Life has it challenges. Sometimes, a person can have more than what seems like their fair share. Some might say that about Rhonda Cagle. She’s lived through the loss of her husband, just 18 days following his cancer diagnosis. She nearly lost her daughter. She fell in love and married again, only to come close to losing her husband to COVID. And she herself is a survivor of Stage III breast cancer. Yet despite the challenges, Rhonda has done remarkable things for the community and for education as the founder and president of Leverage Consulting Agency. She works with public school systems to help them grow, thrive, and succeed. She is a board member at the Mayo Clinic, focusing on cancer research and screening. She is a volunteer member of the Board of Contributors for The Arizona Republic, writing about K-12 education. She’s a loving mom to three adult children … and two rescue dogs. Breast Cancer is complex, as are the treatment options that clinicians and patients must traverse. Stage III breast cancer has up to a 57% chance of recurring up to 20 years after diagnosis. Josh and Michael look at one of the practice-changing papers that tried to address how to manage high-risk patients. This study is called Soft and Text. A large, expanded-cohort pooled analysis of neoadjuvant immunotherapy for patients with resectable Stage III melanoma has reported very high rates of durable survival. The findings from the world’s biggest center of expertise in melanoma were announced at ESMO Congress 2024. The study included patients from clinical trials and real-world studies who had pure immune checkpoint inhibitor neoadjuvant therapy, or combinations including BRAF/MEK targeted therapy. After giving her talk in Barcelona, lead investigator Georgina Long, AO, PhD, MBBS, FRACP, Professor and Co-Medical Director at the Melanoma Institute Australia, University of Sydney, gave Oncology Times reporter Peter Goodwin the details.

Yuliya Linhares, MD is a medical oncologist specializing in the comprehensive treatment of lymphoma and serves as chief of Lymphoma Services at Miami Cancer Institute. In this video, Dr. Linhares provides an overview of cutaneous T-cell lymphoma (CTCL) and discusses some strategies for shortening the diagnostic journey of this rare cancer.The diagnosis of CTCL is often challenging; as a result, delays in diagnosis (and subsequent work-up and treatment) can be significant. Part of the reason is the variability in how individual patients present with CTCL and its subtypes. Because mycosis fungoides progresses slowly, some patients may not experience progression beyond their initial symptoms, even beyond 10 years. Patients with mycosis fungoides or Sézary syndrome also have overlap in manifestations; in fact, Sézary syndrome was once classified as a malignant, leukemic variant of mycosis fungoides but is now recognized as a distinct CTCL subtype.Patients with mycosis fungoides may progress through three phases of skin symptoms. The first may feature little more than transient red, scaly areas of skin on the buttocks and torso. The plaques may be hyper- or hypopigmented. As such, these symptoms can be easily misidentified as common skin conditions such as eczema or psoriasis. The variability of signs and symptoms also adds to the challenge of making a timely, clear-cut diagnosis.In the second phase, patients with progressing disease may develop palpable, scaly, reddish-brown plaques that appear on any portion of the body. Over time, the affected areas of skin may grow, merging with other affected regions. Patients' skin presentation during this stage can vary considerably: Some patients may experience severe pruritus or pain in these scaly bumps, which can result in sleep disturbances and other challenges to quality of life. Other patients may remain asymptomatic other than the skin's appearance.Disease presentation is a bit more consistent in patients who have progressed to the third phase of skin symptoms. Some patients may develop mushroom-shaped skin tumors that can cause skin ulceration and infection. Even for patients with mycosis fungoides reaching this phase of skin progression, malignant spread is uncommon (only 10% will experience metastases to major organs).While patients with Stage III mycosis fungoides experience widespread erythema (over 80% of body surface area), erythroderma is a consistent feature of Sézary syndrome. This rash will often be associated with severe pruritus and peeling.In addition to erythroderma and B2 blood involvement, patients with Sézary syndrome will typically have several other characteristic signs: generalized lymphadenopathy, opportunistic