Last year at the 2025 AAHKS Annual Meeting, our host William B. Kurtz, MD spoke with the James A. Rand, MD, Young Investigator’s Award recipient Michael E. Neufeld, MD, MSc, FRCSC about his study on Synovial Metal Ions in “Nickel Free” vs. Standard Cobalt-Chrome Containing Total Knee Replacement.  Dr. Neufeld shared that the aim of his study was to compare intraarticular synovial fluid levels of metal ions in patients who underwent cemented primary TKA with a hypoallergenic implant vs. a matched cohort of standard cobalt-chromium (Co-Cr) containing implants at a minimum two-year follow-up. A case-controlled study was conducted using prospectively collected data from a single institution – 22 cases and 19 controls.  Interestingly, the study uncovered that patients with hypoallergenic implants had intraarticular synovial Ni ion levels 3.6 times higher vs. standard implant controls, contesting the use of this hypoallergenic implant for Ni allergy/hypersensitivity. Stephen Hunter PT, DPT, OCS, FAPTA received his Bachelor of Arts in physical therapy from University of Utah in 1984, and his clinical doctorate in 2008. He is board certified in orthopedic physical therapy. Currently, Stephen continues to treat patients and is the director of Internal Process Control for Intermountain Health's rehabilitation services. He leads a team of 4 internal process control coordinators who work to establish a culture of quality and standardize evidence-based care across all rehabilitation disciplines and services. He is the past president of NARA (National Association of Rehabilitation Providers and Agencies). He supports research efforts including data collection and clinical implementation. Examples include investigations on Total Knee Arthroplasty (TKA) and low back pain (LBP) resulting in recent publications (Minick 2022, Caplin 2022) and three randomized LBP trials (Brennan 2006, Fritz 2014, Delitto 2020). At the 2024 AAHKS Annual Meeting, Shaun P. Patel, MD and Andrew M. Schwartz, MD sat down to speak with the winner of the 2024 Clinical Research Award. The study’s main author, Simon W. Young, MD talked about the importance of postoperative pain management in primary total knee arthroplasty and how IORA of antibiotics for prophylaxis in TKA is known to increase local tissue concentrations. The study that included 46 patients investigated using IORA Diclofenac to improve postoperative pain management in TKA, compared to IV Diclofenac. Broken up into two groups, one group received IORA Diclofenac and IV normal saline placebo and the second group received IV Diclofenac and 10 normal saline placebo. Forty-three patients undergoing revision of a cemented TKA were included in the study and synovial fluid was collected at the time of revision surgery and analyzed for concentrations. Rand Young Investigator’s Award.  Two of the study’s authors, Anne DeBenedetti, MSc and Charles P. Hannon, MD, MBA addressed the question of what is the safest and most effective dose of intravenous (IV) dexamethasone administered during primary total knee arthroplaty (TKA)? Of the 404 patients undergoing surgery across 11 centers, all were monitored for pain, nausea, vomitting and sleep. The patients were monitored for seven days postoperatively. Miguel began his professional career in 1987 as a controller at VDO AG. He was subsequently CFO of VDO Instrumentos in Spain and the worldwide VDO Instrument Division. Within the Siemens Group, López was CFO of various business units of Siemens AG from 2001 – including the Industry Automation Division from 2008, the Digital Factory Division from 2014 and Siemens Gamesa Renewable Energy from 2017. In the years from 2018 to 2022, he was President & CEO of Siemens in Spain. During this period, he also performed the role of Non-executive Chairman of the Board of Directors at Siemens Gamesa. At the 2023 AAHKS Annual Meeting, the chair of the Digital Health & Social Media Committee Shaun P. Patel, MD and Ahmed Siddiqi, MD, MBA sat down with Drake G. LeBrun, MD and Brian P. Chalmers, MD to discuss their paper presented at the meeting on Does Melatonin Improve Subjective Sleep Quality after TKA? A Randomized, Placebo-Controlled Trial.  This study was ignited from hearing so many patients complain about their quality of sleep following surgery. Drs. LeBrun and Chalmers noticed large improvements over the years in pain control, range of motion, function and things like that, but very little relating to improving sleep for patients.  