This season some of our speakers are guests who will be live in-person at the EIM Align Conference this August 26-28 in Dallas, Texas. On our eleventh episode of season 3, Heidi Moyer, PT, DPT, GCS, CEEAA a Board-Certified Clinical Specialist in Geriatric Physical Therapy (GCS) and a Certified Exercise Expert in Aging Adults (CEEAA) joins us! Heidi is the Program director for Evidence In Motion's Geriatric Certification Program currently. She has previous teaching experience with EIM, Illinois Physical Therapy Association, and for conferences such as National Student Conclave and Combined Sections Meeting. Heidi is an active APTA Geriatrics member, serving as a leader in multiple roles for APTA Geriatrics with the State Advocate Program and Balance and Falls Special Interest Group (BFSIG) as well as for the Illinois Physical Therapy Association, where they are the active chair of the IPTA Geriatric Special Interest Group (GeriSIG). We are so lucky to have her joining us at the Align conference this year! Heidi's Align Session Preview: Heidi will be presenting at the conference during lab 6, “Move Well and OPTIMIZE Always,” alongside Teresa Schuemann, PT, DPT, ATC, CSCS, SCS & Jennifer Stone, PT, DPT, OCS, PHC. The lab is hands-on and interactive. Its goal is to look at the maximization optimization of management and assessment and screening over of athletes across the life span. They will be looking at how you can optimize the performance of athletes across the lifespan, starting at the screening process, then assessment and management. Heidi will be talking about the importance of plyometrics in training older adults and particularly in athletes that are in a plyometrics sport. As well as looking at the senior fitness examination and how you can integrate that in your practice to give you a guidance to how to manage older adults that are athletes. This lab will be presented twice on Friday, once in the morning, once in the afternoon and once on Sunday, in the morning. Here are some of the highlights: In this episode Heidi expands on her experience working in geriatric physical therapy and becoming a geriatric certified clinical specialist. Heidi does say that you do have to be a jack of all trades because you are not just studying one body system or one joint in the body when looking at our older adults. The geriatric population can be present anywhere. She says her favorite geriatric topic to look at is health promotion and wellness and trying to get people moving long after we discharge them. She is very interested in keeping the geriatric population active and healthy. She also is interested in the topic of ageism. Whether that is self-ageism or ageism reflected from someone else. Heidi completed her Certified Exercise Expert for Aging Adults (CEEAA) through the geriatrics academy through the American Physical Therapy Association (APTA). It consists of 3-weekend intensive that included, assessment, interventions, and lastly special populations. There was then a written exam and a practical exam. The whole focus was to provide hand-on skills to be able to get access to outcome measures and get facetime with experts in the field within APTA geriatrics to learn about some of these test and measures that are not taught in entry level DPT programs. Heidi says it was a great way to get clinical skills, to see changes in management with her patients, trace progress closely and more. Heidi says medication reconciliation is within the scope for physical therapists to perform for patients and serves as a safety net to make sure the medications are doing what they are supposed to do. Heidi works hybrid home health currently and the medication reconciliation is her least favorite part. Heidi expands on career negative habits she picked up on and has broken over the years of practicing in the clinic, the fall risk assessment and handling patient falls, using the available assessment tools and more! Heidi's Clinical Pearl for Physical Therapist working with Pharmacist– “If you have an interest in older adults and working with the geriatric population jump on the continuing education train. While the entry level DPT curriculum is obviously good enough to get us pass the exam, it is not sufficient to be an excellent clinician when working with older adults. If geriatrics is your passion or if you are in a setting where you do not have a choice and you must see patients of Medicare age, get into some continuing education classes, and make yourself comfortable. We are dealing with human lives and livelihoods and quality of life. We do not want to take that lightly. On our tenth episode of season 3, Veronica Riera-Gilley is a holistic pharmacist and owner of Prairie Fire Pharmacy Consulting and Functional Medicine Pharmacist with Pharm to Table. Her passion is to maximize health with minimal medication. She is Board-Certified Geriatric Pharmacist, Advanced Mind Body Medicine Practitioner, Certified Lifestyle Coach, and Adaptive Yoga Instructor who helps people embrace healthy living, aging, and dying. In 2004 Veronica earned her Doctor of Pharmacy from Southwestern Oklahoma State University in 2004. Her journey into pharmacy began with the death of a close family friend. The family friend suffered a traumatic brain injury and was then put on a long list of medications. The side effects of the medication were not tolerable for her. She spoke to her physician and said she could not continue like this and was told that this was her new normal and to deal with it. She was given no guidance and continued the medication and shortly after she died of suicide. Veronica became a pharmacist because she wanted to be the healthcare professional that this family friend really needed, someone that could be the voice of reason and say that there are always other options, other ways to do things. That there are safer ways of using medications than others. Sometimes the answer is more medication and sometimes it is no medication at all. That is why Veronica is a pharmacist she wants to be that health care provider for patients who are sick and tired of being sick and tired and want to find a way outside of the pill box. In this episode they expand on what holistic healthcare is and what being a holistic pharmacist is, medication, when it is necessary and when it isn't, how clinicians can benefit from utilizing a pharmacist and more! Here are some of the highlights: Holistic healthcare looks at the whole person and we treat not just the symptoms but the root cause of the problem. Functional medicine is that framework to ask that question of why are you having this symptom? Conventional medicine has often prescribed in medication a pill for every ill. Functional medicine says why are you having this symptom and what is the root cause? How are we going to treat that? It is not always something that comes back to a nutrition deficiency or some other physiologic thing going on in the body. Sometimes it is a emotional or spiritual. The holistic approach to health care is looking at the whole person not only at their physiologic needs but their spiritual and emotional needs as well and giving credit to all of it. When Veronica was in pharmacy school, she was taught that Lifestyle and nutrition is the core of health. When lifestyle and nutrition failed, they were told to use medication. She gets excited when a patient comes in with an ailment that requires them to be on a long-term medication. Veronica says we need to be having the deep prescribing conversations with our patients, this is the planed and supervised process of discontinuing or reducing dosages. She says we should be having these conversations with our patients because most of these medications are band aids and not cures. They are buying us time to make those lifestyle changes are bodies are asking us to make. Poly pharmacy is using multiple medications at once. The greater the medications you use at once the greater the likelihood of having medication related harm or drug interactions. Veronica says she thinks It is important that we find ways for patients to reduce their medication burden for their long term safety. Food as Medicine and Functional Medicine gave her more tools in her toolbox to help her guide her patients on how to reduce her medicine needs and improve their health without reaching for more pills. On our eighth episode of season 3, Dan Rhon, PT, DPT, DSc, PhD joins! Dan is currently a physical therapist, professor and research director for the DScPT Faculty at Bellin College. He graduated from Baylor University with his master's in physical therapy, followed by a DPT from Temple University and DSc from