Send us Fan MailDo you really need a scanner, whole slide images, and AI infrastructure before you can start in digital pathology?In this episode, I argue that you do not.I'm Dr. Aleksandra Zuraw, veterinary pathologist and digital pathology educator, and this talk is about a belief I hear all the time: I don't have the tools yet, so there is no point learning digital pathology. I used to think that too. When I was training in Berlin, there was one Leica 6-slide scanner, and it felt like digital pathology was only for a small group of chosen people. That experience made the field feel distant, exclusive, and not really available to beginners. What changed for me was not a new scanner. It was a small project.I needed a more consistent way to quantify a senescence marker in archived skin samples, so I used a microscope camera, captured images, opened them in Microsoft Paint, and manually marked cells with colored dots. It was scrappy. Very low tech. But it was also digital, consistent, and verifiable. That project became my first real step into digital pathology and helped me get my first job in the field, where I worked between pathologists and image analysis scientists on biomarker quantification and patient stratification problems. That is the core point of this episode: knowledge unlocks technology.Scanners matter. AI tools matter. But the deeper bottleneck is whether enough people understand how to use these tools, ask good questions, and connect pathology expertise with digital workflows. That is why this episode is really about readiness. Not readiness of the hardware. Readiness of the people.I also talk about Dr. Taladzer from Pakistan, whose story makes this point even more clearly. At the time, Pakistan had around 220 million people, about 500 pathologists, and zero scanners. She still started learning digital pathology during COVID using a microscope and camera, joined the Digital Pathology Association, taught herself from papers and online resources, and kept going even after multiple AI vendors rejected her because she did not have whole slide images. Eventually, she found a DIY image analysis platform, learned to annotate and train models on static images, completed projects quickly, and went on to publish more than 10 digital pathology papers without ever using WSI.Why should you listen?Because this episode is for pathologists and lab leaders who are interested in digital pathology but still feel stuck at the beginning. It is for people waiting for permission, perfect infrastructure, or a formal roadmap. And it is for trailblazers who came back from a meeting or conference energized, but need a practical way to turn that energy into action before it fades.I also address an important AI question near the end: How do we know an AI model is good enough for pathology? To investigate scan consistency over time, a control slide was prepared using a tissue microarray stained with Hematoxylin and Eosin (H&E). Ink dots were applied to the slide to define a consistent scanning region. The slide was scanned 77 times over six months using an Aperio AT2 whole slide scanner at 40x magnification.Image analysis was performed using HALO software by Indica Labs. Both the entire scan area and individual tissue punches were analyzed to assess total stained area and stain intensity, quantified by optical density (OD) for both hematoxylin and eosin. A linear regression model was applied to data from all individual punches and the full scan region. Additionally, a two-way ANOVA was conducted to compare OD values of hematoxylin and eosin between the first 10 scans and the last 10 scans. Key findings were that hematoxylin showed a statistically significant decline in both stained areas and OD over time, while eosin demonstrated a statistically significant increase in stained area, but a decrease in OD. These results suggest potential degradation of staining quality or imaging consistency over time. Possible contributing factors include slide bleaching, light source variability, annotation region size, or other imaging conditions. These will be the focus of future investigations to better understand and control variability in longitudinal slide scanning studies.

