If you have a stress fracture and you're trying to figure out whether it is safe to run again, should you get an MRI or a CT scan? Many runners assume there is one perfect imaging test that can tell them exactly when they are cleared to run. Unfortunately, it is not that simple. In this episode of the Doc On The Run Podcast, Dr. Christopher Segler explains the real difference between MRI scans and CT scans for stress fractures, what each test actually shows, and why imaging alone often does not give runners the “green light” to return to training. This is a story about loss - In memory of Kira Johnson Charles welcomed fatherhood with joy. He and Kira were so excited to start their family, and their second pregnancy would bring them back-to-back boys. The first pregnancy had been healthy, but ended in an emergency surgical birth as Baby Charles experienced heart decelerations that became concerning. Welcoming their second child during a transition from Atlanta to Los Angeles provided an opportunity for them to seek world-class care. Kira and Baby Langston were healthy throughout the pregnancy. Kira and Charles took intentional steps to select a provider that met their needs. They opted for a scheduled surgical birth with a provider who had excellent reviews and had privileges at Cedar Sinai Hospital. Their doctor assured them that this was the safest approach to delivery due to her history. A Vaginal Birth After Cesarean (VBAC) was never presented as an option.  Their provider, Dr. Kasanji introduced them to Dr. Naim as a backup provider for the procedure. Spoiler alert - It was a racket. Dr. Kasanji could no longer stand for surgeries continued to accept patients, billed for prenatal care, and  then transferred their care to an incompetent, negligible, and callous butcher. We said what we said.  *The following timeline is not exact, but it's not exaggerated.  2:00 pm - Surgical birth began. 2:02 pm Incision complete on a patient with a previous cesarean scar. Cue internal bleeding. Just before 4:00 pm - Signs of hemorrhaging. Charles rings the alarm. 5:00 pm - CT Scan ordered STAT 6:00 pm - No CT Scan. Charles rings the alarm. 6:15 pm - Declared Surgical Emergency 7:00 pm - No CT scan. Kira is shivering uncontrollably.  8:00 pm - No CT scan. Charles rings the alarm. The CT never happened. Charles rings the alarm! 9:00 pm - Your wife just isn't a priority right now. 10:00 pm - No CT scan.  11:00 pm - No CT scan.  12:00 am - Baby, I'm scared.  12:00 am - “blind surgery” to find the bleed. It's not a big deal. She'll be back in 15 minutes. Incision. Crashed. Condition is critical, continuing to work on her.  Bring my wife back.  In a blink of an eye Kira was gone. There is nothing routine about not surviving a non-emergent surgical birth. The care, compassion, and skill that Kira deserved from her medical team never showed up.  There's a biblical verse that people quote when faced with loss, “Weeping may endure for a night, but joy comes in the morning.” It's intended to provide encouragement for a person to move forward. But what if it was intended to give permission to grieve. “For a night,” gives the illusion of temporality. Night is symbolic of darkness and death. Could it be, “Weeping may endure for darkness or weeping may endure for a death. Grief has cycles, it does not end. But joy comes in the morning. Joy comes in the light. It's the grief cycle of darkness and light - night and morning.  Charles Johnson shared his birth story that ended in the tragic, senseless, and avoidable death of his wife, Kira Dixon Johnson. Kira was a healthy 30-year-old woman in her second pregnancy when she trusted her vetted doctor and the reputation of an accomplished hospital system to ensure safe delivery of her child and her recovery. A rushed surgical birth that nicked her bladder caused internal bleeding; she lost her life after a series of pleas for support when she showed signs of hemorrhaging. Charles describes the formula that accompanies nearly every story of a Black woman losing her life in childbirth as, “We expressed our concerns, our concerns were dismissed or delayed, and by the time they did something, it was too late.”  The attending doctor, Dr. Naim, was later found negligent by the California Medical Board. Charles was inspired to keep fighting for better care and accountability. He founded 4Kira4Moms as a force multiplier in spreading the message of systemic change in medical care. This includes better care provider training, transparency, and accountability. The disproportionate impact of maternal mortality on Black women is at the center of his legislative advocacy, better doula and midwifery support, and increased awareness and education among fathers and non-birthing parents. Charles believes that revolutionary care  is that there has to be a fundamental standard of care in this country for all birthing people and doctors, midwives, whoever is performing these services, and who's getting paid for these services, their compensation needs to be tied to their ability to meet that standard period. Charles brought their baby home without Kira. He has faced fatherhood headfirst, allowing his sons to be his guiding force. They sustain him. They are the tools in his mental health toolbox, along with consistent therapy. He works for Kira's legacy and the future of change. She'd burn it all down. Charles has lit the match. Broadcast from KSQD, Santa Cruz on 11-13-2025: Dr. Dawn discusses a New England Journal of Medicine study examining radiation exposure from medical imaging in over 4 million children showing increased hematological cancer risk. Head and brain CTs deliver highest bone marrow doses, with under-1-year-olds receiving 20 milligrays compared to background radiation of 1 milligray yearly. The study found 3,000 cancers in 4 million children over roughly 10 years, with relative risk increasing 1.6-fold per CT scan. However, methodological flaws include combining US and Canadian cohorts with different data quality, potential reverse causation where imaging detected pre-existing cancers, and arbitrary 6-month latency assumptions are significant flaws in this study.. Despite small absolute risk increases given low baseline cancer rates, she encourages parents to question necessity of repeat scans and request alternatives like MRI when appropriate. She reports on cutting-edge CRISPR therapy using lipid nanoparticles to deliver molecular