; HINTS, TriAGe+ and ABCD2 scores in ED for patients presenting with possible posterior circulatory strokes. How accurate are they and can we reliably incorporate into our practice? Next up is traumatic cardiac arrest. This has obviously got a very high mortality rate. One of the interventions that might improve mortality (for some mechanisms and patients) is a resuscitative thoracotomy, however this is a really significant intervention and we want to ensure we are targeting it at the patients that may benefit. A great paper has just been published from London Air Ambulance which might just help us to identify those patients who would benefit with more certainty, along with those where futility has already been reached. Finally we take a look at acute kidney injury (AKI) and its association contrasted scans. Contrast induced nephropathy (CIN) has historically been a concern, particularly for patients with chronic kidney disease, with contrast causing direct injury and limited blood flow. But recent studies have questioned the actual risk of CIN & there's been a more recent RCEM statement on the topic, but we thought this might be a nice opportunity to refresh and look at a recent paper on the topic. For the first time since the announcement of his cancer diagnosis, Charles was seen leaving Clarence House, accompanied by Queen Camilla, acknowledging well-wishers with smiles and waves.Prince Harry was informed of Charles's cancer diagnosis last week and consented to delay his visit to the UK until after the news was made public.After flying in from California today, Harry spent approximately an hour at Clarence House with King Charles, undertaking the trip from the United States by himself, leaving Meghan Markle and their children back home.It is unclear where Harry is staying.It's reported that King Charles personally reached out to inform Harry, along with his brother Prince William and siblings Princess Anne, Prince Andrew, and Prince Edward about his health situation last week. Following the official announcement of the King's condition by Buckingham Palace last night, Harry then proceeded with his plans to visit the UK.William currently has no plans to see Harry.Tomorrow, William is scheduled to resume his royal duties with an investiture at Windsor Castle and an evening event for the London Air Ambulance.Insiders close to Harry have indicated his openness to meeting with William during his visit, emphasizing that his main purpose in coming to the UK was to be with his father. One source noted, "The Duke's primary reason to travel to the UK is to visit his father. If the opportunity were to arise to see the Prince of Wales then the Duke would have gladly accepted it."A source close to William said: “The prince's main focus is for his wife, who is recovering from surgery, his three children and now his father. There are no plans for Harry's visit to act as some kind of vehicle for reconciliation.”

Let us begin. Matthew Sullivan: Very well, Dan. Thanks for having me on. Dan: Nice. So you have a bit of a history, which, I mean, maybe people already recognize the accent. But do you want to kind of talk about where you came from in your career and kind of how it evolved to what you are doing today? Matthew: Yes, of course. So I am originally from England and I moved to Southern California about seven years ago. And before that, I had spent about the last previous twenty-something years and really running my own businesses in finance, telecommunications, and technology. I think back in the late 90’s I had the amazing privilege of working closely with Sir Richard Branson as well for a few years and it was involved with a number of companies like Virgin Cosmetics, Virgin Clothing. I was a trustee of the London Air Ambulance that was sponsored by Virgin, so that was a fantastic time. I moved here with a big changed in circumstances back in England, so fresh start over here. And the first thing I did was just launch myself into real estate, which is something that I always wanted to do when I was in the UK, but I got distracted by the internet and other sorts of platforms. So, it was really a great opportunity to explore this fantastic universe of real estate. Dan: Nice. Okay, so a couple of things before we dive into some of our big topics here. But I am curious, I do not know a lot about UK real estate. I have heard that it operates a little bit differently, it is more constrained, there is not the type of supply of real estate that we have in almost all the markets in the US. Would you mind shedding some light on that assumption for me? Matthew: Yes. Well again, I did not have any great exposure to the real estate markets in the UK, so all of my real operating experience comes from the US. But I can tell you from my experiences as an investor, as a business person in the UK, property there, obviously it is a much smaller country, so you have got far fewer people and the number of opportunities is nowhere near as numerous as they are in the US. Real estate in