Today, we're diving into one of the most challenging and high-stakes procedures in trauma medicine—prehospital resuscitative thoracotomy (RT) for traumatic cardiac arrest (TCA). Joining me is Dr Mike Christian, the senior author of a recent study published in JAMA Surgery, which examines the impact of prehospital RT in London's Air Ambulance (LAA) system over two decades. TCA is often seen as a condition with a poor prognosis but for select patients—particularly those suffering from cardiac tamponade—prehospital RT has been shown to improve survival rates when performed within minutes of arrest. This study is the largest of its kind, analysing over 600 cases and shedding light on the effectiveness of RT, the critical time windows, and the factors that influence outcomes.In this episode, we'll explore the study's key findings, discuss the operational and ethical challenges of performing RT in the field, and consider what this means for the future of prehospital trauma care. Over the past three decades, the role of physicians in pre-hospital care has grown significantly. In this episode, we are joined by Dr Mike Christian. Beginning his career as a paramedic, Mike is now a critical care physician, researcher, and educator in PHEM. He specialises in interprofessional team care for critically ill patients in Canada. He has held leadership positions in PHEM and hospital systems in the UK and in Canada. Mike's research demonstrates the transformative impact of inter-professional teams, which bring together physicians, paramedics, and other specialists to enhance care delivery and improve patient outcomes. This episode explores the evidence behind these teams, shedding light on their ability to reduce mortality and improve survival for critically ill and injured patients. Thoracotomy: The current research and outcomes with Zane Perkins

In this session we will examine the latest research to emerge from Zane Perkins and Mike Christian around Resuscitative Thoracotomy (RT). The research has been led by Zane and Mike examined over 600 retrospective thoracotomy cases from the LAA database spanning 20 years, looking at the survivors, the pathology (exsanguination versus tamponade), the pre-arrest rhythms, the morbidity and mortality, the time of intervention versus outcome as well as other markers. I wanted to explore the results of this research and the potential implications on practice. Zane Perkins is a consultant Trauma and General Surgeon at the Royal London Hospital, a consultant Physician for London's Air Ambulance, and an Honorary Senior Lecturer at the Centre for Trauma Sciences, Queen Mary's University London. Current thinking on Thoracotomy practice Examine the research in more detail from primary & secondary outcomes Survival rates - Who are the current survivors? What are the main domains of pathology Exsanguination: Results of outcome and pre-arrest rhythms What it shows around intervention and decisions around exsanguination Tamponade: results of outcome and pre-arrest rhythms What it shows around intervention and decisions around tamponade Differentiated decision making and prospective changes to SOP Final thoughts and take-home messages. In this episode, WEM Founder Mark Hannaford is joined by WEM Conference Speaker, HEMS Doctor and former Paramedic Mike Christian.    Mike tells us how he took the leap from working as a Paramedic in Canada, and fuelled by curiosity, decided to go back to University to train as a Doctor. Following his graduation, his interest in infectious diseases and the emerging SARS outbreak led him to further Critical Care Training and an Infectious Diseases Fellowship. This launched him down a path that has included time in the Canadian Military and as part of the London Air Ambulance team as a HEMS Physician.    Alongside Mark, Mike considers some of the best lessons he took from his earlier training and has kept with him even today - from building rapport with patients, to being resourceful in front-line environments and the power of asking good questions.    Mike also offers some advice to those wishing to pursue similarly unique careers paths to his own; from taking risks and pushing yourself out of your comfort zone to utilising your 'imposter syndrome' to drive you to be a better clinician. In this episode, Matt Walton joins WEM Trauma Lead Eoin Walker to provide an update on his experience as a Junior Doctor on the frontline in London. Matt details the rising level of fatigue amongst colleagues as they've spent a number of weeks tackling and treating the enormous amount of unwell patients presenting at A&E, including fellow staff members. Matt comments on the bravery and resilience of the staff as they continue to provide care.  Eoin and Matt also consider the need to have 'difficult conversations' much earlier in the patient journey. Matt highlights the importance of being proactive to ensure their priorities, thoughts and last wishes are carried out. Matt describes how his role has morphed in the face of COVID as his role as a clinician is stripped away when supporting patients unable to be with their family in their final moments.   The pair also discuss the incredibly sad news that a number of NHS staff have died due to COVID19. Matt draws on his own experience to share what he hopes will be useful tips for others faced with similar situations. Matt shares the mix of emotions him and his team faced, from disbelief to sadness and denial, as well as the importance of acknowledging that such sad news will affect your performance and takes steps to compensate for this. Matt also shares his thoughts on how acts of remembrance can be carried out amid the pandemic, reflecting on a common practise used by the military to put aside time after the firefighting is over to reflect on that person, permitting people to experience the sadness and grief that may sometimes be suppressed due to high caseloads. Matt shares his thoughts on his latest paper, co-authored by Esther Murray and Mike Christian that provides a toolkit for those wanting to help support staff mental health at this time. The paper is split into three key forms of assistance; individual, teams and organisations. Matt considers how some of these ideas have already been implemented within his own hospital, from basic needs such as tea and coffee facilities to practising good leadership and encouraging open discussion.  Matt also takes a moment to consider the efforts of the nation and the positive impact this is having, from adhering to the lockdown to the heartwarming 8pm 'Clap for the NHS' every Thursday evening and the epic fundraiser Captain Tom Moore.      Works Referenced:  Matthew Walton, Esther Murray and Michael D. What if we ran Church Online not like a Pastor would run a church, but like a missionary would run a missional movement. Crazy idea, I know. But that's really what we set out to do through Christ Fellowship Miami years ago. To that end, we're introducing Mike Christian to The Church Digital Podcast today. Mike worked with Jeff over at Christ Fellowship Miami as the Online Small Groups director. Mike was a vital part of Christ Fellowship's Online Strategy of Groups and Microchurch movement. The effectiveness of the movement was grounded in a simple-to-say, hard-to-execute philosophy: how can Church Online effectively create disciples for the purpose of multiplication. Honestly, Mike had a little bit of experience in this area before Church Online. Mike served as a missionary in India for a number of years, and was actually had his passport revoked by the Indian government. While India was trying to squash Mike's ability to disciple people in the country, Online Tools (and the Holy Spirit, of course) have allowed Mike's ministry to expand far beyond anyone would think possible. As a result, churches are being planted and multiplied in India, in Nepal, in Kenya... all because of a man's faithfulness to be obedient to make disciples of all nations utilizing Church Online. The "Iron Warrior" Mike Christian is back on the bodybuilding scene and is ready to pick up right where he left off years ago. The bodybuilding legend talks about how bodybuilding saved him from a life of crime on the streets and how he became a huge favorite among the elite competitors of his time, the muscle magazines, and the bodybuilding fans throughout the world. Mike reminisces about how thrilled he was with his second place finish against 8-time champion Lee Haney at the Mr. Olympia and how the magazines pitted them against each other as rivals but he realized that he had gone as far as he could go. When Vince McMahon and his upstart WBF organization came around and wanted the charismatic stage showman so badly, Mike jumped all over the opportunity and the $300,000 contract he was given. Unlike many of the other IFBB professionals that took the same route, Christian never once regretted his decision. Mike also talks about the successful international gym clothing line he created, Platinum Everywhere, and all of the opportunities that it afforded him. Christian suffered a lot of challenges through his life and he tells you about a lot of them during this interview.