Baylor University. He completed a clinical fellowship at Brooke Army Medical Center (orthopaedic manual physical therapy) and a postdoctoral research fellowship at the University of Utah. He has served as the Director of Research, Department of Physical Medicine & Rehabilitation, Madigan Army Medical Center, and both the Director of Physical Therapy and then the Director of Clinical Outcomes Research at the Center for the Intrepid, Brooke Army Medical Center. He has over 50 publications indexed in Medline/PUBMed and he has received collectively over $10 million in research funding from the National Institutes of Health (NIH) and the U.S. Congressionally Directed Medical Research Program (CDMRP). Dan currently does primarily research in the military health system & a couple of academic programs mentioned above. In this episode they discuss some of the impactful trials Dan has led, like comparing cortisone injections in physical therapy in patients with shoulder pain and knee osteoarthritis. They also discuss the new trial Dan is undertaking looking at lifestyle and behavior change and how it is at the core of our health and holistic health and what it is. Dan says after you have been in clinical practice for a while you know that change and behavior is extremely difficult to get to happen by just saying something and telling people to do something. When you run a trial, you are trying to get the patients to respond in a certain way and be compliant because this is the care that you believe is optimal and you just need them to do it. But equally as challenging is you try to get clinicians to deliver certain type of care. Listen and find out things Dan believe play into behavior change and what he thinks about educating patients to promote behavior change & more! We all need models to work under, but I do not think I appreciated the communication piece, the therapeutic alliance, the spending a little less time if I must prioritize my time, on an exam and spend a little bit more time listening to a patient and just how powerful that piece of it is. Becoming less mechanical tends to pay off more in the long run. I didn't realize how powerful that connection was earlier on and realized as I get patients that are not getting better, what I could do better and what I am doing wrong. On our seventh episode of season 3, Lori Michener, PT, PhD, ATC, FAPTA, joins to discuss the high-value treatment she has conducted for shoulder pain. Lori has been a professor at the University of Southern California in the Division of Biokinesiology & Physical Therapy for 7 years and is also a director at the University of Southern California Clinical Biomechanics Orthopedic and Sports Outcomes Research. At the beginning of her career, Lori trained as an athletic trainer and a physical therapist then went into college athletics and taught for six years in a typical undergraduate institution. She taught athletic training and pre-med, pre-physical therapy, and pre-occupational therapy students. She says it was a great opportunity for her to learn how to be a teacher. She then went back and got her PhD in biomechanics and orthopaedics at Hahnemann now Drexel University and taught for 15 years at Virginia Commonwealth University. Now she has been in Southern California for the last 7 years at the University of Southern California. In this episode, they focus on why she chose to focus on the shoulder for her speciality area of study. They also discuss the decision of pursuing a Ph.D. and how she came to the conclusion to do so and more! Here are some of the highlights: Lori says no matter what area you are interested in if you have questions about pursuing a Ph.D. reach out to her and she is happy to talk about it. She says it took her about 2 years to come to the decision that she wanted to get her Ph.D. The advice she gives to anyone looking to pursue their Ph.D. program is to explore a lot of different programs because they are all different. Paul asks Lori what outcome measures she thinks we should be using to measure our patients with shoulder pain? Lori lists several tests she has used and for what over the years: The Penn Shoulder Score (PPS) – a condition-specific self-report measure. It is a 100-point scale that consists of 3 subscales, including pain, satisfaction, and function The American Shoulder and Elbow Surgeons Shoulder Score (ASES) which is a mixed outcome reporting measure. It has 10 questions but some of the questions can be limited depending upon the patient's abilities. Shoulder Pain and Disability Index (SPADI), which is a self-administered questionnaire that consists of two dimensions, one for pain and the other for functional activities. The pain consists of five questions regarding the severity of an individual's pain. Functional activities are assessed with eight questions designed to measure the degree of difficulty an individual has with various activities of daily living that require upper-extremity use. The DASH outcome measure – the disabilities of the arm, shoulder, and hand questionnaire is a 30-item questionnaire that looks at the ability of a patient to perform certain upper extremity activities. The questionnaire is a self-report questionnaire that patients can rate difficulty and interference with daily life on a 5-point Likert scale. Patient Satisfaction Score– a direct question, how satisfied are you with the use of your shoulder presently? 100 is full, 0 is not satisfied. Some of these outcome measures use legacy measure, some are specific questionnaires if your legacy measure does not capture that and then some anchor patient acceptable symptom state or patient satisfaction with the use of your body part that is injured. Lori's Clinical Pearl – “I wish I would have known that connecting with the patient is more important than what you're doing with the patient. I don't think there is a magical set of exercises or manual therapy you can do, how you connect with the patient and deliver care is more important. I try to remember that when I walk in the door, I am in patient mode and the shield is up and I am present with the patient. On our sixth episode of season 3, guest David Bruton Jr., DPT joins! David is an Align Conference keynote speaker this year! He is a former professional football player in the NFL for 8 years 7 with the Broncos and 1 with the Washington Football Team, from 2009-2016. David played at the University of Notre Dame where he graduated with two degrees, political science, and sociology, in 2009. He is a member of the CU Physical Therapy Scholarship Board, Founder of Bruton's Books, and an advocate for change. He graduated from the University of Colorado DPT program in December 2021 and owner of Between the Lines Physical Therapy, helping people return to their game! In this episode they focus on what led David to decide to become a physical therapist, why physical therapy always played a huge role in David's life, his unique role having experienced concussion protocols playing in the NFL, and speaking about them to the senate and more! Here are some of the highlights:David has always enjoyed reading especially when he was injured and couldn't practice in the NFL. He always enjoyed a good book that expanded his mind. In 2015 he wanted to expand his volunteer role in the NFL community with literacy and he wanted to have an impact at the ground level with something that was more his passion. He wanted to be involved in the project. Since he established and founded his foundation, Bruton's Books, they have been able to conduct multiple projects to create opportunities for Colorado's Youth. Their mission is to help low-income children in grades K-3 become strong readers through tutoring and providing books to underfunded schools, libraries, and classrooms. David has opened his own clinic, Between The Lines Physical Therapy. David's Clinical Pearl – “One of the biggest things as a new grad that I learned is that your evaluation does not have to be 100% complete in that first session. This week on the nineth episode of the season Larry Benz, Tim Reynolds and Bryan Guzski are joined by Hunter Hoffman, PhD. He is the Director of the Virtual Reality Research Center at the Human Photonics Lab at the University of Washington Dept of Mechanical Engineering in Seattle and he is affiliate faculty in the University of Washington Depts of Radiology and UW Dept of Psychology. He also collaborates with researchers in UW departments of Radiology, Psychology, Rehabilitation Medicine, Orthopedic and Sports Medicine, and Anesthesiology. Since 1993, he has developed, Virtual Reality Monitoring World, SpiderWorld (For treating spider phobia), World