Recently, a horrific and unimaginable birth event made national (USA) news. A case of shoulder dystocia resulted in an extremely morbid and rarely witnessed complication. On August 16, 2023, an extremely important review article was released (ahead of print) in the American Journal of ObGyn. This article summarizes important key aspects and tips for the proper performance of rescue maneuvers for shoulder dystocia, with the aim of keeping both the mother and the baby safe. In this episode, we will review these critical and vital tips for safe resolution of shoulder dystocia. Should the buttocks rotate upwards with McRoberts maneuver? What is the Ruben I procedure? We'll get to all of this, and more, in this episode.

Encore: Leah and Her Midwife Handle a Complication

Show Notes: Leah is a registered nurse engaged to a UFC fighter, Brandon Davis. They have just grown their family with a new baby! She had a homebirth with her first baby. When asked how she decided to have a homebirth, she said she's always been fascinated with it, but she doesn't have any friends or family members who had had homebirths previously.  That being the case, it took a large amount of research on her part to come to the decision and know for certain she wanted to give birth at home. As a registered nurse, she did have some negative reactions from coworkers and friends about her decision. “I think that labor is best left untouched.” Leah's fiancé was very uncomfortable with the idea of a homebirth. Early on, she asked him to visit several care providers, and he quickly disagreed, saying she needed to give birth in the hospital like every other “normal” mom! Not wanting to argue, Leah began seeing an OB. She continued with this care provider up until week 26, when she began feeling very uncomfortable with the idea that she was now going to be having a hospital birth. She decided to set up a quiet little meeting with a local midwife without involving her fiancé until afterwards. She immediately felt connected with this midwife and said, “This is it.  I know that whatever my birth story is going to be, you are going to be a part of it.” Leah continued to see both her OB and her midwife up until week 36, where she completely transferred care over to her midwife.  Though she liked her OB, Leah began feeling pushed to do things she didn't want to do, including a  3rd trimester ultrasound and weekly cervical checks after 35 weeks. She felt that her prenatals were far more in depth with her midwife; they discussed issues like nutrition, which never happened with her OB- not because she didn't care, but simply because there was just not enough time. In fact, her OB was so busy, that she had a patient due every single day of the month that Leah was due. She had warned Leah that she would likely not be the person delivering her baby anyway. Leah's midwife was able to discern that her baby was posterior, and explained how that had the potential to complicate, or at least prolong, labor. She recommended chiropractic care and yoga to help get baby in a more optimal position.  She appreciated how much more hands-on her care was with her midwife. The OB pushed for a 3rd trimester ultrasound to see baby's position, but her midwife already knew baby's position by consistent belly palpation.  “I felt like she was almost a sister or a mom to me, as well as a care provider.” Leah decided to work up until the day that she gave birth.  She was healthy and her baby was healthy, so she decided to continue. During her 39th week, she got up on a Friday to go to work and noticed some bloody show. She let her midwife know, and asked her what that meant in relation to when labor would start.  Her midwife responded that it could be any time between the next 24 hours and week! Leah went to work and worked the full 12 hour shift. She was on her feet all day, and by the time she got home, she was feeling crampy and “not good,” but she hadn't realized at that point that she was on the cusp of labor. Leah tried to go to sleep early, but couldn't sleep. Although she was feeling crampy, she thought that maybe she was just experiencing Braxton Hicks contractions.  At about 3:30 in the morning, the discomfort was feeling less tolerable.  She thought that maybe she should start timing them to see if they were actually contractions.  At this point, she was having contractions every five minutes.  At 4:30, she called her best friend who lives hours away and let her know that she thought today was going to be the day, so get in the car and head her way whenever she was up and ready. At 5, Leah called her midwife and let her know what she was feeling. An hour later, she called her midwife back and let her know that she was still consistently contracting. At 8:30, Leah's midwife arrived. Leah says, “I remember walking to the door and saying, ‘Hey…I feel like crap.'” Her midwife checked her and said, “Strong work, girl! You're a 6!”  Leah was very happy to know that her contractions were doing something! Leah's midwife had an assistant with her, who also acted similar to a doula during the experience. In the late afternoon/evening, she was checked again and was at 9cm with her bag of waters still intact. Her midwife gave her the option of continuing with labor uninterrupted or breaking her water.  