BUFFALO, NY- March 27, 2024 – A new #research paper was #published in Oncotarget's Volume 15 on March 19, 2024, entitled, “Durvalumab and tremelimumab before surgery in patients with hormone receptor positive, HER2-negative stage II–III breast cancer.” In this new study, researchers Haven R. Garber, Sreyashi Basu, Sonali Jindal, Zhong He, Khoi Chu, Akshara Singareeka Raghavendra, Clinton Yam, Lumarie Santiago, Beatriz E. Adrada, Padmanee Sharma, Elizabeth A. Mittendorf, and Jennifer K. Litton from the University of Texas MD Anderson Cancer Center, Brigham and Women's Hospital, Dana-Farber Brigham Cancer Center, and Harvard Medical School conducted a clinical trial to assess the feasibility of enrolling patients with Stage II or III hormone receptor positive (HR+)/HER2-negative breast cancer to pre-operative dual PD-L1/CTLA-4 checkpoint inhibition administered prior to neoadjuvant chemotherapy (NACT). “This feasibility study was conducted to begin testing the hypothesis that dual checkpoint blockade would increase TIL and enhance the response to subsequent NACT in patients with stage II or III HR+/HER2-negative breast cancer.” Eight eligible patients were treated with upfront durvalumab and tremelimumab for two cycles. Patients then received NACT prior to breast surgery. Seven patients had baseline and interval breast ultrasounds after combination immunotherapy and the responses were mixed: 3/7 patients experienced a ≥30% decrease in tumor volume, 3/7 a ≥30% increase, and 1 patient had stable disease. At the time of breast surgery, 1/8 patients had a pathologic complete response (pCR). The trial was stopped early after 3 of 8 patients experienced immunotherapy-related toxicity or suspected disease progression that prompted discontinuation or a delay in the administration of NACT. Two patients experienced grade 3 immune-related adverse events (1 with colitis, 1 with endocrinopathy). Analysis of the tumor microenvironment after combination immunotherapy did not show a significant change in immune cell subsets from baseline. “There was limited benefit for dual checkpoint blockade administered prior to NACT in our study of 8 patients with HR+/HER2-negative breast cancer.” DOI - https://doi.org/10.18632/oncotarget.28567 Correspondence to - Haven R. On this episode of Baddie 2 Baddie, April and Cynthia dive into the world of paramedical tattooing and its importance for women of color, particularly survivors of cancer.  Joined by Nina Austin, a three-time cancer survivor and speech pathologist, and Zuri, a permanent makeup artist, and epidemiologist based in Houston, the baddies learn about Nina's 3D areola tattoo and how permanent makeup such as scar reduction, eyebrows and more can restore self-confidence and a sense of normalcy in survivorship.   This episode highlights the need for representation and skill as it relates to women of color, self-care and insurance coverage for nipple mound and areola restoration. Zuri also offers advice on how to safely get permanent makeup services and who to trust for the best results.   About our guests: Nina is a three-time cancer survivor.  Nina is a third-generation breast cancer survivor who is BRCA+ and has battled breast cancer twice.  Most recently, she battled uterine cancer.  By day, Nina is a speech pathologist in Houston.  Nina bravely volunteered to be a live model, and received her 3D areola tattoo on stage to ensure that women of color were appropriately trained to provide paramedical tattooing on women of color.    Zuri has been a lifelong lover of makeup. In 2018, she expanded her services to semi-permanent makeup and has since gained 10 certifications in the field. Zuri's artistic eye coupled with attention to detail has allowed her to progress in the world of both makeup both traditional and semi-permanent. Zuri is an avid learner and holds a Bachelors degree in Biology from Texas Southern University and two Masters's degrees from Texas A&M University. As a perfectionist , she aims to make each client look their best with an emphasis on the integrity of the skin and stable healed work. Zuri is continually upgrading, learning, and developing new techniques, and her understanding of art and skin science transfers wonderfully into the art of micropigmentation. Zuri is a licensed tattoo artist in the state of Texas and was awarded the PMU Women of Color Award for Best New Artist in 2019, SHEEO of the Year in 2022, and the Best Speaker in Science of Permanent Makeup Award in 2022. Outside of PMU, Zuri is a trained scientist. With a background in the sciences, Zuri developed a training program that hyperfocuses on bridging the gap between the art and science of PMU. Zuri avidly believes that is the science of PMU that yields stellar results and this is what sets us apart in service and training. We focus on the scientific principles of skin physiology, pigment chemistry, the science of skin, and infectious diseases to perform safe services with lasting stable results. Outside of Artistry By Zuri, Zuri is employed as an Epidemiologist.  You can find Zuri on the web and Instagram by visiting @artistrybyzuri About our Hosts April is a cool high school counselor in South Carolina. She was diagnosed at 35 with Stage II, Triple Negative Breast Cancer and BRAC1+. April is a single southern belle looking to share her testimony and advocate for women of color who have been diagnosed with breast cancer. She is the proud mother of a miniature pincher named Gigi. Cynthia was diagnosed with Stage II Invasive Ductal Carcinoma in 2018 one year before she was old enough to begin regular mammograms. After a lumpectomy, chemo, and radiation, Cynthia is currently undergoing hormone therapy. To cope, Cynthia shared her journey candidly on social media.  This led her to educate and advocate about breast cancer and health disparities. Cynthia lives by the mantra, "If you've got to go through it, GLOW through it."  An educator by day, at night Cynthia glows by volunteering her time as a Baddie Ambassador with For The Breast of Us, an Advocacy Ambassador for the Susan G. Komen Center for Public Policy, a certified global educator with the Know Your Lemons Foundation, a member of Alpha Kappa Sorority, Inc. and a proud Bajan Baddie!