infections, and alopecia. The liver and possibly the spleen will be enlarged, and patients often have very thick, coarse skin on the soles of the feet and palms of the hands (i.e., palmoplantar keratoderma).Diagnosis is usually made with a patient history, complete physical exam, blood tests, biopsy of skin lesions, computed tomography imaging, and sometimes lymph node biopsy and/or bone marrow biopsy. These methods can also be useful in determining the stage of disease, especially whether the lymph nodes have been involved and whether the cancerous cells have spread to blood and other organs. A combination of two checkpoint inhibitors used as neoadjuvant therapy for macroscopic, resectable Stage III melanoma brought a highly statistically significant improvement over the standard of care: surgery followed by checkpoint inhibition (therapeutic lymph node dissection followed by adjuvant therapy with nivolumab, pembrolizumab or, in BRAFmut melanoma, dabrafenib + trametinib). This research was reported from the ASCO 2024 Annual Meeting and highlighted the NADINA trial from the Netherlands. After his session at ASCO, the lead author of NADINA, Christian U. What Deb Krier thought was a routine mammogram turned into a Stage Zero diagnosis of breast cancer. Stage Zero became Stage III, which was upgraded to Stage IV HER 2+ invasive breast cancer. Deb opted for a double mastectomy with no reconstruction. She was subsequently diagnosed with basal cell carcinoma, then papillary thyroid cancer. Through 33 surgeries, she has survived all three cancers.

In today's episode, we're talking about a profoundly touching topic—caring for someone with melanoma. Today, we're discussing the roles, challenges, and triumphs of those who stand by their loved ones in the fight against melanoma—caregivers. They are the unsung heroes who provide the support and strength needed to face each day anew. Our guests: Sue Capozzi: Sue has devotedly served as a caregiver for her husband, John, who, despite his current stability, was diagnosed with Stage III melanoma in 2019, which progressed to Stage IV the following year. Throughout his challenging treatment journey, John has faced numerous hurdles but is currently doing well. Now retired, Sue and John reside in Little Egg Harbor, New Jersey, a stone's throw away from the beach. They cherish their time sailing, kayaking, and enjoying moments with their dog, always mindful to apply copious amounts of sunscreen. The couple enjoys the company of their married son, a granddaughter, and eagerly await the arrival of another grandchild in the spring of 2024. Driven by the importance of melanoma education, Sue became a Peer Connect mentor in 2022. In this short span, she has provided guidance to several caregivers, finding deep fulfillment in offering support. Sue hopes to extend the same level of support and understanding she has received, contributing positively to the lives of others on their melanoma journeys. Nancy Earle: Nancy devoted herself as a caregiver to her son, Will, following his Stage III melanoma diagnosis in 2016, which escalated to Stage IV within the same year. She took a two-year hiatus from her career to support him daily through his illness and treatment. Together, they navigated the complex landscape of medical opinions, trials, and treatment centers. On March 4, 2018, Nancy and her family bid a heartfelt farewell to Will. Before retiring, Nancy had a dynamic career as a lawyer and event producer. Now, she relishes her retirement in Bellport Village, New York, alongside her husband. They take joy in their three surviving sons, their partners, and their grandchildren. Driven by a desire to honor Will's altruistic spirit, Nancy engages with several organizations. She serves on the advisory board of INTERSOS, a Rome-based international NGO, working to raise awareness of its humanitarian programs. Locally, she contributes to the environmental committee and has begun writing a book chronicling Will's courageous battle over his final two years. In 2021, Nancy became a Peer Connect mentor, driven by the critical need for melanoma awareness. She supports caregivers, some fortunate to have loved ones in remission and others mourning their loss. Offering a beacon of information and comfort is crucial for caregivers, who often grapple with fear and emotional turmoil. Ann Camden is a prolific writer and past Burn guest. She was diagnosed Stage III at 38, and then Stage IV at 45. In this episode Ann reads her essay “Fast Will Make Me Furious” from the 2023 “Body” issue of Wildfire. Ann's story is about what happened when she had a severe allergic reaction to chemo.
