Thank you for downloading another episode of Tahoe TAP! As always, Mike Peron & Rob Galloway are keeping it local with a quick round up of top news from around the Tahoe region. Then, the majority of our show will be spent getting to know about the new hospital building at Stateline, Nevada from Dr. Clint Purvance, President & CEO of Barton Health. At 18 years old, Dr. Purvance became an EMT, which inspired him to pursue a lifelong career in health care. He now serves as the President and CEO of Barton Health in South Lake Tahoe. He began his medical career at Barton in 1999 as an emergency physician and later held the position of Chief Medical Officer from 2007 to 2015. In November 2015, he assumed the role of CEO, bringing a clinician's perspective to healthcare leadership. ​ Dr. Purvance earned his medical degree from the University of Nevada School of Medicine in 1996 and completed his residency in Emergency Medicine at the University of Michigan in 1999. He is board-certified by the American Board of Emergency Medicine and holds a Certified Physician Executive (CPE) credential from the Certifying Commission in Medical Management.  Under his leadership, Barton Health has expanded its services to include specialties such as urology, psychiatry, cardiology, and neurosurgery. Dr. Purvance has also overseen the development of the Robert Maloff Center of Excellence, a facility dedicated to orthopedics, sports performance, and wellness services. ​ A long-standing member of the Lake Tahoe community, Dr. Purvance and his wife, Shawna, have five children. They enjoy outdoor activities such as paddleboarding, skiing, and hiking in their free time.

Show Notes To celebrate Pet Dental Health Month, the Skin Flints team looked a bit further afield this month, exploring gum health and Canine Chronic Ulcerative Stomatitis with Hannah van Velzen. Chapter 1 – Understanding the Oral Mucosa and Inflammation (02:53) John welcomes Hannah, who introduces herself and her journey into veterinary dentistry, from her studies in the Netherlands to her current role leading the dentistry referral service at Fitzpatrick Referrals. She highlights the small but growing number of veterinary dentistry specialists in the UK. (05:46) Sue asks for a basic overview of the oral mucosa, as it plays a key role in CCUS. Hannah explains that gingiva surrounds and seals the teeth, preventing bacteria from entering the body, while mucosa covers the rest of the mouth. The mucogingival junction marks the boundary between the two and helps differentiate between gingivitis and mucositis. She describes the different types of mucosa, including lingual (tongue), palatal (roof of the mouth), alveolar (bone covering), vestibular (cheek and lip folds), buccal (cheeks), and labial (lips). These structures vary in thickness and function, with keratinized areas like the tongue and hard palate providing protection, while thinner, non-keratinized areas aid in saliva flow and bacterial clearance. (13:24) John then asks Hannah to define common inflammatory conditions affecting the mouth, including: Gingivitis – Inflammation limited to the gingiva, without mucosal involvement. Mucositis (stomatitis) – Inflammation affecting the mucosa, which is central to CCUS. Periodontitis – Inflammation of the structures supporting the tooth, which can lead to tooth loss. Hannah emphasises the importance of accurately defining oral lesions to guide diagnosis and treatment.    Chapter 2 – What is CCUS? How Can It Be Diagnosed? (18:43) John introduces Canine Chronic Ulcerative Stomatitis (CCUS), asking how it relates to previous terms like CUPS (Canine Ulcerative Paradental Stomatitis) or contact mucositis. Hannah explains that CCUS was formerly known as CUPS, but the name changed as research showed that 40% of lesions occurred in areas without teeth, making the term "paradental" inaccurate. The condition is chronic, meaning it develops gradually rather than suddenly. (23:22) Sue asks how a primary care vet should determine whether a dog with oral ulcerations has CCUS or another condition, such as pemphigus vulgaris, lupus, or uremic stomatitis. Hannah acknowledges that many inflammatory and autoimmune diseases look similar and that no single exam finding confirms CCUS. She advises vets to follow key diagnostic steps: Perform a thorough dental cleaning and radiographs to rule out periodontal disease. Differentiate gingivitis (gum inflammation) from mucositis (mucosal inflammation). Take a biopsy if mucosal inflammation is present, as periodontal disease should not cause mucositis. Look for "lymphoplasmacytic infiltrates" on biopsy, which strongly suggest CCUS. If the biopsy findings suggest CCUS, referral to a dentistry specialist is recommended. If results are inconclusive, a dermatologist may need to investigate potential autoimmune conditions. (27:33) Sue asks whether "kissing lesions" (ulcerative lesions where mucosa touches the teeth) strongly indicate CCUS. Hannah agrees that they are a key sign, but notes that plaque build-up can also cause similar inflammation. A dental clean should be performed first—if inflammation persists despite clean teeth, CCUS is more likely. (28:31) Sue then asks if certain breeds are predisposed to CCUS. Hannah confirms that small breeds and terriers are overrepresented, particularly: Cavaliers, Labradors, Maltese, and Greyhounds. Greyhounds are prone due to poor dental health and periodontal disease. Spaniels may also be affected, though this is not yet confirmed in literature. Some affected dogs have severe gingivitis and mucosal inflammation despite excellent dental hygiene, making CCUS harder to recognise. (31:33) John asks how easy biopsies in the mouth are Hannah stresses that biopsies should always be done under general anaesthesia for pain control and a thorough oral exam. She typically uses a punch biopsy, ensuring a portion of normal tissue is included to help distinguish inflammatory from autoimmune causes. She highlights the importance of sending clear photos and case details to assist pathologists in interpreting results. Additional tests like immunohistochemistry may sometimes be useful.   