I review specific options that have proven helpful to my listeners, including Dr. Heather Fork's résumé and LinkedIn courses, Dr. Gretchen Green's Expert Witness Startup School, real estate investing programs, and the RAL Room Mastermind for residential assisted living. I also discuss my $100,000 investment in a real estate syndication that originated through one of these programs. Managing 1,900 patients in a corporate system left her feeling like she was "flipping patients like burgers every 10 minutes" despite her love for rural medicine and helping patients heal. This classic 2019 interview reveals how Dr. Salyer built a cash-based micropractice after giving her employer eight months' notice and fighting through non-compete challenges. Her membership model allows 60-75 minute patient visits focused on root cause treatment, using specialized testing and natural interventions that aren't available in conventional settings. Beyond her clinical transformation, Dr. Salyer discovered an unexpected calling as a creativity advocate for physicians. Her "Right Brain Rescue" program teaches medical professionals to tap into flow states and innovative thinking, leading to speaking engagements at residency programs and medical conferences where she promotes the restoration of joy and innovation in medical practice. Dr. Schloe's journey began during residency, when he purchased his first investment property with a physician loan and just $58 out of pocket. That early experience showed how house hacking and strategic real estate investing can be accessible even during medical training. He explains how residential assisted living differs from traditional large-scale facilities, offering personalized care in smaller, home-like settings with far better caregiver-to-resident ratios. These homes not only improve care quality and outcomes, but they also cost less than institutional care. We also explore the strong investment potential behind this model, from the massive care gap created by the aging baby boomer population to the various ownership options physicians can pursue, whether running their facility or investing passively. With their medical background and income potential, physicians are uniquely positioned to identify quality care and participate in a real estate opportunity that's both profitable and socially meaningful. She describes the world of Medicare administration, where Contract Medical Directors are responsible for creating Local Coverage Determinations—policies that guide what treatments and services Medicare will cover based on medical evidence. With the introduction of the 21st Century Cures Act, these policies now require greater transparency and rigor, opening the door for physicians with strong clinical and writing backgrounds to contribute meaningfully to federal healthcare decision-making. Dr. Loveless also outlines what the role involves day-to-day, including frequent travel, collaboration with subject matter experts, and the long timelines often needed to develop and implement coverage policies. She offers a grounded look at Medicare administration, challenging misconceptions while showing how physicians can move into policy work that influences care access and quality for millions of people—bringing clinical expertise into one of healthcare's most impactful arenas. He shares how the success of his practice inspired the creation of Atlas MD's electronic medical record system, designed specifically for direct care practices. Unlike traditional systems overloaded with insurance-driven features, this EMR streamlines clinical work by removing unnecessary administrative functions. He also introduces his latest venture, Not Health Insurance, a fixed indemnity plan designed to complement DPC by covering major medical costs like hospitalizations, cancer treatment, and emergency procedures. Unlike ACA plans, this model returns money directly to patients and takes advantage of significant uninsured discounts from hospitals that often go unnoticed. By removing insurance barriers from routine care while still providing support for serious medical needs, this approach creates a more efficient and accessible healthcare experience. It also addresses common concerns about physician access and system sustainability, offering a model that prioritizes both patient outcomes and professional satisfaction. While working as a biller and coder during his pre-med years, he saw how much time and money was lost to bureaucracy. Inspired by his father's straightforward trash collection business—“pay once a month, pick up once a week”—he applied the same logic to healthcare: simple, transparent, and direct. His model strips away insurance entirely, offering wholesale medications, low-cost labs, and unlimited access via calls, texts, and in-person visits—all without billing headaches. By eliminating the administrative layers that bog down traditional practice, Dr. Umbehr created a system that puts the focus back where it belongs: on the patient. The result is a more sustainable way to practice medicine that helps prevent physician burnout by allowing doctors to spend time-solving problems, not just documenting them for insurance. His practice has since expanded with partners and new locations, and he's developed tools like custom EMRs and consulting services to help others make the shift. At