Trade Center World, an immersive virtual reality simulation of the 9/1 attacks for treating civilian PTSD with VR exposure therapy, SnoWorld, the first VR world designed for treating pain and VR DBT Mindfulness Skills learning World (in collaboration with Marsha M. Linehan et al). In 1997, Hoffman and clinical researcher David Patterson originated the technique of using immersive virtual reality for pain distraction during painful medical procedures, at Harborview Burn Center in Seattle. Hoffman, Patterson, and Walter Meyer MD have recently conducted joint research using VR distraction to reduce pain in children with unusually large severe burn wound injuries at Shriners Hospitals for Children in Galveston Texas. Hoffman is also collaborating with researchers at the University of Montreal, using VR distraction to reduce the pain of young children (average age 2 years old) during burn wound care. SnowWorld went on a one year exhibit tour at the Smithsonian Cooper Hewitt National Museum of Design Triennial in Manhattan, and the Boston Museum of Contemporary Art. Hoffman was identified by FastCompany.com as one of the Fast 50 people most likely to influence the next 10 years. Dr. Hoffman says he started out using immersive virtual reality in the 1990s and his colleague and him were the first publish using virtual reality as a distraction. A lot of their early research done was with burn patients during physical therapy range of motion exercises. It worked well for reducing the pain of children and adults with severe burns. As the they were recovering from the burns the skin contracts so there are two things happening, the muscles atrophy from not being used and the skin contracts so the range of motion exercise helps maintain the elasticity of the skin also it helps to strengthen the muscles. As a universal problem with physical therapy, all living organisms avoid pain, so it is hard to get people to do things that make them feel pain. The nice thing about virtual reality is it reduces your pain while you are wearing the helmet. Using it as a simple distraction was easier to get people in. Hunter and his team then developed a question to measure success, How much fun did you have during your wound care or physical therapy? The medical community said that was inappropriate but what was found was that the patients fun went from a 0 or 1 to a 7or 8 out of a 10. Some people pushed back and said they must not of understood the question. Hunter says whether they did or not the answer to the question is how much pain the patient had during their physical therapy, and they are answering that they had fun. It is easier to get someone to do something they remember doing that was fun than painful. In dealing with persistent pain patients Hunter says virtual reality has a lot of potential for at home patient work because it can be more motivating by being intrinsically interesting. To get long-term benefits, he sees an opportunity in changing the persons memories by performing movements with less pain. Avatars are being introduced into virtual reality where you do not need anything on your hand and this is good news for patients with chronic pain. Often, chronic pain patients do not like having any equipment on their bodies. If you start getting more effective treatment you are going to start seeing an increase in retention of physical therapy. On our fifth episode of season 3, guest DeAndre Caldwell, DPT, ATC, MDT, TPS and Jessie Podolak, PT, DPT, FPS join us. This episode will dive into the concept of spirituality and pain and how that might fit into practice. They both will be speaking at the Align conference in August. Jessie is the Program Director for EIM's Pain Science Fellowship, and she owns and operates her community's first direct-pay physical therapy practice, seeing a variety of patients with acute and chronic pain conditions. DeAndre is a physical therapy program manager at ChristianaCare Rehabilitation Services in Smyrna, DE. He is passionate about treating those with low back pain and chronic pain. They focus their discussion on DeAndre and Jessie's Align conference speaker topic, “Beyond Taboo: Accessing Spirituality to Enhance Healing,” integrating that into the clinic, the biopsychosocial model and more! Here are some of the highlights: DeAndre talks about the biopsychosocial model and how it is interesting that spiritual is not part of it, it seems like it is missing. Often someone's spiritual make up is part of them. When you talk to patients you can hear it is engrained in them, but it is now engrained in us to not talk about politics or religion at work or at school from childhood. DeAndre thinks what happens is when patients get to the clinic that way of thinking is still there so they are bringing themselves but part of them is missing so they aren't being their authentic selves because they have a feeling they can't talk about certain things in the clinic. He says that goes for clinicians as well. When asked how they integrate spirituality into the clinic Jessie says integrating open ended questions for patients like, “Where do you draw strength from?” can help. She says we are so used to asking clients for impairments and what is wrong and what are you struggling with? But when we lean into green flags like questions, What is good in your life? Where do you have hope? What gives you hope? Even asking patients what their goals are, Jessie says she usually asks patients about goals and asks for a fun goal and a functional goal and sometimes she says clients will say things that will shock you and you have to maintain that unconditional positive regard for the human before you? DeAndre's Clinical Pearl – “The power of coming to work your authentic self, show up as who you are. There is a scripture that says do not hide your light under a bushel. Be yourself and let your light shine and if that light is reflected, great you guys can explore that, if not, either way you are still going to have a good interaction. Just let your light shine and be your authentic self.” Jessie's Clinical Pearl – “I hope all our young clinicians are already doing this but take care of yourself, recognize this is a part of our health. Take care of our physical health and mental health because we cannot pour from an empty cup. One of the things that has filled my cup is connection to other people and to something bigger than myself. On our fourth episode of season 3, guest Rob Manske, PT, DPT, MPT, Med, SCS, ATC, CSCS, who is a Professor and former Chair in the Doctoral Physical Therapy Program at Wichita State University in Wichita, Kansas joins us. In addition to his full-time faculty appointment, Rob is a physical therapist and athletic trainer for Ascension Via Christi Health and serves as a Teaching Associate at the University of Kansas Medical Center Department of Rehabilitation Sciences in Kansas City, and the Department of Community Medicine for the Via Christi Family Practice Sports Medicine Residency Program. Most importantly Rob continues to practice weekly – spending approximately 10 hours per week treating a variety of knee and shoulder conditions! Rob graduated from WSU in 1991 with a Bachelor of Arts in Physical Education, a Master of Physical Therapy degree in 1994, and further earned a Master of Education degree in Physical Education in 2000. He received his DPT from the Massachusetts General Institute of Health Professions in 2006. Rob has been an APTA Board Certified Sports Physical Therapist since 2002. Rob is also a Certified Strength and Conditioning Specialist (CSCS) through the National Strength and Conditioning Association, and a Certified Athletic Trainer (ATC) through the National Athletic Trainers Association. Rob has been nominated and received numerous awards for excellence in teaching at the local, state, and national levels – receiving the APTA sports award in 2018. To date, Rob has edited/published 12 books, multiple chapters, articles, and home study courses related to orthopedic and sports rehabilitation They focus their discussion on shoulder disorders, lab techniques, special and functional tests, Rob's Align Conference topics & more! Here are some of the highlights:Rob shares how he felt right when he got out of physical therapy school for the first time in three years, he was really struggling to feel like he was helping people to feel better. He thinks the beauty of the PT profession is if you keep trying and learning, as you get older you become better and more skilled and not so frustrated as you were early on in your career. Mark also says doing fellowship helped him tremendously. Rob says today physical therapy