the US is a multi-layered industry, which comprises everything from huge public company investments down to individuals that are fixing and flipping. There is a large private money universe. One of the things that you do not have so much in the UK, so you do not have that same depth to the private capital. And also there is just nowhere near the scale of real estate opportunities that you see over here. If you look at just California for example, the size of the Californian economy is not far off the size of the UK economy and so that just puts it into perspective. It is much bigger over here, much faster-moving, and is much more the bedrock of the economy as a whole. Matthew is the founder and full-time chief executive officer and director of Quantm.One, Inc and has held this position since the Company’s incorporation in December 2017. Since January 2015 Matthew has been the founder and president of Crowdventure, LLC, a real estate crowdfunding company. Matthew has also been a director of Secured Real Estate Income Strategies, LLC, (a real estate fund) since June 2016 and has been a co-founder and director of Secured Real Estate Income Fund I, LLC since May 2015. He is also the owner and president of Carbon Retirement, LLC, and has held this position since November 2014. In the late 90’s he spent a number of years working alongside Richard Branson and his corporate finance team and was involved in a number of high profile Virgin projects, including the Virgin Global Challenger – the first attempt to fly around the world in a hot air balloon. He was involved in the creation of the Virgin Clothing Company, Virgin Cosmetics, Virgin Helicopters, Virgin Executive Jets and a number of other Virgin companies and was appointed a director and Trustee of Virgin’s London Air Ambulance service. Building equity in your home unlocks massive leverage and cash flow, but thanks to an antiquated system, very few people ever get to put that to use. With $17.8 trillion of equity in single family homes, and 14.7 million homeowners with 50% or more in equity, this is a huge untapped asset class that’s right in front of us. An equity-based model takes a different approach because the investors are partners in the ownership of your home, rather than lenders, so you don’t lose your base equity. How some people are able to predict economic shifts and downturns For many people, the occurrences of this year have been a shock and seem to have come out of nowhere. The question is are you born an entrepreneur or do you become one? Matthew wasn’t one of those people who were entrepreneurs at the age of 7, but he has always been fascinated with challenges that were not the focus of his current job. He realized that he was essentially unemployable at that point. Solving Problems Finance is essentially a set of rules and processes and you can navigate that world the same way as technology. There are a lot of similarities between crowdfunding and the blockchain. Real estate is great because it is real and tangible, it’s something that we often take for granted. Combining real estate and technology comes with a lot of opportunities to create new efficiencies and increase the value of real estate as an asset class. Real estate is a tool, but it’s often slow and cumbersome. QuantumRE is trying to change that by creating a new asset, blockchain real estate. The Impact of Blockchain Real Estate When the blockchain was first introduced, most people didn’t see the importance of it, including Matthew. The blockchain is basically a layer of truth that cannot be changed, altered, or influenced. It means that you don’t require a third party to ensure an element of trust. The blockchain turns all of that trust infrastructure into programming language that ensures certain events will happen without any outside trust requirements. For real estate, if you could automate the process of buying a home you can increase the liquidity of real estate immediately and make international transactions more easily. The real friction with real estate is the time and the processes that are involved with changing ownership, and the reason for that is the world that it evolved. Blockchain technology will take real estate from analog to digital and eliminate the need for trusted third parties. Cryptocurrencies are a way of exchanging value on the blockchain, the key to the blockchain becoming widely adopted is people trusting the stability of future cryptocurrencies. QuantumRE plans to issue its own cryptocurrency that is backed by a portion of the equity in your home. The issues with HELOC’s is that lenders often stipulate how you spend the money, with QuantumRE the money is yours to spend as you want. It can actually be an additional source of funds for people who have reached the limit of their ability to borrow but still have equity in their property. Where did QuatumRE come from? Start by looking at the problem you are trying to solve. Great ideas are nice, but they don’t matter much without solving a specific problem.
