This is a story of a woman who didn't realize she had neurological conditions that were perhaps altering her abilities until she was in her forties. You ask how is that possible? Well, Sadie thought that everyone was just like her. She assumed everyone else's brain worked just like hers. Her biggest cheerleader was her grandmother, who since Sadie was a little girl, has catalogued everything her granddaughter has written or drawn.  Adele says of her grandmother "she is perfect and just brimming with fantastic folksy wisdom.""Do I Know You, A Faceblind Reporter's Journey into the Science of Sight, Memory, and Imagination,"  is Sadie's recently released book and there's a wealth of information for anyone who thinks perhaps they are alone in the discovery of being neurodivergent. Dr. Capone has authored or co-authored over 300 publications in peer-reviewed medical journals, book chapters, and publications from clinical trials. He is a scientific reviewer for the leading journals within ophthalmology and retina and an editorial board member of the Journal of the American Association for Pediatric Ophthalmology and Strabismus.  As an innovator, Dr. Capone has several co-assigned patents, has engaged in surgical instrument design and software and biopharmaceutical start-ups, and serves on several scientific advisory boards.

Dr. Nischal joined UPMC Children's Hospital of Pittsburgh as chief of the Division of Pediatric Ophthalmology, Strabismus, and Adult Motility in 2011. He directs pediatric program development at the UPMC Eye Center and is a professor of ophthalmology at the University of Pittsburgh School of Medicine. Dr. Nischal has pioneered the use of new surgical techniques, previously used only in adults, in children with corneal disorders. He is one of the world's foremost pediatric eye specialists and an author of more than 100 published research articles.

Nora Schmidt noticed that her daughter Maddie's right eye was crossing inward, one day after she had gone to her eye doctor for a simple eye appointment in which everything looked fine. This situation, which according to her Eye Doctor and a Pediatric Opthamologist could have been a common condition known as Strabismus, but  instead turned out to be DIPG. Nora will talk about Maddie's journey which lasted for just over 10 months before her passing from this Pediatric Brain Cancer on December 31st of 2022.

All right now on to the show.   01:36 Hey, everybody, welcome to the program, doing a little urban broadcasting today. So I'm in downtown Santa Fe, actually, my old office is just over there. I closed that office in 2010. But this is a very sweet part of Santa Fe, it's on the west side, pretty quiet and easy going. So I thought I would stop and take a question from a parent is very concerned about a four year old daughter Julie. So Julie has been diagnosed with farsightedness and lazy eye and she's got an eye turning in. And her eye doctor said that she needs to wear these glasses full time, mom sent me the prescriptions about a plus six. And he also said that she only has about three more years to be able to improve the lazy eye. And he wants to schedule her for eye muscle surgery. Because the only way to fix this I turn is with eye muscle surgery. Well, of course, she is appalled by this. She wants other opinions. And so she's known me a long time. And so I wanted to give my point of view. So first thing I want to say is that with kids, their visual system is very malleable, very plastic in the sense that it's versatile, it's changeable. And when you start putting glasses on children are doing things like surgery. There's even a technique where you can use eyedrops that can actually reduce a lazy eye. When you do all these invasive treatments, what ends up happening is it basically reinforces the problem. You know, the eyes are the only part of the brain that are outside the cranial vault. And since the eyes are brain tissue, especially in children, it's a great opportunity to improve one's vision by doing physical therapy exercises, because what you're doing there is you're creating new pathways, new ways for the child to use their visual system in the midst of using both eyes together. So with the glasses prescription, you know the way glasses are prescribed is that the doctor is going to put in eyedrops that paralyze the focusing muscles. We call this dilating eyedrops. And once you do that, you have lost the opportunity to be able to measure the organic focusing flexibility of the visual system. And the reason is, is because those muscles are paralyzed because of the eyedrops. And so the prescription that the doctor is going to give is always way too strong. So in this particular case, one of the things we have to look at is First of all,