students have more information to learn than when he was in physical therapy school and that students are overloaded and when they graduate it takes them a couple of years to figure out what they are doing. He says as a student you must be intentional and learn the order of things and suggests that you need to do that work best for you as a clinician, like how to problem solve and learn what questions to ask your patients. You must learn how to listen. The biggest mistake that clinicians make is not listening to their patients Rob stresses. Rob says he learned the ability to have a sound clinical examination process and all the things he didn't learn in school that they are teaching now by continuing to practice, reading about clinical reasoning, and continuing working. Rob thinks the return to sport testing is not done as often as it should be for upper extremity patients and there are a lot of them offered that could be done. Rob's Align Session Preview: Rob will be talking about special testing, and how it gets a lot of criticism specifically shoulder special testing. He helped Dave Magee on the latest edition of the Orthopedic Physical Assessment, 7th Edition, Rob admits he may be a little biased toward special testing. He says if you look at a lot of different special tests that are done throughout the body, they really are not that great at speciosity. They are better at detecting sensitivity and ruling out the bad things. According to Rob shoulder issues probably are not going to kill you so shoulder special tests are there to help aid him to the conclusion of the root cause of the patients. issue is. He will expand upon those test types at the Align Conference. Rob's Clinical Pearl – Everybody has a certain way that they treat things, my way of treating is not the only way of treating to get somebody better, it works for me, and your way works for you and if your patient is getting better, I think that is great. I've learned you must listen to the patient; you must determine what is not working, and you have to be able to self-reflect on what you are doing with the patient and what they are doing and if it is working or not. The older I get the way more conservative I am. I am very systemic and start off very simple and work to a more complex approach with my patients. I would rather not set my patients back weeks to try to gain a week. Listen to your patient. Be nice, and be kind to your patients. You should see it as an honor that patients come see you. People that are hurt and in pain are coming to you to try to help them. Even if you do not agree with the patient, you can still be nice. Truly want to help the patient. Patients know if you are there just to get a paycheck and you are going through the motions or if you really care about them and want them to get better. If you really care about people, you want your patients to get better, that means you like your patients and I think then you are going to be a way better clinician and more likely a way happier satisfied one! This week on the eighth episode of the season Larry Benz, Tim Reynolds and Bryan Guzski are joined by Mike Rienold PT, DPT, SCS, ATC, CSCS, C-PS. He is considered a world-renowned leader in the field of physical therapy, sports medicine, fitness, and sports performance. He is a noted author, lecturer, consultant, researcher, and clinician. As a physical therapist, athletic trainer, and certified strength and conditioning specialists, Dr. Reinhold has used his experience in a variety of settings to help restore, optimize, and enhance their performance. He has most notably worked extensively with a variety of professional athletes from every major sport, with emphasis on the care of throwing injuries in baseball players. Mike is the co-founder and president of Champion Physical Therapy and Performance, a physical therapy and performance training facility just outside Boston in Waltham, MA, Mike is also currently the Senior Medical Advisor for the Chicago White Sox and the Director of Baseball Performance at Northeastern University, where he has been instrumental in the restructuring and building of the high performance and medical teams. Mike expands on the many different roles and projects that he has taken on during his career. He attributes the processes that he has put into place to manage projects and complete tasks. Mike has always loved art and music. He does everything on his website himself, the design, some of the coding and design just purely because he says it stimulates his brain differently. He is always trying to do something to grow and learn. Mike says, “Creative comes in a lot of different ways and there are a lot of ways to apply creative abilities. I am in a position where I am more of a mentor based leader than he is a dictator. It isn't about delegating and barking orders, it is about putting people in the right position, giving some baseline systems, and then letting them grow and make things better. When you make that shift mentally, I think it changes the way you approach everything.” Regarding the next generation of physical therapist hoping to become sports physical therapists Mike recommends reading the 365 degree leader. He says, “ You can always lead from within, you can lead horizontally and vertically.” There are multiple ways to lead within. On social media Mike says, “If you make too many statements online, I don't want to say you become close minded but maybe you are starting to look at the research to support your past public claims a little bit more than keeping an open mind. As things evolve, you don't want to dig in too deep on an opinion because you run the risk of having a little bias that you (not in a negative way) want to egotistically defend that thought versus I want to keep an open mind. That is a negative about social media, everything is out there for everyone to see, so no one wants to look silly. There are ways to do it, share your journey, I have ton of past students that were doing it right and it is working. On the sixth episode, Larry Benz, Tim Reynolds and Bryan Guzski are joined by National Association of Black Physical Therapists (NABPT) co-founders Dr. Oluremi Onifade, PT, DPT, M.Ed, CCVT, CCI & Dr. DeAndrea Bullock, PT, DPT, CCI, CDP. Dr. Oluremi Onifade is the current NABPT President, APTA GA DEI Committee Chair, and IDEAL Chief Brand Officer. She was born in Kenya to two immigrant parents from Kenya and Nigeria. The youngest of four moved to the states as a toddler. She attended Spelman College where she development a track to allow her to become a Physical Therapist. At Spelman, After the initial rejection to PT school, Dr. Onifade pursued a Master's of Kinesiology at Auburn University. While at Auburn, she earned a graduate assistant position in which she was an educator at a juvenile detention center for a year. Participating in this program she laid the foundation for future endeavors to educate and recruit people of color into the field of physical therapy. She earned her Doctor of Physical Therapy in 2013 from Emory University. After completing training in Neurological specialty in 2014, she went into Home Health in North Georgia. Currently she is in her seventh year treating both orthopedic and neurological cases at an outpatient clinic in Ellenwood, Georgia, where she serves as a minority partner and Clinic Director. In 2016 Dr. Onifade along with Dr. DeAndrea Bullock decided to co-found National Association of Black Physical Therapists (NABPT). Currently they have over 150 members in which the foundation paves the way to increase the numbers of the African Diaspora in the physical therapy field while increasing the knowledge and attendance of the black community for physical therapy. Dr. DeAndrea Bullock is the current NABPT Vice President and a Certified Diversity Professional Assistant Director of Clinical Education for the Doctor of Physical Therapy Program at Emory University of Medicine. She graduated from Kennesaw State University with a Bachelor of Science degree. She earned her Doctorate of Physical Therapy from Emory University in 2013. Dr. Bullock has been practicing physical therapy for nearly 5 years for Wellstar. She started her career in acute care and inpatient rehab at Kennestone hospital. She then transitioned to Neuro Outpatient. Dr. Bullock now works at Emory University as an Assistant Professor. She enjoys mentorship and service projects and has helped mentor high school female athletes. Dr. Bullock is passionate about exposing resources to underserved communities and underrepresented minorities. Due to her own background, Dr. Bullock recognizes the importance of creating opportunities