Take a listen and find out if melatonin improves the quality of sleep for patients who recently undergone surgery. Prestonica ostrvske nacije biće domaćin grupama od A do D, pri čemu će se A i B igrati u Araneta Koliseumu, dok će ekipe iz C i D igrati u „Mol of Azija“ areni u Pasaju, predgrađu Manile. U grupi D svoje takmičenje počeće reprezentacija Crne Gore! Njihov inicijalni zadatak nije težak, budući da će se za prolaz boriti sa Egiptom, Meksikom i Litvanijom. Ako se kockice sklope za selektora Boška Radovića, ni prvo mesto nije van dometa, uzevši u obzir silne kadrovske probleme koje Litvanija trenutno ima. Prođu li dalje, Crnogorce u ukrštanju čeka niko drugi do „Drim Tim“ SAD, koji igra u grupi C. Entoni Edvards se rano nametnuo kao lider ove ekipe, koja je svakako i dalje prvi favorit na turniru, bez obzira na činjenicu što u sastavu možda i nema nekih „prepoznatljivih brendova“...mada, ko je slušao naš podkast poslednjih godina, prepoznaće baš svakoga. U konkurenciji su ovde i Grci, kao i Novi Zeland i Jordan. Grupa A je grupa u kojoj se nalaze domaći Filipinci, čija je glavna uzdanica na ovom turniru s.t.r. „Džordan Klarkson“. Edinov i Milošev omiljeni privatnik će ovde imati priliku da se raspištolji kao nikada do sada, a može li to dovesti do nekih opipljivih rezultata? Možda i prolaza? Dominikanska republika je prilično jaka sa Karl-Entonijem Taunsom, a Italija...pa...nećemo sad o Italiji. Rano je. Srbija igra u grupi B, i njima smo, naravno, posvetili i najviše pažnje. A ono o čemu možda nismo pričali previše je to što će naši momci igrati u istoj hali u kojoj je moćna Jugoslavija 1978. osvojila svoje drugo svetsko zlato. Kā rīkoties katastrofas vai nelaimes gadījumā raidījumā Kā labāk dzīvot atgādina Valsts ugunsdzēsības un glābšanas dienesta (VUGD)priekšnieks pulkvedis Mārtiņš Baltmanis un Neatliekamā medicīniskā palīdzības dienesta (NMPD) Operatīvā vadības centra vadītāja vietniece Diāna Tkačenko. Tālruņa numuri, uz kuriem zvanīt nelaimes gadījumā, vai kad cilvēks jūt apdraudējumu savai vai otra cilvēka veselībai vai dzīvībai, ir 112 vai 113. Numuru 112 jāizvēlas, ka ir jebkāds apdraudējums, 113 – ja nepieciešama medicīniskā palīdzība. Diāna Tkačenko skaidro, ka NMPD dispečeri, kas pieņem zvanus, noskaidro adresi, kas noticis un izvērtē, vai konkrētā gadījumā ir jānosūta NMPD brigāde. Ir algoritms, pēc kura izvērtē, jo nevar reaģēt uz katru zvanu, ir arī jārūpējas par resursiem.  Mārtiņš Baltmanis norāda, ka pamatlieta, kas jāatceras, zvanot uz palīdzības tālruni, ir pateikt, kur atrodamies. "Ja nezinām adresi, varbūt kādus lielus objektus atceramies, braucot pa lielceļu, starp kurām pilsētām esam. Kaut kādus orientierus. Tas ir viens no svarīgākajiem. Pats notikuma raksturs arī ir svarīgs, jāpasaka, vai tas ir ceļu satiksmes negadījums vai ugunsgrēks, vai redz tvaikus. Dispečers ir apmācīts izvaicāt cilvēku, ja arī viņš būs stresa stāvoklī. Galvenais nomierināties un klausīties jautājumu, un atbildēt. Pamatlieta – kur atrodamies, kas noticis, vai ir cietušie," skaidro Mārtiņš Baltmanis. VUGD ir trīs zvanu centri Latvijā, zvanu no Daugavpils var apstrādāt arī Rīgas centrā, visi zvani "stāv" rindā. Diāna Tkačenko norāda, ka vienmēr jāvadās pēc trim jautājumiem - kur, kas un kam? "Tas nozīmē, mēs gribam zināt, kur noticis negadījums, kas ir noticis, kam nepieciešama palīdzība," skaidro Diāna Tkačenko. Arī NMPD zvanu centri ir centralizēti, tāpēc dispečeri nezinās ikvienā vietā kādu pagriezienu vai apzīmējumu kādai vietai, ko zina tikai vietējie. "Līdz ar to ir jābūt saprotošiem un adrese ir jānosauc skaidri, nosaucot pilsētu, nosaucot ielu. Nekliedzot, mierīgi izelpot un tad nebūs problēmas, izsaucējs nepārdzīvos, kāpēc mani nesaprot, nedzird. Arī dispečers šo izsaukumu noformēs daudz ātrāk un veiksmīgāk. Un palīdzība nokļūst ātrāk," turpina Diāna Tkačenko. NMPD rekomendē ieslēgt telefonu skaļruņa režīmā, lai ir brīvas rokas un var sniegt palīdzību cilvēkam. "Bieži mums nākas saskarties ar to, ka sniedzam palīdzību telefoniski, līdz brigādes dodas uz notikuma vietu. Notiek reanimācija, notiek asiņošanas apstādināšana, vai bērnam ir krampji. Tev ir jābūt brīvām rokām. Tu dzirdi, ko saka dispečers un dari," norāda Diāna Tkačenko.

