Amy is currently a Hospice Compliance RN for Compassus. Prior to her current position, she held leadership positions in hospice, palliative care, and quality with BJC Homecare in St. Louis. She was an integral part in the development and opening of Evelyn's House, a 16-bed hospice inpatient house, in St. Louis. She also assisted in the implementation of a system-wide palliative care program. She has volunteered with HPNA and HPNF for several years as an award reviewer, member of the planning committee for the annual assembly, and a member of the HPNA Professional Development Advisory Council. She is especially proud to have facilitated support for the Emerging Leaders Award with HPNF and the BJC Institute for Palliative and Supportive Care.   Catherine Pyke, M.A. Ed.  Catherine Pyke worked for thirty years in non-profits in the San Francisco Bay Area, primarily as a program officer for the Hearst Foundations and more recently as a Philanthropic Advisor for the Gladstone Institutes in Mission Bay. She is the founder of Centerpiece Philanthropy, a consultancy that provides philanthropic advising and coaching, grant writing, strategic planning and group facilitation. She is the author of Jane Lathrop Stanford, Mother of a University. In addition to serving on the HPNF board, she serves as the Communications Chair for the San Francisco Region of the Order of St John, an international volunteer organization whose mission is to support St John of Jerusalem Eye Hospital and social and health care needs in the U.S. She recently completed two terms on the board of the Episcopal Impact Fund in San Francisco. An alumna of Scripps College in Claremont California, with a Masters in Education from Stanford University, she came to appreciate the contributions of innovative philanthropists, while visiting colleges, universities, medical centers arts organizations and social service agencies throughout the West. Certified in Advanced Hospice and Palliative Nursing, she provides care for patients in all settings, both pediatric and adult in Palliative Care and Hospice services. She has presented at regional and national meetings on a variety of topics related to serious and advanced illness as well as operation of community-based programs and has served on the American Academy of Hospice and Palliative Medicine (AAHPM) Task Force for Quality, and on the Home-Based Workgroup for the Center to Advance Palliative Care. She has been part of the long-term care faculty for Bootcamp at the CAPC annual seminar for 3 years and facilitated Virtual Office Hours since 2018. Rikki is the author of several book chapters in APRN textbooks and Core Competencies and contributed to a number of articles published in HPM journals.  She has been part of the core team for several Project Echo projects at Four Seasons. Rikki has provided both didactic education and mentoring to APRNs on Palliative Care and Hospice to local students over multiple years.  Most recently she has attained certification as a Nurse Executive and was inducted as a 2023 Fellow in Hospice and Palliative Nursing. She has volunteered on the HPNA Clinical Forum planning committee, the AAHPM/HPNA Annual Assembly Abstract Review Committee, served as the Co-Chair for the Community Palliative Care SIG and currently serves as State Ambassador for North Carolina and on the FPCN Application Review Committee.  Yvonne Ruathaiwat, MSN, RN, GCNS-BC, NE-BC, CCM, PHN Yvonne Ruathaiwat has a diverse clinical and leadership background across the care continuum. Yvonne most recently served as the Chief Operating Officer/Senior VP of Clinical Services for Hospice of the East Bay in Northern California. Her previous experience involved operations and quality improvements for Palliative Care service line, transition of care programs, post-acute ambulatory care, skilled nursing facility partnerships, and population health management for medical respite/high utilizer groups. Yvonne's past roles ranged from advanced practice nurse to adjunct professor.   Yvonne obtained her Master of Science degree as a Geriatric Clinical Nurse Specialist with specialty in Nursing Education. Marianne Fingerhood, DNP, ANP-BC, joins us to share her journey into the field of addiction medicine and discuss the importance of providing medication treatment for addiction within a primary care setting. She talks about the essential role nurse practitioners (NPs) play in treating addiction, how crucial it is for NPs to have proper support, and the benefits of a professional community. Marianne touches on the dynamics of having a husband, Dr. Michael Fingerhood, who also treats addiction. She describes the unique ways nurses can combat stigma related to addiction and shares about the Words Matter Pledge initiative within the Johns Hopkins School of Medicine. Marianne also emphasizes the significance of resilience as a provider and offers words of wisdom to future NPs and addiction medicine practitioners. Dr. Marianne Fingerhood has been a nurse since 1985, a nursing educator since 1990, and a nurse practitioner in primary care since 1999. In all settings, she has brought a passion for sharing knowledge, whether with patients, students, or colleagues. In her clinician role, she is the champion for expanding access to treatment for substance use disorders (SUD) in the primary care setting. As the Adult Gerontology Primary Care Nurse Practitioner program director at Johns Hopkins University, she created a curriculum related to compassionate treatment of chronic pain, safe opioid prescribing, and recognition and treatment of SUD. She directs the Supporting Nursing Advanced Practice Transitions (SNAPT) Fellowship for nurse practitioners entering practice post-graduation. Anemia presents a significant challenge in the management of patients with low-risk myelodysplastic syndromes (MDS). Given the considerable potential for symptom burden associated with anemia in patients with MDS, health care professionals must prioritize enhancing quality of life when evaluating treatment options. In this episode, CANCER BUZZ speaks with David Sallman, MD, associate member in the Department of Malignant Hematology at Moffitt Cancer Center, and Karley Trautman, DNP, ANP-BC, AGACNP-BC, senior instructor of Medical Oncology at Highlands Ranch Cancer Center, to discuss best practices in the management of anemia in low-risk MDS patients.  “The goal is still personalizing [treatment] for the patient. So say a patient feel[s] better when their hemoglobin is greater than 10 grams per deciliter, as an example. Post-2020, OHNs, RNs, and patients alike were – and still are – burnt out. It can be difficult to discern if the mental and physical symptoms patients exhibit – fatigue, “brain fog,” issues with sleep, shortness of breath, etc. – are caused by Long COVID or a rise in mental health conditions. Dr. O'Keefe and our host Stephanie T. Weinsier, DNP, ANP-BC, COHN-S, FAAOHN, dive into how to manage Long COVID symptoms and self-care recommendations for OHNs and RNs. Exocrine Pancreatic Insufficiency (EPI) is a condition caused by damage to the pancreas that results in reduced capability to produce or deliver digestive enzymes which break down fats, carbohydrates or proteins — or to a decrease in the enzyme activity in the small intestine. EPI is most commonly associated with chronic pancreatitis (in adults), cystic fibrosis (in children), diabetes (both type 1 and type 2) and other etiologies such as acute pancreatitis, pancreatic cancer or resection, celiac and Crohn's disease. Consequently, individuals with EPI may suffer malnutrition and fat-soluble vitamin deficiency; loss of bone mass; growth and immune deficiencies; and may experience poorer outcomes such as longer hospital stays and lower survival rates of underlying medical conditions secondary to malnutrition. “I think oftentimes people think this is just a radiology procedure that is rather benign. That's really the role of the oncology nurse, just to be [an educator], support, emotional support, and a coach,” Lisa Parks, MS, APRN-CNP, ANP-BC, nurse practitioner in hepatobiliary surgery at The James Cancer Hospital and Solove Research Institute at The Ohio State University Comprehensive Cancer Center in Columbus, Ohio, told Jaime Weimer, MSN, RN, AGCNS-BS, AOCNS®, manager of oncology nursing practice at ONS, during a discussion about what oncology nurses should know about transarterial chemoembolization administration and their role surrounding that procedure. Therefore, it is no longer really used in metastatic colon cancer. TACE is used in hepatocellular cancer. It also was used more than 10 years ago to treat metastatic neuroendocrine cancers. But recent research has showed that neuroendocrine cancers respond to this embolization without the use of chemotherapy. By eliminating chemotherapy, we also eliminate the potential for side effects.” TS 3:29  “TACE, or TAE, is usually completed more than once in the course of a patient's treatment. Depending on the tumor burden of the liver, the procedure can be segmentally completed on a liver lobe, or you can do the procedure on the right lobe and then follow-up treatment in about six weeks in the left lobe.” TS 5:45  “This is something that isn't even really taught in medical school. So it's really important to understand that even though this is a postprocedural side effect, there are certain things that you have to be aware of. So, the most common side effect that you will see is right upper-quadrant pain, and this is very common. And if the left side of the liver has received the therapy, this pain can radiate to the epigastric area and the patient will describe it as chest pain. And when you have the patient point to that area where he's having pain, it's often epigastric and it's just a referred pain, it's not cardiac pain, typically. You can get a EKG and troponin, but those are almost always negative and it's just really part of this embolization syndrome.” TS 14:30  “As far as what the oncology nurse needs to really be aware of pre-TACE or pre-TAE, I just want to emphasize the importance of patient education. The patient and their family need to understand again, it's not a surgery, it's a radiology procedure, and that the patient is going to have abdominal pain and nausea and vomiting that will last for several weeks and that is why they are not kept in the hospital for three weeks until these symptoms dissipate. Oftentimes these symptoms will be present until they get reimaged at the medical oncologist and then it's time for them to come back and maybe get another phase of their procedure that they are supposed to have as part of their treatment plan.” TS 17:44  “I do want to let you know, though, that patients that have a significant spike of their transaminases over 1,000, those patients are of great concern of going into liver failure. So, the nurses need to let the patient know that they will be monitored and kept in the hospital until we start to see a downtrend in those transaminases before they will be discharged.” TS 19:48  “As an oncology nurse and medical oncology, [it's] education, education, education. Also being able to triage these patients on the phone, talking them through how to keep themselves hydrated. . . . So I just think it's really a coaching job of the oncology nurse. A lot of reassurance, a lot of suggestions on how to get through this very uncomfortable difficult procedure.” TS 21:16  “I want it to be clear that if you're doing local regional therapy, TACE or TAE, this is considered a palliative procedure. You are not going to get a cure with this treatment. In this situation, neuroendocrine carcinoma, it's already metastatic if you're treating the liver. And with hepatocellular, again, it's still palliative because you're not doing a surgical resection on this patient. And every TACE experience for every patient, I've had patients that've and I've had six of these procedures, every experience they get is different.” TS 28:11  “When the patient comes back to our floor after having the treatment, it's just very important for those nurses to know the ‘abnormal normal,' to know that some of the things that they're seeing, the hypertension, the severe pain, the severe nausea, is actually normal and the provider will work with them to try to come up with a regimen that will make the patient as comfortable as possible.” TS 33:22