in different communities. She feels that when given the opportunity, young kids and adults can create the life that they want as opposed to reacting to life as it unfolds. They discussed how they were able to create the NABPT organization and how physical therapist outside the organization can support the organization. As well as how the physical therapy profession can recruit more students of color to enroll in physical therapy programs, financial barriers surrounding physical therapy programs, mentorship and more. On our third episode of season 3, guest Mark Jones, BSc (Psyc), PT, M App Sc, who is an Adjunct Senior Lecturer in the University of South Australia with 35 years' experience teaching undergraduate and postgraduate physiotherapy joins us. Mark graduated from the University of Florida with a B.S. in Psychology and then completed his Physical Therapy studies at the University of Iowa. Having developed an interest in manual therapy Mark travelled to Australia to study Manipulative Physiotherapy and after completing his Graduate Diploma in 1985 under Geoff Maitland he completed his master's degree by Research in 1989. The title of his thesis was “Facilitating Students' Clinical Reasoning in Physiotherapy Education”. Mark has a special interest in biopsychosocial health care and the teaching and assessment of clinical reasoning in physiotherapy. He has conducted and supervised research in the areas of clinical reasoning and musculoskeletal physiotherapy with over 90 publications including three editions of the text “Clinical Reasoning in the Health Professions” and the text “Clinical Reasoning for Manual Therapists”. His latest text “Clinical Reasoning in Musculoskeletal Practice” was published in 2019. Mark has presented 30 Keynote Conference presentations and has taught professional development courses in clinical reasoning and musculoskeletal physiotherapy across 26 countries. They focus their discussion on the conversation of clinical decision making and reasoning that is the focus of Mark's Align Conference discussion. Mark has dove deep into the technique of clinical reasoning to grow and develop it. Here are some of the highlights: Mark gave a little insight on how he got interested in clinical reasoning - In 1985 when Mark said he was studying clinical reasoning it was not considered as much as it is now in physical therapy programs. He studied with Robert S. Maitland in Australia and was very intrigued by the thought process he taught without it being overt. Robert didn't have students think about their thinking, he gave them principles to follow. Mark came across the work Arthur S. Elstein, who wrote what probably still is considered the seminal study and paper in clinical reasoning in medicine, he immediately saw that applicably to physiotherapy. Everything that Arthur said about that process that people go through and how important your knowledge is to the success of that process, the concept of patterns, Mark connected that to what physical therapist did. He did his master's degree surrounded around that. Looking at educational strategies to strategize reasoning. I began to theorize what this would look like in physiotherapy - “I found out that good clinicians have always reasoned. It would be arrogant to suggest that it was not always here. The beginning of Physios, there was reasoning always inherent in what they did. We didn't think about our reasoning back then, it wasn't explicit. Maybe that was what I with others contributed to the process. I started theorizing and writing about it suggesting things as to what this would look like in physiotherapy. In our program we started thinking about what it should be because every program that teaches then must start making decisions on the process you want to facilitate and judgements you think are important.” Mark's Align Session Preview: “Physios are noisy.” Mark shares that he will share a podcast and a book at the conference in his lecturer that expands on this quote above. Mark's Clinical Pearl – “My Favorite definition of an expert is somebody who has a lot of experience, they know a lot, they do what they do well, and they can communicate their reasoning well. But they also know what they don't know. They do not have to have a supervisor look at them anymore, they can be on their own and recognize if they didn't understand information or know what a medicine was. They have that mediative congeniality and that is why they have become an expert; they have become aware of their limitations, and they act on it. Ongoing learning cliché is very true, to all of us and our students but it is also being honest. Welcome back to a NEW season of PT Elevated where we are broadening our topics to include more researchers but still focusing on topics that you can use in your clinic every day. This season we will have some of our speakers as guests who will be live in-person at the EIM Align Conference this August 26-28 in Dallas, Texas. On our second episode of season 3 we welcome Dr. Keith Smart, DSc (Hons), Physio, MSc, PhD, who is a keynote speaker at our Align Conference happening this August! Keith is a physiotherapist who worked clinically as a physiotherapist in several hospitals in the United Kingdom and Ireland. He specialized in the field of ‘musculoskeletal physiotherapy.' He completed his research master's degree in 2004 and a PhD in 2010, during which he was the primary investigator on a number of research projects that investigated physiotherapists clinical reasoning of pain, and mechanisms-based classifications of low back pain. He has since acted as a co-supervisor to a number PhD candidates as well as undergraduate and masters-level physiotherapy students. He has also undertaken several systematic reviews related to pain. He is now a full-time academic physiotherapist at University College Dublin but maintains a small clinical caseload as a Clinical Specialist Physiotherapist within the field of musculoskeletal physiotherapy and orthopaedic triage at St Vincent's University Hospital, Dublin. He has ongoing research interests related to pain and advanced practice physiotherapy. They focus their discussion on the conversation of pain mechanisms that is also Keith's speaker topic of discussion at the Align Conference, and he expands on what his speaker discussion will also cover at the conference and more! Here are some of the highlights: Keith published a series of 3 influential papers about 10 years ago on mechanisms of pain and broke them down to how we might be able to categorize where our patients fit in those categories. They discuss how he got interested in pain science initially and what about his findings surprised him. On a broadscale Keith tells how he used questionnaires, clinically when assessing pain and why. Listen in to hear more surrounding the topic of pain mechanisms. Keith's Align Session Preview – Keith will be presenting virtually in August at the Align Conference; he is looking forward to it! He will be presenting on the broad topic of pain mechanisms, keeping it clinical. He hopes to be able to explain to the attendees what some of the limitations and shortcomings of mechanisms-based patients of pain are. Also, what that looks like in clinical practice, how we might begin to distinguish between different types of mechanistic categories of pain and how we might tell them apart in our patients. We are always taking a path, and if you are always saying yes without evaluating whatever the opportunity is, you are saying no to something else.” Karen says we must be realistic when we set expectations with people dealing with chronic pain. Our first episode of season 3 welcomes Dr. Julia Treleaven who is a keynote speaker at our Align Conference this August. Julia is a senior lecturer and researcher at the University of Queensland in Australia and a part of the head and neck research unit. She has been researching whiplash and neck pain since 2000 and in 2004 completed her PhD focusing on the neck's influence on sensorimotor control. She has continued her research in this area and now is also looking at the role of neck dysfunction in those with headache, dizziness and post-concussion due to concomitant whiplash-type injury. She has over 100 publications in this area, written several book chapters and is an author of the recent book “Management of neck disorders- an evidenced based approach.” Julia works part-time as a physiotherapist in a private practice managing patients such as those with, whiplash, cervicogenic dizziness and post-concussion syndrome. They focus their discussion on sensorimotor control and what Julia's speaker discussion will be focusing