scissors targeting the ANGPTL3 gene controlling LDL cholesterol production. Recent setbacks in several other CRISPR trials raise issues for unexplained liver toxicity. Concerns include off-target gene editing effects and partially repaired DNA creating mutated proteins triggering autoimmune reactions. Dr. Dawn emphasizes restricting gene therapy to life-threatening genetic diseases with no alternatives until safety improves. Stanford scientists used AI model Evo trained on 9 trillion gene samples to design 300 new bacteriophages from scratch, with 16 phages successfully killing E. coli bacteria. AI tools now predict protein structures, design custom drugs, create antivenoms, invent antibiotics, and break down PFAS forever chemicals. The research represents evolution through computation and requires guardrails on AI's ability to manipulate biological structures. An emailer shares the Rosencare model where hotel chain owner Harris Rosen created self-insured health coverage featuring direct provider contracting, imaging facilities charging one-third to one-half traditional costs, transparent pharmacy benefit management, and zero or $5 primary care copays. Employees receive proactive screening for colonoscopies, mammograms, cholesterol, diabetes, and hypertension during clinic visits. Ninety percent of medicines including insulin cost nothing, with remaining drugs $0-25, and hospital admissions cost flat $750. The model saved $600 million while providing superior preventive care by eliminating insurance middlemen and focusing on early chronic disease detection when 75-85% of costs originate. Dr. Dawn explains abdominophrenic dyssynergia causing bloating unrelated to gas or food. The diaphragm descends and abdominal wall muscles relax, pushing organs forward after meals. CT scans showed lettuce-related bloating involved no intestinal gas changes but demonstrated this abnormal muscle reflex. Randomized trials showed biofeedback training with chest-lifting and abdominal wall contracting exercises before and after eating for four weeks improved symptoms 66%. She warns that constant bloating in postmenopausal women unrelated to eating requires ovarian cancer screening. She discusses how genes drive personality using dopamine receptor gene DRD4 polymorphisms as an example. The 7-repeat variant present in 48% of Americans creates receptors binding dopamine poorly, associating with ADHD, pathological gambling, alcoholism, drug dependence, and bulimia, plus personality traits of novelty-seeking, impulsiveness, and optimism. The 2-repeat DRD4 variant common in Asia correlates with lower anger and higher forgiveness. DRD2 variations enhance the memory of negative outcomes, creating pessimistic bias and avoidance behavior. She presents the KETO trial showing "lean mass hyper-responder phenotype" where very low-carbohydrate dieters averaging age 55 maintained LDL cholesterol of 272 for five years but showed identical coronary artery calcium scores and plaque burden as matched controls with LDL under 150. Despite extreme LDL elevation, the very low insulin levels from carbohydrate restriction prevent LDL oxidation, the inflammatory "loading" process enabling arterial damage. She concludes with unusual cancer symptom where recurrent pain in specific body locations after alcohol consumption, lasting 1-2 days, occurs in 5% of Hodgkin lymphoma patients and in other cancers when alcohol induced blood vessel dilation and inflammatory chemical release in cancer-containing lymph nodes causes pain after drinking.

Kopecky, M.D.   Coronary artery calcification (CAC) scanning can help improving our assessment of the risk of heart attack or stroke in thoughtfully selected patients. Not everyone needs a CAC scan! The results of a CAC scan can be particularly helpful in deciding whether a patient should start medications to reduce their risk. Coronary artery calcium scanning is *not* used to follow a patient's risk over time (to see whether the risk is decreasing or increasing).   Topics Discussed: How is a coronary artery calcification (CAC) scan done, and what can the results tell us about a patient's cardiovascular risk and management? Who should consider having a CAC scan? Should a patient discuss the possibility of a CAC scan with their doctor, and should they just self-refer to a screening center? What changes can we make in patient management based on CAC scan findings? How should we monitor whether the management is improving the patient's cardiovascular risk? Nicole Scott and her husband, Keith, worked at the same Ohio hospital.  When Keith noticed abdominal pain in his left side, he thought it was muscle-related because of the physical nature of his job.  The pain radiated to his groin, then what had been random occurrences of the pain became more frequent.   Nicole urged Keith to seek urgent care.  An emergency room doctor called for a CT scan, thinking Keith had at worst, a hernia.  However, the CT scan revealed a mass on Keith's kidney.  An MRI and partial nephrectomy resulted in a diagnosis of papillary renal cell carcinoma, a form of kidney.   Nicole and Keith were shocked by this diagnosis, but after she got over the shock, Nicole assumed the role of Keith's caregiver.   She said without question, the toughest part of her caregiver experience was the emotional piece.  At first, there was a tremendous amount of fear and depression, as she thought Keith might not survive, but all the while she told herself she was caring for the love of her life, had to put up a positive front and be a source of encouragement for her husband.   Nicole said there was some at-home care involved, but much of her duties consisted of making sure that Keith made it to all of his doctor visits, most notably the successful surgical procedure and post-treatment care.   Caregiving can be a round-the-clock challenge, and Nicole said she came close to suffering from caregiver burnout, but she always did what needed to be done, at all times realizing hers was a high-stakes task.   By way of advice, Nicole Scott says to anyone suddenly finding themselves in the role of caregiver to take a deep breath and never lose hope.  She says a caregiver needs to take of themselves.  She says it is “not selfish” to ask a friend to pitch in or to make time for some ‘me time,' as that will make a caregiver a better caregiver.   Additional Resources:   The Kidney Cancer Association: https://www.kidneycancer.org