Chapter 3 – Treating CCUS: What Are the Options? (35:44) John asks how CCUS is treated and whether treatment varies by severity. Hannah explains that CCUS treatment is multi-step and includes: Dental Cleaning & Plaque Management: Full dental cleaning is the first step. Extractions are considered only for teeth that contribute to inflammation. In mild cases, cleaning + home care (brushing, antiseptics) may suffice. Home Management & Pain Control: Some owners can maintain oral hygiene, others cannot. Pain relief options include NSAIDs, paracetamol, gabapentin, or amitriptyline. Feeding tubes may be used in extreme cases for pain-free nutrition. Medical Management for Severe Cases: Two main protocols exist: Cyclosporine + Metronidazole (immune modulation & bacterial control). Doxycycline (low dose), Pentoxifylline (ulcer management), and Niacinamide (vitamin B3). The choice depends on vet preference and patient response. Long-Term Management & Research Gaps: Some dogs may eventually stop medication once inflammation is controlled. More research is needed to determine which cases respond best to which treatments. Avoiding full-mouth tooth extractions remains a key goal. (45:14) Sue highlights the lack of published research on CCUS and urges vets to seek specialist advice before extracting all teeth.

A rate critical to attain better care for patients, I'm gonna say, is enlightened leadership—maybe dyad leadership—at a clinical organization. I am saying this because without enlightened leaders, it'd be harder to build from the blueprint that Beau Raymond, MD, talks about today on the show. I'd say an enlightened leader is someone—and this is my definition, but it's a term that Tom Lee, MD, brought up first in an earlier episode (EP445)—an enlightened leader really cares about providing better patient care at an affordable price. They have a manifesto to that end, if you will. They also have studied, likely, and understand how change management works because every improvement requires change. They get the bit about people, processes, and technology being intertwined and what operational excellence means. Further, they are probably doing or considering many of the things that Robert Pearl, MD, talked about in episode 412. On the opposite end of the spectrum, there's a new term floating around called administrative harm. There's a study. Admin harm refers as much to what administrators—who I refuse to call leaders at this context because I'm talking about the not good administrators, so let's be clear—but I'd say administrative harm results from what the administrators choose not to do as much as what they choose to do. It is actually a thing to be an enlightened leader, especially in these profit-driven times. It's really tough, actually, and nothing anyone should take for granted. So, maybe this whole show is kind of a shout-out to the enlightened leaders out there. Thanks for doing what you do. Okay, so this said, and it needed to be said, let's talk blueprint for better care in the conversation that follows. Dr. Beau Raymond says, step 1, right out of the gate, set clear goals. Then step 2, engage others throughout the organization to together build the framework needed to achieve said goals. Engaging frontline folks and others is really the only way that any proposed framework will actually work in the real world. Listen to the shows with Karen Root (EP381) and Ashleigh Gunter (EP447) for just one proof point after another that what I say is based in fact. Step 3 of the blueprint to better outcomes that Beau Raymond, MD, talks about today is get your data. We talk a lot about plan sponsors and the getting of data, but same thing applies to clinical organizations. For clinical organizations, the getting of data means longitudinal data. The need for longitudinal data has come up in multiple shows, most recently the one with Dan Nardi (Spotlight Episode), and this is just one example of why getting the whole bag of data really matters. Dan said on that earlier show, it's often a thing that oncologists are unaware of how many of their patients are winding up in the ER for nausea after chemo, which, by the way, is the most common cause for readmission. And the reason for this is lots of patients travel to their oncologist but go to a local ER in a different health system. The show with Brendan Keeler (EP454) about the Particle v Epic lawsuit in general dustup over who gets the data is super relevant here. That's what I was thinking when I was talking with Dr. Raymond, and maybe it just popped in your head, too. Or just continuing this topic of the importance of longitudinal data, how many specialists, in almost any specialty, see a patient and then don't know what happened to that patient subsequently? Or even primary care in transactional models? So, step 3 here is get your data and also, as part of that, figure out how to make sure everybody understands the data and also understands that it is fair. Eric Gallagher (EP405), Dr. Raymond's dyad counterpart over at Ochsner, talked about this some in that