In this episode, Dr. Bill Hunter, Founder, President, and CEO of Canary Medical, presents the early interpretation and exciting potential of data taken from implantable prostheses. In this case, the data comes from Zimmer Biomet's IQ Knee. This product uses a variety of sensors to record a wide array of activity metrics from inside the knee to provide data that might be useful for doctors: step count, cadence, stride length, distance traveled, and gait speed.The prosthesis counts with a battery that works similarly to a pacemaker's and a programmed transmitter that provides information several times a day, but not in real-time. It also counts on a recorder that allows physicians to see what is happening inside the knee and identifies movement abnormalities by registering characteristic patterns that can be interpreted by physicians or physiotherapists.What is the point of this? These types of technologies seek to match physiology to other health problems, especially in post-op TKA patients. Ideally, with a set of parameters and standards, this would be able to determine whether a patient has a normal range of motion or not. The ultimate goal of this technology would be not only to understand gait movements but how abnormalities could be used to trace other issues on the clinical side.Listen to this episode and learn about the future of implantable data!

In part two, we looked at resources to fight these denials. Today we have a first-person patient experience. Beckie Hill is someone who had to battle to have the surgeries she needed, and it's a battle she won, twice. So I asked her to share her story for the newsletter and she graciously agreed! The rest of this piece is in her words: I injured both my knees on 08/17/2019 while working. I had x-rays done, followed by a round of physical therapy, massage, medication, activity limitation, taping, and injections. It did not improve my range of motion, activity tolerance, and/or pain. At that time, my physiatrist and primary care physician referred me to an orthopedic surgeon. I saw him on February 3, 2020. He recommended surgery right away, for both knees, and requested authorization for the surgeries through my worker's compensation insurance.On 02/19/2020, the utilization review (UR) company for the worker's compensation insurance provided the following decision: “Recommend denial. Failed to meet the criteria for TKA since her BMI is ##, and the Medical Treatment Guidelines (MTGs) require the BMI to be less than ##”.The UR makes a recommendation to the worker's compensation insurance and they can then agree or disagree with the UR's recommendations. Worker's compensation chose to deny surgery.I began the search of scholarly articles supporting knee surgery in larger-bodied patients. [Editor's note – you can find a collection of those here.] I reached out to professionals within the weight-neutral community for articles, ideas and suggestions. My surgeon also provided documentation to worker's compensation. He also had phone calls with the medical director. On 04/20/20, my surgeon informed the medical director that I would be in a wheelchair within 6 months.Worker's compensation still denied the surgery.Worker's compensation directed me to lose weight or undergo intentional weight loss surgery. The suggestions and denial of medically necessary surgery re-activated a pre-existing eating disorder of atypical anorexia and bulimia. I lost weight, but I was also very sick. I met with an intentional weight loss surgeon during this time, told them about the need for knee surgery, direction to lose weight, and my eating disorder. This surgeon emphatically indicated that weight loss was contraindicated for me from a nutritional, medicinal, and surgical basis. ***Please note, I would NOT have undergone surgery and have been offered weight loss medications many times and refuse to take them.Worker's compensation was provided with documentation from the intentional weight loss surgeon and still denied the surgery.I ultimately retained an attorney to help me navigate this system given the stress and toll the process had taken on me.I was ultimately provided with a cane, walker, an electric scooter, and a disabled parking permit. I lost the job that I was injured at because they were not able/willing to accommodate my work restrictions. Commuting to my place of employment after that became very difficult, and that employer was not able to continue to accommodate my work-from-home status as they had during much of the pandemic. My third job wanted me to work more hours than I was released to do, even though I did try.My lawyer, worker's compensation, and the attorney general had months of discussion and both knee surgeries were ultimately authorized. I had my right knee surgery on 12/29/2021, 680 days after the orthopedic surgeon's recommendation. In June of 2022, my surgeon provided surgery information for the left knee surgery and worker's compensation denied that surgery, again due to BMI. It was ultimately authorized and occurred on 08/10/2022, 904 days after the initial surgery recommendation.Did I ever get close to the BMI that the MTGs “required”? No, in fact, based upon the UR reports I have seen, it went up. It was far more important for me to nourish my body so that I could try to navigate life and recover from surgery if it was/were ever authorized.Not only has the denial of the surgery for each knee been physically traumatic, but it has also been professionally, mentally, emotionally, and financially devasting. I am an “N” of 1. I am not a BMI number. I am a human with a body that is different than every other body on this planet.I am doing okay with the knee recovery. Both surgeries went well and I have almost full ROM in my right knee, and I am about 5 weeks out from the left and doing more movement than the first surgery. There were no complications during or post-op, no issues with wound healing, and while I still have some pain, my surgeon did not anticipate being at maximum medical improvement for one year post-op.I had a surgeon willing and able to do the surgery. And yet, an archaic system and humans within that system made decisions about my body and my life using a system created in the 19th century. The BMI is an abhorrent way for insurance companies and medical providers to render decisions about people. It was not developed to make these decisions, and yet “we” are still making medical decisions and judgements based upon BMI alone.—I am so grateful to Beckie for sharing her story, even though it's a story that should not have to be told, because it never should have happened. At the 2021 Annual Meeting, a podcast was recorded that questioned the rationale of utilizing more expensive, lower-nickel components for TKA based on a patient's nickel or chromium allergy. Co-hosts Stefano A. Bini, MD and Garen D. Steele, MD were joined by Charles M. Lawrie, MD, MSc, and Ryan M. Nunley, MD, who sought to determine how much metal is present in the knee joint after performing a knee replacement with standard components vs. “nickel-free” components.