This week in honor of National Infertility Awareness Week, we have selected this episode from March of 2022 to re-share. It features Melanie Childers sharing her story and feelings around her cancer-induced loss of fertility. Certified resilience and leadership coach Melanie Childers was diagnosed at 34 with Stage II, hormone-positive breast cancer. Today, Melanie reads her piece “No” from Wildfire Magazine's 2018 “(In)fertility” issue. A decade ago, cancer made Melanie's fertility choices final. A cancer diagnosis that affects reproductive organs that comes well after menopause is not going to land the same way the same diagnosis will decades prior. It is heartbreaking, maddening, painful in a way that is hard to articulate. Melanie wrote her piece 4 years ago, and today has the retrospect of 10 years to share her reflections. April and Melanie will talk about what processing and grief looks like now and how cancer clarifies life's path. They'll also discuss how writing is a powerful way to process wounds, and allows for a way to revisit previous versions of ourselves when we read our written words later in time. Melanie's piece is included in Wildfire's upcoming book “Igniting the Fire Within: Stories of Healing, Hope & Humor.” This episode contains explicit language.More about Melanie and The Bad Bitch Entrepreneur podcast: https://www.melaniechilders.comhttps://www.instagram.com/the_underbelly/More about this episode's sponsor, “Life Reconstructed” author Kim Harms: https://kimharms.netPurchase a copy of the “(In)Fertility” issue: https://www.wildfirecommunity.org/shop/digital-download-infertilityIda Sukrungruang's essay: https://brevitymag.com/issues/issue-49-may-2015-gender/after-the-hysterectomy/Buy the Wildfire book “Igniting the Fire Within: Stories of Healing, Hope & Humor, Inside Today's Young Breast Cancer Community”: https://www.amazon.com/dp/B0BJVJ629F?ref_=pe_3052080_397514860Get the free Wildfire email newsletter: https://www.wildfirecommunity.orgLearn about Wildfire writing workshops: https://www.wildfirecommunity.org/workshopsShop Wildfire merch & more: https://www.wildfirecommunity.org/shop*Free* The Burn Writing Companion: Guided Prompt Journal (Vol. 1): https://www.wildfirecommunity.org/the-burn

In this episode of BackTable, Dr. Bagrodia interviews Dr. Sia Daneshmand, chief of urologic oncology at the University of Southern California, about the research trials and treatment of stage II seminomas. --- EARN CME Reflect on how this Podcast applies to your day-to-day and earn free AMA PRA Category 1 CMEs: https://earnc.me/oOcODJ --- SHOW NOTES FIrst, Dr. Daneshmand defines stage II seminomas, which are seminomas that have spread outside of the testicles to the retroperitoneal lymph nodes. Stage II seminomas are further subclassified into IIa, b, or c depending on size and number of affected lymph nodes. This disease stage can present on Initial seminoma diagnosis or occur after a relapse of a stage I seminoma. There are 3 options for standard treatment for stage II seminomas: radiation therapy, chemotherapy, or retroperitoneal lymph node dissection (RPLND). Dr. Daneshmand recommends a monotherapy option to minimize toxicities and explains which treatment modalities are best for each subtype of stage II seminomas. Then, the doctors discuss common side effects of each modality. Radiation may cause cardiotoxicity, fatigue, enteritis, and secondary malignancy. Chemotherapy may cause neurotoxicity, nephrotoxicity, infertility, and tinnitus or hearing loss. The doctors also discuss RPLND as a therapeutic option. Dr. Daneshmand explains that some patients may be hesitant at first to choose this option, as this has been a large open surgery with lengthy hospitalizations in the past. However, his technique involves only making a small midline incision and using an extraperitoneal approach. His RPLND patients usually only stay in the hospital for one day and fully recover by 2 to 3 weeks. He also discusses prospective RPLND trials on stage II seminomas, including his own research project. Both doctors agree that patients should be involved in shared decision making in order to weigh the cure rate against the comorbidity rate of each therapy. Finally, Dr. Daneshmand speculates on the future of seminoma treatment, which he believes lies in advancements of individualized treatment based on biomarker data.

This week Bri and Jer are joined by Amanda Vegter, a 4th-year veterinary student at Colorado State University who found out she had Stage II breast cancer during her studies. She shares how she discovered a lump but didn't get it checked out because she was "too busy" (I mean, hey… Vet school is busy). After travelling to Africa and contracting tick bite fever, Amanda decided to get it checked and was diagnosed. She goes into the effects of forced/early menopause and how they're often overlooked in breast cancer treatment. Amanda reveals how they were nearly as bad, if not worse than the cancer treatment itself, which no one warned her about.