on at the Align Conference and more! Here are some of the highlights:Julia discusses how important it is to understand why sensorimotor control is important in the head and neck. She discusses the different tests that can be used to find and diagnose a problem and find out where it may be coming from and if the neck or head is involved or not. Julia says today, "We are understanding more how to assess patients to decide whether the head or neck is involved or not." She talks about how to handle joint restrictions and more! Julia's Align Session Preview – She will be going over the Clinical Assessment of Sensory Motor Control in the neck and tests that she thinks might be useful clinically to help differentiate if it is coming from the neck or the vestibular system. Then some ideas of how to integrate that into training for patients as well. Julia's Clinical Pearl – “Really think about your examination: what you are feeling, what you are seeing? Try not to rely so much on pain, then try to put it together, does it make sense? If it is not making sense then you probably have missed something or you need to assess if there is anything else you can look at. If it is making sense, then fantastic! Really just focusing on what you are feeling and seeing and if it is making sense." Now current & incoming physical therapists will have pain science incorporated from the beginning of their careers. Adriaan discusses the algorithm of 3-3-1 they developed to teach to PT students to get them to understand pain science - The three types of pain, nociceptive, peripheral neuropathic and central pain. We examine them different, treat them different and allocate our resources to treat them different. To treat persistent pain, you need 3 things: to think different (cognitive), movement & calming the nervous system down. Finally, knowing every case of pain is unique. Adriaan: “There is nothing more challenging in clinical practice then to change a person's life. To change a person's life where they are going attend a wedding or go to a family reunion or lift a grandchild again, those things matter. I can probably remember a handful of people that I manipulated their neck and they felt better but I almost remember every person along the path that I changed their life. Morris, SPT will graduate from Ithaca College's accelerated Doctor of Physical Therapy program in May 2022 and will start Emory University's Orthopedic Residency program in August 2022. She currently serves on the APTA Student Assembly Board of Directors as Secretary and DEI Project Committee Liaison, and as the Student Member of the PTJ Editorial Board. Devon is an active member of Ithaca College Physical Therapy Students of Color (ICPTSOC) and hopes to inspire other women of color to pursue leadership positions within the physical therapy profession. He completed his transitional DPT graduate from the Post Professional Doctorate of Physical Therapy Program at Des Moines University, Class of 2010. At this time, he is in the candidacy phase in the PhD PT program at Nova Southeastern University. Currently Dr. Zimney is an Assistant Professor within the Department of Physical Therapy at the University of South Dakota, Senior Faculty with International Spine and Pain Institute (ISPI), and researcher with Therapeutic Neuroscience Research Group and USD Center for Brain and Behavior Research. His primary teaching, research, and treatment focus is with pain neuroscience, therapeutic alliance, and evidence-based practice for orthopedic injuries of spine and extremities. He has published multiple peer reviewed research articles in these areas. Past work experiences have been with various community-based hospitals working in multiple patient care areas of inpatient, skilled rehab, home health, acute rehab, work conditioning/hardening and outpatient. He has completed the Advanced Credentialed Clinical Instructor program through the American Physical Therapy Association and is a Certified Spinal Manual Therapist (CSMT) and assisted in the development of the Therapeutic Pain Specialist (TPS) through the ISPI certification program; and has a Certification in Applied Functional Science (CAFS) through the Gray Institute. For more information on Jessie: Jessie received her Master's Degree in Physical Therapy from the College of St. Catherine, Minneapolis, in 1998. She completed her transitional DPT from Regis University, Denver, in 2011. She has been teaching pain science and manual therapy techniques at continuing education courses since 2013. Jessie currently owns and operates her community's first direct-pay physical therapy practice, seeing a variety of patients with acute and chronic pain conditions. She has special interests in manual therapy, Pilates, spine and running injuries. She is a certified clinical instructor through the APTA and has completed her Therapeutic Pain Specialist certification through ISPI. Read the full transcript below: Karen Litzy: 00:00 Hey everybody, welcome back to the podcast. I'm your host, Karen Litzy coming to you live from the align conference in Denver, Colorado. And I am fortunate enough to be sitting here with Kory Zimney and Jessie Podolak and we're going to talk about the workshop that they did yesterday and will probably do again tomorrow on moving our language and why language matters around people with persistent pain. So my first question is why does it matter? Jessie Podolak: Well, words are powerful. We started off by just doing some cool quotes that words change worlds, right? And words can pierce like a sword. The tongue of the wise brings healing. And that's just ancient wisdom, right? We've known that words just have so much power. They shape our perceptions, they shape our action. We know even from the research, just how we look at something. So for example, one of the studies we cited was if crime is presented as a beast, okay, crime is a beast versus crime as a virus. Jessie Podolak: 01:12 When crime is presented that way with just those two words. And we survey people and we say, what should we do about crime? Those who hear crime is a beast, 71% say we should increase law enforcement. 51% of those who hear crime has a virus say we should increase law enforcement. So the word evokes more of an action response when we hear the beast versus virus. And other one was the economy, is the economy stalled or is it ailing? If the economy is stalled, we jump start it, right, stimulus package. If it's ailing, maybe we take measures that are really going to do long term change. We look at education levels or socioeconomic things and what can we do with this economy? So words shape so many things in general and in healthcare, the word surrounding pain, can evoke a lot of fear. Jessie Podolak: 02:08 They can evoke a lot of a knee jerk reactions of what needs to get done. It can kind of force us to look at these more short term solutions. And I think that's been a theme emerging throughout this conference is that there's so many things that we do that are helpful in the short term but can actually be harmful in the long term. So the words that we have surrounding pain, probably lend themselves many times to short term solutions. And if we want to look at really a sea change in how we approach pain, we've got to think and consider our language. Kory Zimney: 02:45 When we look at what we're just talking about, you know, a lot of people, I think they look at it and they go, well that's just a little change. You know, it was only 20% different. What's the big deal? And to me, you know, and it's all about nudges, that a lot of times it's just these little changes that can make huge difference for some people. And I get for a lot of people it probably wouldn't make a big difference, but if it did make a difference for a person, why wouldn't I want to try to maximize every little opportunity that I could get? And I know some people look at it like, well, I don't think language is that be all, a lot of people I can tell arthritis and they don't have a problem because I used that word and I get that. But what about that one person that it did make a difference for? How do you know it didn't make a difference for somebody? And if we have good evidence that shows that these little changes can make a difference, why wouldn't we try to maximize every little bit of that? Karen Litzy: 03:33 Yeah. And I think that harks back to Kory to what you said this morning about everyone in the room has probably treated one person in pain and that's great. You treated one person, but you can't extrapolate what works for one person to a population. And so I agree that I think in as much as saying, do no