Leah decided to wait and see and did not want her water broken at that time. She labored on for another hour and began feeling exhausted. At that point, she decided to have her water broken.  She as laboring on the bed at this time, but her midwife told her she should try to decide where she wanted to be when they had the baby since they were getting close.  She labored for about 26 hours total, but only spent about 45 minutes in the pool.  She decided she wanted to go back to the labor pool. Leah's midwife told her that would be fine… but she was worried that they wouldn't actually have time to get the birth pool set back up before baby arrived. Leah decided to go for the next best thing- her shower! At this time, she was already feeling pushy.  With her next contraction, she made a clear pushing sound, so her midwife came back into the room…. Only to realize that Leah had already birthed the baby's head! Her midwife came behind her and provided perineal support to help prevent tearing. After a few pushes, Leah's midwife that the baby wasn't coming out.  She asked Leah to give her everything she had during the next push.  At this point, Leah was doing all that she could, and wondered what was happening.  Her midwife told her that she would need to get out of the shower if baby didn't come with the next push. Baby still did not come, so Leah stepped out of the shower/tub and her midwife had her get onto her hands and knees on the bathroom floor. Her midwife had her alternate lifting one knee into a runner's lunge, and then the other knee.  Back and forth, back and forth.  Baby still was not coming, though Leah's midwife did free a nuchal cord that was wrapped twice around his neck. At this point, about 5 minutes had passed. Leah's midwife had her lie down on her back and lift her knees up as high as possible (this is called the McRoberts maneuver).  Baby came at this point!! Leah's baby was not crying or vigorous when he was born- he was absolutely exhausted. It took a bit of stimulating him to finally get a solid response.  Of course, he was attached to the cord this entire time, so he was still receiving oxygen via the placenta.  Even after this entire situation, Leah did not tear at all during her birth. She and her midwives spoke after everyone had calmed down. Leah was curious to hear how this situation would have played out in the hospital.  She learned that she certainly would have received an episiotomy, which she mentions is not a very helpful intervention because a shoulder dystocia is not a skin-on-skin issue, but rather a bone-on-bone issue.  Leah says that she felt so good postpartum that she probably didn't take it quite as easy as she should have. Postpartum bleeding ended up being somewhat of an issue. She had a hard time just lying in, so she admits that she ended up doing too much and being on her feet too much! She felt such a sense of empowerment after the experience, and so did her fiance. He was very upfront that he didn't want her to give birth at home, but after the experience, he couldn't stop bragging about her. He was so happy with their birthing experience.   Episode Roundup This is an amazing example of a rather large complication being handled at home. Notice how calmly and skillfully Leah's midwife managed the shoulder dystocia.  She had her get up, then hands and knees, then alternating lunges, and finally had her move onto her back with her legs pulled up.  She also knew the importance of course of keeping the cord attached so that Leah's baby still had a direct flow to oxygenated blood, even if he wasn't quite breathing yet. Just because you're feeling empowered and strong doesn't mean you should go make dinner! It can be challenging to find time for rest and relaxation. We are often consumed with work, and the busyness of life makes it hard to slow down and connect with Jesus. However, it is essential to recognize that work and rest are complementary. They are not opposing forces, but rather two sides of the same coin that draw us back to the same source. In his new book, Sacred Strides, speaker, musician and coach Justin McRoberts explores the idea of work and rest and how they can be transcendent experiences that connect us with the heart of the divine. McRoberts argues that work is not the enemy, but rather, can bring us closer to God. He suggests that rest is not about divorcing ourselves from the world, but about disconnecting from the machinery that keeps us from having transcendent experiences. McRoberts' perspective is refreshing because it challenges the notion that work and rest are at odds with each other. It is not a matter of choosing one over the other, but rather finding congruence between the two. When we work from a place of rest, we can experience the joy and fulfillment that comes from doing meaningful work. Conversely, when we rest from a place of work, we can recharge our batteries and reconnect with the divine. Work and rest are