Neurodiversity is something paramedics encounter every day, whether we consciously identify it or not. It is present in the child who will not tolerate the blood pressure cuff, the adult who cannot answer questions in sequence, or the patient who appears agitated under blue lights but settles in a quiet room. Autism and ADHD are not rare edge cases; they are common neurodevelopmental differences that shape how people process information, regulate emotion, experience sensory input, and respond to stress.Both are frequently hidden. Many adults remain undiagnosed. Others mask extensively, particularly in public or professional settings, meaning the first time their coping strategies fail may be during illness, injury, or crisis. In the pre-hospital environment, defined by urgency, noise, bright lighting, unfamiliar touch, and compressed decision-making, those coping mechanisms can collapse quickly. What follows may look like resistance, inconsistency, hostility, or disengagement. In reality, it is often cognitive overload or dysregulation.Autism and ADHD also overlap significantly. Executive dysfunction, sensory sensitivity, communication differences, emotional lability, and difficulties with working memory can coexist. For paramedics operating under time pressure, rapid behavioural interpretation becomes almost automatic: cooperative or not, intoxicated or sober, compliant or difficult. When neurodivergence is not considered, behaviour is framed as intentional rather than neurological.Today on the Pre-hospital Care Podcast, I'm joined by David Birch to unpack this in practical terms. We explore how ADHD and autism actually present on scene, beyond stereotypes, and why behaviours such as poor eye contact, restlessness, blunt communication, or inconsistent histories are so often mislabelled as non-compliance or intoxication. We examine the clinical risk of those assumptions, from premature diagnostic closure to inappropriate use of sedation or restraint.Most importantly, we discuss what paramedics can do differently. Not complex protocols or lengthy interventions, but small, deliberate, patient-centred adjustments: simplifying language, reducing sensory load where possible, allowing processing time, offering clear structure and predictability, and reframing behaviour through a neurodiversity-informed lens. In pre-hospital care, few diagnoses carry the same urgency as stroke. But not all strokes present the way we expect, and that's where the risk lies. In this episode, we explore the challenge of recognising posterior circulation strokes, using both clinical insight and lived experience. I'm joined by Shane Devlin, a registered paramedic and newly appointed PhD researcher focusing on posterior stroke recognition. His work builds on frontline experience and a growing body of evidence that highlights just how easily these patients can be missed.Alongside Shane, this conversation is grounded in a personal story: my brother, Ronan Walker, who suffered a posterior stroke 4 years ago in his late 30's. Many people will be cared for at home by specialist community palliative care services. Despite this, hospitalisation rates remain high for people receiving specialist community palliative care services. Carers face physical, emotional and financial burden caring for someone with palliative care needs at home.   What this paper adds Provides a deeper understanding of the contextual and individual determinants of why people receiving specialist community palliative care access unplanned hospital care. Provides insight into specialist palliative care carer behaviour at the end of life. Highlights the importance of the carer's role in decisions surrounding acute care access for people at the end of life.   Implications for practice, theory or policy Further research is needed to capture the perspective of people receiving specialist community palliative care regarding the reasons for unplanned hospital use. The number of unplanned hospital presentations for people receiving community palliative care may be reduced through the provision of better formal support for their carers. The goal is to help care teams see patients as whole people beyond their diagnoses and to give patients an opportunity to share interests, hobbies, and fun facts that can help build trust and meaningful human connections. This About Me Initiative is being expanded from inpatient care to outpatient settings, such as infusion centers. Paul explains, "The About Me section in our EHR Epic is something that NYU Langone developed with the inspiration of Dr. Hochman, and it's essentially a dedicated discreet space in the medical record to allow patients to declare themselves, allowing them to give them a space that is transparent to all the users of the electronic health record, which are many and vast in their rules. So let them articulate something beyond their disease process or their health. So often, we reduce records to pneumonia, a broken hip, or an infection. The patient is so much more than that, and patients want to share that with us. We know that from practicing for years, Kathy and I both know that the way you connect with patients is through a narrative. Our currency is narrative. This is a location where either a patient or, if they choose to defer, can have one of their nurses or care team members provide a small sample of who they are outside of the care environment."   Katherine elaborates, "So I'm a hospitalist. Every Tuesday, I come into 16 to 18 new patients. And at NYU Langone, our length of stay keeps getting shorter and shorter, which is fantastic for patients because we want to get them in and out of the hospital as quickly and as safely as possible. A first time mum living on the land of the Wurundjeri and Bunurong Peoples. She shares her pregnancy story and how her frequent engagement with the hospital system due to her debilitating hyperemesis gravidarum influenced her decisions around what model of care she wanted and hoped for. Ultimately, despite back and forth and advocating for the same, L was unable to access midwifery continuity of care or a homebirth through the publicly funded model. He serves as the association's chief liaison on these issues with the Centers for Medicare and Medicaid Services and the Medicare Payment Advisory Commission. Working closely with the senior finance and policy executives in the Federation's member companies, Steve develops and carries out both issue-specific and general strategic plans designed to advance the finance and payment related regulatory and legislative interests of the FAH.Prior to joining the Federation, Steve served as Associate Vice President, Policy Coordination and Communication for the Health Industry Manufacturers Association (now AdvaMed), the national trade group representing the medical technology industry. Before moving to Washington, Steve held a succession of increasingly senior management and policy positions in Massachusetts. During his time in the Bay State, Steve served as Legal Counsel to the Lieutenant Governor, Legislative Counsel for the Executive Office of Health and Human Services, Executive Director of the Disabled Persons Protection Commission, and Legal Counsel and Policy Director in the Office of State Health Planning. Steve also taught health law and policy as an Assistant Professor at Simmons College Graduate Program of Health Administration.At the federal level, Steve served in the Food and Drug Administration's Office of Legislative and Congressional Affairs. He also worked in the Environmental Protection Agency's Office of General Counsel.Steve earned a J.D. degree from American University's Washington College of Law; a Master in Public Health degree in Health Administration from the University of North Carolina School of Public Health; and a Bachelor of Arts degree in Anthropology/Zoology from the University of Michigan.
