Grab your tissues for this moving and powerful episode! Flora Sadri-Azarbayejani, DO, MPH, FAAFP, FASAM, joins host, Zach, to talk about her path to and passion for practicing addiction medicine. During the conversation, she shares stories and experiences that have been impactful on her journey and shaped both how she practices medicine – emphasizing the critical component of compassion – and her perspectives on addiction. Dr. Flora Sadri-Azarbayejani attended Boston University, where she earned an undergraduate degree in biomedical engineering and a dual master's degree in epidemiology and biostatistics, and medical science. She attained a doctorate in medicine from the University of New England College of Osteopathic Medicine. Dr. Sadri-Azarbayejani worked as an epidemiologist in infectious disease at the Massachusetts Department of Public Health; completed residency and became a family physician and eventually chief medical officer at The Community Health Center of Franklin County (Massachusetts); and, after becoming board certified in addiction medicine, has worked in the addiction medicine space since 2014 in both inpatient and outpatient settings and has opened programs in both arenas.   Dr. Sadri-Azarbayejani serves as the medical director for substance use services at Lowell Community Health Center and medical director for an inpatient addiction treatment center. She is active in various medical societies, including the Massachusetts Medical Society, the American Academy of Family Physicians (AAFP), and the American Society of Addiction Medicine (ASAM). She has been involved with the development of CARF guidelines for outpatient addiction treatment programs and hopes to make an impact on passing legislation of supervised consumption sites in the near future. Episode 45: Osteoporosis Update. Dr Linares (endocrinologist) explains the basics of screening and treatment of osteoporosis, referring frequently to the updated guidelines of osteoporosis by AACE and ACE (2020). This group of residents will start in July 2021 and will graduate in July 2024. We hope you enjoy your time with us.Today is March 22, 2021.Implanted pacemakers and defibrillators are equipped with a switch that responds to magnetic forces to stop them when needed. Magnetic interference between these cardiac implantable electronic devices (CIEDs) and mobile devices have been investigated for years. It has been established that magnetic fields stronger than 10 gauss can deactivate these cardiac devices, causing pacemakers to give asynchronous pacing and ICDs to stop tachyarrhythmia detection.The Heart Rhythm Society journal, published in October 2009 (that was 11 years ago), an association between portable headphones and significant electromagnetic interference (EMI) in patients with implantable cardioverter-defibrillators (ICD) and pacemakers (PM). 100 patients with implanted devices were tested with different portable headphones. Headphones effectively deactivated implanted devices when held less than 2 cm from skin on the left side of chest. There was not interference when headphones were placed farther than 3 cm. In this study, normal functioning of the devices was restored in 29 out of 30 cases when the headphones were removed from the patient’s chest. The recommendation from that study was to recommend patients to keep their portable headphones at least 3 cm away from their implanted device.More recently, in January 2021, the same journal posted the effect of iPhone 12 on ICDs deactivation. iPhone 12 and MagSafe technology, which allows faster wireless charging, contain strong magnets. iPhone 12 successfully deactivated a Medtronic Inc. ICD when tested by a group of investigators in a patient[2]. The official Apple Support website posted on February 25, 2021, “To avoid any potential interactions with these devices, keep your iPhone and MagSafe accessories a safe distance away from your device (more than 6 inches / 15 cm apart or more than 12 inches / 30 cm apart if wirelessly charging)”[3]. Other devices such as fitness tracker wristbands, and even e-cigarettes have been involved in deactivation of ICDs.Bottom line: Make sure your patient discusses with you or their cardiologist before buying wearable or mobile technology that may interfere with their implanted cardiovascular devices.This is Rio Bravo qWeek, your weekly dose of knowledge brought to you by the Rio Bravo Family Medicine Residency Program from Bakersfield, California. This is not a complete analysis of those guidelines. For a comprehensive explanation of the guidelines, visit the AACE or ACE websites. The recommendations from these organizations may be different than the ones given by the American Academy of Family Physicians (AAFP) or the United States Preventive Services Taskforce (USPSTF), which are organizations we are more familiar with as family physicians.The questions analyzed during this conversation includes:When would you consider a DEXA scan to screen a woman younger than 65 for osteoporosis? What to do when the report says Osteopenia (T score -1.0 to -2.5)? Let’s mention the recommended dose of Vitamin D and Calcium. What is the FRAX score?  