episode. So did Kenny Cole, MD (EP431), interestingly, also from Ochsner. Amy Scanlan, MD (EP402) mentions it as well. Step 4 in the blueprint to measurably better outcomes that I discuss with Dr. Beau Raymond, data collection and data management probably need to be system-wide because … yeah, longitudinal and etc. But the “What are you gonna do now with the insights that you derived from the data?” is pretty local. The obstacles and enablers are going to be different depending on the geography. For example, an area with a large Vietnamese population and a big variation in colorectal screening rates as a priority, just logically, is gonna have a program that is in no way suited to roll out in an area with, say, a large Black or African American population with high hypertension rates. Priorities and programs are just different depending on the geography. So, step 4 here is, ask each region, based on the data, what fixes they're going to own. What will they take ownership on and commit to improving? What I thought was interesting in this interview is kind of the way that equity comes up between ethnic groups or between genders. In and of itself, obviously, striving for equity is critical. But also, if you're trying to improve quality across the board and you see disparities in care, figuring out what is going on with the group experiencing the worse outcomes is also just operational excellence. You don't want to be a solution looking around for a problem, after all; so, you need to figure out the actual problems for the actual people experiencing the problems to avoid that. Those are the only solutions that are actually gonna work. Step 5 is to learn from each other. Maybe not a whole program is flat-out transferable from one geography to another, but that doesn't mean that nothing is transferable either. As usual, it's about being thoughtful and nuanced and finding that productive middle. At Ochsner, they do these cool weekly primary care huddles to share learnings and goings-on that Dr. Raymond explains in the show that follows. Throughout all of these steps in this blueprint, there is obviously a need to align how the practice or system is getting paid for the time and capital expenditures, of course. And Dr. Raymond addresses this and interestingly says something similar to what Dr. Tom Lee (EP445) and Scott Conard, MD (EP391) have said on earlier shows: that a lot of times compensation for improving care, if you do it in an operationally excellent way, can be revenue positive for systems with a combination of both FFS (fee for service) and value-based reimbursement. Underline, however, the part about having an enlightened leader who cares about clinical quality for that to work out. Dr. Beau Raymond, my guest today is chief medical officer for Ochsner Health Network. Ochsner Health Network, by the way, includes Ochsner and some other health system partners. There's also a bunch of small independent practices of one to two docs. He graduated from Harvard University and then pursued a medical degree from Rutgers New Jersey Medical School, followed by an orthopaedic surgical residency at the Cleveland Clinic Foundation. During his clinical training he had the opportunity to spend some time working at a clinic at a ski resort in Vermont, and spending a few months at the Nuffield Orthopaedic Centre in Oxford, England. In addition to a full-time clinical practice as an orthopaedic surgeon, Dr. Alhadeff has also worked in many administrative and leadership roles in hospitals and health systems over the years, serving as department chair, executive board member, Finance Committee Chair, President of the Medical staff, and President and CEO of OSS Health.  He completed a Masters in Medical Management, MMM, at Carnegie Mellon University and completed additional healthcare leadership training with the Harvard Business Schools Executive Education programs and the American Association of Physician Leadership, AAPL. He discusses his efforts as president of the Tennessee Society of Addiction Medicine (TnSAM) to address stigma and other issues and shares his primary goals in this leadership role. Dr. Kirby highlights some impactful moments during his career and offers advice for the next generation of addiction medicine providers. Dr. Jason Kirby serves as the chief medical officer for Landmark Recovery and Praxis by Landmark Recovery and is currently the president of TnSAM. He is an expert in population health policy, healthcare economics, and human behavior and is dual board-certified in addiction medicine and family medicine. He received his medical degree from the West Virginia School of Osteopathic Medicine and his MBA from Point Park University. Dr. Kirby also is a fellow and leader of the American Society of Addiction Medicine (ASAM), having served or currently serving on its Finance, ASAM Criteria Strategy Steering, Annual Conference Program Planning, and Levels of Care Certification Oversight Committees. Recently, he co-authored the withdrawal management chapter for The ASAM Criteria, 4th Edition. He previously served as medical director of addiction and recovery services at St. Peter's Health Partners in Albany, NY, and practiced at St. Peter's Hospital as their chief of behavioral health. Welcome to the third of a six-part series focused on the integration of palliative care into the practice of surgery.  Malignant bowel obstructions occur in 5-43 percent of patients with a diagnosis of advanced primary or metastatic intra-abdominal malignancy and both acute care surgeons and surgical oncologists are called upon to manage this condition.
