Professor Lorimer Moseley is neuroscientist, who specialises in the complexities and mind-boggling nature of pain - what it is, why it exists, how it works and when it can go wrong.For most of us, pain is a fundamental part of being alive, and staying alive and yet none of us will ever experience the exact same pain as someone else, which makes it incredibly difficult to understand.Every day, we stub our toes and burn our tongues. Some of us break bones and suffer from more serious illnesses and conditions.What you feel when your skin is broken or a ligament is torn is there to tell your brain to be careful, that something is wrong and needs to be protected.But what happens when doctors can't find any damage? Bill is a Physical Therapist and, together with Mike Robertson, founded Indianapolis Fitness and Sports Training, known as IFAST, located in Indianapolis, Indiana. Over the past decade, IFAST has quietly become one of the predominant hubs of education in the industries of PT and S&C. Former Bill mentees include Zac Cupples, Mike Camporini, and what feels like 25% of medical and physical prep staff in the NBA and MLB. This past January, Tim had the pleasure of going out to Indy for 3 weeks to learn from Bill. This upcoming January, Tim will be heading back out for 3 months to continue his study as he and Bill attempt to improve Tim's movement profile enough that he may be able to avoid hip surgery. Bill's a fiercely independent thinker, and one of the most intellectually honest individuals on the planet. As such, his style of communication may not be FOR everyone. But, as you'll hear from him, that's kind of the point. The topic of the conference was the launch of the revised Persistent Pain guidance first published in 2019.  The COVID-19 pandemic has changed how we live and access services. In this episode, Drs. Mark Hoffman and Amy Park invite Dr. Isabel Green, fellowship director of MIGS at Mayo Clinic, to speak about persistent pain in endometriosis patients. --- SHOW NOTES Dr. Green begins by defining endometriosis as endometrium-like tissue that grows outside the uterus, but emphasizes the complexity of the disorder with its varying phenotypes and inadequate knowledge about the disease process. Patients with superficial endometriosis can have terrible pain while others with deep disease may have little pain. It's also common for people to have comorbid conditions, like IBS, fibromyalgia, migraine, myofascial pain, and central sensitization disorders. This makes it a difficult disease to diagnose and treat, and highlights the need to tailor therapy for individual patients. The doctors discuss the lack of data to support pain medications like gabapentin or amitriptyline, and even the typical treatments of birth control pills or excision surgery can fail to help patients. The doctors then go on to discuss the pathophysiology of endometriosis. Dr. Green believes it may be different for different people as retrograde menstruation fails to explain all cases of endometriosis. There is research on immune dysregulation and inflammatory cytokines that could show the disease is systemic and not just contained in the pelvis. Additionally, some believe it could be cells transforming similar to a malignancy or even a nervous system issue, but more research is needed on the topic. There is a lot we still don't know about endometriosis. Next, Dr. Green explains the challenges in diagnosing endometriosis. Patients have variable symptoms, people may dismiss pelvic pain and normalize the symptoms, and ultrasound/MRI is only good at visualizing deep lesions. It is often years by the time patients go to surgery and endometriosis is visualized. The episode ends with the doctors discussing how to manage patients long-term, especially if the typical treatment of hormonal and pain medications and excision surgery fail. Listening to the patient's history, knowing exactly what the patient's lesions looked like during surgery, repeat imaging, and learning the degree of neural involvement can help tailor the next steps. Back in the 80s, he began seeing an increasing number of fibromyalgia and chronic fatigue cases, but because the medical world was unfamiliar with this condition, patients were sent to therapy. From there, he started connecting the dots to conditions such as mold toxicity and started helping his clients, even though it wasn't the "norm" in the medical world. From there, his journey into mold and understanding how it affects the body took off. This includes studying with Dr. Shoemaker. 6:00: Dr. Nathan goes through his other books and what sparked his desire to keep releasing information about mold, healing, and treatments. 9:30: Dr. Nathan discusses his idea of how many individuals are suffering from mold exposure, his experience in treatment, and the common symptoms he's seen. He goes over mold allergy and mold toxicity as well as the difference between the two. He discusses the influence of genetics and the various other components that result in adverse health reactions. 14:00: Dr. Nathan addresses the misconception that since mold exists in nature, it's no problem for our health. He also discusses mycotoxins, a toxic byproduct of some mold species.16:30: Dr. Nathan discusses why he agrees to disagree with Dr. Shoemaker on the idea that it's not mold growth in a home that's causing health issues, but actinomyces.