its core, his philosophy is simple: basic healthcare should be affordable. It is so affordable, that it's “too cheap to insure” without compromising quality. He dedicated over 30 years to the U.S. Army, taking on key leadership roles including Deputy Commander and Acting Commander of the Army Medical Research and Materiel Command, as well as CEO of the Walter Reed Health Care System. After retiring from military service, he transitioned into consulting as a Specialist Executive at Deloitte, one of the world's leading advisory firms—a unique path for a physician. Dr. Jaffin discusses how the leadership experience he gained in the military helped him succeed in consulting, where he provided strategic guidance to healthcare systems such as the VA and the Department of Defense. He talks candidly about the realities of life in consulting—frequent travel, the value of clinical experience in building client trust, and the importance of clear communication. He also emphasizes that while an MBA can be helpful, it's not essential if a physician has already built a strong foundation in leadership and professional relationships. He also shares practical advice for physicians considering consulting careers, such as taking on leadership roles, developing a strong network, and building credibility in a specific area of expertise. For those thinking about leaving clinical practice, he offers a reassuring perspective: it's still possible to make a meaningful impact by improving healthcare systems and policies that affect thousands of patients—not just one at a time. After experiencing burnout from corporate medicine's unattainable metrics and assembly-line patient loads, Dr. Hakimi discovered DPC—a membership-based model where patients pay a flat monthly fee (around $150) for unlimited access to their physician through calls, texts, and visits. John and Dr. Hakimi discuss how this approach eliminates insurance bureaucracy while delivering remarkable benefits: deeply discounted labs, imaging, and medications; unhurried appointments; and the freedom to practice medicine according to one's own values rather than corporate metrics. With only 150 patients (compared to thousands in traditional practice), Dr. Hakimi operates with minimal overhead, no staff, and complete autonomy—creating a practice that not only better serves her "members" (she doesn't call them patients because "they're not sick") but has restored her professional joy. Their candid conversation reveals how physicians at breaking points with corporate medicine might find salvation in this model without needing to abandon clinical practice or compromise their incomes. First appearing in the early 2000's, this type of practice is characterized by the elimination of third party payers. In most cases, insurance billing is replaced by a subscription or “membership” model in which patients prepay for their care on a monthly or quarterly basis. The typical DPC practice agrees to provide a comprehensive set of outpatient services that meet the needs of most patients. Hospitalizations and high-cost surgeries are generally covered by traditional insurance. Under this model, the elimination of insurance billing results in lower overhead, less paperwork, fewer employees, and better access by expanding the use of texting, phone calls, email, and telemedicine. Office visits are generally longer and more comprehensive with less time spent on paperwork. While originally designed for primary care, there are examples of specialty-based DPC-style payment arrangements. John describes why this can be the solution to burnout and a way to recover the joy of practicing medicine while earning a very good income. He also explains that creating a DPC practice requires extensive planning and significant financial resources, but less than opening a traditional practice. And he offers his thoughts on transitioning from employment to your own DPC practice. You'll find links mentioned in the episode at nonclinicalphysicians.com/direct-primary-care-practice/

https://grundymdconsulting.com/ (650) 649-5352 | (513) 993-3045 Integrating Medical Knowledge into Legal Strategy Dr. David Grundy, a medical-legal consultant with over 20 years of experience, recently appeared on The Attorney Post to discuss how medical expertise plays a vital role in legal cases. With a strong background in emergency and forensic medicine, as well as peer review, Dr. Grundy provides attorneys with the insights they need to navigate complex medical evidence. The Work of Grundy MD Consulting As the founder of Grundy MD Consulting, Dr. Grundy offers pre-litigation medical opinions, case de-risking services, medical damages assessments, and expert testimony. Diverse Experience Strengthening Legal Cases Before launching his consulting firm, Dr. Grundy served as an associate professor of emergency medicine and worked internationally, gaining valuable experience that now informs his legal consulting work. His ability to interpret medical records and identify critical medical connections has proven invaluable in personal injury and medical malpractice litigation. Medical-Legal Consulting Focus Areas Dr. Grundy primarily works on personal injury cases, which make up 90% of his practice, while medical malpractice accounts for the remaining 10%. His ability to identify causation, assess future