I see you a lot on social media and reached out because I know that not everyone has an advanced directive.Discussing advanced directives can be uncomfortable, but it's crucial when making decisions for someone else. Family members often feel obligated to pursue aggressive care, which might not align with the patient's wishes. An advanced directive can guide family members or friends, ensuring decisions reflect the patient's desires.Dr. Tanno, how did you get into this field? Your experience highlights the importance of advanced directives, especially in hospitals where critical decisions are made. Please share your journey and the background of Advanced Direct MD.Thank you for having me. During my time in the ICU, I noticed that very few patients had discussed care planning with their families. Only 36% of people have an advanced directive, often skewed towards older individuals. This lack of planning leads to family members making uninformed decisions, often opting for more aggressive and sometimes futile care. I realized that advanced directives should be created with physicians, not just attorneys. This led me to teach people how to create comprehensive advanced directives through courses and lectures, providing real-life examples to clarify their importance.Let's discuss some tips for our listeners on creating an advanced directive. Firstly, you don't need an attorney to make an advanced directive. You can download your state's form online. However, it helps to create it with someone knowledgeable to ensure it's legally sound and up to date.Tip one is to name your medical decision-maker. Without one, your spouse or adult children might be the default, which can lead to conflicts. Name someone explicitly to avoid these issues. Tip two is to have a detailed conversation with your decision-maker about your medical wishes. Teach them the answer to the question: "I would want to be kept alive as long as I am still able to ___." Now that you've thought about who will speak and advocate for you should you be unable to speak for yourself, what will you include in your Advance Directive? This can be an emotional and difficult process. It's not a one-and-done document. You can and should review your AD when you have a new diagnosis or if you've learned something that has changed how you think about dying. -Note- Around the two-minute mark, I mentioned primary care providers not able to spend 15 minutes to discuss your AD. It's not because they're not interested: it's more that they are pulled in so many directions they may not be able to sit with you during a regular checkup. As part of our community health partnership with Valley Health, we talked with Dr. Jim VanKirk, a board-certified palliative care specialist and Medical Director of Valley Health's palliative care program. Dr. VanKirk explained the role of palliative medicine, which is to work with patients with serious (possibly life-threatening) illnesses and help alleviate burdensome symptoms, while the patient is still pursuing aggressive treatments. He clarified that hospice care is a later stage of palliative medicine, often when the patient stops further life-prolonging therapies and focuses on comfort. Dr. Jeff Harris joined Dr. VanKirk to talk about advance care planning. Dr. Harris is part of a volunteer group of retired physicians who offer their (free) consultative services to help families work through the details of setting up medical directives. Dr. Harris discussed the importance of discussing end-of-life plans with patients through the use of an Advance Directive, such as '5 Wishes'. He explained the benefits of walking through this document with individuals, including the opportunity to discuss other practical and emotional considerations. He also noted that the hospital provides support for these discussions through volunteers and meeting space, with no charge to patients. Dr. Harris emphasized the value of having conversations about end-of-life wishes ahead of time, which can help prevent disagreements among family members and make the process easier for everyone involved. He also highlighted the importance of having these wishes legally documented, such as through the 5 Wishes form recognized by Valley Health. He explained that 5 Wishes was chosen for its wide recognition across multiple states, making it more likely for an individual's wishes to be honored regardless of where they are. People interested in discussing healthcare decisions can call 540-536-5437 to book a one-hour meeting with one of the four specialists.    

Joanne Eason, President of Five Wishes, knows it's important to document our end-of-life wishes so our voices are heard when we can't speak for ourselves.   Five Wishes was established over 25 years ago through a series of listening tours, asking people what was important to them.  From those results, the Five Wishes document was formed with guidance from the American Bar Association and other palliative care providers across the U.S.  It's designed for individuals to use when sitting down with their families, a tool for talking about what's important to the person.    Five Wishes has incorporated three questions into their Five Wishes document: How comfortable do you want to be? How do you want to be treated? What do you want those around you to know about you?  Those answers are the voice of the patient.  Where health systems use certain forms to document decision-makers and specific medical directives, Five Wishes is about not just the wishes of the person, but who they are. It can be used like a workbook and comes in both digital and paper forms.  The workbook is a living document and should be revisited as healthcare status changes or your spokesperson changes.   Five Wishes can also be utilized by hospice agencies, healthcare organizations, and businesses. It's legal in almost every state in the U.S. so it's a very flexible document.  If you live in one of only four states (New Hampshire, Kansas, Ohio, or Texas) you can still use the Five Wishes Paper but may need to take an extra step.  Five Wishes is available in 30 languages, as well as Braille. We were on the screen today for a conversation with Dr. James VanKirk, Director of Palliative Care for Valley Health, based at Winchester Medical Center about advance medical directives. This is part of an ongoing community health partnership with Valley Health where I talk every month to physicians, administrators, nurses, etc. from Valley Health about a wide range of topics regarding healthcare. Dr. VanKirk explained what palliative care is: a specialized, interdisciplinary approach to improving comfort and quality of life at any stage of serious illness by addressing symptoms, communications, and next steps. We talked about National Healthcare Decision Day that happens on April 16 each year. He told us that it's actually observed for the entire week but advance medical directives can be done at any time of year. Dr. VanKirk talked about the different types of advance medical directives, how to start the conversation with loved ones, how to decide who you'd like to be "your person" and the steps you should take to ensure your wishes are on file somewhere like your local hospital. An advance directive is a form you can complete so that you can be in charge of your health care if you become unable to make healthcare decisions for yourself because of injury or illness. “It always seems too early until it's too late.” An Advance Directive includes two important parts: Choosing someone to be your voice when you cannot speak for yourself. This person is commonly called a health care agent, or may also be known as a Health Care Proxy, Substitute Decision-Maker, or Medical Power of Attorney. Living Will. DNR. Advance Directive. POLST.  You know these forms are important. But are they the same? Do you need all of them? What are they for? And when would you need them?   Forms are location specific. Do an online search for "[where you live] and [name of form]" or ask your lawyer to get the right version for you.   Recap of summary:  Everyone over the age of 18 should specify their wishes and designate a health care proxy, ideally with an Advance Directive or Living Will and Medical Power of Attorney If you're close to the end of life or frail and the risk of complications from resuscitation would not be worth it, that's when you want a Do Not Resuscitate (DNR) order If you're close to the end of life and don't want to artificially extend your life and you want to be sure that your wishes are honored (and it's available in your area), get a Physician Orders for Life Sustaining Treatment (POLST)