Michael Lievens: his strabismus journey & translating Fixing my Gaze

Denise introduces Michael Lievens, whose website strabismusworld.com was one of the few blogs available when she started vision therapy. Following his journey and feeling less alone was so helpful as she began vision therapy! Michael shares how he developed accommodative strabismus at age three as a result of undetected far-sightedness, underwent three eye surgeries which left him with constant double vision and finally discovered vision therapy as a result of searching English language resources because no resources were available in his native language of Dutch. In 2017, Michael founded a Belgian strabismus nonprofit organization which undertook the project of translating Susan Barry's book Fixing my Gaze into Dutch. He also has a Dutch website, scheelzien.org, a Dutch YouTube channel and FaceBook groups in Dutch and English. His association organizes annual patient meet ups where VTODs (vision therapy optometrists) are invited to speak. Proceeds from the Dutch translation are being rolled forward into his latest project of translating Fixing my Gaze into the French language. He is also creating a French chapter of his organization, which will cater to the other side of Belgium, France and other French-speaking territories. Can you see him say hi? Yes, I can.   01:08 He's 18 months old. And I noticed since birth one of you I don't know if it's one or both or depending is looks like it's slightly off. I don't know if you receive the pictures I sent.   01:21 Just okay. And I attempted a golden I like an ophthalmologist but I kind of walked out politely because they just wanted to do too much dilate. I just didn't want to do that just yet. And I'm wondering if you could help assess what the problem might be. And if there's something natural or exercise or something holistic that could maybe be treated or   01:43 want to make sure it's not an issue and it's not going to get worse and you know, something I have to be ahead of.   01:50 Absolutely. So Alexandre is 18 months old. Is that Is that correct? Yes. Okay, and tell me a little bit about your gestation period, the birth process. Anything unusual there? I went through IVF for about eight years. I had him when I was 51. I actually carried him through to 37 weeks. And we were supposed to go to 40 i My pregnancy was fine. No issues. No you no nausea, no cravings, nothing. It's just up to 37 weeks. i My blood pressure was starting to elevate so my OB wanted to to take him out and we did a C section. He was born four pounds along the weight 15 ounces. And they said he was just under five pounds. So they had to put him in a NICU and they said it's because he had a little low blood sugar and low body temperature. We were both in the hospital for about five days. Me to regulate my blood pressure and then my son to regulate his body temp and his sugar. And then we released and since then he's you know, he's seeing a holistic pediatrician and he's taking vitamin D probiotic, and DHA and NACA what? He's been healthy hasn't been sick.   03:05 Great. Okay.   03:08 So, after the birth, brought him home, what is his? What is his mobility? Does he move around a lot? Is he on his tummy at all? Do you know what's kind of active mobile activities? Are you engaging with him when he was when he came home from the hospital or in general? Well, you know, after so he's 18 months. So we've got you know, a little bit of track record here. Obviously, you know, is there any mobility on the floor? Is he scooting, calling? Tell me about his his mobility experience? Yes, he's right now he's walking. He loves to activity walks, almost runs around the house. He's very active.   04:01 Let's see, we take him outside and I just stay focused. But this is his baby nurse. This is MISA. Hey everyone in this podcast, I talk about strabismus, that means the two eyes are working together. And this therapist presented a case to me where the child has one eye that straight and one either wanders out, and there's an alternating situation. And she said that a doctor recommended that the child get some dimestore glasses, some magnification glasses, and put some scotch tape on the outside part of both sides of the glasses, the right lens and the left lens, and you'll hear my response. And what I recommend in terms of helping anybody who's got the two eyes not working together. The question is the eyes or one or both eyes are turning out we call that either alternating exotropia or just exotropia is that it's an over divergence, it's like, not able to converge. So if you're using dollar store glasses, and you're trying to tape, the person is going to have no reference point on why you're doing that. There's no learning, there's no reason why Why are you doing that to me. And so they're probably going to just do more of it, or they're going to find another compensating adaptive response. It starts ultimately I feel with developing more of the midline. So and I would even take it to more of the Moro reflex, that midline development is really the key in the eyes feeling safe enough to coming in. So you know, whenever there's strabismus, we start off and we do some of the activities just during the activity if we can include one of the eyes. So we give each I chance, whether it's the marzen ball, the ball bounce, I mean almost anything, even vestibular driven activities with the eyepatch. So in other words, we want to allow each eye to grow in their processing. And then the reflex work. So again, this is where what I'm learning about the fear paralysis reflex, that maybe that's the place to start, because the I'm turning away from life. So the metaphor is the left eyes The mother eye, the right eyes, the father I which I am I fixating more with Is it the right or the left it you know, she usually uses the right one the left. Okay, so so the left is the mother, I'm turning away from the Mother, I'm turning away from my creativity. And I'm looking more with my father I masculine. So you can kind of I was joking with one of my 86 year old patients the other day, and I said I can look at the child's eyes. And I can pretty much tell what the parents relationship is. It's in the child's eyes in their reactions and how they're aiming their eyes. Because what children do is we internalize the primary relationship. But we don't know what to do with that internalization. And a lot of times we internalize it through the eyeballs. And so when they start doing some output things, you see the reflection of what they're living in. So I think that this divergence situation, I don't think, if anything, what I feel works better with the divergence is putting the tapes on the inside part. Now you might say, Well, that makes no more than it brings the midline awareness. And it really, what's what needs to happen in strabismus is that there's an inability to understand your middle, whether it's your visual metal, your body, metal or both. And by putting the tapes in the middle, you are disrupting the habit of one is straigh...