medical costs, and clarify complex medical findings allows attorneys to build stronger cases and secure better outcomes. The Shift to Medical-Legal Consulting In 2017, Dr. Grundy participated in a CME course led by Dr. Armin Feldman, a leader in the medical-legal consulting field. This course introduced him to the potential of medical consulting for attorneys, leading him to transition from clinical medicine to consulting full-time. Over the past three years, he has refined his approach, providing attorneys with clear, data-driven medical analyses. Navigating the COVID-19 Pandemic During the pandemic, Dr. Grundy worked at Seton Medical Center, treating a large and diverse patient population. The hospital also handled cases from San Quentin correctional facility, presenting additional medical and logistical challenges. Despite the difficulties, the healthcare team remained dedicated, providing critical care throughout the crisis. Post-Pandemic Changes in Communication and Social Norms Dr. Grundy has observed noticeable changes in social interactions since the pandemic, with conversations becoming less predictable and more fragmented. These shifts have affected both personal and professional interactions, altering how individuals engage in legal and medical discussions. Clarifying the Role of Consulting Physicians in Legal Cases With the vast majority of medical malpractice cases settling before trial, consulting physicians are instrumental in the pre-litigation phase. However, many attorneys struggle to differentiate between consulting physicians and expert witnesses. Dr. Grundy stresses the need for greater awareness of these distinctions to improve collaboration and case outcomes. A Passion for Helping People Through Medical-Legal Consulting Beyond his technical expertise, Dr. Grundy is deeply committed to assisting attorneys and their clients. He recognizes societal challenges, including the presence of sociopathic tendencies in certain individuals, and approaches his work with a focus on ethical, effective medical consultation. Dr. Grundy's expertise in emergency and forensic medicine has positioned him as a trusted resource for attorneys navigating complex medical cases. His consulting work continues to bridge the gap between medicine and law, helping legal professionals achieve better results for their clients. "Doing medical-legal consulting is a great way to use your medical training in a non-clinical field that really helps people. One of the many things we can do is help attorneys refute the question of a pre-existing medical condition. Often opposing counsel will try to lay off accident injuries as not related to the accident or injury in question." Armin Feldman is a medical consultant to attorneys. He shares his story and discusses his KevinMD article, "How a medical-legal consultant helped refute a possible pre-existing medical condition argument." "To a reasonable degree of medical probability, it was my opinion that the long hours the client spent typing, in addition to the poor ergonomics of his workstation, contributed to him being pre-disposed to a cervical injury sustained after lifting heavy boxes for several days. I was able to back up my opinions with evidence from the medical literature." Armin Feldman is a medical consultant to attorneys. Show NotesLast year, when I interviewed Armin Feldman, MD, I learned about a new medical specialty called forensic consulting. In today's episode, Dr. Feldman, a former psychiatrist, gets into the nitty-gritty of what it takes to practice forensic consulting. He answers questions regarding the required training, liability risk, marketing strategy, potential earnings, and how this specialty differs from working as an expert witness. While this type of practice requires extensive medical knowledge, it is more of a nonclinical medicine role ideal for retired physicians looking to continue to utilize their medical knowledge and for actively practicing physicians looking for part-time work. If you are interested in forensic consulting, Dr. Feldman has taught over 1,600 physicians how to get started. Show NotesInterview January 8, 2021Many thanks to Armin Feldman, MD, for sharing his secrets of the new medical subspecialty, “forensic medicine.” A psychiatrist and psychoanalyst by training, Dr. Feldman initially focused his energies on traumatic brain injury patients. However, for the last 13 years, he has assisted attorneys in pre-trial and pre-litigation medical-legal case preparation. Rather than function as an expert witness, Dr. Feldman delves into the medical literature to define each case's merits, interviews every client, and prepares a report. His work helps attorneys negotiate and arrive at prompt and favorable settlements. These are not medical malpractice cases but involve disability, personal injury, or workmen's compensation. To date, Dr. Feldman has accumulated a vast experience, assisting in over 3,000 cases. Dr. Feldman has already shared his experience with this type of medical-legal consulting with over 1,600 physicians. If you would like to learn more about this meaningful nonclinical work that helps people, increases your medical knowledge, and pays well, you can join Dr. Feldman at his next online seminar, April 24 and 25, 2021. To register: www.medlegal2021.comFor more information: www.andrewwilner.com
