First appearing in the early 2000's, this type of practice is characterized by the elimination of third party payers. In most cases, insurance billing is replaced by a subscription or “membership” model in which patients prepay for their care on a monthly or quarterly basis. The typical DPC practice agrees to provide a comprehensive set of outpatient services that meet the needs of most patients. Hospitalizations and high-cost surgeries are generally covered by traditional insurance. Under this model, the elimination of insurance billing results in lower overhead, less paperwork, fewer employees, and better access by expanding the use of texting, phone calls, email, and telemedicine. Office visits are generally longer and more comprehensive with less time spent on paperwork. While originally designed for primary care, there are examples of specialty-based DPC-style payment arrangements. John describes why this can be the solution to burnout and a way to recover the joy of practicing medicine while earning a very good income. He also explains that creating a DPC practice requires extensive planning and significant financial resources, but less than opening a traditional practice. In today's episode, Dr. Feldman, a former psychiatrist, gets into the nitty-gritty of what it takes to practice forensic consulting. He answers questions regarding the required training, liability risk, marketing strategy, potential earnings, and how this specialty differs from working as an expert witness. However, for the last 13 years, he has assisted attorneys in pre-trial and pre-litigation medical-legal case preparation. Rather than function as an expert witness, Dr. Feldman delves into the medical literature to define each case's merits, interviews every client, and prepares a report. His work helps attorneys negotiate and arrive at prompt and favorable settlements. These are not medical malpractice cases but involve disability, personal injury, or workmen's compensation. To date, Dr. Feldman has accumulated a vast experience, assisting in over 3,000 cases. Dr. Feldman has already shared his experience with this type of medical-legal consulting with over 1,600 physicians.