harm, changing words around that might connect with someone I don't think is going to be incredibly harmful. By reframing words that maybe we know might be a little harmful. Like arthritis or what are some other ones from yesterday? Jessie Podolak: One for me was wear and tear. How often do we say wear and tear. And what's the first thing that pops into your mind when you hear wear and tear? What's an object? Yeah, the tire. And what's that gonna do? It's gonna blow. Jessie Podolak: 04:24 Right? So if I say you have wear and tear, what is kind of even a subconscious thing? They're just waiting for it to blow. And how does that influence your movement? How does that influence the adventure you have in life? How does that influence your whole being? Just knowing I have wear and tear for some people they might say, well I don't care. I'm going to wear it out. I'm going to grind that thing to the ground. But for others they might say, oh my gosh, these tires have to last me another 20 years. I better take really, really good care and back way off. So wear and tear is a hot button one for me. Kory Zimney: 05:03 But yeah, so it's just those little phrases that are so easy for us to throw around. But we have to recognize that the lens that the patient looks through is probably different than the lens that me as the therapist with all my education and training on how I look through it. And I think that's just, again, taking that patient perspective is something that we all can hopefully try to do a little better sometimes. Karen Litzy: 05:28 Yeah. And one thing from yesterday's class that I had never heard of before was negative effective priming. So can you explain what that is and then how we use it maybe not even knowing we're using it as therapists. Kory Zimney: 05:43 Yeah. It’s really kind of what you talk about is kind of what you start thinking about. And so if I'm telling you how you're going to lose, if you don't do your exercises, you won't be able to do these things. And just create more of a negative type of attitude to everything, in everything the patient sees then will be directed more towards the negative. Where if you can flip it to more of a positive type outlook as far as when you do this, you'll be able to do these things and you can do that. And again, always flipping it to more of a positive direction. So again your just priming them, nudging them, turning them towards things that they can do as compared to, you lost this, you won't be able to do that. So, it's those little shifts and changes to focus on those positives. As a clinician, you know, you struggle like our patient’s so negative. And then we come up with these negative phrases sometimes and it's like, well, how are we helping prime them the right direction? Karen Litzy: 06:34 Right, and what are some examples of maybe common negative priming that we may do as therapists? Kory Zimney: 06:41 If you don't do your exercises, you know, that shoulder's gonna only get worse. You know, if you're overweight, you know, this puts lots of extra pressure on your knees, they're more likely to wear out. It’s just those little negative type of things. It's so easy. We can look at, we were talking about what they lose, you know, the kind of the gain aspect or the loss aspect. And oftentimes we tend to talk about the losses and patients will get focused on that, on the negatives. That's just human nature that we focus on negatives. As a clinician, if we're adding to that, it's only going to multiply more. Back in younger days as a clinician, I'd always get so proud of, you know, if I could get their problem list to 10, I thought, how cool am I am double digits. Kory Zimney: 07:24 You know what I mean? Just get that problem list as long as possible, you know, but really looking at the optimism list, what things can they do? You know, what things can they do better? And you know, isn't that, how cool is that? That you can do that? In focusing on those things and what they can do better, what things they can do instead of on what things they've lost, what things they couldn't. So that's that kind of priming a kind of nudging more into a positive direction compared to our traditional, you got dysfunction, you can't do this, you're broken. Jessie Podolak: 07:50 Yeah. And even the way we asked that question, Lindsay had just a really nice thing this morning that she talked about with goals instead of, you always think of, you know, what are your goals? And that's kind of an obscure thing, but I think she asked it in a way that was something like, tell me something that you'd like to do more of, be better at, or return to doing that you currently can't. It flipped it because it started, you know, there's this great quote from a Ted talk that I love by Kelly McGonigal called making stress your friend. It's awesome. She has this quote in there near the end where she said, you know, it's so easier to run towards something than away from something. And if you look at your patients, what are they right in their goals? Jessie Podolak: 08:29 I want to get rid of this pain. I want this away from me. I want to avoid it. It's so overtaking their life that they're running from it. But if we can just direct people towards what is to come and even get them to maybe cast a little vision, which I know is scary. Right? And you don't want to have false hope. We talked a lot about that, about how to balance reality and honesty. And sometimes to say, I'm not sure how this is going to turn out, but I'm with you in it. Right? But I think, you know, this is the worst I've ever seen, or man, this is the biggest trigger point I've ever felt, no wonder you hurt. Those things come from a place of pity or sympathy which it's well intended, but it's not as far on the empathy and compassion scale that we want. Jessie Podolak: 09:26 We want that empathy and compassion of, I see where you're at and where you've been, but I'm with you as we go forward, I guess how I look at it. Karen Litzy: Absolutely. And I think that sentiment of yes, I'm with you, but being honest, so doesn't mean everything's pie in the sky. And I think that's where people, when they hear about this, explain pain, quote unquote or PNE, they think, oh, you're just talking away the pain and you're not being honest. You're not being realistic. But that's not what we're saying when you're talking about language and talking about communicating with someone who has persistent pain. So one of the examples we used yesterday was like hippo A and we said, you know, yes, you're, you may have pain and we're going to work on strengthening. There is a chance you might need surgery, but if you do, you'll be stronger going in. So you have to be honest, you can't say to someone with severe hip OA, you'll be fine. Just do a couple exercises. It's just not realistic. And then when the person isn't fine, that's a steep fall. Jessie Podolak: 10:18 Yes. And it goes back to this, not swinging too far on the pendulum away from the bio, it's still bio-psychosocial. And how do you explain something that there are biomechanical issues in a way that's not scary that still honors the bio, but that kind of de-catastrophizes or softens, it's really just about softening and responding. Like watching the patient's nonverbals. You can tell when you're starting to freak somebody out. And so then you make the adjustment and you just be very, very present. Jessie Podolak: 11:12 So it's certainly our language, but like, as you know, Kory talked about is communication. And I really like what Jonie said about pain neuroscience communication versus just education, I the smart therapist I'm going to teach you, silly patient about how this works. No, this is about communication and dialogue and how do we do that? Karen Litzy: Yeah. And Kory, I think you said this yesterday, but correct me if I'm wrong, I think you said that the body is not fixed rather a robust ecosystem that has the ability to change and grow. Kory Zimney: 11:54 Yeah. And that was actually a TPS grad that we have that talked about that. The beauty of the amazing plasticity and I mean I go back to when I used to, you know, work somewhere in our rehab unit and when a patient came in with a stroke, you knew there was brain damage and you could see the MRI report. But the beauty is you had no idea what they might be able to function and do afterwards, right? Because you'd look at those areas that were destroyed, where the infarct was and stuff like that. And some of them amazingly regained function and the ability to walk and their ability to transfer and get out of bed. So you just always had this ultimate optimism, you know, as the