intertwined. There is a common belief that people are either good at resting or good at working, but this is a false dichotomy. Work and rest are complementary and that they are both necessary for a fulfilling life. Rest is essential for wholeness. It can be difficult to capture the balance. Many of us have been taught a narrative of being rescued from all the bad things in the world, including work. We have been conditioned by hustle culture to believe that our limitations are problematic and that we must constantly strive to do more. This narrative is not rooted in the biblical narrative of wholeness, which teaches that our limitations are actually part of what makes us human and that work can be a sacred and fulfilling practice. Rest is a necessary gift. The podcast emphasizes the importance of rest in our lives. It is not just a luxury but a necessary gift that we need to embrace to maintain our well-being. Rest is not just about taking a break from work, but it is also about finding joy in being present in the moment. It is about embracing our belovedness and allowing ourselves to be who we are. Belovedness destroys our need for success. Our belovedness annihilates the need for success. McRoberts argues that we live in a culture that glorifies success and achievement, which often leads to burnout and a sense of emptiness. He suggests that by embracing our belovedness, we can find a sense of wholeness that transcends the need for success. She has been training as an actor/performer since the age of 5 and as a professional performer has not only performed Canada wide, but has performed Internationally. She graduated George Brown Theatre School's Classical Theatre Conservatory Program and was immediately launched into a professional career predominantly as musical theatre performer. She has received awards and accolades along the way such as the Second City Comedy Award from her theatre school, A Dora Mavor Moore Award as part of the ensemble of The Wizard of Oz at Young People's Theatre in Toronto, Ontario, and has received several nominations for her work on stage. Some of her favourite credits include Rock of Ages at Stage West Calgary, All Shook Up at Stage West Mississauga, The Baker's Wife with Talk Is Free Theatre performed at the iconic Winter Garden Theatre in Toronto, April Fools at the Segal Center in Montreal where she was able to put her musician skills to work as an actor/musician and her performance in For Both Resting and Breeding which was performed in Adelaide, Australia. Born in Australia, Nicholas McRoberts studied piano, composition and conducting at the Melbourne Conservatory, the Victorian College of Arts, the CNSM in Paris and the Ecole Normale. He studied conducting with Robert Rosen, Jorma Panula and Dejan Savić.Today he is based in France. His works include operas, symphonies, ballets, voice, piano and chamber works.His Symphony No. 1 “From the Old World” was premiered by the Sofia Philharmonic on the 27th of August 2022 with the composer conducting. His "Adagio for Strings" was commissioned for the Nürtinger Chamber Orchestra and first performed on November 14, 2021, conducted by Friederike Kienle. It was written during the Covid lockdowns and recorded with the Janaček Philharmonic in March 2021. The French première was given by the Ensemble Orchestral de Biarritz, conducted by Yves Bouillier in July 2022. In September 2019 he was named conductor of the Orchestre Démos du Grand Verdun with the Philharmonie de Paris. In 2018 he was named artistic director of Opéra Montmartre in Paris. In 2017 he collaborated with the French choreographer Nawel Oulad on a ballet Les Tisseuses de Silence and a duo Femme au Piano for the Semaine de la Danse in Paris which were performed in the Festival Les Aliennes and the Festival Appel de la Lune. His Violin Concerto was recorded in 2017 with the Janáček Philharmonic Orchestra. His operas include "Lyon" (2016) premiered by the Ruse State Opera in July 2016, and "Nera" (2017) adapted from the play "Devojka Modre Kose" (The Girl with the Midnight Blue Hair) written by the Serbian playwright Vida Ognjenović. His composition "Festival Fanfare" was the recipient of the 2002 OpenBook Award for Sacred Music. Zach McRoberts is a former college basketball player at Vermont and Indiana University. He joins Jimmy to discuss their shared high school experience at Carmel, winning back-to-back state titles for the Greyhounds under head coach Scott Heady, and growing up in a basketball family. Zach's brother, Josh, was a highly recruited player out of Carmel High School, being named a McDonald's All-American and projected as a lottery pick in the 2005 NBA Draft if he declared. Josh would go on to play 11 years in the NBA for 7 teams, including his hometown Indiana Pacers. *Pulling back the curtain on the foster system, a foster family and case workers explain the joys and challenges of fostering * (0:06) Tom and Anne Marie Malysz of St. John Fisher Chapel University Parish in Auburn Hills discuss how becoming foster parents has changed them, relying on the Lord's help. (2:09) In Part 1, the Malysz family talks about