In today's podcast episode, I speak with physical therapist, chiropractor, researcher and educator, Greg Lehman. Greg has been in the rehabilitation field for more than 20 years. Following his undergraduate degree in Kinesiology, he was awarded the Canadian Society for Exercise Physiology Gold Medal for highest academic performance in Kinesiology. This degree allowed him to obtain certification as a Strength and Conditioning Specialist and Certified Fitness Appraiser and to work as a Strength and Conditioning coach for Sir Wilfrid University's Men's Basketball team and for Queen's University Women's Varsity Hockey Team. Greg's success in university resulted in being awarded a graduate scholarship (NSERC) to the University of Waterloo to be one of only two Masters students per year studying at the Occupational Biomechanics Laboratory, a world leader in Spine Biomechanics, exercise prescription and athletic performance. As a faculty member at the Canadian Memorial Chiropractic College, Greg developed a research program that produced more than 20 publications on exercise biomechanics, golf fitness and the science of manual therapy. He has taught more than 25 graduate students in Spine Biomechanics and Research Instrumentation and supervised more than 50 students and 20 undegraduate research theses. He was subsequently awarded a Researcher of the Year award by the Ontario Chiropractic Association. These days, most of Greg's time with patients is spent one-on-one using exercise, load/stress management, manual therapy techniques and education. Both pain, injury and performance are influenced by a number of factors in an individual's life. Greg's approach addresses the multitude of these contributors and he primarily works with my patients to come up with strategies where they are actively involved in their recovery. Physios have been lambasted awhile for not understanding the basics of strength and conditioning principles. I reject this idea. There two areas we explore in this podcast. 1. The idea that strength and conditioning basics are relevant for rehabilitation 2. Whether physios actually know these basics. I make an attempt at trying to understand the arguments againsts physios and try to provide a competing view. Here is how the AI summarized it (not really accurate but I don't want to piss off the robots): Recovery Mechanisms in Rehabilitation Programs Greg discussed the importance of understanding what facilitates recovery in rehabilitation programs, emphasizing that it's not always about building strength. He argued against the judgment of programs based on arbitrary ideas of what needs to change, as the actual mechanism of recovery can vary. He also addressed the concept of tolerance, suggesting that improving a person's ability to tolerate loads can lead to recovery, even without necessarily building strength. Pain also doesn't always mean tissue damage as we are mostly taught to believe. Author Ryan Whited has been helping athletes at his gym, Paragon Athletics, for many years using the biopsychosocial model of pain. In this book, he shares his own experience with pain, what the research says about pain, what the biopsychosocial model of pain is and Paragon's Training as Treatment model that anyone can use to work their way through non-traumatic injuries.Ryan Whited is a personal trainer, elite climber, and the founder of Paragon Athletics, a gym that empowers athletes with sport-specific strength and conditioning to complement their performance goals. His passion for working with athletes and investigating the nature of biomechanics and human performance is the basis for Paragon's innovative Training as Treatment program. My guest for this podcast is Greg Lehman. Greg is a physical therapist, chiropractor, and researcher. Our main subject was a new study that found very positive results for Cognitive Functional Therapy (CFT). I thought Greg would be a good guy to ask about the strengths and weaknesses of the study, and how to interpret its meaning in light of the broader literature.Greg is a popular writer and teacher whose main interest is reconciling pain science with biomechanics. Part of his approach involves looking at many different kinds of therapies that seem to work, and then asking: what do all these different methods have in common? It's a good question that generates insight. One of Greg's strengths is a broad knowledge of the relevant research. Therefore, he always supports his arguments with specific citations, and he also has a good context to interpret the meaning of a news study. For example, if a new study comes out showing that therapy X works or doesn't work, he can probably think of other studies with different results.Another great thing about Greg is that he is always willing to challenge his own biases. So if a study comes out validating his ideas, he will be the first one to stand up and point out its limitations.In this podcast we talked about the strengths and weaknesses of the new CFT study, and how to put it in the context of the larger literature. There is certainly no shortage of "advice" out there about running form. If your social media habits are anything like mine, you can't scroll down your feed more than two or three times without seeing some video or image depicting "bad running form" or "good running form" or some sort of transition between the two. Heck, I've put up quite a number of these posts myself! BUT! What does the evidence say? What aspects of running form have been shown to influence running performance? What about injury rates? These are very important factors to consider before you start messing around with your running technique. Today we are joined by the one and only Tom Goom, aka The Running Physio. Many of you will be familiar with Tom, over the last decade he has gained quite a reputation for his ability to translate running injury research into easy-to-digest messages. Tom teaches clinicians all over the world through his Running Repairs Courses and has written for Running Fitness, Men's Running UK, the British Journal of Sports Medicine and the Journal of Orthopaedic and Sports Physical Therapy. His website, Running-Physio.com is the go-to resource for clinicians and runners who want to know what the science actually says about running injury management. To put it simply, there is no one better positioned to give us a "thousand-foot view" on running form research.
