Janine, she is a Jersey Girl and a 25 year veteran of the entertainment industry having created and implemented tactical public relations campaigns, events, and marketing promotions for numerous studios including HBO, Universal and Cohen Media Group, where she led the publicity campaign for the Academy Award®winning film, The Salesman. More recently she has made the transition into the development and production creating 2nd Chapter Productions.  She is producing and directing her first documentary feature “Unheard: The Ears of Menieres”.  This is a passion project that will unravel the mystery surrounding Meniere's Disease. With this documentary Janine hopes to take people on a journey to understanding, on a deeper level, what living with these symptoms is really like using visual and audio simulations so the audience actually experiences what its like to have vertigo, tinnitus and hearing distortion. It is a common cause of hearing loss. Scientists don't yet know the cause. They think that it has to do with the fluid levels or the mixing of fluids in the canals of your inner ear. Symptoms occur suddenly and can happen as often as every day or as seldom as once a year. This week I am talking with Dave Giugno, about his vestibular journey with Meniere's Disease which ultimately led him to a left sided labyrinthectomy. His symptoms began Christmas Eve 2014 and became chronic in 2018. Prior to his vestibular journey he sang baritone in the church choir and was an avid photographer. He has turned a hobby of video editing into a passion project for Menieres patients on his YouTube Vlog titled “Meniere's Disease Warrior”. He shares openly about his day to day life with this disease. He is also the cofounder of two groups on FaceBook, Meniere's and Vertigo without Borders and Meniere's and Vertigo with Christian borders. His wife began the support group Partners of Meniere's and Vertigo without Borders, which is directed more to the significant others and partners of loved ones with these vestibular disorders. Dave has found living with Meniere's has given more patience and has also allowed him to watch his youngest son grow. Simple everyday words seem like a struggle.  You might be experiencing the cognitive changes people often refer to as “Brain Fog” Brain fog isn’t a medical condition. But it is an experience lived by people. And it impacts their quality of life. There are symptoms, a cluster of symptoms that indicate whether what you’re going through could be brain fog- but that’s not really the area to focus on- more so- what can you do about it. The experience though varies from person to person. One person’s experience of brain fog could be very different from another. It could be very light and minor.  Or it could be so severe and significant that it’s already an impairment to your life. How you are dealing with memory problems, poor concentration, and lack of mental clarity.... When it comes to your ability to do things such as drive a car, manage your finances, pursue your hobbies. Have fun interactions and make conversations. Know this one important thing. Brain fog doesn’t have to be a permanent fixture in your life. There are tests that you can do. There are treatments available. You don’t have to live the rest of your life behind that glass pane. Life can become clear again! So make your move. If you know the cluster of symptoms associated with brain fog then you’ll know whether or not you’re experiencing it. The next step is to understand what could be causing your brain fog. Because when you know what’s causing it then you know what to do next. It is often that the causes determine the action. Address the things causing your mental fogginess. Take charge of your brain health and get clear. When your mental focus is taking a dive, that’s the signal for you to dive in as well.  Take things into your hands.  Attack the causes associated with it.  Because that’s what you have to be. You have to stay vigilant and safeguard your cognitive fitness for as long as possible. So that you can get control and live your life on your terms.   About Our Host David Norris is an occupational therapist who has been in this practice for 20 years. He has dealt with a lot of clients who seek help with their memory loss problems. David began asking himself about how these people can get ahead of these problems. It is then that he started teaching his clients how to improve their brain health to prevent memory loss. David Norris is also the director and founder of Occupational Therapy Brisbane.   