What is an easy work up we can do to rule out a secondary cause of osteoporosis before sending patient to you? The new guidelines divide patients in two categories: “High risk/no risk of fractures” and “VERY High risk/prior fractures”, What’s the difference in management between those two categories? (alendronate in high risk vs abaloparatide in very high risk). How can you tell the patient has a good response after 1 year of treatment (Dexa scan, bone turnover markers)? What is a drug holiday? ___________________________Now we conclude our episode number 45 “Osteoporosis Update”. Dr Linares explained what the FRAX score is and mentioned the different options we have for treatment of osteoporosis. DEXA scan continues to be the gold standard for screening, diagnosis and monitoring of osteoporosis. I mean, that's why we encourage our patients to track with they're their eating, or patients with diabetes to monitor their blood sugar so that they can use that information to to make healthier choices to better improve their overall health and well-being.  Fertility awareness based methods are such an incredibly effective tool to educate and empower women, and honestly, engage men back in the conversation of family planning.” - Dr. Marguerite Duane   Dr. Marguerite Duane is a board-certified family physician and co-founder and Executive Director of FACTS, the Fertility Appreciation Collaborative to Teach the Science.  She serves as an Adjunct Associate Professor at Georgetown University, where she directs an introductory course on natural or fertility awareness based methods of family planning. She is a practicing direct primary care physician and she has served on the board of the American Academy of Family Physicians (AAFP) and the Family Medicine Education Consortium (FMEC). After receiving a Bachelor of Science with Honors and a Master of Health Administration from Cornell University, she earned her medical degree from the State University of New York at Stony Brook and completed her family medicine residency at Lancaster General Hospital. During her residency, she was surprised to hear her senior resident explain to a postpartum patient that there is a way women can learn to manage their fertility without any medical side effects, such as those that occur from hormonal birth control.  She wondered how it was possible this topic hadn't been covered in her medical training. This insight planted the seed that changed the trajectory of her career. Dr. Duane began to focus on learning more about these methods, for her own personal health as well as that of her patients.  She went on to complete training in the Creighton Model of natural family planning and has since made it her passion to educate other healthcare providers and patients. The opportunity was to provide science-based dietary supplement information on the products that were part of the Nature Made brand. The reach of the family physician is broad with, nearly 1 in 4 doctor visits in the United States being made to a family physician. Four years ago, Pharmavite became a founding partner of a new AAFP education initiative to bring the Nature Made’s science based messages to family physicians. Overall support from the AAFP leadership and response from the AAFP membership to the dietary supplement education programs has been overwhelmingly positive. On September 26, 2013, Doug MacLean, Pharmavite’s CEO, accepted the American Academy of Family Physicians Foundation’s Outstanding Service Award on behalf of Pharmavite at the AAFP Foundation Dinner. It is the Academy’s highest recognition for vendors, suppliers and companies that support the medical profession or their employees. “The depth of this honor is that the nominations are made by the AAFP state chapters, members and staff to the AAFP Board of Directors,” said Doug MacLean. “It speaks to the strength of the relationship between our two organizations and our common vision of optimizing health and wellness for individuals, our communities and people around the world.” The 11th year will break new ground with Pharmavite’s creation of an initiative called Nature Made Cares About Family Medicine. It is designed to raise funds to support Family Medicine Cares, the AAFP Foundation’s humanitarian program to establish new free medical clinics. The initiative encourages physician engagement with and education about dietary supplements with patients. “The Nature Made Cares About Family Medicine program adds a new dimension to Pharmavite’s support of the Foundation’s work,” said Craig Doane, Vice President of Strategic Partnerships at the AAFP. “We know our members place a high value on the humanitarian efforts of the organization, and they consistently request and need patient education materials for their day-to-day practice. The innovative design of this new program allows both goals to be achieved.” Nature Made Cares About Family Medicine is a part of Pharmavite’s strategic partnership with the AAFP. The partnership allows Nature Made to reach AAFP’s physician members with Nature Made brand information and dietary supplement-focused educational materials.