traditional neuro type of Rehab Therapist, when somebody would come in with their stroke or spinal cord and in their ability to be able to do things. But for some reason in the orthopedic world, we just have this like, oh, well, yeah, sorry. Karen Litzy: 12:38 Yeah, sucks to be you. Kory Zimney: 12:44 We just create this, like the body can't be adaptable to these things. And now that they've done the imaging studies on normal people, we're all walking around this stuff. We've all had this beautiful adaptability, whether it was from a neurological orthopedic, any kind of change that's gone on on our body, but we don't ever appreciate, and look at that from that optimistic again in realistic sense, you know. But again, we know that if you have a little tear in your meniscus that might be an issue. Yes, it's a huge bucket handle and you can't straighten your knee out and it clicks every step. Yup. That's probably a major deal. But otherwise a lot of people can get by with that. No, I don't know with absolute certainty, but the beauty is we should be able to find out in four to six weeks because we can train the body, help it become more adaptable. We can explore different motions and movements and see how you do with it. And if it still doesn't, the awesome thing is we do have surgical options, to make that better. And so that's just that beauty of appreciating the adaptability of the human body. And I don't know that we, for some reason, we seem to have lost that appreciation to some degree. Karen Litzy: 13:46 Yeah, and I think that's something that I know I'll be using with my patients just to say, listen, you are this robust ecosystem, and I think if we share that with all of our patients, I think they may have a mind shift change there. Jessie Podolak: Yeah. If you think of ecosystems, so many things go into it. Yeah. Right. It's not just the musculoskeletal. I think just that if people could really view the body as juicy and more robust and just multifactorial, and I think that's where maybe we got off track is we just started seeing the body as a machine. Karen Litzy: Which I have to say is my pet peeve. I hate when people say, your body's just like a car. I'm like, no, it's not because the car doesn't breathe. We're not mechanics. We're not this. Like that is not how it works. Where I'd like to think as people we’re a little more complex and in a very good way, right? So now what would be the thing that you want people to take away from why language is important when it comes to working with people with persistent pain. Kory Zimney: 14:56 For me it's just being mindful of that, you know, taking that moment and again not to as a therapist, don't overthink it either. Don't think, oh, what words can I say? And if I said arthritis all crap, their patients going to catastrophize and never be able to walk again. No. But just be mindful of it and be present with your patient. Because when you're truly present with your patient, you can see that look in their eye and you can get that sense that they may be getting a little bit worried or catastrophizing or a little anxious and stuff like that. So it's that ability to just be present and mindful that words do matter. But again, not so overly mindful that you freeze and you don't act either. We still have to just be human, just being a part of that. And again, that's just that communication piece that really is what we're talking about. Jessie Podolak: 15:38 I would just echo what Kory said. It's just be with your patients. Care, invest in them. Some of the patients who it takes every ounce of energy they have just to make it to your appointment. Realize that they're giving you the trust and kind of the gift of their time and their precious energy. And so, even when you have that busy day, even when you know you're kind of sucked dry, just to give them that time that you have with them and to slow down a little bit, listen, be mindful and you know, I just think it's just about being a little softer, just softening out the rough edges and being that safe place. You know, Louis Gifford, one of our heroes said reassurance is an analgesic and sometimes we can't reassure that that hip is going to not need surgery, but we can reassure that I'll be with you. We’re in this, I'm in this with you. So that's what I would say. Karen Litzy: Awesome. Science News and the media shared some recent research on what morphine does in terms of pain reactivity as well as what is happening with post-surgical opioids. We welcome Dr. Peter Grace to discuss these findings and to unpack this information to help us in this battle against the opioid crisis. Peter has a balanced viewpoint that should give us some good information to consider, going forward. Additionally, Tim, Jeff, and Peter discuss Peter’s current research on understanding the mechanisms of pain and the role of peripheral immune cells and neuropathic pain. They also discuss the transition from acute to chronic pain and the effect of opioids and morphine substances on moving into that persistent pain pattern. Peter is an Assistant Professor in the Department of Critical Care Research, at the University of Texas MD Anderson Cancer Center. His research aims to understand the neuroinflammatory mechanisms that drive chronic pain, in order to identify new treatment strategies. We welcome Ash James to Pain Reframed! This week, we are talking about reducing fear, encouraging a variety of movement, and getting to the root of what’s going on with a patient’s body. We want to ensure patients don’t catastrophize, be afraid of their bodies, nor let negative thoughts and other things get in the way of living and enjoying life and full movement and ability. Ash has worked in various sectors of musculoskeletal physiotherapy, including private practice, professional sports and occupational health. He is now a Clinical Lead for a large Occupational Health Company, runs external courses and workshops, and is looking at ways to improve the care of people within the community. Mark also, specifically, shares how his base level of pain increased significantly and immediately after seeing an image of his fracture, continuing to show how cognitive our pain really is. Mark is an amazing musician who has a passion for the guitar: specifically his 1980s Martin. Mark also has Multiple Sclerosis and is very close to the release of a new CD, featuring incredible musicians from around Colorado. Sometimes patients will hear mixed messages and contradictory advice, which can be a huge problem in their treatment and faith in the system, especially when they have high concern and fear levels. Thankfully, Dr. Becky Vogsland has been working with an amazing team to create a system where a common message is focused on to truly help patients. Becky’s story and their results are phenomenal! Becky has practiced at the Minneapolis VA in the areas of traumatic brain injury, polytrauma, orthopedics and chronic pain since 2007. To better serve her patients, Becky has obtained certifications as an Orthopedic Clinical Specialist, Certified Manual Trigger Point Therapist, Certified Spinal Manual Therapist, and in Rehabilitation Pilates and TRX. Certain regions of our country have a particularly challenging time dealing with the opioid crisis. These regions, not only have the challenge of seeing improvements with the crisis, but are also still seeing a rise in opioid use. In addition to his teaching and research at Curtin University, he works in clinical practice as a Specialist Musculoskeletal Physiotherapist (as awarded by the Australian College of Physiotherapists in 2005). He is recognized internationally as a leading clinician, researcher and educator in the management of complex musculoskeletal pain disorders. Tim, Jeff, and Peter discuss being authentic with patients and walking alongside with them in ways the medical industrialized complex doesn’t. Peter shares his success story of deeply caring for patients for a long time and being willing to drop his pride and ego when the patient wasn’t getting better. Beth has a focus on pain catastrophizing as well as treating some of the key issues that prevent patients from succeeding with other pain treatments. Clinically, Beth provides psychological services to patients with catastrophic burn, spinal cord injury or amputation. Her desire to specialize in the management of chronic pain was inspired by her clinical experiences and by her own personal experience with chronic pain. Beth brings a wealth of knowledge to the episode as they discuss some key aspects to managing patients with chronic pain as well as altering systems to maximize positive treatment effects.