Brain Fog And Stress What Does Brain Fog Feel Like Philosopher Gilbert Ryle’s “Concept of Mind” is an argument against principles that the mind and body are separate. Brain fog is a cluster of different symptoms and experiences.  It is a silent phenomenon. To others, the people who have it look normal. For those living with it, it is a pervasive experience. They have less energy, retreat more, and are a bit depressed. Some people feel hungover, some can’t think clearly, and others find simple words become such tongue twisters. One person’s experience of brain fog is entirely different from another’s. What Are 5 Brain Fog Symptoms Memory problems. Lack of mental clarity or difficulty of putting things together. Poor concentration and attention. Inability to focus. Mental fatigue or an impairment to do things that matter. 12 Causes Of Brain Fog Lack of Sleep Makes A Fuzzy Mind A lot of people aren’t sleeping properly. Broken, interrupted sleep means you don’t get the brain flushing benefits. When you get a deep nourishing sleep, your brain undergoes a wash and rinse cycle associated with mental health.  Chronic Stress And A Tired Brain Stress plays a role in impacting your immune system.  It triggers a whole host of changes that ultimately lead to mental fatigue. Stress makes it harder for you to think and rationalise things. Hormone Changes Lead To Bad Memory Thyroid dysfunction can manifest as brain fog. It can impact your sense of arousal, memory, and mood. Memory loss and hormonal changes are both associated with menopause. Hypothyroidism or not having enough hormones can lead to difficulty with concentration and also affects verbal memory. Depression And Brain Fog People with depression experience a cognitive dysfunction.  Reaction times are slower. The ability to solve complex problems and have a sense of clarity are affected. Comparing 2005 with 2015, there’s been an 18% increase in people living with depression. Older people ages 65 and up are living with the profound impacts of depression. Food and Nutrition Affects Brain Health Brain fog can be a symptom of the lack of a nutrient rich and diverse diet. A healthy thriving brain needs vitamin Bs such as vitamin B12.  Allergies and your consumption of food with preservatives or additives have been documented to be associated with brain fog. A MTHFR genetic mutation leads to low folate levels in the body and impacts methylation, which is responsible for allowing people to operate at high levels. Lifestyle factors set you up to experience brain fog. Medication Can Make You Fuzzy I put together some time ago a ‘Fuzzy 17’ list of medications associated with brain fog. Blood pressure lowering drugs have gained notoriety for that. Too little, too much, or too much of a multitude of different vitamins can be a factor for you to experience brain fog. People who undergo cancer treatments such as chemotherapy and radiation report having a foggy brain. Neurological Conditions Impact Clarity Having anxiety impacts cognitive dysfunction. Neurological disorders such as Multiple Sclerosis and Dementia, autoimmune diseases like arthritis and lupus, dehydration, and conditions such as Meniere’s, all contribute to poor cognitive fitness. Poor Gut Health Means Poor Mental Clarity We’ve already looked at the relationship between the gut-brain access. Sensitivities in the gut and the immune system response affects your cognitive well-being. Gut health is an important variable contributing to your brain fog. Fatigue Makes You Too Tired To Focus Fatigue can be due to too much physical exertion or a lack of incidental movement  Obesity and grief are both associated with fatigue. Toxins Are Toxic To Your Brain Toxins are in our environment. Some mimic the functions of hormones. Some block and mimic the activities associated with sugar regulation. Alcohol Flooding Your Brain? Alcohol has after effects and your liver has to deal with it. Sustained alcohol consumption and abuse will impair your cognitive performance. Genetics May Up Your Brain Fog Risks The MTHFR snip or single nucleotide polymorphism is a genetic variable associated with cognitive dysfunction. Brain Fog Testing: What Do You Need To Do? How is your mental health? What is your diet like? Are you getting enough physical activity? Or is it too much? Are you taking any medications or supplements? Are they doing the job they need to do? Check for symptoms such as hair loss, changes in skin moisture, weight gain, or changes in your nail conditions. Get some blood work done. Check your fasting glucose levels, any underlying infections, or nutritional deficiencies. Test your kidney, liver, and thyroid function. It might be worthwhile to get an MRI or CT Scan. Investigate your allergies or consider getting