Tutaj wkleimy o respiratorach.   Jednym z tematów wywiadu była kwestia tak zwanej afery respiratorowej. W sprawie, przypomnijmy, chodziło o to, że zakup respiratorów powierzono Andrzejowi Izdebskiemu, który nigdy nie miał nic wspólnego ze sprzętem medycznym, za to zasłynął jako handlarz bronią. Był wielokrotnie podejrzewany o nielegalny obrót sprzętem wojskowym. Andrzej Izdebski nie tylko nigdy nie dostarczył respiratorów, ale też znikną z zaliczką, którą wiał na poczet zamówionego sprzętu medycznego. Wkrótce po ujawnieniu afery, niedoszły handlarz respiratorów zmarł w Albanii, a jego ciało zostało poddane kremacji. Ta oficjalna wersja wydarzeń do dziś budzi wiele wątpliwości. Czy Izdebski tak naprawdę nie żyje, czy też może zniknął wraz z pieniędzmi.   Prowadzący podcast Witold Jurasz spytał ministra Siemoniaka o to, czy może potwierdzić ustalenia Onetu, że Andrzej Izdebski był współpracownikiem Polskich Wojskowych Służb Specjalnych. Przy czym, jako ów współpracownik dopuścił się bardzo poważnego oszustwa, jeszcze na długo przed tym, jak przy okazji respiratorów oszukał państwo polskie. Rzeczony handlarz został wskazany przez polskie służby specjalne jako wiarygodny partner dla Stanów Zjednoczonych, dla których miał kupić za amerykańskie pieniądze broń dla syryjskiej opozycji. Podobnie jak w przypadku respiratorów - broń nie została dostarczona, a pieniądze się rozpłynęły.   Jeśli powyższe doniesienia okazałyby się prawdą, znaczyłoby to, że polskie służby, polecając Ministerstwu Zdrowia Andrzeja Izdebskiego, musiały doskonale zdawać sobie sprawę, że mają do czynienia ze złodziejem. Witold Jurasz pytał też, czy prawdą jest, iż w Agencji Wywiadu, która poleciła Izdebskiego, pracowali oficerowie służb wojskowych, przy czym działo się to w czasie, gdy służbom szefował były współpracownik zlikwidowanego już WSI. Rzeczona osoba swoje stanowisko objęła, co tylko dodaje całej sprawie pikanterii, w okresie rządów PiS.   Pozostaje jeszcze jedno palące pytanie - czemu afera ta nie została rozliczona. W odpowiedzi minister koordynator służb specjalnych stwierdził, że powyższa sprawa jest przedmiotem doniesienia do prokuratury i jest jedną z kilku priorytetowych kwestii, o których Tomasz Siemoniak rozmawia z prokuratorem generalnym. Jak stwierdził gość podcastu, sprawa respiratorów dobrze opisuje schemat i niechlubne tradycje działania polskich służb specjalnych, w których dochodzi do pomieszczenia działalności służbowej i prywatnych interesów. Zapytany o to, czy może potwierdzić wszystkie wymienione przez Jurasza fakty, minister odparł, że nie może ich potwierdzić, ale nie może też im zaprzeczyć. Co obydwaj prowadzący podcast jednoznacznie odebrali jako potwierdzenie podanych wyżej informacji.   Przedmiotem rozmowy jest też zatrzymanie polskiego zakonnika przez białoruskie KGB. Prowadzący podcast pytają, czy Polska planuje konsultacje, koordynację, a być może wręcz oddanie inicjatywy w tej sprawie Stolicy Apostolskiej. Prowadzący zadają też jednak ministrowi Siemoniakowi pytanie fundamentalne, czy Polska faktycznie werbuje księży lub zakonników. Tomasz Siemoniak jednoznacznie zaprzecza. Prowadzący podcast pytają też swojego gościa o zniknięcia kolejnych białoruskich opozycjonistów i zastanawiają się, czy Polska roztacza w dostatecznym stopniu opiekę nad przebywającymi u nas działaczy opozycyjnych.   W dalszej części podcastu Witold Jurasz i Zbigniew Parafianowicz pytają swojego gościa, czemu nasz kraj zgodził się na przekazanie do Rosji zatrzymanego w Polsce za szpiegostwo Pawła Rubcowa, nie uzyskując w zamian zwolnienia z rosyjskiego więzienia przebywającego tam po skazaniu za szpiegostwo Mariana Radzajewskiego czy też przebywającego w białoruskim więzieniu Andrzeja Poczobuta. Tomasz Siemoniak stwierdza, że Stany Zjednoczone w zamian za Rubcowa przekazały Polsce niezwykle cenne informacje. Przy okazji wątku współpracy ze Stanami Zjednoczonymi Zbigniew Parafianowicz zadał pytanie, czy prawdą jest, że Amerykanie w coraz większym stopniu ograniczają współpracę wywiadowczą z państwami europejskimi, w tym również Polską?   Nie zabrakło również tematu aktów sabotażu dokonywanych przez rosyjską agenturę w Polsce. W tym kontekście wciąż aktualna pozostaje dyskusja, czy powinniśmy odpowiedzieć na te działania w ofensywny sposób.  Witold Jurasz zadał Tomaszowi Siemoniakowi też pytanie o to, czy w służbach specjalnych jest właściwy balans pomiędzy konieczną i potrzebną kontynuacją kadrową, a wyczyszczeniem tych, którzy mają skłonność do nadmiernego wysługiwania się każdej kolejnej władzy. Zbigniew Parafianowicz postaci takie, wywodzące się z polskich służb specjalnych określa mianem piwotalnych członków. Zostaje też poruszona sprawa działalności polskich służb specjalnych za wschodnią granicą. Prowadzący podcast interesują się tym, czy fakt, iż Rosjanie i Białorusini nie są w stanie wyłapywać prawdziwych polskich oficerów wywiadu, tudzież współpracowników polskich służb specjalnych, świadczy doskonałej kondycji naszego wywiadu, czy też rzeczywistość jest zdecydowanie bardzie ponura – nie prowadzimy żadnej znaczącej aktywności za wschodnią granicą.

Janelle Yip's path to becoming a pro skier was more unexpected than unconventional. She grew up as a skier on the slopestyle path, and while she loved it, being a comp skier wasn't in the cards for Janelle. So, after a gap year led her to Revelstoke, Janelle never gave up on her pro skier dreams, giving herself a 5-year window to make it happen. In year one, Janelle met the crew that would become “The Blondes," altering the trajectory of her life.