how they became involved in fostering, the expectations they had going in, and how divine providence played a role in helping the couple agree to take on the responsibility. (6:11) The couple takes an orientation class, undergoes an arduous licensing process, and is paired with a case worker. Despite stereotypes to the contrary, the couple says they were pleasantly surprised by the painlessness of the process. (7:02) The Malysz family welcomes their first foster children, an infant boy and his two-year-old sister. They discuss how their biological children reacted and what it was like. Since then, they've fostered a total of five children. (8:20) The couple discusses the challenges and joys of fostering, including the knowledge they are helping a family in need. For the Malyszes, their Catholic faith is a big component of it all. (11:28) The couple admits it's bittersweet when a child leaves their home, because they don't often get further contact with the child. But they hope that somehow, the child's future was changed for the better as a result of their stay. (13:30) In Part 2, Kristie Harden, director of child welfare for Catholic Charities of Southeast Michigan, and her staff explain the ins and outs of the foster care process. Hardin discusses the paperwork, training and background checks involved. (16:15) Hardin discusses how a child becomes removed from his or her birth family and is placed in the foster system. The goal isn't to place a child with a foster family, however. The goal is always reunification with the child's birth family. (17:25) Lyda McRoberts, one of CCSEM's case workers, talks about how the system encourages reunification by working with parents and children on treatment plans and services. She is in constant communication with a child's birth parents, and supervises planned visits. She also conducts regular checks with the foster family to ensure the child's wellbeing. (20:07) McRoberts talks about what a supervised visit looks like. It's awkward at first, she admits, but important to see how birth parents interact with and care for their children. (24:07) While reunification is the ultimate goal, it's not one that's often met. That makes case workers' jobs seem exhausting and thankless, but the interest of children and the cooperation of foster families makes the sacrifice worth it, McRoberts says. (24:49) Ross Henson, a licensing specialist with CCSEM, discusses the process of ensuring a foster family is ready to take on a child, including home visits. Henson discusses the shortage of foster families willing to take on children, making his job even more important. (29:48) Hardin offers an appeal to those considering becoming foster parents: Educate yourself, but don't be afraid of the process. It's not scary, and for those who are called, it can be one of the most rewarding experiences in the world. In this episode of the I Am A Champion show, the guys are joined by Coaches Steven McRoberts, and Taj Dashaun to discuss how we can thrive through transitions in both life and sports. In 25 years as a collegiate head coach, including 17 at the Division I level, Steven McRoberts owns a 619-265 (.700) overall record with 23 winning seasons and 19 years with 20 or more victories. He ended the 2021 season ranked 19th among active Division I coaches in total victories and 26th in winning percentage. A 10-time conference coach of the year, McRoberts' teams have won a combined 13 conference regular season, tournament, and national postseason tournament titles. He has mentored a total of 20 All-Americans, 70 all-conference players, 11 conference players of the year, 12 Academic All-Americans, and 29 total specialty award winners at the conference level. McRoberts came to Missouri State after a six-year tenure at Ole Miss, where he posted a 111-82 record and guided the Rebels to the 2017 National Invitational Volleyball Championship title, the first postseason national championship for any program in the school's history. He has also coached at the University of Tulsa, University of Central Arkansas, and Lubbock Christian University. He has been married for 18 years to his lovely bride, Stacy, and they have three kids: Jackson (17), Mollie (14), and Nate (9) Taj Dashaun is an Author, Speaker, and Athlete Transition Coach. When his football career ended, Taj struggled heavily with depression. Because he was so focused on the game, he never fully developed a clear vision for his life. After several years of stumbling around in the emotional darkness of uncertainty, Taj began to take his eyes off of himself and focus on serving others. He now runs holistic programs for retired athletes who, like he once was, are struggling to adapt to life after the game. Not only does Taj help former players land jobs and start businesses, he helps them find fulfillment in every area of their lives. It's become his personal mission and passion to reach out and help others overcome the fear, frustration and uncertainty associated with life post-athletics. Justin McRoberts is an American author, speaker, retreat leader and songwriter from the San Francisco Bay area.