a sleep study. It might be worthwhile to get an MRI or CT-scan. Start a diary of your symptoms, the things happening in your life, your stress, your mood can help you keep a track record that could be the start of a relevant conversation later with your GP. Get Some Brain Clarity with Treatments The causes determine the actions needed to be taken. For example, if low iron levels are causing your poor cognitive function then add iron supplements or change your diet. For medication problems, work with your GP to determine what’s best for you. Get some good quality sunlight exposure. Connect with others and find joy and pleasure in the activities you do in your life. Recovery Is Possible Brain fog does not need to be an ongoing life condition. You can do something about it. Pay attention to the signs and be vigilant about safeguarding your cognitive fitness. You can tackle this, cut your risks down, and gain control. Put together the right team to develop a personalised action plan relevant to your particular situation. Check out today's questions:  Karin: Hello Dr Cabral, I listen to your podcasts every day and never stop learning something new. I was diagnosed with candida overgrowth and I really want to know, is there any truth behind the dietary recommendation to avoid the consumption of yeasts and mushrooms (not medicinal)? And if so, why? As a biologist I'm really interested in the mechanism behind it.Thank you so much!Karin. Carolyn: How can I lower my LDL to below 50. Would my liver be producing too much LDL for some reason other than from my diet?? I am 64 year old female diagnosed with coronary heart disease which runs in my family. Father died at age 66 and one brother has had heart bypass surgery. My total cholesterol is 176, HDL 72, Triglycerides 69 and LDL 88. I am on statin 40 mg. for 1.5 years. I have always exercised and still do, 4 days a week. 3 cardio and one strength class per week for 60 minutes each. I eat fresh fruit and vegetables and lowfat yogurt, egg beaters, oats and grains, some dark chocolate and salmon or cod. Occasionally eat shrimp. I take vitamin supplements and also was diagnosed with osteopenia 10 yrs ago and took extra calcium as recommended by doctors for past 10 year which I am afraid contributed to my heart disease. Now I get my calcium from food sources and small amount in multi vitamin. Heart disease was diagnosed with chemical stress test which I requested because of family history as I have no symptoms of heart disease. My calcium score test 10 years ago was zero and now it is 56 - my diet and exercise has been the same other than I am older and started taking calcium supplements. Also have had a stressful life during that time frame but now I have gotten my stress under control. Never had high blood pressure ever. Jada: Hi Dr.Cabral, Thanks for all of the wonderful information you make available for people! I’m a 37 year old woman who isn’t overweight, pre-diabetic, smoker or drinker who has been experiencing heart disease symptoms. I get extremely short of breath by doing nothing to the point of feeling as if I’m suffocating and will prevent me from sleeping, chest pressure that can cause me to dry heave and will last for days/weeks at a time, heavy arms, weakness, being woken from sleep not breathing, sometimes SVT episodes, hair loss, tachycardia, fatigue, brain fog, inability to retain info and recall words, hormonal issues with possible endometriosis now, Hashimotos but thyroid is stable. These symptoms mildly started in 2012 after a stressful event but cardiac symptoms worsened in 2017 out of nowhere. My EKGs are now borderline and I’ve had one abnormal EKG after I was given Albuterol which showed Ischemia and mild T-wave depression. I’ve been to 9+ cardiologists, NDs and a couple functional medicine doctors that don’t know what is wrong and they all tell me my heart is fine. I’ve had stress test, Stress MRI, echos, holter monitors(showed PACs,PVC and occasional SVT) but everything looks “fine.” I’ve really lost my quality of life and while some days my symptoms calm down, I have many days where I can barely move because I’m so shortness of breath with such a tight chest! This is terrifying! I’ve had one doctor tell me this could be related to stealth infections or mycotoxins. Your expertise is much appreciated! Lauren: Dear Doctor Cabral, I’ve been listening to your podcasts for about 4 months, I love everything you do, its amazing how one person can be such a wealth of knowledge in so many different areas of expertise. I’m female, 31, 68kg, no children, BMI fine. I’ve been working with a local practitioner to help heal from a variety of