Do you work with people who have pain AND neurologic conditions? In our world it would be rare to find someone who doesn't have at least a few patients or clients who experience pain either as a result of their diagnosis or even pain that is completely unrelated but persists. In this episode we had the privilege of picking the brain of one of the best pain neuroscience educators around, Dr. Adriaan Louw, PT, PhD, senior faculty, pain science director and vice-president of faculty experience for Evidence in Motion. As a leader in the field of pain neuroscience, Adriaan has authored and co-authored over 100 peer-reviewed articles related to spinal disorders and pain science. Physical pain is a universal human experience. And for many of us, it's a constant one. Roughly 20 percent of American adults — some 50 million people — suffer from a form of chronic pain. For some, that means having terrible days from time to time. For others, it means a life of constant suffering. Either way, the depth and scale of pain in our society is a massive problem.But what if much of how we understand pain — and how to treat it — is wrong?Rachel Zoffness is a pain psychologist at the University of California, San Francisco, School of Medicine and the author of “The Pain Management Workbook.” We tend to think of pain as a purely biomechanical phenomenon, a physical sensation rooted solely in the body. Adriaan discusses the algorithm of 3-3-1 they developed to teach to PT students to get them to understand pain science - The three types of pain, nociceptive, peripheral neuropathic and central pain. We examine them different, treat them different and allocate our resources to treat them different. To treat persistent pain, you need 3 things: to think different (cognitive), movement & calming the nervous system down. Finally, knowing every case of pain is unique. Adriaan: “There is nothing more challenging in clinical practice then to change a person's life. To change a person's life where they are going attend a wedding or go to a family reunion or lift a grandchild again, those things matter. I can probably remember a handful of people that I manipulated their neck and they felt better but I almost remember every person along the path that I changed their life. We are starting the year off on a strong note with one of the world's experts with regard to pain neuroscience education. My guest is Professor Adriaan Louw. Adriaan is a physical therapist, pain scientist and author in the field of pain neuroscience, Director of the Therapeutic Neuroscience Research Group and Director of Pain Science for Evidence in Motion. His main area of research focuses on teaching patients and healthcare providers more about pain. He has years of experience teaching pain neuroscience education to health professionals. He's authored numerous books on the topic and published over 70 peer-reviewed papers related to pain science. In this episode, we discuss pain neuroscience education plus why it is important for both practitioners and people living with pain. What types of clinical outcomes can you expect when you deliver effective pain neuroscience education? Whether or not it should be delivered as a standalone intervention or part of a larger multimodal treatment approach coupled with other cognitive interventions such as Cognitive Behavioral Therapy, Acceptance and Commitment Therapy or mindfulness. Throughout the interview, we discuss how you can effectively use pain neuroscience education and couple it with other cognitive interventions. Sarah Haag graduated from Marquette University in 2002 with a Master's of Physical Therapy. Sarah has pursued an interest in treating the spine, pelvis with a specialization in women's and men's health. She went on to get her Doctorate of Physical Therapy and Masters of Science in Women's Health from Rosalind Franklin University in 2008. In 2009 she was awarded a Board Certification as a specialist in women's health (WCS). Sarah also completed a Certification in Mechanical Diagnosis Therapy from the Mckenzie Institute in 2010. Most recently, Sarah completed a 200 hour Yoga Instructor Training Program, and is now a Registered Yoga Instructor. Sarah plans to integrate yoga into her rehabilitation programs, as well as teach small, personalized classes. Sarah looks at education, and a better understanding of the latest evidence in the field of physical therapy, as the best way to help people learn about their conditions, and to help people learn to take care of themselves throughout the life span. Karen Litzy started her physical therapy career in an inpatient hospital in Scranton, Pa. Moving to New York a few years later she had the opportunity to work for the New York public school system, Broadway musicals and orthopedic outpatient clinics. While the work was rewarding, she always felt like she could do more to serve her clients. As she was searching for ways to provide a more comprehensive approach to practicing physical therapy she became overwhelmed with requests from clients to be seen in their home or office. This was an opportunity to provide not just convenience, but a different kind of practice. By adopting a “concierge” model, she could dedicate a full hour of one-on-one treatment to each and every client. Now she had ample time to evaluate, treat and re-evaluate. The concierge model allowed me the time to provide vital client education. Her clients would now benefit from a comprehensive home education program. As part of her commitment to her clients and her career, She is constantly engaging in continuing education. She has been lucky enough to learn directly from some of the best in the profession. She has received certificates from Dr. David Butler, Dr. Lorimer Moseley, Dr. Adriaan Louw, Dr. Paul Hodges, The Institute of Physical Art, The American Physical Therapy Association, Hospital for Special Surgery, and many more. She graduated from Misericordia University with her masters degree in Physical Therapy in 1997 and then graduated from the same university in 2014 with a Doctorate of Physical Therapy. She is the host of the podcast, Healthy, Wealthy, and Smart.