dis-eases and more in discovered alone the way. I use to suffer with seasonal allergies (hay fever, pollen (Adelaide high pollen rate), animals, grass etc ever since I can remember). I now suffer constantly all year around from chronic itchy eyes (TRIED EVERYTHING), it’s very debilitating as I wake myself up from itchy my eyes in my sleep therefore I never feel well rested. I use to live in Dubai, where my allergies didn’t flare up but suffered regular ear infections as I was a flight attendant. My IGE is through the roof; 780 H and my eosinophils has been really high for a while now too. I have had asthma, eczema since I was a baby and suffered on and off with anxiety/depression since I was 16 (on/off meds). About 23, I was diagnosed with PCOS/celiac dis-ease and discovered had I H-pylori, which I took a millions AB’s for. I recently got tested for MTHFR, which I tested positive for homozygous C677T and tested positive parasites Blasto (AB’s didn’t kill). I discovered some mould (killed with vinegar) in my home and also have a hypoallergenic dog. My symptoms and trying to treat are: chronic itchy eyes (TRIED EVERYTHING), acne (since I can remember), headaches, emotional, eczema/dry skin, high eosinophils, candida, parasites, brain fog, parasites. Past conventional treatment for acne: 2 courses of Roaccutane, Doxycycline, steroid injections, hormone blockers, even got a job a skin clinic, hoping I would heal. Working with my local practitioner and bicom therapy; we are addressing the parasites, possible candida and healing of the gut, with hope all of the other symptom will decrease. I eat relatively healthy, gluten free, dairy free, exercise/sweat 4-5 times per week, drink plenty of water, fast minimum 12 hours per day. Have a good sleep hygiene however never get a good night sleep. I would say I’m pitta/vata dosha type. Due to supplements, I’ve got my homo from 10.1 down to 4.4, b12 has increased, folate has increased, vit d has increased. I have really dry flaky skin on scalp/face and dry eyes but acne! I have tried low histamine diet (all vary depending where you source it) however it conflicts with the MTHFR diet and Ayurveda diet. I use your daily nutritional shake, do I require the additional supplements on top? I have read your amazing book RBE, use your daily nutritional shake, currently doing your allergy protocol and about to start your 21 day detox and parasite protocol. I would love to do the big five, especially the OAT test but as you know all these supplements and labs are very expensive. I know you have done podcasts before on a variety of the above topics however as you know not one size fits all and I don’t think anyone out there has the exact health history as myself. I'm a 5' 9", 71kg predominantly Vata body type, and I'd like to gain a little size, without harming my health otherwise. In recent years I have suffered the symptoms of Menieres disease, however between your fantastic podcast and my local naturopath's help, I feel like I can say I've finally got on top of it, touch wood. The only problem is, I sometimes feel increased symptoms, mainly ear fullness and slight hearing loss in my right ear, within a day or so after workouts with heavy compound lifts, especially squats and deadlifts. I'm concerned the heavy lifts may be too taxing on my CNS and that may be the cause. What do you think? I'd love to know your thoughts on the connection between the CNS and weightlifting. Welcome back to our weekend Cabral HouseCall shows! This is where we answer our community's wellness, weight loss, and anti-aging questions to help people get back on track! Check out today's questions: Cameron: Hi Dr Cabral, my name is Cameron, I'm a 42 year old male from Melbourne, Australia. I discovered your podcast a few months ago, after searching for answers to my medical issues, and since then I've been hooked. You've opened my eyes to the fascinating world of functional medicine, so thank you! My reason for writing is for advice on my Menieres disease that I was diagnosed with about three years ago, after a couple of severe vertigo episodes, along with tinnitus and hearing loss in my right ear. I have seen several ENT specialists, and had an MRI which came up clear, and done various tests to help confirm the diagnosis. As a result, I've been put on a low sodium diet, been prescribed Serc (betahistine) and a diuretic pill, and been told to also reduce caffeine, alcohol, and stress. I've also tried Ginko Biloba, and several different vitamins. The only medication that has noticeably helped is 2mg Valium pills, taken only when symptoms flare. After more sporadic episodes in the last year, usually lasting for a burst of a few weeks at a time, I had a steroid injection into my inner ear, which appears to have stopped the vestibular problems at least for now, however, the hearing problems and tinnitus remain. Hoping to find more help, and after hearing your podcasts, I started seeing a naturopath, who also used to be a pharmacist. She suggested an IGG food sensitivity test, which came back showing a strong sensitivity to egg and dairy. Up until then I loved eggs, and consumed whey protein powder on a regular basis around weight training. I'm currently on a moderate detox, and going to the gym more often to try to help with blood flow. I'd love to hear your thoughts on Menieres disease, and any suggestions as to what else you think could help me. It is a miserable condition, which I would do anything to get rid of. It was particularly high in vegan proteins which are ones I would be more inclined to use, so I am very concerned. My question is about varicose veins (not spider veins but deeper veins). I have very big, bulging veins on one leg and less severe case on the other. They first appeared over 30 years ago after my 1st pregnancy. 8 years ago I had a laser closure procedure that was not successful as it never repaired my veins and they continue to be very enlarged. Here are my 2 questions:- what would you recommend for treating or preventing further development of varicose vein? - i’m doing spin classes 2 - 3 times a week. Can that contribute to the enlarged veins? Should I avoid this type of exercise? And what is recommended? Izziness, We're back with our 2nd day of the Cabral HouseCalls and I'm looking forward to sharing with you more of our community's questions... Let's get started! Leilani: Dear Dr Cabral, I have tried dry brushing twice and each time I start off, I'm fine for around 2 to 3 weeks after which my legs start to itch! I brush for a few minutes before showering and then when I dry my legs after coming out of the shower, they will not stop itching for quite a while. Rubbing with a towel or just my hands make it feel better for a little bit, but then it just starts off again. At first I didn't see anything strange (like a rash) but when I looked closer I could just slightly make out some small white spots...? Eventually the itching will go away (10 - 30 minutes later) but sometimes when I sit on the train (when my legs make contact with something, usuly wearing trousers) it can kick off again. After I stop with the brushing, my legs will continue to itch after showering for up to a month. I only moisturise my body with coconut oil, I have for years now, so I know it's not that. I only use natural soaps from the health food store, also have done so for years, so I don't think it's that. Our towels are cotton, always have been. This occurs whether my legs have been shaved ot not. We don't have a water filter in our shower and the water is quite hard - we've lived in hard water areas for many years and it didn't do anything to my skin, it's just when I start dry brushing. The brush I have came in a gift pack with some natural skin care profucts, so I assume the bristles are natural, but it did 't say. The last time I tried I made a point of not rubbing too hard, just making very gentle strokes. But just before I stopped again the second time, my arms also started feeling itchy although nowhere near as bad as my legs. I did take Roaccutane when I was a teenager, which dried out my lips to this day. Could it have had an effect on all of my skin? I do get stretch marks very easily and whenever I go for a facial I'm told my skin is quite dry. I drink a lot of water and herbal tea during the day, is there something else I can try? Dry brushing seems like such an easy thing to do to benefit your health, it's just so annoying this happens when I try. It is so very much appreciated.   Marie: Hi Stephen, I have an itchy patch on my gluteal cleft and sometimes also around the anus. It gets worse around menstruation. I did have candida overgrowth a couple of years ago and did a cleanse to balance things out. I'm wondering if this could be a yeast issue or something else. I'm in my 30's and have an active lifestyle. I occasionally drink matcha and have chocolate (Hu Kitchen/Eating Evolved) sweetened with coconut sugar. I don't eat dairy & occasionally eat gluten. I mostly shop at the farmers market & whole foods eating little to no processed foods regularly. Thank you in advance for your time and thoughts. What are the causes and what are the treatments? And will lab tests help determine root causes? Thanks!    Stephanie: Hi Dr Cabral, first off love the podcasts, they are a daily ritual for me. I am writing because I just finished the 21 day detox with sole purpose of trying to get rid of lifelong acne. I took prescriptions growing and 15 years ago before I was aware of how bad it was, I took Accutane which got rid of it temporarily but it now seems to be getting more aggressive as I get older. Unfortunately the detox didn't help. I have been eating a plant based diet for about 6 months now and am thinking this isn't diet related at all. I feel great, have lots of energy, sleep well, no stomach pains or bloating or weird cravings. I do have really heavy and long periods and surprisingly have no acne when I am pregnant or breast feeding. I have seen a natural Dr who ran a ton of blood work and said everything looks pretty good but noted my Estradoil and Estrone were a bit high and put me on some progesterone because she said it's a tad low, but i took myself off because I felt horrible on it. She also gave me several supplements but here I am 4 months later with acne. I'm not ready to give up yet, but don't know where to go next. I am not able to purchase every test so would love to know what you would suggest as my next step. The amount of information that I gain from these calls have helped me immensely in my private practice. I have a client who has come to me with ME/CFS. She has had a myriad of health issues since she was a child and has finally been "diagnosed" with ME/CFS. She is very aware of diet and nutrition (bachelors in nutrition), has tried many different alternative methods of healing and still can't seem to find answers. She has had copious amounts of antibiotics around 2014-2015 and right now we are working on rebuilding her gut flora, but her stomach is very sensitive so many things she struggles with including sauerkraut. I was diagnosed with this about 18 months ago which was about 6 months after my initial onset of tinnitus. The ringing and pressurized feeling started in my right ear in August of 2015. At the time I believed it was directly related to stress. I had just moved my family across 3 states, my wife was pregnant with very severe kidney complications, and I had left my old career behind to become a Police Officer. In February of 2016 I had my first experience with brain fog and severe dizziness. Since then I have had seasons where it increases and decreases in intensity. I was also diagnosed with a shift work sleep disorder. Long story short the dizziness and nausea became too intense to continue in my new career as a Police Officer. Menieres has drastically impacted my life and I'm hoping to find out what I can do differently to get my life and health back. A little more info about me that may be helpful...I lift weights about 5 days a week and have always taken care of my body. I generally eat a protein rich diet and especially have been consuming lots of cruciferous vegetables lately. I typically start my days with anti oxidant berry blend smoothies with a handful of spinach and a scoop of Muscle Farm's Combat Powder(tri-blend protein). I've started to limit processed foods but I still eat a lot of bread. I have noticed that my worst days often fall a day or two after a "cheat day" where I probably binged on chips or other similar processed foods. I've become very conscientious of sodium intake but I still experience random bouts of brain fog and dizziness. For a short while I was receiving vestibular therapy and dry needling around my right trapezius/upper back and shoulder area. I found that very relaxing and that it did suppress some symptoms. However, like I said, since then I have had to walk away from my job as a Police officer and no longer have he insurance coverage for V therapy or dry needling. I'm curious to know... I've had a very high protein diet for about 11 years, since I really got into weightlifting. Is there a possibility that I've just trashed my gut and it may be manifesting itself through the ringing and dizziness in my ear? What would be the test you recommend to look into this? Also, for years I have taken Benadryl (1 tablet) at night to help me fall asleep. Is there any way that this could also be causing an adverse reaction affecting my Menieres diagnosis? Along with his three brothers and three sisters, he grew up in an old house with only three bedrooms and one bathroom. Donny has obtained numerous degrees and is also an ordained minister at multiple churches. He has been married to his wife Christal for ten years and is currently serving as Assistant to the Children’s Church Pastor at LifeSpring Community Church. He has also served for four years as a missionary in Honduras, Central America.
